Statistical Techniques based Computer-aided Diagnosis (CAD) using Texture Feature Analysis: Applied of Cerebral Infarction in Computed Tomography (CT) Images

  • The brain is the body's most organized and controlled organ, and it governs various psychological and mental functions. A brain abnormality could greatly affect one's physical and mental abilities, and consequently one's social life. Brain disorders can be broadly categorized into three main afflictions: stroke, brain tumor, and dementia. Among these, stroke is a common disease that occurs owing to a disorder in blood flow, and it is accompanied by a sudden loss of consciousness and motor paralysis. The main types of strokes are infarction and hemorrhage. The exact diagnosis and early treatment of an infarction are very important for the patient's prognosis and for the determination of the treatment direction. In this study, texture features were analyzed in order to develop a prototype auto-diagnostic system for infarction using computer auto-diagnostic software. The analysis results indicate that of the six parameters measured, the average brightness, average contrast, flatness, and uniformity show a high cognition rate whereas the degree of skewness and entropy show a low cognition rate. On the basis of these results, it was suggested that a digital CT image obtained using the computer auto-diagnostic software can be used to provide valuable information for general CT image auto-detection and diagnosis for pre-reading. This system is highly advantageous because it can achieve early diagnosis of the disease and it can be used as supplementary data in image reading. Further, it is expected to enable accurate medical image detection and reduced diagnostic time in final-reading.

A Prospective Study on an Association between Apolipoprotein E ${\varepsilon}4$ and Cognitive Change in Community-Dwelling Elders with Alzheimer's Disease (일 지역 알츠하이머병 노인에서 Apolipoprotein E ${\varepsilon}4$와 인지변화의 연관에 대한 전향적 연구)

  • Objectives : The aim of this study was to examine the prospective impact of the apolipoprotein E (APOE) ${\varepsilon}4$ on cognitive performance in the community-dwelling elderly individuals with Alzheimer's disease (AD). Methods : The total number of subjects was 30 (12 men and 18 women) who were diagnosed with AD from a Korean project of "Early Detection of Dementia". People aged 65-85 years were included in the analysis. The eight neuropsychological domains from the Korean version of Consortium to Establish a Registry of Alzheimer's Disease (CERAD-K) were conducted to test subjects. They have been followed at 24-month intervals with the same assessments at each interval. Their cognitive performance at 2 year intervals was compared by the occurrence of the APOE ${\varepsilon}4$. Results : The impact of ${\varepsilon}4$ allele was significant in the Word List Memory Test (WLMT, F = 4.345, df = 1, p = 0.021) and Word List Recall Test (WLRT, F = 5.569, df = 1, p = 0.033). Conclusions : The APOE ${\varepsilon}4$ allele was significantly correlated especially with verbal episodic memory domain in community-dwelling elders diagnosed with AD.

Clinical Application of I-123 MIBG Cardiac Imaging (I-123 MIBG Cardiac SPECT의 임상적 적응증)

  • Cardiac neurotransmission imaging allows in vivo assessment of presynaptic reuptake, neurotransmitter storage and postsynaptic receptors. Among the various neurotransmitter, I-123 MIBG is most available and relatively well-established. Metaiodobenzylguanidine (MIBG) is an analogue of the false neurotransmitter guanethidine. It is taken up to adrenergic neurons by uptake-1 mechanism as same as norepinephrine. As tagged with I-123, it can be used to image sympathetic function in various organs including heart with planar or SPECT techniques. I-123 MIBG imaging has a unique advantage to evaluate myocardial neuronal activity in which the heart has no significant structural abnormality or even no functional derangement measured with other conventional examination. In patients with cardiomyopathy and heart failure, this imaging has most sensitive technique to predict prognosis and treatment response of betablocker or ACE inhibitor. In diabetic patients, it allow very early detection of autonomic neuropathy. In patients with dangerous arrhythmia such as ventricular tachycardia or fibrillation, MIBG imaging may be only an abnormal result among various exams. In patients with ischemic heart disease, sympathetic derangement may be used as the method of risk stratification. In heart transplanted patients, sympathetic reinnervation is well evaluated. Adriamycin-induced cardiotoxicity is detected earlier than ventricular dysfunction with sympathetic dysfunction. Neurodegenerative disorder such as Parkinson's disease or dementia with Lewy bodies has also cardiac sympathetic dysfunction. Noninvasive assessment of cardiac sympathetic nerve activity with I-123 MIBG imaging nay be improve understanding of the pathophysiology of cardiac disease and make a contribution to predict survival and therapy efficacy.

