연구목적 : 본 연구의 목적은 기공에 기초한 스트레스 관리 프로그램이 산모의 우울, 불안 등의 정서에 미치는 영향에 대하여 알아보고자 하였다. 방법: 임신 16주 이상의 산모들을 대상으로 하였다. 기공프로그램군은 8주동안, 일주일에 2번씩 기공프로그램에 참여한 52명으로 구성되었으며, 교육군은 8주동안 1주일에 1번씩 산모교육에 참여한 40명으로 구성되었다. 평가도구는 우울 수준을 측정하는 척도로서 해밀턴 우울 척도(Hamilton depression rating scale, HAMD), 벡 우울 척도(Beck depression inventory, BDI), 에딘버러 산후 우울증 척도(Edinburgh postnatal depression scale, EPDS)를 이용하였다. 불안 수준을 측정하는 척도로서 스필버거 특성 상태 불안척도-1 (Spielberger trait state anxiety inventory-1, STAI-1), 해밀턴 불안 척도(Hamilton anxiety scale, HAM-A)를 이용하였다. 기공과 교육 프로그램 시작전, 이후 2주, 4주, 8주에 걸쳐 산모의 우울, 불안 수준을 상기 척도로 측정하였다. 결과: 기공프로그램군은 프로그램 시작 전과 8주 후를 비교하였을 때 유의미하게 불안과 우울 수준이 모든 척도에서 호전되었다. 반면 교육군은 시작 전과 8주 후를 비교하였을 때 유의미한 호전은 없었다. 두 집단의 프로그램 전후 변화량을 비교한 결과 기공프로그램군이 교육군에 비하여 통계적으로 유의미한 변화를 보였다. 결론: 본 연구는 기공에 기초한 스트레스 관리 프로그램이 산모의 불안과 우울감을 조절하는데 효과적인 방법일 수 있음을 시사한다.
Purpose: This study was to investigate the degree of postpartum depression and its predictors at six months postpartum. Methods: The subjects were 161 women six months after delivery who were registered with the public health center. The instruments included a survey of various characteristics, the Edinburgh Postnatal Depression Scale(EPDS), husband support, maternal self-esteem, and marital adjustment scale. The data was analyzed using the $x^2$-test, t-test, the Pearson correlation coefficients, and the logistic regression. Results: The point prevalence of postpartum depression at six months postpartum was 14.3%, corresponding to a score of 12 or higher on the EPDS. Postpartum depression was significantly associated with husband support, maternal self-esteem, and marital adjustment. Predictors of postpartum depression identified by the logistic regression analysis include marital adjustment (OR .29 [95% CI .13-.61]) and the delivery method(OR 3.57 [95% CI 1.25-10.23]). Conclusion: Strategies for improving postpartum depression, considerations of husband support and maternal self-esteem are important in research and practice. In addition, interventions for reducing Cesarean delivery and improving marital adjustment are needed.
Purpose: This study investigated the physical and emotional health of South Korean mothers of preterm infants in the early postpartum period. Methods: In this descriptive correlational study, the participants included 91 mothers of preterm infants who were admitted to the neonatal intensive care unit of a tertiary hospital in South Korea. Physical health status was measured using a self-reported questionnaire, postpartum depression using the Edinburgh Postnatal Depression Scale, anxiety using the State-Trait Anxiety Inventory, and guilt using a 4-item scale. Results: Fatigue had the highest score among mothers' physical health problems, followed by shoulder pain, nipple pain, neck pain. The average postpartum depression score was 11.02 points, and 44% of women had postpartum depression with a score of 12 or above. Postpartum depression significantly was correlated with physical health (r=.35, p=.001), anxiety (r=.84, p<.001), and guilt (r=.75, p<.001) and was significantly higher for women with multiple births, and preterm infants who required ventilator and antibiotic treatment. Anxiety also showed a significant difference according to preterm infants' condition. Conclusion: The significant correlations between postpartum depression and physical health, anxiety, and guilt indicate a need for nursing interventions that provide integrated management of mothers' physical and emotional health.
