  • KD는 아직 발병인자가 밝혀져 있지 않아 특별한 검사방법 없이 임상소견에 의존하여 진단하여야 하고 조기진단만이 CAD을 예방할 수 있는 중요한 후천성 심장질환이다. 그러나 발열과 5가지의 고전적인 진단기준에 부합되지 않고 발열과 4가지 미만의 진단기준 또는 전체 6가지 중 3-4가지 진단기준 만이 관찰되는 AKD는 조기진단이 어려워 전형적인 KD보다 CAL의 발생률이 높다. 특히 1세 미만의 영아에서는 모든 임상소견이 사라진 후 아급성기에 나타나는 손과 발의 낙설을 보고 후향적으로 진단을 하는 경우가 많다. 따라서 AKD에서 관상동맥합병증을 예방하려면 오랜 기간 동안 원인을 찾을 수 없고, 항생제에 반응하지 않는 발열이 있을 때에는 고전적인 진단기준에 얽매이지 말고 항상 high index of suspicion을 적용하여 필요할 때마다 CAL의 유무를 조사하고 가능한 빠른 시기에 치료하여야 할 것이다.

Metastasis Related Gene Exploration Using TwoStep Clustering for Medulloblastoma Microarray Data

  • Microarray gene expression technology has applications that could refine diagnosis and therapeutic monitoring as well as improve disease prevention through risk assessment and early detection. Especially, microarray expression data can provide important information regarding specific genes related with metastasis through an appropriate analysis. Various methods for clustering analysis microarray data have been introduced so far. We used twostep clustering fot ascertain metastasis related gene through t-test. Through t-test between two groups for two publicly available medulloblastoma microarray data sets, we intended to find significant gene for metastasis. The paper describes the process in detail showing how the process is applied to clustering analysis and t-test for microarray datasets and how the metastasis-associated genes are explorated.

  • Tomato yellow leaf curl virus (TYLCV), transmitted exclusively by the whitefly (Bemisia tabaci) in a circulative manner is one of the most important virus in tomato. Since the first report of TYLCV incidence in Korea in 2008, the virus has rapidly spread nationwide. TYLCV currently causes serious economic losses in tomato production in Korea. Early detection of TYLCV is one of the most important methods to allow rouging of infected tomato plants to minimize the spread of TYLCV disease. We have developed an ultra-rapid and sensitive real-time polymerase chain reaction (PCR) using a new designed real-time PCR system, GenSpectorTM TMC-1000 that is a small and portable real-time PCR machine requiring only a $5{\mu}l$ reaction volume on microchips. The new system provides ultra-high speed reaction (30 cycles in less than 15 minutes) and melting curve analysis for amplified TYLCV products. These results suggest that the short reaction time and ultra sensitivity of the GenSpector$^{TM}$-based real-time PCR technique is suitable for monitoring epidemics and pre-pandemic TYLCV disease. This is the first report for plant virus detection using an ultra-rapid real-time PCR system.

  • Because damages arising from the occurrence of foot-and-mouth disease (FMD) are very great, it is essential to make a preemptive diagnosis to cope with it in order to minimize those damages. The main symptoms of foot-and-mouth disease are body temperature increase, loss of appetite, formation of blisters in the mouth, on hooves and breasts, etc. in a cow or a bull, among which the body temperature check is the easiest and quickest way to detect the disease. In this paper, an algorithm to detect FMD from the hooves of cattle was developed and implemented for preemptive coping with foot-and-mouth disease, and a hoof check test is conducted after the installation of a high-resolution camera module, a thermo-graphic camera, and a temperature/humidity module in the cattle shed. Through the algorithm and system developed in this study, it is possible to cope with an early-stage situation in which cattle are suspected as suffering from foot-and-mouth disease, creating an optimized growth environment for cattle. In particular, in this study, the system to cope with FMD does not use a portable thermo-graphic camera, but a fixed camera attached to the cattle shed. It does not need additional personnel, has a function to measure the temperature of cattle hooves automatically through an image algorithm, and includes an automated alarm for a smart phone. This system enables the prediction of a possible occurrence of foot-and-mouth disease on a real-time basis, and also enables initial-stage disinfection to be performed to cope with the disease without needing extra personnel.

FDG-PET/CT as prognostic factor and surveillance tool for postoperative radiation recurrence in locally advanced head and neck cancer

  • Purpose: To evaluate the prognostic value of metabolic tumor volume (MTV) and maximum standardized uptake value (SUVmax) on initial positron emission tomography-computed tomography (PET-CT) and investigate the clinical value of SUVmax for early detection of locoregional recurrent disease after postoperative radiotherapy in patients with locally advanced head and neck squamous cell carcinoma (HNSCC). Materials and Methods: A total of 100 patients with locally advanced HNSCC received primary tumor excision and neck dissection followed by adjuvant radiotherapy with or without chemotherapy. The MTV and SUVmax were measured from primary sites and neck nodes. The prognostic value of MTV and SUVmax were assessed using initial staging PET/CT (study A). Follow-up PET/CT scan available after postoperative concurrent chemoradiotherapy or radiotherapy were evaluated for the SUVmax value and correlated with locoregional recurrence (study B). A receiver operating characteristic (ROC) curve analysis was used to define a threshold value of SUVmax with the highest accuracy for recurrent disease assessment. Results: High MTV (>41 mL) is negative prognostic factor for disease free survival (p = 0.041). Postradiation SUVmax was significantly correlated with locoregional recurrence (hazard ratio, 1.812; 95% confidence interval, 1.361 to 2.413; P < 0.001). A cutoff value of 5.38 from follow-up PET/CT was identified as having maximal accuracy for detecting locoregional recurrence by ROC analysis. Conclusion: MTV at staging work-up was significantly associated with disease free survival. The SUVmax value from follow-up PET/CT showed high diagnostic accuracy for the detection of locoregional recurrence in postoperatively irradiated HNSCC.

