A Case Report of Korean Medical Treatment on Parkinsonism Patient Complaining of Motor Disorder and Aphonia (한방치료로 운동 기능장애와 실성증이 호전된 파킨슨증후군 환자 치험 1례)

  • ■Objectives This case study is to report the effectiveness of Korean medicine in Parkinsonism patient's treatment. ■Methods We used the acupuncture, electro-acupuncture, moxibustion, cupping therapy, herbal medicine, especially Palmulgunja-tang to the Parkinsonism patient with motor disorder such as Postural Instability and Gait Difficulty(PIGD) and aphonia. Unified Parkinson's Disease Rating Scale(UPDRS), analysis of gait pattern, voice dB and self-evaluation of speed and volume were used to assess the change of symptoms. ■Results ‌After treatment, the UPDRS score decreased in overall category and the walking pattern has improved. In addition, the improvement was observed in voice volume and in self assessment of the patient. ■Conclusion This case suggests the effect of Korean medical treatment on motor disorder and aphonia in Parkinsonism.

Sodium Picosulphate with Magnesium Citrate versus Polyethylene Glycol for Bowel Preparation in Children: A Systematic Review

  • Purpose: To compare the effectiveness, tolerability, acceptability, and safety of sodium picosulphate with magnesium citrate (PS/Mg) and polyethylene glycol (PEG) in children (≤18 years) preparing for colonoscopy. Methods: Three electronic databases (MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials) were searched till July 2020. Only randomized controlled trials (RCTs) were included. At least two authors independently selected studies and performed risk of bias assessment and data extraction. Results: Four RCTs (n=390), with overall good quality were included. A meta-analysis of two trials (n=224) found no statistically significant difference between the groups with respect to the proportion of patients who had excellent and good scores (≥6 points) according to the Boston Bowel Preparation Scale (relative risk: 0.99; 95% confidence interval [CI]: 0.90 to 1.08). Excellent and good scores were observed in both groups in approximately 90% of children. A meta-analysis of two other trials (n=150) showed no significant difference between the groups with respect to the mean total score for the Ottawa Bowel Preparation Scale (mean difference: 0.20; 95% CI: -0.74 to 1.14). Both regimens provided a comparable safety profile; however, PS/Mg was significantly superior to high volume PEG in terms of tolerability (abdominal pain, nausea, vomiting, bloating/flatulence/fullness) and acceptability (ease of formulation consumption, taste acceptance, need for nasogastric tube, compliance with full dose). Conclusion: PS/Mg provides a quality and safety profile similar to PEG for bowel cleansing; however, it has better acceptance and tolerance in children preparing for colonoscopy.

Effects of Banhahubak-tang on Gastroesophageal Reflux Disease : A Systematic Review and Meta-Analysis (위식도역류질환에 대한 반하후박탕의 효과 : 체계적 문헌고찰과 메타분석)

  • Objectives : This study was designed to investigate the effect of Banhahubak-tang on gastroesophageal reflux disease(GERD) through a systematic review and meta-analysis of randomized controlled trials(RCTs). Methods : RCTs using Banhahubak-tang on GERD was searched in databases such as EMBASE, PubMed, MEDLINE, CENTRAL, CNKI, KISS, RISS, ScienceON, and OASIS. RCTs published up to October 8th, 2021 were included. Meta-analysis was performed by synthesizing outcome data, including Total Effectiveness Rate (TER), Reflux Symptom Index(RSI), Reflux Finding Score(RFS), and Incidence of Adverse Reactions. RevMan 5.4 software was used for data analysis. The Cochrane collaboration bias risk assessment scale was used to evaluate the methodological quality of the included studies. Results : Ten RCTs met the inclusion criteria. The total effective rate was the most commonly used outcome measure. The meta-analysis revealed that the TER in the experimental group was higher than that of the control group(N=2, RR:1.22, 95% CI:1.09 to 1.36, P=0.0004, I2=0%)(N=6, RR:1.22, 95% CI:1.14 to 1.32, P<0.00001, I2=0%)(N=8, RR:1.22, 95% CI: 1.14 to 1.30, P<0.00001, I2=0%). On the other hand, RSI(N=2, MD : -4.29, 95% CI: -4.71 to -3.86, I2=94%), RFS(N=2, MD : -3.28, 95% CI: -3.71 to -2.85, I2=96%), and Incidence of Adverse Reactions(N=5, RR: 0.32, 95% CI: 0.17 to 0.61, I2=0%) in the experimental group were lower than that of the control group. Conclusion : Treatment with Banhahubak-tang was found to be effective on GERD. However the results might be biased because of the poor quality and small sample size of the included RCTs.

