Development and Evaluation of a Stage Matched Exercise Intervention Program for Elders - Application of the Tran Theoretical Model - (노인 운동행위 변화단계별 중재프로그램의 개발 및 평가 - 범이론적 모형의 적용 -)

  • Objectives: This study was designed to develop and evaluate a stage matched exercise intervention program to effectively increase exercise behaviors in urban elders. Methods: The study included three phases: preliminary descriptive data collection, program development, and program evaluation. The data for the preliminary descriptive phase were collected between May and June 2001. The study participants were 89 urban elders who responded a questionnaire that included general characteristics, exercise related experiences, stage, and process of change in exercise behaviors. Data were analyzed using descriptive statistics, $x^2$-test, and content analysis. Development of the program was based on the preliminary data. and a literature review, and was guided by the tran theoretical model. It consisted of strategies to facilitate the process of changes used in each stage. Evaluation of the program was achieved from October to December 2001, using a case study method, in which eight urban female elders participated. Interviews were conducted on a weekly basis in the form of either an individual interview, or group discussion. Each elder subject received education in accordance with the program strategies and education materials. In the case that a subject's stage of change moved into another one, the scores for the process of change were re-measured. The data were analyzed using the content analysis technique. Results: The results were as follows: 1. Elders who participated in the preliminary data collection phase were over 75 years of age, and the majority of them were women. They had a higher educational level, and fewer number of illnesses than the subjects in other studies. Their stage of change was divided into pre-contemplation and maintenance. The social liberation scores were the highest across all stages of change. There was no difference between men and women on scores for processes of change in each stage. 2. The stage matched exercise intervention program that was developed in this study consisted of one counseling type program and three distinguished educational booklet materials. 3. The results of the case studies are as follows: 1) The study participants were 8 women between 75 and 87 years of age. At the first interviews, all of them were in the pre-contemplation stage. All of them reached the action stage before the 7th week. The scores for processes of change that were the focus in each stage increased more than the scores for other processes of change. During the early stages of change, experimental processes increased more than behavioral processes. However. this pattern was reversed during later stages of change. 2) Characteristics of the subjects in each stage were identical as presented at the tran theoretical model. The intervention strategies were effective in the transition occurred in any stage. 3) Barriers for exercise included unwillingness to exercise, fatigue, shortness of breath, and pain. Ways to overcome these barriers were 'learning an alternative exercise method that can be done at home', 'self-promising/ exercise-promising', and 'use of cues to exercise'. 4) The factors that affected the application of the program were consideration of age and personal preference in selecting an exercise pattern, individualized intervention, and use of education materials appropriate to elders. Women over 80 years of age preferred muscle strengthening and stretching exercise, because they can be easily done at home. They also preferred individualized interventions, materials that were easy to read, and education contents appropriate for elders. Conclusion: In conclusion, the stage matched exercise intervention program that considered the characteristics of the elders was effective to facilitate exercise behaviors of the elders.

Effect of Exercise on Blood Concentrations of Ethanol, Lactate and Glucose in Men Showing Facial Flush after Ethanol Ingestion (음주후(飮酒後) 얼굴 붉어지는 사람에 있어서 운동(運動)이 혈중(血中) 에타놀, 유산(乳酸) 및 포도당(葡萄糖) 농도(濃度)에 미치는 효과(效果))

