Tuberculosis in the foot progresses gradually; thus, diagnosis is usually delayed, and early treatment is rarely provided. If osteomyelitis occurs due to delayed diagnosis and treatment, surgical treatment should be considered. We report the case of a 46-year-old man with osteomyelitis of the calcaneus who was diagnosed with multidrug-resistant pulmonary tuberculosis and he was treated with anti-tuberculosis drugs. Bilateral adrenal masses, abscess of both testes and a small wound in the left plantar heel were observed. Both adrenal masses and abscess were regarded as paradoxical reaction of anti-tuberculosis treatment. After 1 month, he developed a pain in the left plantar heel that was compatible with calcaneal osteomyelitis in radiological features. He underwent right orchiectomy for right scrotal abscess aggravation and surgical treatment for left calcaneal osteomyelitis. Mycobacterium tuberculosis was confirmed by polymerase chain reaction. The patient was immobilized by cast for 8 weeks and the heel pain gradually improved.

Bilateral femoral neck stress fractures have been rarely reported. When diagnosed, they are usually limited to athletes or military personnel. A 35-year-old man, previously healthy, visited the emergency department for right inguinal pain. On physical examination, no external wound, tenderness or limitation of motion were found in either lower extremity. Plain radiography showed normal findings but an magnetic resonance image showed a linear fracture in the femoral neck. In this case, bilateral femoral neck stress fractures were found in a young non-athlete adult with no prior medical history of related injuries. Surgery is generally not performed for stress fractures but considering the patient's early mobilization and the need for quick reintegration into society, surgery was done in this case.
Diabetic foot is one of the most devastating consequences of diabetes, resulting in amputation and possibly death. Therefore, early detection and vigorous treatment of infections in patients with diabetic foot are critical. This review seeks to provide guidelines for the therapy and rehabilitation of patients with moderate-to-severe diabetic foot. If a diabetic foot infection is suspected, bacterial cultures should be initially obtained. Numerous imaging studies can be used to identify diabetic foot, and recent research has shown that white blood cell single-photon emission computed tomography/computed tomography has comparable diagnostic specificity and sensitivity to magnetic resonance imaging. Surgery is performed when a diabetic foot ulcer is deep and is accompanied by bone and soft tissue infections. Patients should be taught preoperative rehabilitation before undergoing stressful surgery. During surgical procedures, it is critical to remove all necrotic tissue and drain the inflammatory area. It is critical to treat wounds with suitable dressings after surgery. Wet dressings promote the formation of granulation tissues and new blood vessels. Walking should begin as soon as the patient's general condition allows it, regardless of the wound status or prior walking capacity. Adequate treatment of comorbidities, including hypertension and dyslipidemia, and smoking cessation are necessary. Additionally, broad-spectrum antibiotics are required to treat diabetic foot infections.
Purpose: The aim of this study was to compare the activation of shoulder stabilizer muscles to variations of manual resistance during three-dimensional shoulder rehabilitation exercises. Methods: A total of 13 participants were included in this study. To normalize each muscle's activity, a maximal isometric voluntary contraction was performed by all participants. After receiving 30 minutes of training in three-dimensional shoulder rehabilitation exercises, participants randomly performed PNF arm and scapular patterns according to the intensities of manual resistance. The activities of the upper trapezius, lower trapezius, and serratus anterior were measured during these patterns. All exercises were performed for five seconds, and the average of three seconds, excluding the first and last seconds, was used for data analysis. Results: Lower trapezius activity was significant among manual resistance intensities. In both the PNF arm and scapular patterns, using 80% manual resistance of maximum resistance showed higher activity of the lower trapezius muscle compared to 20% of the maximum resistance. Conclusion: It is expected that PNF arm and scapular patterns, with varying intensities of manual resistance, can be used for early rehabilitation of patients with shoulder impingement syndrome.
