Fertility preservation for patients with breast cancer: The Korean Society for Fertility Preservation clinical guidelines

  • With advances in the methods of cancer treatment used in modern medicine, the number of breast cancer survivors has been consistently rising. As the number of women who wish to become pregnant after being diagnosed with breast cancer increases, it is necessary to consider fertility preservation in these patients. However, medical doctors may be unaware of the importance of fertility preservation among cancer patients because most patients do not share their concerns about fertility with their doctors. Considering the time spent choosing and undergoing treatment, an early referral to a reproductive specialist is the best way to prevent a delay in cancer treatment. Since it is not easy to make decisions on matters related to cancer diagnosis and fertility, patients should be provided with enough time for decision-making, and to allow for this, an early referral will provide patients with sufficient time to choose an appropriate method of fertility preservation. The currently available options of fertility preservation for patients with breast cancer include cryopreservation of embryos, oocytes, and ovarian tissue and gonadotropin-releasing hormone agonist treatment before and during chemotherapy. An appropriate method of fertility preservation must be selected through consultations between individual patients and health professionals and analyses of the pros and cons of different options.

Neutrophil Functions and Cytokines Expression Profile in Buffaloes with Impending Postpartum Reproductive Disorders

  • The study was conducted to correlate the periparturient immune status in terms of neutrophil functions and cytokine expression in peripheral blood mononuclear cell culture with impending postpartum reproductive disorders in buffaloes. Forty pregnant buffaloes were observed for occurrence of postpartum reproductive disorders (PRD), i.e., metritis, endometritis and delayed uterine involution etc., during one week prepartum to four weeks postpartum period. A representative number (n = 6) of buffaloes that did not develop any PRD were included in group I (healthy, control), while the animals which experienced PRD were assigned into group II (PRD, n = 8). The blood samples were collected at weekly interval from one week prepartum to four weeks postpartum period considering the day of calving as 'd 0'. Differential leucocytes counts, superoxide and hydrogen peroxide production activity in isolated neutrophils and the mRNA expression profile of cytokines i.e., IL-2, IL-4 and IFN-${\gamma}$ in PBMC culture were studied in all the samples. A higher total leucocytes, neutrophil and band cells count along with impaired neutrophil functions i.e., lowered level of production of superoxide and hydrogen peroxide before parturition and during early postpartum period were observed in buffaloes developing PRD. Further, a lower expression of IL-2, IFN-${\gamma}$ and IL-4 mRNA in PBMC culture was observed at calving in buffaloes that subsequently developed PRD at later postpartum. Thus, suppression in neutrophil function and cytokine expression at prepartum to early postpartum period predisposes the buffaloes to develop postpartum reproductive disorders. Hence, monitoring of neutrophils function and cytokine expression profile would be effective to predict certain reproductive disorders at late pregnancy or immediately after parturition in buffaloes. In future, this may be a novel approach for determining suitable management and therapeutic decisions for prevention of commonly occurring reproductive disorders in farm animals.

Assessment of Cervical Cancer Risk in Women between 15 and 49 Years of Age: Case of Izmir

  • Background: The aim of the study was to determine risk factors for cervical cancer for women in Izmir. Methods: This cross-sectional, descriptive field covered a population of 4319 women of reproductive age (15-49) (household registration in the Mukhtar's office-2007). A total of 1,637 women were included in the sample given a four-part questionnaire through face-to-face interview by visiting the women in their homes in order to determine socio-demographic factors, obstetric history, genital hygiene and the use of family planning methods. In addition, during the data collection process, the women were given group training in order to raise awareness of cervical cancer. The number and percentage distributions of the data were calculated. Results: While the average age of the women was $31.9{\pm}9.77$ (Min: 15.00-Max: 49.00), education level of 43.4% of them was elementary school only. It was determined that 70.3% of the women experienced at least one pregnancy, 71.0% had vaginal delivery and 75.9% used a contraceptive method. In the study it was determined that among the cervical cancer related risks vaginal delivery, vaginal lavage and having three or more pregnancies had the highest rates, while having sexual intercourse before 16 years of age and having more than one sexual partner constituted lower rates. The rate of the women who stated not having a smear in the last three years was 82.4%. Conclusions: Considering the case in terms of having Pap smear test, women's awareness on the risk factors and early diagnosis of cervical cancer was found to be low. Due to this reason, awareness of women has to be raised through education.