  • To elucidate the effect of exercise on blood concentrations of ethanol, lactate and glucose in men who show facial flush after ethanol ingestion, 59 healthy male college students were studied. After 6 or more hours of fasting, the subjects were administered 3 ml of 25% ethanol solution(Soju) per liter of total body water. For control experiment Soju was replaced with the same dose of water. Exercise performed was vertical jumping on a rebounder for 3 min immediately after drinking. The subjects were classified into 6 groups: water ingestion(W), flushed (F) and non-flushed (N) groups after ethanol ingestion, water ingestion and exercise(WE), flushed(FE) and non-flushed (NE) groups after ethanol ingestion and exercise. Blood ethanol concentration in the exercise groups(NE, FE) was lower until 60 min after drinking than that in the non-exercise groups(N,F). Factor k representing the rate of ethanol absorption was markedly lower in the exercise groups than in the non-exercise groups. The flushed groups(F,FE) showed higher blood ethanol level than the non-flushed groups (N,NE) from 30 to 120 min after drinking. Blood lactate concentration in WE group was elevated immediately after exercise and returned to the resting level at 60 min after exercise. Ethanol increased blood lactate level from 30 to 120 min after ethanol drinking, Exercise after ethanol ingestion produced a sharp increase and then drop in blood lactate level which was stilled significantly higher than the resting level all the way through 120 min. Blood glucose concentration was decreased at 15 min after exercise. Ethanol-administered groups except F group showed a steady decrease in blood glucose level from 30 through 120 min. Heart rate was elevated by ethanol only in the flushed groups. Heart rate in F group was significantly increased at 4 min after ethanol and was maintained at high level until 120 min. In WE and NE groups, heart rate was significantly increased immediately after exercise and returned to the resting level at 60 min. The FE group, however, showed a consistently elevated heart rate throughout the 120-min experimental period. Taken together, the exercise alone produced a delayed ethanol absorption, a prompt increase in heart rate and blood lactate level and a decrease in blood glucose level early in the recovery period from exercise. After ethanol administration, blood lactate was elevated and blood glucose was lowered from 30 to 120 min. Flushed subjects showed rapid increase in heart rate after ethanol drinking and higher blood ethanol level than non-flushed ones from 30 to 120 min after drinking.

The Effect of Functional Postural Trunk Exercise on Pain, Activities of Daily Living, Range of Motion, Deep Cranio-cervical Flexor Muscle Endurance in Neck Pain Patient by Acute Whiplash-Associated Disorders (체간의 기능적 자세운동이 급성 편타성-관련손상에 의한 경부통 환자의 통증과 기능장애수준, 관절가동범위, 두경부 심부굴곡근 지구력에 미치는 영향)

  • PURPOSE: The main objective of this study is observing the effects of the functional postural trunk exercise (FPTE) on deep cranio-cervical flexor (CCF) muscle endurance applied on neck pain patients suffering from the acute whiplash-association disorder (WAD). METHODS: The study was tested with 47 patients with neck pain. All patients were equally treated with the ordinary therapy, only experimental group (n=23) was treated with FPTE: assumption of an upright lumbo-pelvic and spinal postural position, adding a neck lengthening manoeuvre in addition. Patients attended physical therapy for 3 times a week, for 4 weeks. Visual analogue scale (VAS) for pain, neck disability index (NDI), range of motion (ROM), were recorded both before and after the intervention. Also muscle activity in the CCF test was employed to analyze the changes between before and after. RESULT: After 4 weeks of training intervention, a remarkable increase in muscle endurance, rotation, extension ROM (p<.05) and decrease in pain and NDI (p<.05) in both groups. Further, the differences between groups were muscle endurance (F=60.350, p<.01), pain, and the left rotation ROM (p<.05). CONCLUSION: From the results, the experimental group had significant the increase in muscle endurance, and the significant decrease in pain. Whilst further research in this category is necessary, these observations suggest that applying exercise in early diagnosis can be of help to treat the neck pain patients suffering from the WAD.

Factors Influencing Osteoporosis (폐경기 전.후 여성의 골다공증 영향요인)

  • Purpose: This study was carried out to identify factors influencing osteoporosis in women at pre- and post-menopausal state. Methods: The subjects of this study were 52 pre-menopausal and 125 post-menopausal women who were assessed of bone density in one general hospital. The data were collected through review of clinical records and telephone interviews using a questionnaire. Results: In the pre-menopausal women, the factors influencing osteoporosis were regular exercise (protective factor) and age (risk factor). Regression analysis showed that the factors attributable to osteoporosis included educational level, weight, age and number of pregnancy, accounted for 41.89% of the total variance. In the post-menopausal women. the factors influencing osteoporosis were age (risk factor), low educational level (risk factor), low economical state (risk factor), high parity (risk factor), and intake of coffee (protective factor). Regression analysis also showed that factors attributable to their osteoporosis included age, educational level, number of delivery, intake of the coffee, regular exercise, number of pregnancy and duration of oral pill intake, accounted for 37.41 % of the total variance. Conclusion: In pre-menopausal women, regular exercise was one of the most powerful determinant of their bone mass. Therefore, it is necessary to participate in a regular exercise program to maintain peak bone mass density prior to the onset of menopause. In post-menopausal women, increased age was the most influencing factor of their bone mass. Therefore, it is essential to establish early diagnosis and management of osteoporosis after menopause.