Different Effects of Cognitive and Non-exercise Physical Leisure Activities on Cognitive Function by Age in Elderly Korean Individuals

  • Objectives: We aimed to examine the effects of various leisure activities on cognitive impairment in young-old (aged 65-74 years) and old-old (aged ${\geq}75$ years) adults. Methods: In total, 10,279 elderly Korean individuals from the 2014 Korean National Survey on Older Adults' cohort were enrolled in our study. Cognitive impairment was assessed using the standardized score of the Mini-Mental State Examination for Dementia Screening, whereas leisure activities were recorded via self-reporting of the extent and type of leisure activity the subjects involved in over the past year. Multivariate logistic regression was used to assess the effect of leisure activities on cognitive impairment, while controlling for potential covariates. Results: The subjects were more likely to participate in cognitive activities than in non-exercise physical activities. After controlling for selected covariates, involvement in cognitive activities was found to be a significant predictor of cognitive impairment in both the groups, whereas involvement in non-exercise physical activities was not a predictor of cognitive impairment in individuals aged ${\geq}75$ years. Moreover, depressive symptoms, rural residence, and hearing difficulties were common predictors of cognitive impairment among elderly-Korean-individuals. Conclusion: Leisure activity involvement may help delay cognitive impairment, which is often concomitant with aging. Hence, an early intervention service may significantly benefit both young-old and old-old individuals.

The Relationship Between the Quality of Sleep and the Cognitive Function, Depression, and Activities of Daily Living in the Institutionalized Elderly (요양시설 노인의 수면의 질이 인지기능 및 우울감, 일상생활활동과의 관련성 연구)

  • Purpose. The purpose of this study was to determine the relationship between sleep quality and cognitive function, depression, and institutionalized elderly activity. The goal is to improve the quality of life through early intervention in the sleeping problems of the elderly hospitalized in the nursing home. Method Twentythree patients who were diagnosed with CDR (clinical dementia grade) 0.5 ~ 2 stages by a psychiatrist from September 3 to 30, 2017 were measured for sleep status, depression, cognitive function and activities of daily living at the same time once a day. The collected data were analyzed using SPSS (v.21, IBM, USA) program. Results The correlation between sleep status and cognitive function, depression, and activities of daily living was not statistically significant, and there was a moderate correlation between cognitive function and depression. Conclusion The results of this study suggest that the development and application of a program that activates the cognitive function that is institutionalized with a positive correlation between cognitive function and depression will be effective in decreasing depression in the elderly patients hospitalized in the nursing home.

Literature Review on the Development of Cognitive Function Improvement Program for the Elderly in Community (지역사회 노인의 인지기능 향상 프로그램 개발에 대한 문헌적 고찰)

  • Objective: This study was to compares and analyzes programs applied to improve cognitive function in patients with mild cognitive impairment and early dementia in the community to find out their effectiveness. Methods: In this study, 12 papers were finalized by searching for "elderly", "cognitive", "community", and "program" using the database of the Research Information System (RISS), National Assembly Library, and Korean Studies Information (KISS). Results: Programs for cognitive function were in the order of cognitive stimulation program, arts and crafts, and exercise program. In the program, rather than applying the cognitive stimulation program alone, the program was operated by combining leisure or exercise, music, art, and handicraft. The time was shown to be 30 minutes. The most frequently used evaluation tool was MMSE, followed by GDS and BBS. By cognitive domain, cognitive stimulation program and memory, satisfaction in psychology, and balance ability in exercise were evaluated the most. In the cognitive area, various cognitive stimulation areas were included, and in the exercise area, basic exercise, muscle strength exercise, joint exercise, and balance exercise were applied. Conclusion: Therefore, developing a program to improve cognitive function for mild cognitive impairment, it will be possible to prepare guidelines to establish and development.

Application of False Discovery Rate Control in the Assessment of Decrease of FDG Uptake in Early Alzheimer Dementia (조기 알츠하이머 치매의 뇌포도당 대사 감소 평가에서 오류발견률 조절법의 적용)

  • Purpose: Determining an appropriate thresholding is crucial for PDG PET analysis since strong control of Type I error could fail to find pathological differences between eariy Alzheimer' disease (AD) patients and healthy normal controls. We compared the SPM results on FDG PET imaging of early AD using uncorrected p-value, random-field based corrected p-value and false discovery rate (FDR) control. Materials and Methods: Twenty-eight patients ($66{\pm}7$ years old) with early AD and 18 age-matched normal controls ($68{\pm}6$ years old) underwent FDG brain PET. To identify brain regions with hypo-metabolism in group or individual patient compared to normal controls, group images or each patient's image was compared with normal controls usingthe same fixed p-value of 0.001 on uncorrected thresholding, random-field based corrected thresholding and FDR control. Results: The number of hypo-metabolic voxels was smallest in corrected p-value method, largest in uncorrected p-value method and intermediate in FDG thresholding in group analysis. Three types of result pattern were found. The first was that corrected p-value did not yield any voxel positive but FDR gave a few significantly hypometabolic voxels (8/28, 29%). The second was that both corrected p-value and FDR did not yield any positive region but numerous positive voxels were found with the threshold of uncorrected p-values (6/28, 21%). The last was that FDR was detected as many positive voxels as uncorrected p-value method (14/28, 50%). Conclusions FDR control could identify hypo-metaboiic areas in group or individual patients with early AD. We recommend FDR control instead of uncorrected or random-field corrected thresholding method to find the areas showing hypometabolism especially in small group or individual analysis of FDG PET.