Purpose: This study aimed to elucidate the prevalence of breast feeding and maternal psychosocial predictors affecting the breast feeding practice at six months postpartum. Method: One hundred and sixty one mothers at six months postpartum were conveniently recruited in two public health center in Jeju-city. Subjects completed a well-structured questionnaire regarding the feeding types of the child, maternal self-esteem, the Edinburgh Postnatal Depression Scale (EPDS), husband support, and marital adjustment scale. The data was analyzed using the $X^2-test$, t-test, Pearson correlation coefficients, and logistic regression. Result: The prevalence of breast feeding at six months postpartum was 29.9%. Predictors of breast feeding practice identified by the logistic regression analysis include first feeding type after delivery, marital adjustment, readiness for maternal role and work state. Conclusion: The prevalence of breast feeding practice was lower than WHO desired at six months postpartum. Therefore, the findings support the need for breast feeding education including interventions focused on improving readiness for maternal role and marital adjustment.
목적 본 연구는 산욕초기 미숙아 어머니의 산후우울과 예측요인을 파악하기 위한 서술적 조사연구이다. 방법 연구대상자는 일 지역 두 곳의 신생아중환자실에 미숙아가 입원 중인 미숙아 어머니로 분만 후 2-3주이며, 배우자가 있는 기혼여성 101명을 대상으로 하였다. 자료수집기간은 2010년 6월부터 2011년 1월까지였다. 연구도구는 산후우울(Edinburgh Postnatal Depression Scale), 산전 우울, 주관적 아기 건강상태, 의료인 지지, 배우자 지지 측정도구를 사용하였다. 자료는 SPSS win 18.0 program을 이용하여 기술통계, ttest, ${\chi}^2$-test, multiple logistic regression으로 분석하였다. 결과 산욕초기 미숙아 어머니의 산후우울 발생률은 81.6%였으며, 산후우울 예측요인은 분만방법(OR, 5.57; 95% CI, 1.25-24.77), 주관적 아기 건강상태(OR, 0.34; 95% CI, 0.16-0.70) 및 의료인 지지(OR, 0.52; 95% CI, 0.28-0.97) 등으로 나타났다. 결론 대부분의 미숙아 어머니들은 산욕초기 산후우울을 나타내었기 때문에 조기 중재를 목적으로 한 스크리닝이 필요하다. 또한 산후우울은 제왕절개로 분만하였으며, 주관적으로 아기 건강상태가 불건강하다고 인식할수록 그리고 의료인 지지가 낮을수록 발생비율이 높게 나타났다. 따라서 의료인들은 미숙아 어머니들이 제왕절개로 분만한 경우 더욱 관심을 기울어야 하며, 주관적으로 자신의 아기를 부정적으로 인식하지 않도록 도와야 한다.
Purpose: This study explored the prevalence of prenatal and postpartum depression in Korea and its influencing factors from 20 weeks of pregnancy to 12 weeks postpartum. Methods: Using a prospective cohort study design, data on women's depression and its influencing factors were collected at 20, 28, and 36 weeks of pregnancy and at 2, 6, and 12 weeks postpartum. The participants were 219 women and 181 spouses during pregnancy; and 183 mothers and 130 spouses after childbirth. Depressive symptoms were assessed by the Edinburgh Postnatal Depression Scale and influencing factors were measured by the Postpartum Depression Predictors Inventory-Revised, parity, and spousal depression. Results: The prevalence of maternal depression was 10.5% to 21.5% before birth, and it was 22.4% to 32.8% postpartum. The prevalence slightly decreased during the prenatal period but peaked at 2 weeks postpartum. Antenatal depression was influenced by low socioeconomic status, lower self-esteem, having experienced prenatal depression, having experienced prenatal anxiety, a previous history of depression, lower social support, lower marital satisfaction, and higher life stress. The factors influencing postpartum depression were lower self-esteem, having experienced prenatal depression, having experienced prenatal anxiety, lower social support, lower marital satisfaction, and higher life stress, as well as infant temperament and maternal blues. Parity and spousal depression had no impacts. Conclusion: The prevalence and influencing factors of maternal depression changed over time. Nurses need to screen women accordingly during the perinatal period and should provide education or counseling to prevent depression and promote adjustment to parenthood.