Integrated Rehabilitation for Children with Cerebral palsy: A Prospective Observational Study

  • Objectives: This study aimed to analyse: 1) the clinical effectiveness and safety of traditional Korean Medicine (TKM) and frequency of TKM therapies used and their relationship with conventional treatments. Methods: This prospective observational study enrolled children with cerebral palsy (CP) aged 6-78 months (n=126). The children who used herbal medication for >30 days or acupuncture treatment >12 sessions within 6 months were defined as the integrated rehabilitation (IR) group; the remaining participants were included in the conventional rehabilitation (CR) group. Results: Changes in the Gross Motor Function Measure-66 (GMFM-66) were greater in the IR group (6.4±6.1) than in the CR group (4.6±5.8). The reduction in the number of other health problems was greater in the IR group than in the CR group. The GMFM-66 improvement was greatest in gross motor function classification system level 1. There was no between-group difference in the frequency of rehabilitation therapy (10.9±6.6 and 12.0±9.9 in the IR and CR groups, respectively). Conclusion: TKM may offer additional benefits in terms of the GMFM-66 score and other health problems. However, there is a need for further randomized controlled trials involving a restricted CP type and a controlled treatment type and intensity to confirm these findings.

Improving the effectiveness of document extraction summary based on the amount of sentence information (문장 정보량 기반 문서 추출 요약의 효과성 제고)

  • In the document extraction summary study, various methods for selecting important sentences based on the relationship between sentences were proposed. In the Korean document summary using the summation similarity of sentences, the summation similarity of the sentences was regarded as the amount of sentence information, and the summary sentences were extracted by selecting important sentences based on this. However, the problem is that it does not take into account the various importance that each sentence contributes to the entire document. Therefore, in this study, we propose a document extraction summary method that provides a summary by selecting important sentences based on the amount of quantitative and semantic information in the sentence. As a result, the extracted sentence agreement was 58.56% and the ROUGE-L score was 34, which was superior to the method using only the combined similarity. Compared to the deep learning-based method, the extraction method is lighter, but the performance is similar. Through this, it was confirmed that the method of compressing information based on semantic similarity between sentences is an important approach in document extraction summary. In addition, based on the quickly extracted summary, the document generation summary step can be effectively performed.

Safety and Effectiveness of Intra-articular Injection on the Ipsilateral Adhesive Capsulitis after Breast Cancer Surgery (유방암 수술 후 동일측 유착관절낭염에 대한 관절강내 주사 치료의 안전성 및 효과)

  • Objective: To confirm the safety of Intra-articular (IA) injection on the ipsilateral adhesive capsulitis (AC) after breast cancer surgery. Methods: Between January 2017 and May 2020, we retrospectively studied 29 patients after breast cancer surgery who underwent IA injection in the glenohumeral joint for AC in aseptic procedure. Results: There were no side effects or complications such as lymphedema or cellulitis in the patients. There was a significant improvement in pain score and range of motion (ROM) at the 1st, 3rd, and 6th months visits compared to the baseline (p<0.05). The presence or absence of axillary lymph node dissection and radiation therapy had no significant difference in improvement of ROM. But, in rotator cuff syndrome (RCS) group, there was a significant difference in improvement of shoulder IR in patients without RCS. Conclusion: IA Injection on the ipsilateral AC after breast cancer surgery was safe and even effective to improve pain and shoulder ROM. Ipsilateral IA injection can be a good treatment for breast cancer surgery patients suffering from AC.