  • The improving in physical activity of Disability elder is the most important and effective way of prevent and treatment. Especially, It improves ADL(Activities Daily of Living) by decreasing restrictive function and dependence. This study was researched on change by comparing before and after about ADL of Women elders who were instructed self rehabilitation exercise in bedside and have been take that. The whole subjects are 92 persons. Elders aged 65 to 69 are 32 persons. elders aged 70 to 74 are 22 persons 38 persons are over the age of 75. This study shows that the whole subjects have been improved ADL. especially, "Take a bath" in detail items of ADL comes to the most improvement rate. Next, "Sit to stand on the bed and transfer" comes to the second highest rate. According to findings upon the age group. "Take a bath" is also the highest improvement rate. Elders aged 60 to 65 show "Sit to stand on the bed and transfer" is the second highest rate of improvement" and elders aged 70 to 74 show that "control urine" is the second highest rate of improvement. and elders over the age of 75 shows that "sit to stand on the bed and transfer" is the second highest rate of improvement. This study have some problems on having possibilities of results because hospitalized persons have answered imperfect statement and being compare progressed for short term with rehabilitation exercise. so, There are some causes that is early to be generalized.

Comparison of Serratus Anterior and Abdominal Muscle Activity During Push-up Plus Exercise With Hip Adduction and the Abdominal Drawing-in Maneuver

  • Background: The serratus anterior (SA) is a muscle that performs protraction of the scapulothoracic joint and plays a role in stabilizing the scapula. Imbalances or weaknesses in SA activation are associated with a variety of shoulder dysfunctions, making selective SA strengthening important for rehabilitation. Objects: We aimed to compare the muscle activation of the pectoralis major (PM), SA, external oblique (EO), and internal oblique (IO) during the push-up plus (PUP) exercise with isometric hip adduction (HA) and abdominal drawing-in maneuver (ADIM). Methods: Nineteen healthy male participants performed three PUP exercises: standard PUP, PUP with ADIM, and PUP with HA. Surface electromyography was used to measure and analyze the muscle activity for PM, SA, EO, and IO. Results: PUP with HA showed the lowest PM activity and highest SA activity, and no significant difference was observed between PUP and PUP with ADIM. PUP with ADIM showed significantly the highest EO and IO activity, followed by PUP with HA and PUP. Additionally, PUP with HA showed the lowest PM/SA ratio, and no significant difference was noted between PUP and PUP with ADIM. Conclusion: PUP with HA was able to show high SA muscle activity while reducing PM muscle activity. In addition, PUP with HA can lead to higher EO and IO muscle activity than standard PUP. This exercise could be used as a practical exercise method to selectively strengthen SA and improve scapular muscle stability during early shoulder rehabilitation.

Effects of Ovarian Cycle on Hemodynamic Responses during Dynamic Exercise in Sedentary Women

  • This study tested the hypothesis that effects of the menstrual cycle on resting blood pressure carry over to dynamic exercise. Eleven healthy females were studied during the early (EP; low estrogen, low progesterone) and late follicular (LP; high estrogen, low progesterone) menstrual phases. Stroke volume (SV), heart rate (HR), cardiac output (CO), systolic blood pressure (SBP), diastolic blood pressure (DBP), and total vascular conductance (TVC) were assessed at rest and in response to mild and moderate cycling exercise during EP and LP. During EP, compared to LP, baseline SBP ($111{\pm}1$ vs. $103{\pm}2$ mmHg), DBP ($71{\pm}2$ vs. $65{\pm}2$ mmHg) and mean arterial pressure (MAP) ($84{\pm}2$ vs. $78{\pm}1$ mmHg) were higher and TVC ($47.0{\pm}1.5$ vs. $54.9{\pm}4.2$ ml/min/mmHg) was lower (p<0.05). During exercise, absolute values of SBP (Mild: $142{\pm}4$ vs. $127{\pm}5$ mmHg; Moderate: $157{\pm}4$ vs. $144{\pm}5$ mmHg) and MAP (Mild: $100{\pm}3$ vs. $91{\pm}3$ mmHg; Moderate: $110{\pm}3$ vs. $101{\pm}3$ mmHg) were also higher, while TVC was lower (Mild: $90.9{\pm}5.1$ vs. $105.4{\pm}5.2$ ml/min/mmHg; Moderate: $105.4{\pm}5.3$ vs. $123.9{\pm}8.1$ ml/min/mmHg) during EP (p<0.05). However, exercise-induced increases in SBP, MAP and TVC at both work intensities were similar between the two menstrual phases, even though norepinephrine concentrations were higher during LP. Results indicate that blood pressure during dynamic exercise fluctuates during the menstrual cycle. It is higher during EP than LP and appears to be due to additive effects of simultaneous increases in baseline blood pressure and reductions in baseline TVC.

