수직고경 설정을 위한 안면 계측방법에 관한 비교 연구 (A COMPARATIVE STUDY ON THE SEVERAL FACIAL MEASUREMENT METHOD FOR VERTICAL DIMENSION)

  • This study was designed as a reference of vertical dimension in prosthetic treatment. The author analyzed six facial measurements, namely, (1) the height of lower face at maximum intercuspal position, (2) the height of lower face at resting position, (3) midface, (4) external ear and lateral wall of orbit, (5) interpupillary distance, (6) distance between pupil and mouth in the 100 Won-kwang Univ. Dental collage students(50 : male, 50 : female), who have normal occlusion, no posterior prosthesis, no experience of orthodontic treatment, and no deformity of facial soft tissue and no temporomandibular dysfunction. The results of this study were as follows : 1. The length of midface was shortest and the inter-pupillary distance was longest in both male and female. 2. The length difference with the length of midface and lower face at maximum intercuspal position was 5.64mm in male and 2.23mm in female, so the lower face was longer, 3. The facial measuring component, similar to lower face at maximum intercuspation, was the length of between medial wall of external ear and lateral wall of orbit. It's difference was 1.3mm in male, 1.77mm in female, and the lower face was shorter. 4. The difference of lower facial length in resting position and maximum intercuspation was 2.48mm in male, 2.24mm in female, the length of resting position was therefore longer. 5. The most clost correlation with the height of maximum intercuspal positioning lower face was resting lower face in both groups.

The Clinical Application of Sound-Protection

  • Bekesy measured the sound transformation system of the middle ear 49 years ago. According to his reports, a ratio between the size of ear drum and the size of oval window is 17 : 1, and the lever function of the ossicles is physiologically 1.3 : 1. Therefore, the hearing might be aggravated to 27.5㏈ in the case of the vanishment of 3 ossicles. In 1952, Wullstein reported 5 types of tympanop-lasty and the fourth type among them was especially named for the sound-protection. The oval window is only exposed by the sound pressure and the round window is not exposed. According to the application by this idea, the post-operative hearing might be improved until 27.5㏈. Mean while, in 1942, Onchi verified through his experiment that the results of Bekesy's measurement was not completely conformed to Onchi result. Bekesy measured the sound pressure on the stapes plate of the oval window, on the other hand, Onchi measured the sound pressure on the surface of the perilymph of the oval window after removing the stapes plate(Fig. 1).(Figure omitted) The difference of their experiment is recognized that the impedance of the stapes plate exists or not (Fig. 1). Both Audiogrums are compared as Fig. 2. The result of IV type of tympanoplasty is success ful in 54% as the Table 1. (Table Omitted) The reason of unsatisfactory is caused by the thick and unmovable window-membrane and by the closing of air passage to the round window. The closing of the air passage to the round window is occurred by the adhesion between the grafting membrane and the surface of promontorium. In order to preserve this adhesion, I produce to transplantate the mucous membrane of the lip to the bone surface of tympanic cavity after removing the granulation tissue of the tympanic cavity and to form a membranous canal for the sake of air passage (Fig. 3). (Figure Omitted) The post-operative hearing by this method is shown as Fig. 4, 5. In other words, the post-operative sound pressure entered into the cochlea directly, by way of the oval window only, not by way of the round window, as a theorie of the sound protection. Cryptotia recurrence lowering technique with additional acellular dermal matrix graft

  • Background: Cryptotia is a congenital anomaly in which the upper part of the retroauricular sulcus is absent and buried underneath the temporal skin. Various surgical techniques have been reported for the correction of cryptotia following Kubo's V-Y plasty in 1933. Conventional methods using a local skin flap, skin grafting, tissue expansion, Z-plasty, and any of these combined approaches can result in skin deficiency of the upper auricle. The aim of this study was to develop a new method that improves cosmetic results and has fewer complications. Methods: This study involved four patients in whom five cryptotia deformities were corrected using V-Y plasty and Z-plasty. After elevation of the flap, acellular dermal matrix (ADM; MegaDerm) that was over 5 mm in thickness was applied to the cephalo-auricular angle and positioned to enhance the projection of the ear. Lastly, the flap was transposed to complete the repair. Results: Between January 2014 and February 2018, cryptotia correction with ADM graft was performed in four patients. None of the patients developed a recurrence of cryptotia, and there were no postoperative complications such as wound infection, seroma formation, and dehiscence. In addition, the procedures resulted in a favorable cosmetic appearance. Conclusion: Based on these findings, i.e., no recurrence and a favorable cosmetic result, when using an ADM graft, it is suggested that this technique could be an alternative method of cryptotia correction. It could also lessen donor-site morbidity when compared to autologous cartilage grafting and be more cost-effective than using cartilage from a cadaver.