Long-term condition monitoring of cables for in-service cable-stayed bridges using matched vehicle-induced cable tension ratios

  • This article develops a long-term condition assessment method for stay cables in cable stayed bridges using the monitored cable tension forces under operational condition. Based on the concept of influence surface, the matched cable tension ratio of two cables located at the same side (either in the upstream side or downstream side) is theoretically proven to be related to the condition of stay cables and independent of the positions of vehicles on the bridge. A sensor grouping scheme is designed to ensure that reliable damage detection result can be obtained even when sensor fault occurs in the neighbor of the damaged cable. Cable forces measured from an in-service cable-stayed bridge in China are used to demonstrate the accuracy and effectiveness of the proposed method. Damage detection results show that the proposed approach is sensitive to the rupture of wire damage in a specific cable and is robust to environmental effects, measurement noise, sensor fault and different traffic patterns. Using the damage sensitive feature in the proposed approach, the metrics such as accuracy, precision, recall and F1 score, which are used to evaluate the performance of damage detection, are 97.97%, 95.08%, 100% and 97.48%, respectively. These results indicate that the proposed approach can reliably detect the damage in stay cables. In addition, the proposed approach is efficient and promising with applications to the field monitoring of cables in cable-stayed bridges.

Quantitative Analysis of Construction Site Layout: A Usability Evaluation Study (건설현장배치 수준의 정량적 평가: 사용성평가 방법을 활용하여)

  • Construction site layout is attracting attention as efficient use of construction site space greatly affects the duration and cost of the overall construction. Therefore, there are many studies that automate and optimize construction site layout planning. However, the usability of construction site, which consists of goal variables of the studies, has still been unknown. Therefore, the authors present the evaluation criteria for usability of construction site layout and evaluate the usability of domestic construction sites through user survey. Furthermore, the difference in usability between construction site managers and construction site workers was confirmed. As a result of the survey, domestic construction site layout had a low effectiveness and had the lowest score in the environment category. In addition, construction site workers scored lower overall than construction site managers. Through such usability evaluation results, it contributed to the construction site layout theory by assessing current construction site layout practice and suggesting an improvement direction for automating site layout planning.

Electroacupuncture Treatment for Primary Dysmenorrhea: A Review of Randomized Controlled Trials

  • To evaluate the effectiveness of electroacupuncture treatment for primary dysmenorrhea (PD), a literature review of a randomized controlled trials (RCTs) was performed where electroacupuncture was used as a treatment intervention in patients diagnosed with PD. Relevant clinical studies (N = 226) were retrieved from multiple databases according to the study inclusion/exclusion criteria, and interventions and outcomes were analyzed. As a result of the review, there were 6 RCTs which met the criteria. In all 4 studies that measured pain indicators (visual analog scales), electroacupuncture showed significantly positive changes. In addition, positive changes were observed in most indicators (MMDQ, PGF2a, PGE2, 6-keto PGF1, TXB2, clinical efficacy score, uterine arterial blood flow parameters, and blood viscosity). 2 studies showed that electroacupuncture had a stronger therapeutic effect than NSAIDs. No major side effects were reported. Electroacupuncture may be an effective and safe treatment for PD however, further RCTs are required.

A Pilot Study on Effects of Critical Pathway Application for Hwa-Byung (화병 표준진료지침 적용효과에 대한 Pilot 분석)

  • Objectives: To investigate effectiveness and satisfaction of applying critical pathway (CP) to Hwa-byung outpatients. Methods: Subjects were 32 outpatients who were diagnosed with Hwa-byung between January 1 and 2021 to October 31, 2021. Among these patients, 18 patients were applied with CP and 14 patients received treatment without applying CP. Their medical records and administration records were retrospectively analyzed. Data were analyzed by mean, standard deviation, and t-test using SPSS 26.0 program. Results: Mean total treatment period significantly decreased in the CP group compared with the non-CP group. Medical expenses were classified by treatment period, per visit, and patient charges per type of visit. When analyzed specifically by detailed items, there was a decreasing tendency in total medical expenses, uncovered medical expenses, and patient charges but an increasing tendency in covered medical expenses, although some of these changes were not statistically significant. Satisfaction score increased in the CP group compared with the non-CP group in general, although not all increases were statistically significant. Conclusions: Applying CP may contribute to the reduction of medical expenses and improvement of medical service quality. Further research on the development of CP for various diseases and the application of CP under various circumstances is needed.