Objective: Cortical Visual Impairment (CVI) is a leading cause of pediatric blindness and the most common form of pediatric visual disability, particularly prevalent among children with cerebral palsy (CP). This systematic review synthesizes the latest research on various interventions for managing CVI, focusing on studies published in the last decade. Design: A systematic review Methods: A comprehensive search was conducted in March 2024 across several databases including MEDLINE, CINAHL, Embase, and Web of Science. Studies were selected based on inclusion criteria set under the PICOSD framework and were limited to those involving human subjects, published in English, and conducted within the past ten years. The selected studies included randomized controlled trials, observational studies, and case reports focusing on rehabilitation, therapy, and surgical interventions for CVI. Results: Out of 221 studies screened, 5 met the inclusion criteria and were reviewed in detail. These studies covered a range of interventions including physiotherapy, sensory integration training, visual training programs, neuromotor rehabilitation, and surgical procedures aimed at improving visual function and overall quality of life for CVI patients. Conclusions: The studies demonstrate the potential benefits of structured, early intervention programs that incorporate family involvement and are tailored to the unique needs of children with CVI. However, there remains a significant need for further research to establish evidence-based practices in this field.

This study was an attempt to encourage the development of a rehabilitation delivery system and programs as a substitute service for hospitalization on the case of car accident patients, such as hospital based home health care nursing services. Various substitute services for hospitalization are required to curtail the length of stay for inpatients who were hospitalized with car accident compensation insurance. It focused on developing an estimation an early discharge day for car accident inpatients based on detailed statements of treatment for 111 inpatients who were hospitalized at the General Hospital in 1997. This study had four specific purposes as follows. First. to find out the utilization of medical services. Second, to estimate the time of early discharge and income increasing effect based on early discharge for those patients. Third, to identify the factors affecting total medical expenditure and the length of stay for those inpatients. Forth, to figure out the need of utilizing home health care nursing service for accident patients. In order to analyze the length of stay and medical expenditure for inpatients who were hospitalized due to car accidents, the authors conducted micro- and macro-analysis of medical and medical expenditure records. Micro-analysis was done by nominal group discussion of 4 expertise with the critical criteria, such as a decrease in the amount of treatment after surgery, treatments, tests, drugs and changes in the test consistency, drug methods, vital signs, start of ROM exercise, doctor's order, patient's outside visiting ability, and stable conditions. In addition to identifying variables affecting medical expenditure, and the length of stay and income effect due to early discharge day, the data was analyzed with a multiple regression analysis and linear regression analysis model by SPSS-PC for windows and Excell program. Results of this study were as follows. First. the mean length of stay was 50.3 days. whereas the mean length of stay due to early discharge was 34.3 days at the hospital. The estimation of time of early discharge depended on the length of stay. The longer the length of stay, the longer the length of time of early discharge : for instance a length of stay under 10 days was estimated as correlating to a mean length of stay of 6.6 days and early discharge of 6.5. The mean length of stay was 217.4 days and the time of early discharge was 110.1 respectively. The mean medical expenditure per day was found to be 169.085 Won and the mean medical expenditure per day showed negative linear trends according to the length of stay at the hospital. The estimation results of the income effect due to being discharged 16 days early was around 2,244,000 won per bed. However. this sum does not represent the real benefits resulting from early discharge, but rather the income increasing amount without considering medical prime cost in the general hospital. Therefore, further analysis is required on the cost containments and benefits as turn over rate per bed as the medical prime costs. The length of stay was most significant and was positive to the total medical expenditure, as expected. Surgery and patient's residential area was also an important variable in explaining medical expenditure. The level of complications was the most significant variable in explaining the length of stay. There was a high level for need a home health care nursing service which further supports early discharge for accident patients. In addition, when the patient was discharged. they needed follow up care for complications suffered during the car accident. $86.8\%$ of discharged patients responded that they needed home health services after early discharge. From these research findings, the following suggestions have been drawn. Strategies on a health care delivery system must be developed in order to focus on the consumer's needs and being planned for 21 century health policy in Korea. Community based intermediate facilities or home health care should be developed for rehabilitation services as a substitute for hospitalization in order to shorten the length of stay would be. A hospital based home health care nursing service. it would be available immediately to utilize by patients who want rehabilitation services as a substitute for hospitalization with the cooperation of car insurance companies.
