구조화된 환자교육이 뇌졸중 환자의 조기재활에 관한 지식과 활동수행에 미치는 영향 (The Effect of the Structured Education on the Early Rehabilitation Knowledge and Activity Performance of the C.V.A. Patients)

  • This study has been attempted to set up the strategies of the nursing which can promote the activity performance for early rehabilitation for the patients by examining the effect of the structured patient education on the early rehabilitation knowledge and activity performance of the C.V.A patients. The study method has been done by investigating the experiment group and control group in advance through the question papers and interview and observation on 65 patients who had been hospitalized at oriental medicine hospital of K Medical Center from July 1st 1995 to the end of Sep, 1995. The analysis of the collected material had been done for the homogeneity test in which general characters of experiment group and control group had been tested by X²and the homogeneity test of ADL by t-test. To test the hypothesis the t-test had been given for the difference of the early rehabilitation knowledge and activity performance between the two groups and the correlation between early rehabilitation knowledge and activity performance had been tested by Pearson's Correlation Coefficient. The result of the test of the hypothesis is as the below. 1 The 1st hypothesis “The experiment group which had received the structured education should be higher in the early rehabilitation knowledge than the control group” was supported(t=4.45. p=.000). 2. The 2nd hypothesis “The experiment group which received the structured education should be higher in the early rehabilitation activity performance than the control group”was supported(t=2.11, p=.036). 3. The 3rd hypothesis “The higher the early rehabilitation knowledge of the patient the higher the activity performance degree” was rejected (r=.1546, p=.219). In conclusion, the patients who received the structured education showed the increase in the degree of early rehabilitation knowledge and activity performance, so it has been judged that education has been prerequisite in increasing the knowledge and activity performance of early rehabilitation.

청력손실아동의 재활에 관한 고찰 (A Study on Rehabilitation in Hearing Impaired Children)

  • The human baby appears to be born with preexistent knowledge of language. This specialized neural structure in the brain awaits auditory experience with language to trigger it into functioning. The auditory-linked acquisition of language is a time-locked function related to early maturational periods in the infant's life. The longer auditory language stimulation is delayed, the less efficient the language facility will be. It is for these reasons that it is urgent to fight the hearing problems of children with all the skill, knowledge and insights of which we are capable, the so called 'rehabilitative process'. An understanding of the timetable and the origin of prenatal to early life development of auditory mechanism will help in planning the aural rehabilitation. Further interests and studies are needed to establish the systematic structure of rehabilitative audiology.

Unilateral hypoglossal nerve palsy after mild COVID-19: a case report

  • Post-acute coronavirus disease (COVID-19) syndrome is defined as persistent symptoms or delayed complications after COVID-19. Several cases of cranial nerve invasion related to COVID-19 have been reported. However, to our knowledge, no cases of solitary unilateral hypoglossal nerve paralysis after mild COVID-19 without intubation have been reported to date. Herein, we report the case of a 64-year-old man with unilateral hypoglossal nerve palsy as a complication of COVID-19. He complained of dysarthria and tongue discomfort 2 weeks after COVID-19 onset. Brain and neck computed tomography, magnetic resonance imaging, ultrasonography, and blood tests ruled out other possible causes. The patient's nerve palsy was rapidly diagnosed and improved with early rehabilitation. Understanding of the pathology of COVID-19 is still limited. Physicians should focus on patients' symptoms and their relationship to COVID-19, and investigate complications immediately. This case highlights the importance of early detection and rehabilitation of post-acute COVID-19 syndrome.

Early Rupture after Coiling of Unruptured Aneurysm

  • We describe a case of an unruptured basilar top aneurysm that was associated with early rupture after incomplete coiling. A 62-year-old woman with a history of several small infarctions has undergone coiling of unruptured basilar top aneurysm. Two weeks after initial coiling the patient presented with Hunt and Hess grade IV subarachnoid hemorrhage consistent with a ruptured basilar top aneurysm. Repeat angiography revealed a rupture of recanalized basilar top aneurysm. Second embolization with additional coils resulted in complete occlusion. However, her neurological status was not improved afterward and she was transferred to department of rehabilitation one month after hemorrhage with comatous state. To our knowledge, this is the first case of fatal early rupture after coiling of unruptured aneurysm. It has been speculated that coiling could cause injury to aneurysmal wall and facilitate rupture.

  • Early detection and intervention is critical in improving prognosis of developmental disorders. Developmental delay can have many different causes and the clinical features of developmental delay are diverse according to its etiologic causes and severity. The vast and rapid growth of the child's neurobehavioral repertoire from birth through adolescence requires the physician's abundant experience, knowledge, and understanding of development. Here, we summarize instruments for use in neurodevelopmental assessment of infants and toddlers, focusing on motor development, which is the most frequent complaint listed in the developmental clinic.