Colonization of Pathogens in Earphones and Observation of Effective Sterilization Methods and Cycles

  • The use of earphones has recently been widely used around the world. In currently, students wear earphones a lot in a daily life. The types of earphones are open-earphones, Canalphones, and headphones. Many students don't periodically to sterilization their earphones. Therefore, it can be an incubator that can induced ear infections. The objective of this study was to detect the pathogenic bacteria from the earphones used by the students. A total of 3 type earphones swabs were collected by sterile cotton swabs. The swabs were inoculated onto BHI agar and incubated aerobically 48 hour at 37℃. 16s rRNA PCR, electrophoresis and sequencing were performed to confirm the identification of all the bacterial isolates. As a result, 24 pathogens were identified in sequencing. Three types of earphones were sterilized in three ways: ultraviolet (UV), 70% ethyl alcohol, and antibacterial wet tissue. If you use earphones for a long time without disinfecting them for a long time, it causes various diseases such as external ear infections. The findings of this study the users periodically to sterilization their respective earphones.

혈관기원종양으로 오인된 귓바퀴의 색소 모기질종: 증례 보고 (Pigmented Pilomatricoma on the Ear Resembling Vascular Tumor before Surgery: A Case Report)

  • Pilomatricoma is characterized by a semi-transparent epidermis, especially pigmented pilomatricoma, containing melanocytes in basaloid cells, which are dark and purple, resembling vessel-derived skin masses. If the vascularity at doppler ultrasound is high before surgery, it may be misdiagnosed. A 10-year-old female patient visited our clinic because of a mass in the right ear triangular fossa. Ultrasonography was performed, and a vascular-origin tumor was suspected because of the high vascularity. The excised mass was diagnosed as pigmented pilomatricoma by a pathologist. Pilomatricoma is mistaken for other masses owing to its various phenotypes. A misdiagnosis can lead to misdirected strategies which can cause delayed treatment and can result in an increase in the size of the pilomatricoma, making the sequalae of surgery more complicated. For proper treatment, careful examination and evaluation are required before surgery.

Er:YAG 레이저와 Er,Cr: YSGG 레이저가 염증유발 마우스조직에 미치는 영향 (The Effect of ER:YAG Laser & ER,CR:YSGG Laser on the Tissue of the Inflammation-Induced Mouse)

  • Purpose: This study was performed to find out the effects of the Er:YAG laser (Key Laser) & Er,Cr:YSGG laser (Water Laser) on inflammatory tissues. Materials and Methods: It was performed on about 20 g, 6 weeks male ICR mouses. They were grouped into the control (negative), the inflammation induced 'control'(positive), Er,Cr:YSGG laser exposured group after inducing inflammation, Er:YAG lasere exposured group after inducing inflammation each 15 mouses. The mouses were applicated 0.5% DNFB 1 cc on ear skin twice a day for 4 days until symptom expression. After laser exposure, ear tissues were extracted and defined gene expression by RT-PCR. Then, tissue staining, lymphocytes observation, electromicroscophic laboratory were carried out. Results: Interleukin-$1{\beta}$ was expressed much less in the A-laser exposed group. Interleukin-$1{\beta}$ & Tumor Necrosis Factor-${\alpha}$ were expressed 7 times lesser in the A-laser exposed group. The number of Lymphocytes related to inflammation was decreased rapidly in the A-laser exposed group in vivo. he number of cavity recovered normal was a little bigger in the A-laser exposed group after 5 days Conclusion: The expression of IL-$1{\beta}$ & TNF-${\alpha}$, hitologic change, observation with electron microscope shows that Erbium laser exposure causes lesser inflammation with A-laser rather than B-laser.

고구마뿌리혹선층(Meloidogyne incognita)에 대한 국내 자생식물의 살선충 활성 (Nematicidal activity of Korean native plants against root-knot nematode, Meloidogyne incognita)

  • 국내 시설재배지에서 고구마뿌리혹선충(Meloidogyne incognita)의 발생으로 많은 경제적인 피해가 발생하고 있으나 이를 효과적으로 방제 할 수 있는 천연물 기원의 친환경 자재가 거의 없는 실정이다. 국내의 자생식물 중 뿌리혹선충에 활성이 있는 식물 종을 선발하기 위하여 자생식물 26과 43종으로 부터 메탄올 조추출물을 얻은 다음, 기주식물인 토마토 뿌리로부터 얻은 뿌리혹선충을 조추출물이 1,000 mg $kg^{-1}$수준으로 포함된 24-well tissue culture testplate에 $1\times10^2$ 마리 수준으로 처리하고, 해부현미경 하에서 살선충 효과를 검정하였다. 실험에 사용한 43종의 식물시료 중 쇠비름(Portulaca oleraceae L.)과 쇠별꽃(Stellaria aquatica SCOP.) 조추출물의 살선충 효과는 각각 00.0%되 40.6%이었다. 그러나 대부분의 식물시료 조추출물은 살선충 효과가 30% 미만으로 낮았다. 쇠비름과 쇠별 꽃 조추출물에 함유되어 있는 활성물질을 단리 할 경우 친환경농가에서 뿌리혹선충 예방 혹은 방제용도로 활용될 수 있을 것이라 사료된다.

