  • Korean society is experiencing rapid changes which are certain to shape the lives of the older people and their families. So, the purpose of this study was to improve the quality of the elderly welfare service in Korea. Even though the interest about the elderly welfare in Korea is rapidly on the increase, the reality of Korean Society still leaves much to be desired, quantitatively as well as qualitatively. Pay attent to this point, now going to grasp the situation of not the handicapped elder people but the general elder people actual life condition and demand for welfare. So this study focused on the demand of not out-of home service but in-home care service. It was based on the data from by Korea National Statistical Office, having observed 6,139 elderly people. The results were as follows. They hope to get the elderly welfare services about health examination service, nursing survice, supporting service for household. That was, the most of them wanted in-home care service than out-of home care service. To ensure effective care to the elderly, it needed development and settlement of welfare service in face of their daily living. And, even though they recognized that they should cover the elderly life expenses themselves, the rate was hit that indicated their adult children as a parents supporter. This means that we should consider not only to the elder people but also to the family which contained the old people as the elderly welfare service. As a remedy, we can find the cooperation between elder welfare service at the social welfare organization and counsel, education at the family strengthen center. To improve the quality of the elderly welfare service in Korea, the elderly welfare service should focus not only on the physical and spiritual health maintenance but also active understanding living environment and growing ability to arbitrate between individual and living.

일 지역 취약가구 독거노인의 건강상태에 따른 가정지원과 도움요구 (Home Care Support and Support Requirements According to Health Condition in the Poor Elderly People Living Alone)

  • Purpose: To identify the health condition, home care support, support requirement of poor and elderly people living alone. Method: Data was collected through self-administered questionnaires and analyzed by descriptive statistics, t-test, ANOVA and correlation. Survey involved 269 conveniently selected who have a social support in H city. Result: Perceived health condition of subjects was bad to moderate (mean score: 2.22). There were significant home care support differences according to gender, religion, education level and dwelling pattern. Support requirement was influences only by the dwelling pattern. Perceived health condition showed a positive correlation with home care support of friends and neighbors, and a negative correlation with support requirement (medical, material, economic emotional support). Conclusion: These findings are expected to make a positive contribution to create an ideal intervention for public visiting nurses and social workers to improve the quality of life in poor and elderly people who live alone.

일본의 재가간호 및 방문간호 -새로운 개호보험제도의 실시를 앞두고- (A Study on Home Care and Home Visiting Nursing in Japan)

  • Japan has been prepared for aging society from 1970. In 1970, the percentage of distribution of population of 65 years old and over was 7.1%. It is similar to present percentile of the elderly in Korea. Therefore, it will be needed to study about home care and home visiting nursing in Japan at present. This study was aimed to prepare the fundamental documents for home care nursing in Korea and to know the background of new health care system of Long-term Care Insurance in Japan, by studying home care and home visiting nursing in Japan. With the continuing aging of the population, especially the increase in the number of latter stage elderly, it is predicted that there will be an increase in the number of the elderly who are bedridden and suffering senile dementia. To ensure that these people will be able to continue living in the communities and homes they are accustomed to, surrounded by their families and neighbors, Japan substantially improve and expand in-home services. There were also long-term effort to reach the level of services outlined in the Gold Plan and the New Gold Plan within the decade between FY 1991 and FY 1999 in the field of health care and welfare. Under this plan, the most noticeable change was occurred in home care, home was permitted as the field of care and visiting nursing was established in law. Through this 10- Year Strategy for Promotion of Health and Welfare Services for the Aged, many problems have been improved and solved, but some problems remained such as inadequate service supply and consumption of medical insurance for the elderly. Japan will be a society composed 25% of elderly people of total population in 2020, and it will be soon faced with a shortage of welfare and medical facilities and manpower. As for equalizing the benefits and cost burdens, and other future arrangements for health care and welfare, Long-term Care insurance system was established in 1994. This system will be enforced from April 2000 and use present facilities and services. To know home care and home visiting nursing in Japan, we need to consider present conditions well and to take notice of changes and measures to cope with an aging society continuously.