Breast Cancer Knowledge and Screening Behaviour among Women with a Positive Family History: A Cross Sectional Study

  • Background: Breast cancer is the commonest type of cancer among women, and in Malaysia 50-60% of the new cases are being detected at late stages. Do age, education level, income, ethnicity, relationship with breast cancer patients and knowledge of breast cancer risk factors influence breast screening practices? This study revealed interesting but significant differences. Objectives: To assess the knowledge of breast cancer risk factors and early detection measures among women in a high risk group. Materials and Methods: A cross sectional survey of one hundred and thirty one women relatives of breast cancer patients was carried out. Participants were selected through purposive sampling, during hospital visits. A self-administered questionnaire was used for data collection. Results: The majority of the respondents (71%) had poor knowledge of the risk factors for breast cancer. Income, relationship with a patient and practise of breast cancer screening predicted performance of mammography, $R^2$=0.467, F=12.568, p<0.0001. Conclusions: The finding shows inadequate knowledge of breast cancer risk factors and poor cancer screening practise among women with family history of breast cancer. Poor knowledge and practise of breast screening are likely to lead to late stage presentation of breast cancer disease. Some important predictors of breast cancer screening behaviour among women with positive family history of breast cancer were identified. An understanding of the strengths and significance of the association between these factors and breast screening behaviour is vital for developing more targeted breast health promotion.

주관적 기억장애 및 경도인지장애 노인의 의미연상과제 수행 특성 (The Characteristics of semantic association task performance in elderly with subjective memory impairment and mild cognitive impairment)

  • 초기의 인지 감퇴를 판단하는 요소로 의미지식 및 의미 연관관계의 범주별 손상 유무가 주목을 받고 있다. 본 연구에서는 정상 및 주관적 기억장애, 경도인지장애 노인을 대상으로 의미연상과제를 사용하여 인지 감퇴의 정도에 따라 관찰되는 의미 하위범주별 수행과 오류유형의 차이를 살펴보고자 하였다. 연구 결과, 의미연상과제 범주별 정반응 점수와 반응시간에서 세 군 간 유의한 차이를 보였으며, 하위범주 중, '기능'에서 가장 높은 수행력을, '상위'와 '부분/전체'에서 가장 낮은 수행력을 보였다. 또한, 오류유형별 산출 횟수는 정상에서 경도인지장애 노인으로 갈수록 유의하게 높아졌으며, 무반응은 주관적 기억장애 노인부터 유의하게 증가하였다. 결론적으로, 인지 감퇴가 진행될수록 의미연결망에서 범주별 손상의 정도와 과정이 다르므로, 기억장애 노인의 인지적 감퇴를 확인하고 추적 관찰하기 위한 지표로 의미연상과제 수행력을 활용할 수 있을 것이다.

Effects of early clinical and basic laboratory exposure program on premedical students: a questionnaire survey

  • Background: Because premed students do not take courses related to medicine during their first 2 years, they cannot establish their identity as students at medical schools, making it difficult for them to set goals as future doctors. We conducted an early clinical and basic laboratory exposure program for premed students and studied the effects of the program and student satisfaction levels. Methods: We performed an early clinical and basic laboratory exposure program for premed students for 2 days and evaluated the effects of the program and student satisfaction with it. The program consisted of two types: type 1, where two to four students formed a group, which was assigned to a particular department to participate and make observations during ward rounds, outpatient clinics, examinations, procedures, and surgeries (in the case of basic laboratory work, the students partook in experimental observations); and type 2, where one student followed a medical school professor to observe the professor's day. After the program ended, an online survey was conducted to investigate the effects on students, their thoughts, and satisfaction levels. Results: In total, 114 students (91.2%) responded to the survey. Approximately 94% of them were satisfied with the program. They found that the program would be useful for deciding on future career paths, gaining knowledge about a department of interest, studying for a medical program after premedical studies, and befriending residents and professors in certain departments. Conclusion: Early clinical and basic laboratory exposure programs are recommended for premedical students.

Neuralgic Amyotrophy Manifesting as Mimicking Posterior Interosseous Nerve Palsy

  • The upper trunk of the brachial plexus is the most common area affected by neuralgic amyotrophy (NA), and paresis of the shoulder girdle muscle is the most prevalent manifestation. Posterior interosseous nerve palsy is a rare presentation in patients with NA. It results in dropped finger on the affected side and may be misdiagnosed as entrapment syndrome or compressive neuropathy. We report an unusual case of NA manifested as PIN palsy and suggest that knowledge of clinical NA phenotypes is crucial for early diagnosis of peripheral nerve palsies.

전방십자인대 재건술의 합병증 (Complications of Anterior Cruciate Ligament Reconstruction)

  • Although the number of anterior cruciate ligament reconstruction is increasing, complications after primary ACL reconstruction are more difficult to determine. Intraoperative and postoperative complications can lead to ultimate failure of a primary reconstructive procedure. Therefore, surgical success in ACL reconstruction requires detailed knowledge and technical advancements about ACL reconstruction. Preoperatively surgeon must pay attention to selection of grafts and methods of fixation, and intraoperatively, attention to the harvest of graft, passage of graft, intraarticuar placement of the graft, notchplasty, proper tensioning of the graft, and others. Postoperative complications must be detected early, including infection, abnormal healing responses, arthrofibrosis, graft rejection, and reflex sympathetic dystrophy. Careful patient selection, appropriate surgical timing, careful surgical technique, and supervised preoperative and postoperative rehabilitation can minimize postoperative complications.