소의 일중 체온변화 Data Base 구축에 관한 연구 (Establishment of Data Base for Body Temperature Change in Cattle)

  • 센서 네트워크 및 무선 주파수 식별 기술을 이용하여 가축의 건강상태를 자동으로 모니터링 하기위한 기술에 활용하기 위하여 정상시 소의 체온 data base를 구축하였다. 체온측정을 위해 거세한 홀스타인 수소 3두를 공시하였다. 동물들은 stanchion barn에서 2주간 적응시킨 후 열전대와 recorder 장치를 이용하여 귀, 목, 머리와 심부 4부위의 온도를 9일간 연속 측정하였다. 측정한 체온의 모든 부위는 하루 중 낮 시간에는 체온이 올라가고 다시 저녁시간에는 체온이 떨어지는 패턴을 보였다. 하루중 심부체온은 $36.1^{\circ}C$에서 $38.2^{\circ}C$의 범위에서 변화하였다. 각 피부 온도는 환경온도 변화에 따라 차이를 보였다. 귀, 목, 머리 온도의 하루중 변화는 각각 $28.5{\sim}36.3^{\circ}C$, $28.0{\sim}36.1^{\circ}C$, $28.2{\sim}35.0^{\circ}C$를 나타내었다.

만성 및 급성 염증 모델에서 강황추출물의 효과 (Anti-inflammatory Effects of Turmeric (Curcuma longa L.) Extract on Acute and Chronic Inflammation Models)

  • 본 연구에서는 강황 유래의 curcuminoids가 없는 다당체를 이용하여 급성 및 만성 염증 모델에서의 항염증 효과를 검증하고자 하였다. 급성염증에 대한 효과를 알아보기 위하여 carrageenan에 의한 족부종 유발과 xylene에 의한 귀부종 유발을 일으킨 모델을 이용하여 수행하였다. Carrageenan에 의한 족부종 유발 실험에서 강황추출물과 관절염 치료제로 알려진 diclofenac sodium을 처리하였을 때는 부종의 증가도가 낮은 것으로 나타났다($P{\leq}0.05$). 또한 다른 급성염증 모델인 xylene application 마우스 모델에서도 대조군에 비하여 diclofenac sodium과 강황추출물(31.5, 63, 126 mg/kg)을 농도별로 경구투여 하였을 때 염증 억제율이 각각 78.13%, 68.75%, 62.50% 및 59.38%로 나타나 염증 억제율이 높은 것으로 나타났다($P{\leq}0.05$). 만성염증에 대한 효과를 알아보기 위한 cotton pellet 육아종 실험에서는 대조군과 비교하여 삼출물과 염증 육아종이 적게 발생된 것으로 나타났다($P{\leq}0.05$). 따라서 강황 유래의 다당체를 경구투여 하였을 때 만성 및 급성 염증에 대한 저해활성이 있는 것으로 나타나 항염증 소재로서 개발 가능성이 있음을 제시하였다.

고양이에서 비디오 이경검사법을 이용한 염증성 폴립의 진단 및 치료 1례 (Diagnosis and Treatment of Feline Inflammatory Polyp with Video Otoscopy : A Case Report)

  • 1년 간 우측 귀에 재발성 외이염을 앓다가 이도 내 종괴가 발견된 19개월령의 중성화된 수컷 잡종 고양이에서 종괴의 제거를 시행하였다. 전신마취 후 2.7 mm 내시경을 우측 이도로 삽입하였으며, 원활한 시야 확보를 위해 이도 내 세척을 동시에 시행하였다. 이도 내에서 4.9 mm 직경의 둥근 붉은색 종괴가 확인되었고 traction-avulsion 방법으로 제거되었다. 제거 후 고막 파열이 확인되었으며, 고실 내의 삼출물 제거를 위해 0.9% 멸균생리식염수로 귀를 플러싱하였다. 제거된 종괴는 조직검사 결과 심한 궤양을 동반한 육아조직으로 진단되었다. 술 후 환축은 특별한 부작용을 나타내지 않았으며, 5개월이 경과된 현재 재발의 징후는 발견되지 않았다. 비디오 이경검사법의 사용은 고양이 염증성 폴립의 치료를 위한 효과적인 방법이 될 수 있을 것으로 생각된다.