Characteristics of Breast Cancer Patients, their Understandings and Attitude towards the Disease (유암환자의 제 특징 및 유암에 관한 태도와 그 인식도)

  • Breast cancer is one of the most feared health problems in women ; Recent studies revealed that it had come up to be the second most in this country and high prevalent disease in the western countries among breast disease in women. However, early detection of the cancer mass is known to be easier than in many other malignancies. This study was performed to investigate the various characteristics of patients of breast cancer ; by the structural variables, menstrual, marital, and child bearing, and also their understandings and attitude towards the disease. A hundred any in- and out- patients of 51. Mary's Hospital and National Atomic Institute, Seoul were sampled. Data were gathered through direct interview by the researcher from February 1976 to January 1977 and the clinical records were used as references. Results are as follows ; 1. Breast cancer revealed to be most prevalent in the forties ; average age of 45.2 years. 2. The average age of menarche revealed to be 15.4 years ; the largest group were the 16-20 years (N=75, 68.2%). In 55 cases (50%) menstruation were normal, 38 (34.5%) postmenopausal and only in 17 (15.5%), menstruation revealed irregularity. 3. The average marital age revealed to be 22.3 years ; the largest group were 21-25 group (N=43, 39.1%). The average duration of marital life revealed to be 24.7 years ; 11-20 years group were the largest (M=34, 30.9%). 4. Most of the patients revealed to have pregnancy experiences(N=100, 90.9%) ; the average rate of experience were 5, 3 times the largest group were 3.4 times group(N= 32, 29.1%). 54 patients (49.1%) revealed to have had abortion experience ; the average were 3.4 times. 5. The largest group(N=77, 70%) had been breast feeding : followed by mixed feeding (N=12, 10.9%) and artificial feeding(N=10, 9.1%). 6. Personal health history revealed that in 20 patients (18.2%) revealed to have the past history of Purulent mastitis, 5 patients(4.5%) of breast cancer and 3 patients(2.7%) of uttering cancer family history. 7. In the one half (N=56, 50.9%, they had some information about breast cancer :27 (24.5%) by mass media, 12 (10.9%) through personal contacts and 17 (15.5%) were not able to classify the source of information. 8. In 55 cases (50%) the canoe, mass were discovered incidentally, in 39 cases (35.5%) by manual detection by self, in 10 cases (9%) by others and in 6 cases(5.5%) by observing subjective symptoms. 9. The average duration lapsed between the discovery of cancer mass and the visit to the hospital revealed to be 9.4 month. Chief reason for the delay revealed to be the non-chaplaincy due to the absence of pain(N=50, 45.5 %) followed by the administration of herb and commercial medication (N=19, 17.3%). 10. The left side breast was more affected than the right side breast, represent by 60 cases in the left and 39 cases in the right. The most frequent site of the breast cancer was the upper- outer quadrant in 53 cases (47.7%), and followed by the center in 20 cases(18 %), and the upper inner quadrant, in 19 cases (17.1%). There was / cases of bilateral carcinoma. The most prominent symptom was painless mass.

The Factors Causing Change of Lifestyle by the Outcome of Oriental Medical Examination (한방건강검진(韓方建康檢診) 결과(結果)에 따른 생활행동(生活行動) 변화(變化)에 영향(影響)을 미치는 요인(要因))

  • This study tried to know the chang of lifestyle and the factors causing of lifestyle change by the outcome of oriental medical examination for labors in one of the automobile factories in Jeon Buk area. The results will be helpful to the effective health care for the labors in factory. Oriental medical examination was done 22 times from May 20 to June 19, 2002. The numbers of labor who received oriental medical examination were 531, and 300 questionnaires among them were collected. The results were as follows: 1) General characteristics of examinee for oriental medical examination; total 300 labors, high percentage in age range $31{\sim}45$ years old, mostly married, high percentage of high school in completion of study. High percentages in drinking, smoking, and working hour were less than 1 time per week, non-smoking, and above 10 hours, respectively. High percentages in working year and salary were $11{\sim}15$ years. 2) The degree of lifestyle change by the oriental medical examination had the highest score with consult of oriental medicine doctor, and the lowest score was from moire typography result. 3) The degree of lifestyle change by medical examination was highly influenced by the subject characteristics that were less than 1 time per week for drinking, non-smoking, and less than 10 years of working year. 4) For the lifestyle change by the cognition of subject, the subjects who had high confidence for oriental medical examination, high recognition for oriental medical examination's requirement, high concern for health. effective cognition for early detection of disease, had high degree of lifestyle change. 5) The variables that cause lifestyle change in Sasang constitutional analysis result were early detection of disease, type of smoking, working year, moire typography result, interview for health. The variables that cause lifestyle change in moire typography result were type of drinking, ages, working year, consult for health, moire typography result. The variables that cause lifestyle change in interview with oriental medicine doctor were constitutional analysis and moire typography result.