Prenatal Diagnosis of Accompanying Alveolar Cleft and Cleft Palate in Fetuses with Cleft Lip Using Prenatal 3D Sonographic Identification and Antenatal Counseling (구순열 태아에서 3D 산전 초음파를 이용한 치조열 및 구개열의 동반 유무 진단 및 산전상담)

  • Purpose: Cleft lip and/or palate is the most common congenital facial anomaly whose incidence is about 1 in 500~1000 live births. As this anomaly may be associated with the serious chromosomal anomalies or the multiple organ abnormalities resulting in the fetal loss or perinatal maternal morbidity and mortality, careful prenatal counseling with early and accurate detection is important. Although conventional prenatal ultrasound(US) examination in midterm pregnancy has been applied for screening of cleft lip, there are definite limitations in the diagnosis of accompanying cleft palate or alveolar cleft. We applied high-resolution 3D US along the serial axial, coronal and sagittal plane so that we could diagnose the cleft palate and/or alveolar cleft in fetuses with cleft lip. Methods: From May 2005 to September 2005, 20 fetuses with cleft lip were examined with prenatal 3D US. Average maternal age was 28.8 years old(24-35 years old), and average gestational age was 24.8 weeks(17.6 to 34.2 weeks). Consecutive axial, coronal and sagittal multislice view were obtained via prenatal 3D US examination and diagnosis of cleft palate and/or alveolar cleft in cleft lip fetuses was followed. Results: With noninvasive and safe prenatal 3D US examination, 17 of 20 cleft lip fetuses were demonstrated to have cleft palate and/or alveolar cleft. Prenatal counseling according to the result was made. Conclusion: Existing prenatal US examination is suitable for screening the cleft lip fetuses but has limitation in identifying the related existence of cleft palate and/ or alveolar cleft. Authors verify the presence of cleft palate and/or alveolar cleft acquiring the successive multislice axial, coronal, and sagittal view with prenatal 3D US examination. Therefore, prenatal 3D US examination could be regarded as a noninvasive and secure screening modality in fetuses with cleft lip for confirming whether cleft palate and/or alveolar cleft is accompanied.

Effect of Artificial Menopause on Diagnosis of Common Cancers in Women: Focusing on Thyroid Cancer, Breast Cancer, and Cervical Cancer (인공폐경이 여성의 다빈도암 진단에 미치는 영향: 갑상선암, 유방암, 자궁경부암을 중심으로)

  • Objectives: The purpose of this study is to determine the impact of artificial menopause on the diagnosis of thyroid cancer, breast cancer, and cervical cancer, and to provide basic data for cancer prevention and early diagnosis in women. Methods: Analysis was conducted using raw data from the 2011-2020 National Health and Nutrition Examination Survey. Among the 79,262 people surveyed in the 2011-2020 National Health and Nutrition Examination Survey, 10,207 people were selected as the final research subjects, excluding men, those under 18 years old, those over 80 years old, those who did not participate in the health survey, those with missing data, and those who were not in menopause. Among them, 248 people were diagnosed with thyroid cancer (2.7%), 225 people were diagnosed with breast cancer (2.5%), and 143 people were diagnosed with cervical cancer (21.5%). Results: First, there appeared to be differences between the thyroid cancer diagnosed group and the non-diagnosed group depending on educational level, childbirth experience, and menopause type. Second, there appeared to be differences between the breast cancer diagnosis group and the non-diagnosis group depending on educational level, menopause age, pregnancy experience, childbirth experience, subjective health status, and menopause type. Third, there appeared to be differences between the cervical cancer diagnosis group and the non-diagnosis group depending on menopause age, subjective health status, and menopause type. Fourth, compared to natural menopause, in the case of artificial menopause, the diagnosis probability of women increased by 2.010 times for thyroid cancer, 3.872 times for breast cancer, and 14.902 times for cervical cancer. Conclusion: For thyroid cancer, breast cancer, and cervical cancer, the probability of cancer diagnosis increases in the case of artificial menopause compared to natural menopause, so it is considered important to avoid experiencing artificial menopause to prevent cancer.

Optimization of liquid phase enzyme immunoassay for determining of progesterone (Progesterone 측정을 위한 액상(液相) 효소면역측정법(酵素免疫測定法)의 최적조건에 관한 연구)

  • This study was carried out to develop an effective liquid-phase double antibody enzyme immunoassay for determining of progesterone. The optimum conditions of assay system, 1st and 2nd antibodies, enzyme conjugate, and time reaction were invested. The bovine plasma progesterone level in dairy cattle and korean native bulls were also analyzed. The results obtained were as follows; 1. The reproducibility of petroleum ether was superior to that of ethyl ether as extract solvent of progesterone in plasma. 2. The optimum dilution rate of 1st and 2nd antibody was 30,000 and 10 times, respectively. Affer the reaction of enzyme conjugate to progesterone 1st antibody, and then 2nd antibody competition reaction was enough for over 1hr. 3. Average plasma progesterone level in 4 pregnant and 9 nonpregnant Holstein was $2.5{\pm}0.5$ and $0.7{\pm}0.2ng/m{\ell}$, respectively. Average plasma progesterone level of 10 Korean native bulls was $0.1{\pm}0.001ng/m{\ell}$ From these results, by using liquid phase double antibody enzyme immunoassay for progesterone is applicable to detect of early pregnancy diagnosis, factorial analysis of reproductive disorder, and also reproductive physiological function such as monitoring of cyclicity during the post-partum period.

