A Case Report of a Stage IV Non-small Cell Lung Cancer Patient Treated with Modified Yieum-jeon Showing Improvement in Malignant Pleural Effusion and other Respiratory Symptoms (비소세포성 폐암 4기 환자의 악성흉수로 인한 증상이 이음전가미방으로 호전된 1례)

  • Objectives: This is a report of a case of a stage IV non-small-cell lung cancer (NSCLC) patient whose malignant pleural effusion & respiratory symptoms have been relieved with Korean medicinal prescription, modified Yieum-jeon. Methods: A 56-year-old male diagnosed with stage IV NSCLC suffering malignant pleural effusion with complaints of cough, sputum, bilateral leg edema, dysphagia, post neck and left scapular pain was treated with modified Yieum-jeon and acupuncture during 16days. After treatment, blood lab and chest X-ray were used to follow the condition of malignant pleural effusion & pulmonary symptoms. Results: Malignant pleural effusion and pulmonary symptoms were improved with modified Yieum-jeon intake without any related adverse side effects. Conclusions: This study suggests Yieum-jeon may play a positive role in improving the malignant pleural effusion symptoms in advanced lung cancer cases.

Effect of Hand Acupuncture on Easing Dysmenorrhea (수지침요법이 월경전후기 불편감 완화에 미치는 효과)

  • The purpose of this study was to identify the effect of hand acupuncture for reduction of dysmenorrhea. 71 female middle school and high school students were selected as subjects from April to May 2001, among which 38 students consisted of the experimental group while 22 students were the control group by convenient assignment. The experimental group participated in hand acupuncture treatment. The experimental treatment was to puncture corresponding points(A4, A5, J1, J3, J23, H2, F7) on both the palm and back of a hand with disposable acupuncture needles and to remove needles after 20-30 minutes' recess during which the subjects reposed themselves on bed. The collected data were processed using the SPSS PC Program and analyzed using $X^2test$ and The results of this study were as follows 1. The symptoms of premenstrual syndrome have decreased, but no significant differences were statistically observed between hand-acupunctured group and non-hand acupunctured group. 2. Subjective pain degree has decreased after hand acupuncture. 3. As for objective pain degree, a significant reduction in voice change was observed between two groups. Facial expressional changes and perspiration degree reduced but showed no significant statistically changes between two groups. 4. Comfort has increased after hand acupuncture. These results suggest that the hand acupuncture therapy works effectively on easing menstrual pain and symptoms as well as partially on reducing discomfort. It is particularly efficacious for a reduction in subjective pain and facial expressional changes in positive respect. Therefore the hand acupuncture therapy can be considered as an independent nursing intervention for dysmenorrhea reduction.

A Study of Analgesic Effect of Twirling Acupuncture on Pain Model of the Formalin Test Using the Infrared Thermal Image Processing (적외선 영상 처리를 통한 Formalin Test 통증 모델에서의 염전 침자극 효과에 대한 연구)

  • Objective: As a manual accupucture method, the twirling-needle treatment has been known more effective in relieving pain than the conventional simple accupuncture treatment. Finding a proper treatment condition is difficult because of the lack of a quantative measurement of the alleviation of pain made by acupuncture. In this research, the authors propose the use of infrared thermal images in a formalin test to quantatively verify the effect of twirling. Methods: After injecting 10%~20% formalin into the tail of rats, the infrared thermal images(ITI) have been obtained to estimate the thermal distribution caused by inflammation. The authors propose a processing method to measure the thermal distribution from the thermal images obtained from the infrared camera as a pain model of the formalin test. Results: The pain model obtained from the infrared thermal image has two phases. The first phase, which is a transient period, is the initial 20 minutes when the pain is developed after the formalin injection. The second phase, which is a steady state, is where the development of pain lasts for 60 minutes or more after the first stage. This characteristic of the proposed model based on ITI is consistent with that of the pain model reported by other researchers whose works are based on the time-course of flinching and licking/biting, following a different concentration of formalin. It is noticed that the response of the thermal distribution obtained from ITI shows very high correlation to the behavioral response in the formalin test performed by Kazuhiro Okuda and four others5). In addition, the authors propose an ITI method to determine the pain-reducing effect of the acupuncture. The thermal distribution obtained from the experiment shows that there is significant pain reducing effect made by the twirling-needle method.