Early Detection of hyperemia with Magnetic Resonance Fluid Attenuation Inversion Recovery Imaging after Superficial Temporal Artery to Middle Cerebral Artery Anastomosis

  • Objective : Cerebral hyperperfusion syndrome (CHS) manifests as a collection of symptoms brought on by heightened focal cerebral blood flow (CBF), afflicting nearly 30% of patients who have undergone superficial temporal artery (STA)-middle cerebral artery (MCA) anastomosis. The aim of this study was to investigate whether the amalgamation of magnetic resonance imaging (MRI) fluid-attenuated inversion recovery (FLAIR) and apparent diffusion coefficient (ADC) imaging via MRI can discern cerebral hyperemia after STA-MCA anastomosis surgery. Methods : A retrospective study was performed of patients who underwent STA-MCA anastomosis due to Moyamoya disease or atherosclerotic steno-occlusive disease. A protocol aimed at preventing CHS was instituted, leveraging the use of MRI FLAIR. Patients underwent MRI diffusion with FLAIR imaging 24 hours after STA-MCA anastomosis. A high signal on FLAIR images signified the presence of hyperemia at the bypass site, triggering a protocol of hyperemia care. All patients underwent hemodynamic evaluations, including perfusion MRI, single-photon emission computed tomography (SPECT), and digital subtraction angiography, both before and after the surgery. If a high signal intensity is observed on MRI FLAIR within 24 hours of the surgery, a repeat MRI is performed to confirm the presence of hyperemia. Patients with confirmed hyperemia are managed according to a protocol aimed at preventing further progression. Results : Out of a total of 162 patients, 24 individuals (comprising 16 women and 8 men) exhibited hyperemia on their MRI FLAIR scans following the procedure. SPECT was conducted on 23 patients, and 11 of them yielded positive results. All 24 patients underwent perfusion MRI, but nine of them showed no significant findings. Among the patients, 10 displayed elevations in both CBF and cerebral blood volume (CBV), three only showed elevation in CBF, and two only showed elevation in CBV. Follow-up MRI FLAIR scans conducted 6 months later on these patients revealed complete normalization of the previously observed high signal intensity, with no evidence of ischemic injury. Conclusion : The study determined that the use of MRI FLAIR and ADC mapping is a competent means of early detection of hyperemia after STA-MCA anastomosis surgery. The protocol established can be adopted by other neurosurgical institutions to enhance patient outcomes and mitigate the hazard of permanent cerebral injury caused by cerebral hyperemia.

Public Awareness and Knowledge of Oral Cancer in Yemen

  • Background: Oral cancer is in increasing in incidence in Yemen and indeed worldwide. Knowledge regarding risk factors and early signs in the general population can help in prevention and early detection of the disease. Purpose: The aim of this study was to assess the level of awareness and knowledge of oral cancer in the general population in Yemen. Materials and Methods: A cross-sectional survey using a self-administered questionnaire was conducted on Yemeni adults aged ${\geq}15$ years old. A total of 543 persons participated, the collected data being analyzed using SPSS software. The significance level was set at p<0.05. Results: Two thirds (71.5%) of the participants had heard about oral cancer. Smoking and smokeless tobacco usage were identified as the major risk factors by 71.5% and 73.7% of the participants, respectively. Only 24.1% and 21.4%, respectively, were able to correctly identify red and white lesions as early signs of oral cancer. Knowledge of oral cancer was significantly associated with age (p<0.01), gender (p<0.05) and education level (p<0.001). Conclusions: The findings suggest that the knowledge regarding oral cancer in this population is low. Therefore, educational programs are highly needed to improve such knowledge.

In vivo Optical Coherence Tomography Imaging of the Mesothelium Using Developed Window Models

  • The mesothelium is an essential lining for maintaining the normal homeostasis of the closed body cavity and a central component of pathophysiologic processes. The mesothelium has been known as the end target for asbestos which induces asbestos-related lung diseases. Malignant mesothelioma (MM) is a rare and fatal neoplasm predominantly due to asbestos exposure. Adaptation of an advanced and reliable technology is necessary for early detection of MM because it is difficult to diagnose this disease in its early stages. Optical coherence tomography (OCT) provides cross-sectional images of micro-tissue structures with a resolution of $2-10{\mu}m$ that can image the mesothelium with a thickness of ${\sim}100{\mu}m$ and, therefore, enable investigation of early development of MM. The mesothelium is typically located at the pleura and tunica vaginalis of the scrotum. In this study, we developed animal window models in the above two anatomical sites to visualize mesothelial layers within the mesothelium. OCT images at the two locations were also acquired.