Purpose: The purposes of this study were in understanding maternal and neonatal risk factors for postpartum depression using Edinburgh Postnatal Depression Scale(EPDS). Methods: Among 788 women, who had delivery include cesarean section in the department of obstetrics and gynecology at OO medical center from May 28th 2008 to October 6st 2009, 72 women filled out EPDS questionnaire sheets. Additional aspects included for the analysis are maternal factors including age, number of children, parity, delivery method, and hemoglobin; and neonatal factors such as weight, sex, gestational age, apgar score, and neonatal intensive care unit admission. Comparison was performed between the women with EPDS score equal or less than 8 and the women with EPDS score equal to or higher than 9 using statistical methods of student t-test for linear variables and chi-square test for non-linear variables. SPSS version 13.0 for windows was used for analysis. Results: Thirty women(41.7%) were included in the postpartum depression risk group (EPDS score ${\geqq}9$). Statistically significant difference(P<0.05) was found in gestational ages of the risk group($36.57{\pm}29.6$ weeks) and the non-risk group ($38.10{\pm}1.97$ weeks). Identified statistically significant risk factors(P<0.05) include cesarean section (OR=3.304 [1.121-9.744]), low birth weight infant(OR =6.500 [1.606-26.314]), preterm delivery(OR=2.857[1.071-7.621]), low apgar score (1minute) after delivery (OR=14.909 [1.750-127.025]). There was no statistically significant difference in maternal age, number of children, parity, hemoglobin, neonatal sex, apgar score (5minutes), NICU admission. Conclusions: Through the results showed, gestational age, delivery method, neonatal weight, apgar score(1minute) were identified as risk factors for postpartum depression. To prevent or minimize postpartum depression, oriental medical intervention is recommended for pregnant women through early detection.
Purpose: This study aimed to investigate the prevalence of postpartum depression (PPD) and stress, as well as factors influencing PPD, among women in Saudi Arabia. Methods: This study employed a cross-sectional online survey and recruited participants during postpartum visits to the Clinic of Gynecology and Obstetrics in Al-Khobar, Saudi Arabia. Data collection was done using Arabic versions of the Edinburgh Postnatal Depression Scale, Perceived Stress Scale, and a sociodemographics and obstetric history questionnaire. Descriptive and inferential analyses were conducted, including multiple linear regression using a stepwise method. Results: Data from the 270 participants showed low levels of postpartum depressive symptoms with a mean score of 2.54±4.5 and low levels of perceived stress with a mean score of 2.49±6.2. While 94.4% of the participants reported low levels of stress and PPD, 5.6% reported elevated levels (≥10 for PPD, ≥14 for stress). The stepwise regression analysis showed significant results (p<.001), accounting for 34% of the variance in PPD. The factors significantly influencing PPD included the type of family, stress, number of abortions, disease during pregnancy, and family income. Importantly, perceived stress emerged as a factor influencing PPD. Conclusion: Although the majority of participants exhibited low levels of PPD, about 1 in 18 showed elevated levels. The identification of significant influencing factors highlights the need for targeted interventions to effectively address mental health concerns in postpartum women.
Purpose: This study was designed to determine the predictors of postpartum depression. Method: One hundred-sixty one women within one year after delivery from one public health center located in the northern area of Seoul were used in this study. The instruments were a survey of general characteristics, the Edinburgh Postnatal Depression Scale, recent life events index, perceived social support from family, Quality of marriage index, parenting stress index, and Rosenberg's self-esteem inventory. Data was analysed using descriptive statistics, Pearson correlation coefficients, and logistic regression. Result: The average item score of the EPDS was 6.67. 12.4% of respondents, who scored above a threshold 12, were likely to be suffering from a depression of varying severity. The fitness of the model for explaining postpartum depression from six variables, plan for pregnancy, family support, quality of marital relation, perceived social support, life events, childcare stress, and self-esteem, was statistically significant and the predictive power of these variables was 90.9%. The significant predictors of postpartum depression were family support and child care stress. Conclusion: Further research is needed to identify the prevalence rate of postpartum depression using more reliable sampling methods from a large general population. Nursing interventions need to be developed for promoting family support and reducing childcare stress.
Purpose: This study was conducted to determine the impacts of maternal depression on the temperament, health problems, and development of 4-month-old infants. Methods: For this longitudinal comparative study, data collected at one month postpartum and at the 4th month of follow-up were evaluated from sixty five pairs. The Edinburgh Postnatal Depression Scale (EPDS) was used to assess the symptoms of depression. Results: Eighteen point 5 percent and15.4% of mothers were diagnosed with postpartum depression at one month post-partum and at the 4th month of follow-up, respectively. The temperament of the infants between the two groups was not different. However, the infants of the depressed mothers had more health problems after 4 months. Depressed mothers had a lower score for the acceptance of Home Observation for Measurement of the Environment (HOME), and the development score of their infants was lower than that of the non-depressed mothers. Conclusion: Maternal depression affects the health and development of infants. Women should be screened for potential risk factors and for symptoms of depression during postpartum periods so that appropriate interventions including emotional support can be initiated in a timely manner.