가정간호환자의 일상생활작동수행능력 변화에 대한 연구 (A study on Changes in ADL Functioning of Residents in Taejon City)

  • This study was done to investigate the ADL differences between before and after home care. For this survey, the Barthel index, an ADU(activities of daily living) assessment, and general history questions were asked. Functional performance, i.e. ADL, was studied in a population a total of 56 men and women aged 65 and older from the city of Taejon. Among the independent subjects, women, 73 years of age and older, married status, 1-2 times taking home care per month, the case answering 'Quite' about satisfactory of home care, elderly dwelling with others and who have helper and spouse, elderly having a snack regularly, are statistically significant. Also ADL differences were found in grooming, getting in and out chair, getting on and off toilet, walking 500 meters on the level. Further studies should evaluate the activities of daily living to predict important disability-related outcomes.

경기도 일부 노인의 노인장기요양보험 방문요양서비스의 영양상태 평가 및 영양관리 현황 (Nutritional Assessment and Management in Long-term Care Insurance's Home Visit Care Service)

  • The purpose of the study was to examine nutritional management and nutritional care demands of home-visit care service and the nutritional status of serviced elderly. The survey was carried out from $5^{th}$ to $21^{st}$ of January, 2011 among 37 In-Home Service institutions. The number of elderly surveyed by 143 care helpers was 281. Analysis was performed for 203 subjects (male: 44, female: 159). The age distribution was from 50 to 99 years. The grading by long-term care insuranceshowed 18,2% on level 1 and 2, and 81.8% on level 3. For the disease treatment, the proportion of implementing diet treatment turned out to be very low. The proportion of subjects living with their children was 45.3%, and living alone was 29.1%. The proportion of home-visit care among types of In-Home Service was 70%. Subjects who needed additional necessary diet management service turned out to be 40.9%. Diet management was the most necessary services from all levels. MNA (specifiy MNA) scores were significantly (p < 0.001) associated with BMI, mid-arm circumference (MAC), calf circumference (CC), intake problems, and weight loss during the last 3 months. For studied elderly, 45.3% were malnourished, and 46.8% were at the risk of malnutrition by MNA score. Based on the result of this survey, the nutritional care was not systematic on the In-Home Service. Prevention and management of the disease was much better than the treatment to improve the quality of life. We conclude that nutrition management is necessary for the elderly. To improve nutritional status of elderly in home care services, systematic nutrition management should be implemented.

노인의 부양유형 선호와 영향요인 (The Preferences of The Elderly for Their Care Types and Contributing Factors)

  • The purpose of this study is to identify the preferences for care type among the elderly and to analyze contributing variables. The sample in this study are 485 elder people who are age 55 and over. The independent variables are divided into five factors according to their characteristics, and the dependent variables fall into two categories of private and public care types. The descriptive statistics and Logistic Regression method are employed for the analysis. The results can be summarized as follows. The elderly prefer individual care paid for by their own support to the other types like the care by children or institution. However, the elderly stiff like and expect family care by their children. The variables that haute affected the preferences of the care types among the elderly are whether they are employed or not, household's the amount of debt, whether they currently live with the oldest child, their level of self esteem and their degree of satisfaction in life. The results show that the elderly don't have various wants or alternatives to their care and regard public care as negative support for the financially incapable or the elderly who have no relatives.

노인요양보장제도와 재가 간호사업 현황 및 과제 (Policy Alternatives on Developing A Home Health Care System for Long-term Health Care Insurance System in Korea)

  • Presented here are policy alternatives for understanding home health care for the long-term health care insurance system which is being developed for elderly people starting 2007. The summary of issues concerning home health nursing care under the long-term care insurance system include; 1) absence of comprehensive and systematic policy in home health care deliverly systems; 2) absence of community based home health agencies that are considered as the community residents in general. In order to overcome these problems and Issues, policy alternatives of home health care should 1) establish a comprehensive home health care policy for homebound persons; 2) establish the foundations for home health care nurses and community based home health care systems; 3) establish home health care facilities and infrastructure; and 4) promote research and development concerning home health care. Conclusively, a home health care system should be built on a comprehensive policy vision based on health policy, especially long-term care insurance system in the near future. Every homebound residents service has to be constructed systematically under suitable facilities considering the consumer characteristics and health conditions. By doing this, the consumer based comprehensive community home health care delivery system can be constructed in view of the long-term health care insurance system for elderly people.