Congenital miliary tuberculosis in an 18-day-old boy

  • Congenital tuberculosis (TB) is a rare disease that is associated with high mortality. Mycobacterium tuberculosis, the causative agent, may be transmitted from the infected mother to the fetus by the transplacental route or by aspiration of infected amniotic fluid. Clinical symptoms and signs are not specific. Miliary patterns are the most common findings in the chest X-rays of many infants with congenital TB. In this case, an 18-day-old boy had jaundice on the fifth day of birth, and fever and respiratory distress appeared on the 18th day. Chest X-ray showed diffuse fine bilateral infiltration. Clinically, pneumonia or sepsis was suspected. Respiratory symptoms and chest X-ray findings worsened despite empirical antibiotic therapy. The lungs showed miliary infiltration suggestive of TB. Gastric aspirates were positive for M. tuberculosis. Respiratory distress and fever were gradually improved after anti-TB medication. Congenital TB is difficult to detect because of minimal or no symptoms during pregnancy and nonspecific symptoms in neonates. Hence, clinicians should suspect the possibility of TB infection even if neonates have non-specific symptoms. Early diagnosis and meticulous treatment are required for the survival of neonates with TB.

Establishment for Improving Productivity of Cattle by Fecal Steroid and Milk Urea Nitrogen Analysis - I. Development of Enzyme-linked Immunosorbent Assay for Progesterone and Milk Urea Nitrogen Analysis in Cattle

  • This study was carried out to determine the blood and milk progesterone by enzyme-linked immunosorbent assay (ELISA), and milk urea nitrogen (MUN) in cows. MUN and protein concentration were determined using automated infared procedures. The optimum conditions of ELISA system was investigated including the first and second antibody titres, bound percent, and enzyme conjugate and also the factors on MUN and protein concentration by sampling procedures and addition of preservatives. Progesterone antibodies did not react to pregnenlone, testosterone, estrone, estradiol-l7$\beta$, aldosterone, cortisol, corticosterone and 11$\alpha$-dehydroxycortisone (DOC), but reacted with only progesterone. The intra and inter-assay coefficient of variation 4.5%, 6.1~9.4% when used of bovine serum. The morning, MUN concentration (17.6$\pm$2.8 mg/100 ml) in the 13 herds was similar to that of evening MUN concentration of the lactating cows from the same herd. A significant relationship between morning and evening milk samples of upper parameters was found r=0.93. Difference in MUN concentration with sampling procedures and using of preservatives were investigated.

Enzyme-linked immunosorbent assay(ELISA) for determination of progesterone concentration in bovine serum (ELISA에 의한 소의 혈청 progesterone 농도 측정)

  • This study was carried out the determine the progesterone concentration for serum by enzyme-linked immunosorbent assay(ELISA) in bovine adult at estrous, pregnant, after patuation and male, female calves of 1 month old, respectively. The results obtained were summarized as follows ; 1. The assay has a sesitivity of $0.1ng/m{\ell}$. 2. Intra and inter-assay coefficient of variation were 4.5%, 6.1~9.4% when used for the determination of progesterone in samples of bovine serum. 3. The percentages of recovery for progesterone added were between 88.0 to 88.9%. 4. Progesterone concentration of adult bovine serum at estrus, pregnant and after 1 day of parturition were $0.37{\pm}0.16$, $7.1{\pm}1.0$, $0.13{\pm}0.02ng/m{\ell}$, respectively. 5. There was no differences in serum progesterone concentration of calves both male($0.16{\pm}0.03ng/m{\ell}$) and female($0.15{\pm}0.04ng/m{\ell}$) on 1 month old. From these results, progesterone determination by ELISA is very applicable to detect of early pregnancy diagnosis, factorial analysis of reproductive disorder, and also reproductive physiological functions such as veterinary therapeutic measures and monitoring of cyclicity.

A case report of embryo transfer with air-transported fresh bovine embryo produced by multiple ovulation in Hanwoo

  • Because multiple ovulation embryo transfer (MOET) in cattle includes several benefits such as wide spreading of genetically superior offspring for long distance, this biotechnological method has been widely applied to Hanwoo. When the recipients are not stayed close after embryo recovery from donor, the embryos are moved to other farms via several vehicles (car, train, and airplane). However, air travel induces lesser oxygen level, increased vibration, lower air pressure, higher noise, and increased exposure of cosmic radiation to living things than ground level. It was still unknown that fresh embryos obtained from multiple ovulation of Hanwoo could maintain their fertility after being transported via air plane, the present case report introduced a clinical case of MOET in Hanwoo after shipping fresh embryos via air transportation. The donor was multi-ovulated via follicle-stimulating hormone series of injection, which was followed by a gonadotrophin-releasing hormone injection and artificial insemination twice. The embryos were recovered by the uterine flushing, packed in ministraws, transported to recipients for 6 h including 1 h air flight, and then transferred to the synchronized recipients. During pregnancy diagnosis of early gestation period, 5 of 7 recipients (71.4%) presented no heat signs and showed fetal sacs with fluid under transrectal ultrasonography. After normal gestation period, all recipients naturally delivered healthy calves (male n = 2 and female n = 3) without abortion, stillbirth, and premature birth. The present case report indicated that transportation of fresh embryos for MOET via domestic flight in Korea did not affect to their fertility.
