  • Purpose: Facial rejuvenating surgery has become a challenge to most plastic surgeons. Patients are demanding fewer complications, a prompt recovery, and more natural results. Current trend of the face lift surgery has been developed into less invasive procedures. Every aging patient in Asia wants to look younger without obvious evidence of surgical correction. Methods: The authors performed the limited dissection face lift with PDS quilting suture on twenty five patients. These five quilting sutures consist of sutures at 1~2 cm posterior to the prominent point of zygoma to the periosteum of the zygomatic arch, at the lateral border of Bichat's fat pad area to the zygomatic arch, at the lateral border of the orbicularis oculi muscle to the deep temporal fascia, at the upper lateral border of the platysma to the periosteum of the mastoid, and at the anterior lower margin of the earlobe to the deep temporal fascia by quilting suture technique to achieve rejuvenation. Results: These procedures could produce a balanced volumetric rejuvenation. This method gave considerable benefit of stable and satisfactory results. It provides reduced operative time, well corrected nasolabial fold and neck wrinkle, and swift recovery with minimal complications. Conclusion: Although it could not replace the classic facelift, this technique can be recommended as an option for patients who do not present with advanced facial aging or not want a more extensive procedure.

  • Magnetic Resonance Image (MRI) has been used as a safe, conventional and harmless diagnostic tool. However, thermal injuries have frequently been reported during MRI scanning due to the heat generated by the reaction with the magnetic field. It is recommended that metal-containing monitoring devices such as pulse oximetry and ECG monitoring leads should be removed prior to the start of the MRI scan, but these monitoring devices are inevitably placed in children or patients in the intensive care unit who have low compliance with the scan. Since the interaction between the metal probe or wire loop of pulse oximetry and the magnetic field can result in high thermal conduction, full-thickness burn can occur over the entire body surface during the MRI examination. Several cases of thermal burns from pulse oximetry on the fingers have been reported. However, we present a case of a full-thickness burn arising left earlobe in a 2-month-old child caused by the high conduction heat from pulse oximetry metal probe.

  • Transcutaneous auricular vagus nerve stimulation (taVNS) is known to be effective in improving symptoms of numerous diseases such as depression and epilepsy by increasing vagus nerve activity through electrical stimulation. The purpose of this study is to investigate the effect of vagus nerve stimulation on the activity of autonomic nervous system and the changes in postprandial blood glucose levels. Seven healthy adults participated in a non-invasive transcutaneous auricular vagus nerve stimulation experiment. taVNS (25 Hz, 200 ㎲, biphasic pulse) was applied to the cymba concha (taVNS group) or the earlobe (Sham-taVNS group) of the left ear. As autonomic nervous system signals, skin conductance level, skin temperature, and heart rate were recorded during the application of taVNS. Postprandial blood glucose changes due to food intake were recorded at 5 min intervals for 25 minutes after taVNS or sham-taVNS. The taVNS showed a significantly lower skin conductance level than the shamtaVNS (p < 0.05). The increase rate of postprandial blood glucose was significantly lower in the taVNS than in the sham-taVNS (p < 0.05). These results showed that taVNS reduced the activity of the sympathetic nerve system and alleviated early rise in postprandial blood glucose. Although further studies in diabetic patients are needed, this study suggest that taVNS has a potential for clinical use to improve postprandial blood glucose.

  • This paper is based on the design and analysis computing model of oxygen saturation with the pulse oximeter using the integral ratio of pulsating components. In our proposed algorithm. we modeled the transmitted optical signal in fingertip or earlobe to DC component $A_{dc}$ pulsating component $A_a\;Sinwt$, noise component $A_{noise}$ and etc.. To separate the pulsating components and DC components efficiently, we defined the signal average to DC components. Also we presented the way to eliminate the noise using integral ratio. To acquire a linearity of correlation graph for pulsating components ratios and non invasive oxygen saturation. we intensively observed on the oxygen saturations in the range of 75-100% in consideration of the error range of simulator. Also, for real time processing we experimented on changing the period of area calculating cycle from 1 to 6. The functional evaluation of the algorithm is compared with the method using the amplitude ratio of pulsating components frequently seen with pulse oximeter. The result was that our algorithm with 4 cycles of area calculating cycle which considered to be best fit by 1% to the existing method. Moreover r , the decision coefficient showing the correlation of regression graph with real data, proved better result of 0.985 than 0.970.

  • In this paper, we correct pulse wave velocity(PWV) with heart-rate and derive regression equations to estimate intima-media thickness(IMT). Widely used methods for diagnosis of arteriosclerosis are IMT and PWV. Arterial wall stiffness determines the degree of energy absorbed by the elastic aorta and its recoil in diastole but there is not correlation between sclerosis and IMT in an existing study. In this study, we will correct PWV with heart-rate and get regression equation to estimate IMT using heart-rate correction index(HCI). We executed experiments for this study. Made up question of physical condition and measured electrocardiogram(ECG), photoplethysmogram (PPG) of finger-tip and toe-tip and ultrasound image of carotid artery. Calculated PWV and IMT using ECG, PPG and ultrasound image. We found that every p-value between PWV and IMT is not significant(<0.05). But p-value between IMT and HCI which is a corrected PWV using heart-rate is significant(>0.01). We use HCI and various measured parameter for estimating regression equation and apply backward estimation to select parameters for regression analysis. Result of backward estimation, found that only HCI is possible to derive proper regression equation of IMT. Relationship between PWV and IMT is the second order. Result of regression equation of E-H PWV is $R^2$=0.735, adj $R^2$=0.711. This is the best correlation value. We calculate error of its analysis for verification of earlobe PWV regression equation. Its result is RMSEP=0.0328, MAPE(%) = 4.7622. Like this regression analysis, we know that HCI is useful parameter and relationship between PWV, HCI and IMT. In addition, we are able to suggest possibility which is that we can get different parameter of prediction throughout just one measurement.