Screening of Preventive Effects of Herbal Prescriptions in Rat Model of Arterial Thrombosis Induced by Ferric Chloride

  • Objectives: This study examined the effects of 8 types of herbal prescriptions prescribed to alleviate dementia symptoms in a rat model of arterial thrombosis induced by ferric chloride ($FeCl_3$). Methods: Thirty minutes before 35% $FeCl_3$ treatment, SD rats were intraperitoneally injected with the 8 types of herbal prescriptions, respectively. We tested the effects of the herbal prescriptions on time to occlusion (TTO) in an arterial thrombosis model using a laser Doppler flow meter. In addition, thrombus weight (TW) and collagen fiber damages were evaluated in the same condition. Results: Herbal prescriptions showed the following rank-order based on their TTO: Chong-myung-tang (CMT) > modified Jangwonhan 02 (LMK02) > Toki-shakuyaku-san (TSS) ${\geq}$ Oren-gedoku-to (OGT) ${\geq}$ Yokukansan (YKS). In particular, CMT (100 mg/kg, i.p.) and LMK02 (100 mg/kg, i.p.) delayed the TTO the most ($14.83{\pm}0.98$ and $13.67{\pm}1.03$ min, respectively) compared with the vehicle group ($7.95{\pm}0.78$ min, P<0.001). In addition, CMT, LMK02, and OGT treatment ($0.63{\pm}0.01$, $0.66{\pm}0.02$ and $0.67{\pm}0.01$ mg/mm, respectively) significantly reduced thrombus weight compared with the vehicle treatment ($0.78{\pm}0.03$, P<0.001) and also alleviated collagen fiber damage (CMT; $28.40{\pm}2.22$%, LMK02; $30.79 {\pm} 4.07$%, OGT; $26.20{\pm}1.48$%) in the vessels injured by $FeCl_3$. Therefore, CMT and LMK02 showed the greatest preventive activity in rat model of arterial thrombosis induced by $FeCl_3$. Conclusions: These results provide experimental evidence for traditional use of herbal prescriptions, suggesting that CMT and LMK02 extracts could be used to prevent vascular injury and thrombosis in the early stages of dementia.

Analysis of Occupational Therapy Intervention Research for Improving Memory: Focus on Single-Subject Research Design in Korean Academic Journals (기억력 향상을 위한 작업치료 중재 연구 분석: 국내 단일대상연구 중심으로)

  • Objective : This study aimed to identify the characteristics and analyze the quality of studies on memory improvement using a single-subject research design. Methods : Six studies were selected through the Research Information Sharing Service (RISS), Korean Studies Information Service System (KISS), and National Digital Science Library (NDSL). Keywords were memory training, stroke, early dementia, mild cognitive impairment, and single-subject research design. The characteristics and quality levels were analyzed between January 2011 and October 2020. Results : Regarding the quality level, the middle level (7-10 points) was 66.7% of the four articles, and the high level (11-14 points) was 33.3% of the two articles. Reversal designs were used in all studies. Independent variables were errorless learning, smartphone application, cognitive training system (CoTras), spaced retrieval training (SRT) with errorless learning, spaced retrieval training, and iPad applications. The dependent variables included memory, attention, electroencephalogram, instrumental activities of daily living, depression etc., which increased after the intervention. The total session, study period, and intervention time were varied. Conclusion : In single-subject research design related to memory training, occupational therapy intervention was confirmed as an effective method for improving memory and attention. The quality level of single-subject research design was moderate or higher, and high-quality level of studies should be conducted by additional design to increase the validity in the future.

The Effects of a Cognitive Stimulation Activity Program on the Mild Cognitive Impairment Elderly's Cognitive Function, Self-Efficacy, and Depression (인지자극 활동프로그램이 경도인지장애 노인의 인지기능, 자기효능감, 우울에 미치는 영향)

  • The purpose of this study was to implement a cognitive stimulation activity program and examine its effects elderly people who is capable of daily life and community activities but suffering from per sistent cognitive decline on the elderly's cognitive function, self-efficacy, and depression. The purpose of this study was to help elderly people who is capable of daily life and community activities but suffering from persistent cognitive decline improve their ability to perform and adapt to activity-based tasks incorporating a variety of actions of different subdomains and difficulty levels, ac cording to their characteristics and cognitive levels. The aim of the program created was to systemati cally improve the subdomains of cognitive functions and eventually preventing these conditions develo ping into dementia in those affected. This study also enabled the subjects to improve their self-effica cy through newly gained positive perceptions and attitudes toward their own abilities by successfully expressing their experience or thoughts about circumstantially and relationally meaningful events and memories, and performing activities that promote sensory stimulations. The group activities conducted as part of the study helped the subjects reduce their symptoms of depression not only by expressing their thoughts and emotions, but also by encouraging empathy with, understanding of, and caring for others. Given that this study confirmed the potential of the cognitive stimulation program designed fo r it as a clinical intervention for elderly people with cognitive impairment, it is suggested as a useful preventive program for early stage dementia.