  • Although the physical therapy of both East and West has been based on an identical philosophy, they have had their own therapy with difference in its form and pattern. In general, cupping is used to diagnose and treat viscera by means of acupuncture point, and myofascial release is also used for both diagnosis and orthopedic treatment on the basis of trigger point and myofascial however, when they have a lot of identical facts such as using both mental and physical aspects of human beings for treatment, keeping nervous action balanced, and recovering depressed nervous functions and relieving the pain. In addition. their identical fact includes that they tend to treat patients by using symptoms and reaction shown in their skin, and that both East and West try to consider myofascia as an integrated totality and as a unified body of organic functions with correlations. Among the principles of myofascial release, recently, it has been very identical that stimulus given to the skin results in synapse to sympathetic nerve through dosal horn cell has an effect on viscera, and that cupping is sued for diagnosis and treatment of viscera. It is required, therefore, to continue to carry out studying on this field.

Effectiveness of herbal acupuncture of Carthami semen(Honghwa) vs its cotreatment with spiral taping for the treatment of acute low back pain : A randomized controlled trial (급성요통의 치료에 홍화약침과 홍화약침+첩대요법의 비교연구 : 무작위 대조군 임상실험)

  • Spiral taping shows great effectiveness in relieving musculoskeletal pains. The purpose of this research is to compare effectiveness of herbal acupuncture of Carthami semen(Honghwa) vs its cotreatment with spiral taping for the treatment of acute low back pain. The target patients were those who satisfied with all of the following conditions. 1. Those who suffered from acute low back pain but without past history of the pain in the last six weeks. 2. The pain wasn't radiated to the lower limbs and were without any neurotic symptoms. A computerized randomization was used to set the patients in two groups. The control group was consisted of 41 patients and treated with herbal acupuncture. The experimental group was consisted of 49 patients and treated with spiral taping as well as herbal acupuncture. The treatment was given two times a week and the nurses measured the ROM and VAS evaluation before every treatment. The patients wrote ODI by themselves before the first treatment and after the 3rd treatment. After three months, an interview of the patients' condition through a follow up survey was done. In terms of acute low back pain in both the control group and experimental group, significant treatment effects were found by assessing VAS, ROM and ODI. By comparing the control group and the experimental group, there was more significant improvement in the experimental group than the control group of VAS and ROM, but no significant effect was found between the groups of ODI. This study shows that the herbal acupuncture of Carthami Semen plays a significant role in relieving acute low back pain. It also shows that spiral taping of the experimental group was more highly effective. As a result, spiral taping can be used with acupunture of Carthami Semen for a highly effective treatment for acute low back pain.

Effects of Sleep Quality, Rehabilitation Motivation, Depression, and Anxiety on Quality of Life in Occupational Accident Patients (산업재해 환자의 수면의 질, 재활동기, 우울, 불안이 삶의 질에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effect of sleep quality, rehabilitation motivation, depression, and anxiety on quality of life in occupational accident patients. Methods: The participants were 138 patients who in and outpatient treatment at a hospital affiliated with the Korea Workers' Compensation and Welfare-service Hospital located in Daegu and Daejeon. Data were analyzed by independent t-test, ANOVA, Scheffé test, Pearson's correlation coefficients, and hierarchical multiple regression analysis using SPSS/WIN 25.0 program. Results: Quality of life had a significant negative correlation with sleep quality, depression, and anxiety, respectively, but had a significant positive correlation with rehabilitation motivation. Hierarchical multiple regression analysis showed that anxiety was a major factor affecting the quality of life of occupational accident patients. Anxiety showed 42.7% of explanatory power for the quality of life of occupational accident patients. Conclusion: Based on the results of this study, in order to improve the quality of life of patients with occupational accidents, psychological interventions for anxiety should be provided first in the early stage of treatment.

Comparison of Efficacy between Medial Branch Block and Collaborative Treatment with Acupuncture on Acute Low Back Pain (급성 요통의 내측지 차단술 단독 치료와 침을 병행한 협진 치료의 효과 비교)

  • Objectives : The purpose of this study was to compare the effects of acupuncture and medial branch block(collaborative treatment) with just medial branch block regarding acute low back pain. Methods : Forty inpatients who received treatment between January 2012 and February 2013 were divided into 2 groups. The East-West treatment group(EW group, n=20) received one treatment of medial branch block and then acupuncture afterwards. The Western treatment group(W group, n=20) received one treatment of medial branch block. Both groups continued to receive manual therapy 3 times a week. Evaluations were made before medial branch block, 7 days after, and 14 days after using the Numerical Rating Scale(NRS). Results : Compared to before treatment, the NRS score of both EW and W groups after 7 and 14 days of treatment significantly decreased(p=0.0001). But only the EW group showed additional improvement between days 7 and 14(p=0.005). Regarding group comparison, the NRS score of the EW group was significantly lower than the W group at 7 days(p=0.037), and even more at 14 days(p<0.0001). Conclusions : Although medial branch block alone significantly improved acute low back pain, collaborative treatment with acupuncture was even more effective, with increased efficacy as time passed. Further research is recommended regarding the effects of collaborative treatment with acupuncture on acute low back pain.