Objective : Questionnaires completed by occupational therapists and patients in institutions conducting a "Pilot Project of Rehabilitation Medical Institutions" were used to determine whether occupational therapy was able to establish appropriate patient support systems for return to home and community. Methods : The questionnaire was completed by 293 occupational therapists from 13 institutions conducting pilot projects and 296 patients from 8 institutions who were able to respond. Results : Although the Pilot Project of Rehabilitation Medical Institutions is being implemented to enable early return of patients to activities of daily living, the most important therapeutic goal is not efficiently applied. In addition, since the health insurance systems utilized in medical institutions are applied as in the pilot project, there is no basis for evaluation and mediation of practical daily activities and vocational rehabilitation for actual return to the community. Conclusion : Through a detailed review of the pilot project, it will be necessary to improve the quality of life through the early return of patients to social activity, and to make practical improvements to reduce unnecessary societal expenditures.

We need substitute services for hospitalization to curtail the length of stay for inpatients due to car accidents. It focused on developing an estimation for early discharge based on a detailed statement of treatment from medical records of 109 inpatients who were hospitalized at General Hospital in 1997. This study has three specific purposes: First, to find the mean length of stay and mean medical expenditure. Second, to estimate the mean of early discharge from the mean length of stay. Third, to analyize the income effect per bed from early discharge. In order to analyze the length of stay and medical expenditure of inpatients the author conducted a micro and macro-analysis with medical expenditure records. To estimate the early discharge we examined with a group of 4 experts decreases in the amount of treatment after surgery, in treatments, in tests, in drug methods. We also looked their vital signs, the start of ROM exercise, the time removel, a patient's visitations, and possible stable conditions. In addition to identifing the income effect due to an early discharge, the data was analyzed by an SPSS-PC for windows and Excell program with a regression analysis model. The research findings are as follows: First, the mean length of stay was 47.56 days, but the mean length of stay due to early discharge was 32.26 days. The estimation of early discharge days was shown to depend on the length of stay. The longer the length of stay, the longer the length before discharge. For example, if the patient stayed under 14 days the mean length of stay was 7.09 while an early discharge was 6.39, whereas if the mean length of stay was 155.73, the early discharge time was 107.43. The mean medical expenditure per day of car accident patients was found to be 169,085 Won, whereas the mean medical expenditure per day was shown to be in a negative linear form according to the length of stay. That is the mean expenditure for under 14 days of stay was 303,015 Won and the period of the hospitalization of 15 days to 29 days was 170,338 Won and those of 30 days to 59 days was 113,333 Won. The estimation of the income effect due to being discharged 16 days was around 2,350,000 Won with a regression analysis model. However, this does not show the real benefits from an early discharge, but only the income increasing amount without considering prime medical cost at a general hospital. Therefore, we need further analysis on cost containments and benefits incending turn over rates and medical prime costs. From these research findings, the following suggestions have been drawn, we need to develop strategies on a rehabilitation delivery system focused on consumers for the 21st century. Varions intermediate facilities and home health care should be developed in the community as a substitute for shortening the length of stay in hospitals. In home health care cases, patients who want rehabilitation services as a substitute for hospitalization in cooperation with private health insurance companies might be available immediately.

In this paper, new method was proposed to classify lung tissues such as Broncho vascular, Emphysema, Ground Glass Reticular, Ground Glass, Honeycomb, Normal for early lung disease detection. 459 Statistical features was extraced from joint histogram matrix based on multi resolution analysis, volumetric LBP, and CT intensity, then dominant features was selected by using adaboost learning. Accuracy of proposed features and 3D AMFM was 90.1% and 85.3%, respectively. Proposed joint histogram based features shows better classification result than 3D AMFM in terms of accuracy, sensitivity, and specificity.
The purpode of this study was to determine the EMG characteristies of 7 subjects with hemiplegic gait receiving therapeautic exercise after stroke. The akin electrode and gait analysis system were used. The normal gait of 6 health volunteers was analysised. The results were following. 1. Gluteus maximus, the extensor of hip joint had high level of activity compared to normal, and had two peak in late stance phase and early Swing phase. 2. Medial hamstring, the flexor of knee joint had low level of activity compared to normal, and had continuous low amplitude pattern. 3. Vastus lateralis, the extensor of knee joint had high level of activity compared to normal, and had not continuous high amplitude. From early stance phase and mid stance phase, activity had high level but after swing phase. similar to normal. 4. Gastrocnemius, plantar flexor of ankle joint had low level of activity compared to normal and had continuous low amplitude. 5. Tibialis anterior, dorsiflexor of ankle joint had similar muscle activity to normal and had continuous low amplitude.