An Approach to Identify Single Nucleotide Polymorphisms in the Period Circadian Clock 3 (PER3) Gene and Proposed Functional Associations with Exercise Training in a Thoroughbred Horse (국내산 경주마의 주기성 시계 유전자(PER3) SNP 및 운동에 따른 기능적 식별 접근 가능성 제안)

  • The period circadian clock gene 3 (PER3) plays a role in the mammalian circadian clocksystem. A regular exercise regime may affect the PER3 transcription in skeletal muscle. Although the effects of day length on circadian and circannual processes are well established in humans and mice, the influence of exercise on these processes in the horse has not been investigated. The present study investigated the expression of the PER3 gene following exercise in a thoroughbred breed of Korean horse. In addition, a comprehensive in silico nonsynonymous single nucleotide polymorphism (nsSNP) analysis of the horse PER3 gene and predicted effects of nsSNPs on proteins were examined. The expression of PER3 in skeletal muscle was significantly upregulated after exercise. Four nsSNPs were functionally annotated and analyzed by computational prediction. The total free energy and RMSD values of PER3 gene showed causative mutations. The results showed that nsSNP s395916798 (G72R) was associated with residues that have stabilizing effects on structure and function of PER3 gene. This study documented role of PER3 gene in phenotypic adaptation related to exercise in skeletal muscle. Further, the SNPs in PER3 could serve as useful biomarkers of early recovery after exercise in racehorses.

The Effect of Fumanet Exercise Program for Life care on Cognition Function, Depression in Dementia (라이프케어 증진을 위한 후마네트 운동프로그램이 치매노인의 인지기능, 우울기능에 미치는 영향)

  • Purpose: As dementia progresses, cognitive function decreasing leads to memory loss, speech degradation, time and space degradation and judgment degradation, which causes difficulties in carrying out tasks related to daily life. It was said that community-based non-drug intervention therapy for early dementia patients was important to participate in entertainment treatment, including activities such as awareness and exercise therapy, exercise rehabilitation, aerobic exercise, and art. Methods: This study conducted 15 experimental and 15 control groups(experimental group : Fumanet exercise, control group : general occupational therapy) for eight weeks at the Daycare Center in Gyeonggi-do to find out the impact of the Fumanet exercise program on cognitive and depression functions of the elderly. The pre-post evaluation used KGDS, MMSE. Results: There were significant differences between the two groups in the function of menopause, memory recall, attention concentration and calculation, and depression, and no significant results were obtatined in memory registration, language function, understanding and fracture. The Fumanet movement was judged to be effective in improving cognitive function and reducing depression for the elderly with dementia. Conclisions: The Fumanet movement was judged to be effective in improving cognitive function and reducing depression for the elderly with dementia.

Effects of Health Belief on Exercise Adherence among college Students in China -Mediating Effect of Planning behavior and action control- (중국 대학생의 건강 신념이 운동지속성에 미치는 영향 -계획 행동과 행동 제어의 매개효과-)

  • The purpose of this study analyzes by confirming the structural relationship between the four variables of Chinese college students' health beliefs, planned behavior, behavioral control, and exercise persistence, this study can establish measures to enhance and improve college students' health beliefs and planned behavior and improve behavioral control. The purpose is to provide basic data to improve the level of continued exercise among college students. The EBI study verified reliability and validity through a preliminary survey targeting 380 people. This survey distributed questionnaires to college students at 52 universities from May 12 to July 21, 2023, and a total of 5,109 copies were finally selected and analyzed, excluding invalid questionnaires. As a result of the study, first, the structural relationship between variables showed that health beliefs had a positive effect on planning behavior, behavioral control, and exercise persistence. Second, planning behavior was found to have a positive effect on behavioral control and motor persistence. Third, behavioral control was found to have a positive effect on exercise persistence. Fourth, planning behavior and behavioral control were found to play a positive mediating role in the relationship between health beliefs and exercise persistence. Therefore, to improve college students' health beliefs, schools should open a health psychology curriculum. Furthermore, there is a need to strengthen college students' beliefs about their health by developing and distributing customized health education programs for college students.