A Study on Cancer Patients' Quality of Life, Perceived Health Status and Susceptibility.Severity for Cancer Recurrence (암환자의 삶의 질, 지각된 건강상태 및 암재발 인지에 관한 연구)

  • Purpose: This study was designed to identify the relationship between quality of life, perceived health status, perceived susceptibility severity to recurrence of cancer and character of object in cancer patients to provide the basic data of effective nursing interventions. Method: The subject of this study was randomly chosen from the patients diagnosed of cancer and being hospitalised or receiving chemotherapy as outpatients, at a Division of Hemato-Oncology, Department of Internal Medicine of a university hospital located in Seoul. The tool used in this study was a measurement tool for quality of life (${\alpha}=.829$), perceived health status (${\alpha}=.903$), and perceived susceptibility and severity for cancer recurrence (${\alpha}=.860$). The collected data were analysed using SPSS PC 12.0 Programme for real number, percentage, average, standard deviation, Pearson's Correlation, t-test and ANOVA, according to the purpose of this study. Result: 1) The score of quality of life in cancer patients was 3.64 point, perceived health status was 2.62 and perceived susceptibility severity to recurrence of cancer was 2.41. 2) There was a significant difference in the quality of life according to general characteristics, such as level of education, occupation, level of activity, pain, medication period, diagnosis, disease stage, purpose of medication, and recurrence. There was a significant difference in perceived health status according to age, level of activity, pain, diagnosis, purpose of medication, and recurrence. There was a significant difference in perceived susceptibility severity to recurrence of cancer according to age, level of education, and pain. 3) The cancer patients' quality of life showed significant correlation with perceived health status, perceived susceptibility severity to recurrence of cancer, pain, job, treatment purpose, relapse. The perceived health status showed significant correlation with perceived susceptibility severity to recurrence of cancer, pain, treatment purpose, relapse. The perceived susceptibility severity to recurrence of cancer showed significant correlation with pain. The relapse showed significant correlation with treatment purpose. The cancer patients' quality of life, perceived health status, and perceived susceptibility and severity for cancer recurrence, as confirmed above, showed differences according to the related factors of each subject, and it was also confirmed that those factors were significantly related with general characteristics. Upon these results, I suggest further studies on the factors that affect the cancer patients' quality of life.

Effects of Moxibustion and Theraband Exercise on Physical and Psychological Variables of the Aged with Degenerative Osteoarthritis (뜸요법과 세라밴드운동이 퇴행성 골관절염 노인의 신체적, 심리적 변수에 미치는 효과)

  • Purpose: To examine the effects of a nursing intervention including moxibustion and theraband exercise on pain, fatigue, flexibility, and depression in aged patients with degenerative osteoarthritis. Methods: This study had a one group pretestposttest design. Thirty people over 60 yr-of-age with degenerative osteoarthritis were recruited from a community health center from November 1, 2005 to June 14, 2006. The 6 week treatment intervention included twice weekly 60 min sessions of of theraband exercise (30 min) and moxibution (30 min). Data was analyzed by SPSS Ver. 12.0. Results: Intervention was associated with significant decreases in pain (t=8.28, p<.001) and fatigue (t=5.19, p<.001), and significant increases in knee flexibility (t=-3.54, p=.001), dorsal flexibility (t=-9.05, p<.001), and plantar flexibility (t=-4.09, p<.001). Depression was significantly decreased after intervention (t=-4.09, p<.001). Conclusions: Moxibustion and theraband exercise should be utilized as an effective nursing intervention for the aged with degenerative oateoarthritis.

Nursing Students' Perception, Experience and Attitude on Complementary and Alternative Therapies (보완대체요법에 대한 간호대학생의 인식, 경험 및 태도 조사)

  • Purpose: This study aimed to provide a basic resource for developing nursing interventions and educational programs for complementary and alternative therapies (CAT) by examining nursing students' perception, experience, and attitudes toward CAT. Methods: Data were collected targeting 230 nursing students from October 20 to November 10, 2014, and analyzed by using descriptive statistics, t-test, ${\chi}^2$-test, and ANOVA with SPSS/WIN 18.0. Results: Results showed that CAT were performed by CAT specialists in combination with hospital treatments for individuals suffering from chronic pain and musculoskeletal diseases. Nursing students obtained information on and experience of CAT through the Internet. CAT were used for psychological and mental stability as a supplementary treatment. The majority of the participants used Yoga followed by massage as a sub-therapy of the program. They said that openness of medical staff and active communication are important for having a positive attitudes toward CAT. There are differences in attitudes toward CAT according to gender, student levels, and previous experience on CAT. Conclusion: It is crucial for nursing students to have right perception, experience, and attitudes toward CAT in order to apply CAT to the clinical field.