Care of the Elderly with Dementia and the Need for Group Homes from Middle and Upper Class Families in Korea

  • Concerns about paid-facilities for the elderly with dementia from middle and upper class families have been discussed lately in Korea. Small-scale, home-like group homes for the elderly with dementia were developed in Sweden in the 1980, but they have not been effectively introduced in Korea where they remain a comparatively new concept. The group home, however, would provide a good alternative to Koreans who feel guilty when they have to leave their frail parents in large-scale facilities or hospitals instead of caring for them at home. The aim of this paper is, first, to define the care of the demented elderly who come from upper and middle class families in Korea and, secondly, to discuss the need for group homes to help care for them. A survey was done by questionnaire and was answered by 577 respondents who belonged to middle and upper class families living in Korea in December 2000. Methods of analysis were frequency, mean, and Chi-square. The results of this study were as follows: 1) the traditional notion that the eldest son should be burdened with the primary responsibility for a demented parent has weakened remarkably; 2) a small-scale long-term care facility was considered the most desirable living environment for the demented elderly; 3) the facilities families needed for professional hospitals for victims of dementia, senior citizen centers, large and small-scale long-term care facilities, day-care centers, and short-stay and service housing. The study also revealed that there was low awareness of the group home. The most preferred type of structure for group homes was a single-family detached house, and the most preferred management system was small-scale that could create home-like atmosphere. Additionally the respondents wanted group homes to consist of 6-8 residents with 2-4 persons per room. The results of this study strongly suggest that policy makers should encourage the development of smallscale group homes as an alternative form of housing for the elderly with dementia.

  • As same time of starting on the Korean Long-term Care Insurance System, the government developed a program to train new qualification of long-term care workers. The number of enrolled long-term care workers are 950,000 persons in 2010. Mostly they are working in home based care work places such as home visit care centers and home visit bathing centers covered insurance. The purpose of this study is to understand the difference of task performance requirement according to long-term care workers' responsibilities which divided into home visiting care and home visiting bathing. The comparison analysis was conducted to task performance requirement in basic management, safety management, administration management, practical services. Key result was found that task performance requirement of long-term care workers whose responsibilities are home visiting care were more higher than the home visiting bathing. Finally, To improve quality of home visiting care and home visiting bathing, it is necessary to provide the fields based continuing education and reflect new reimburse system.

노인장기요양보험제도에 의한 방문간호 표준개발 (Development of Home Visiting Nursing Standards Base on a Long-Term Care Insurance for the Elderly Program)

  • Purpose: The study was aimed at qualitatively enhancing and promoting a home visiting nursing program established in Korea on July 1, 2008, as part of the Long-Term Care Insurance for the Elderly program. Methods: Structural, procedural and consequential aspects of home visiting nursing care wereclassified on the horizontal axis by applying the standard notions for the evaluation of medical care (Donabedian, 1998). At the same time, the home visiting nursing care service support system and the service provision system weredivided on the vertical axis with reference to the accreditation standards for home visiting nursing care organizations suggested by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO, 2008). The data were collected from June 4, 2008 to October 27, 2008, and were analyzed using SPSS ver. 15.0. Results: Twenty-two (proposed) standards, centered on the standard elements under the conceptual framework of the study, were developed, and comprised structural aspects (n=10), procedural aspects (n=6) and consequential aspects (n=6). Those criteria and indicators underwent two content validity surveys among groups of home visiting nursing care research and training experts. The research produced 22 proposed standards, 50 proposed criteria and 166 proposed indicators. Conclusion: The home visiting nursing care standards developed pursuant to the Long-Term Care Insurance for the Elderly Act and the applicability of these standards need to be verified by home visiting nurses. These proposed standards should prove useful in developing an assessment tool to encourage the qualitative enhancement of visiting nursing care in Korea.
