소득계층에 따른 노인들의 건강 불평등 측정: EQ-5D 척도를 중심으로 (An Analysis on Income-related Health Inequality of the Aged Applied to EQ-5D)

  • 본 연구는 EQ-5D를 이용하여 소득계층에 따른 노인들의 건강불평등을 측정하였다. 특히 건강불평등의 다양한 측면을 파악하기 위하여 집중지수를 EQ-5D의 하위차원 및 노인들의 하위집단에 따라 분해(decomposition)하였다. 분석 자료는 2010년 국민건강영양조사 원자료가 사용되었으며 이중 60세 이상인 자를 노인으로 정의하여 분석하였다. 본 연구의 분석결과를 요약하면 다음과 같다. 첫째, 노인들의 EQ-5D 집중지수는 .0254를 기록하였다. 이는 고소득층에 유리한 불평등이 있음을 의미한다. 나아가 EQ-5D를 구성하는 모든 하위차원 즉, 운동능력, 자기관리, 일상활동, 통증/불편, 불안/우울, 기능악화 등에서도 불건강이 저소득층에 집중되어 나타났다. 둘째, EQ-5D 집중지수를 가장 많이 설명하는 하위차원은 운동능력으로 EQ-5D 집중지수의 35.8%를 설명하였다. 그 다음으로 통증 및 불편(17.7%), 기능악화(17.7%), 일상관리(14.6%) 등의 순으로 설명력이 높다. 이러한 결과는 전 연령층을 분석한 선행연구의 결과와 대조를 이루는데, 선행연구들에서 기여도가 매우 낮았던 운동능력(3.9%)이 크게 상승한 반면 불안/우울의 기여도는 약화되었다. 셋째, 성별, 지역, 배우자 유무에 따라 EQ-5D 집중지수를 분해한 결과 세 집단이 집중지수를 설명하는 정도는 비슷하게 나타났다. 세 집단은 각각 건강형평성의 1/10정도를 설명하였다. 넷째, EQ-5D 집중지수를 하위집단에 따라 분해한 결과, 남성보다는 여성이, 도시지역보다는 농어촌지역이, 그리고 배우자가 있는 노인보다는 없는 노인의 내부불평등이 더 크게 나타났다. 내부 불평등이 큰 세 집단의 경우 모두 EQ-5D 평균값도 낮았는데, 이는 저소득 여성노인일수록, 농어촌지역 저소득 노인일수록, 배우자가 없는 저소득 노인일수록 건강관련 삶의 질이 떨어짐을 의미하는 것이었다. 마지막으로 하위집단의 내부불평등이 집중지수를 설명하는 정도는 여성노인집단, 도시지역 노인집단, 배우자가 있는 노인집단이 크게 나타났다.

  • Stomach cancer shows the highest incidence among cancer patients in Korea and it deteriorates the patients' health-related quality of life considerably. This study measured utility weights of severe stomach cancer symptoms using VAS (Visual Analogue Scale), TTO (Time-trade Off) and EQ-5D (EuroQol-5 Dimension) methods. A survey questionnaire was developed to describe the symptoms of severe stomach cancer comprehensively and concisely using VAS, TTO and EQ-5D. A face-to-face interview was conducted for 21 cancer inpatients in a hospital and 25 non-patients. The mean utility weight is 0.498 for VAS, 0.375 for EQ-5D and 0.300 for TTO. Using VAS or TTO methods, there were no statistically significant differences between patients and non-patients while there was significant difference in EQ-5D. The utility weight with VAS and EQ-5D has shown similar trend - higher score in patient group, male and aged while it has opposite trend with TTO.

일부 농촌지역 맞춤형 방문건강관리 대상 취약계층 노인들의 건강 관련 삶의 질 (EQ-5D) 수준 및 관련요인 (Levels of Health-related Quality of Life (EQ-5D) and Its Related Factors among Vulnerable Elders Receiving Home Visiting Health Care Services in Some Rural Areas)

  • Purpose: The purpose of this study is to identify levels of quality of health-related life (EQ-5D) and its related factors among vulnerable elders receiving home visiting health care services in some rural areas. Methods: The subjects of this study were vulnerable elders aged 65 or higher receiving visiting health care benefits by a public health center in a county. Results: Levels of health-related EQ-5D were high when the subjects were males, their ADL and IADL were high. The EQ-5D of the vulnerable elders and social supports had a positive correlation, while their IADL and depression had a negative correlation with the EQ-5D. We can see that the EQ-5D related factors are variables that have significance influence on gender, subjective health status, BMI, IADL, depression levels, and social support. Conclusion: To improve the EQ-5D of the vulnerable elderly, it needs maintenance of independent IADL levels, social supporting systems using small meeting of the elderly in community. It is necessary to maintain independent IADL levels, enhance social supporting systems including small group gatherings for elders living alone by means of places like community halls, and develop specific intervention programs for the prevention and management of depressive elders.

Exploring Concurrent Validity and Item Level Analysis for Two Korean Versions of Health-Related Quality of Life Instrument: EQ-5D vs. WHOQOL-BREF

  • Background: Cross-culturally adapted questionnaires may not be comparable to their original version. Objects: To examine concurrent validity of two health-related quality of life (HRQOL) instruments for the Korean versions of EuroQOL-5 Dimension (EQ-5D) and the abbreviated version of the World Health Organization Quality of Life (WHOQOL-BREF) instrument. Methods: A total of 139 cancer survivors from two rehabilitation institutes was recruited. All participants were registered for palliative rehabilitation care. Both instruments were concurrently administered by health care providers following the second bout of the rehabilitation cares. Rasch partial credit model and Spearman's correlation analysis were used to investigate: 1) dimensionality, 2) hierarchical item difficulty, and 3) concurrent validity using correlations between two instruments. Results: For the WHOQOL-BREF, all items except negative feeling, pain, dependence of medical aid, were found to be acceptable, while all items of EQ-5D were acceptable. There was an evidence of negative correlations between EQ-5D and 4 domains of WHOQOL-BREF. Two correlations were strong (EQ-5D vs. physical health domain, ρ = -0.610, 95% CI = -0.716 to -0.475) and moderate (EQ-5D vs. psychosocial domain, ρ = -0.402, 95% CI = -0.546 to -0.236). Other two correlations were weak (EQ-5D vs. social relationship and environmental domains, ρ = -0.242, 95% CI = -0.401 to -0.075 and ρ = -0.364, 95% CI = -0.514 to -0.207, respectively). Item difficulty calibrations of the two measurements were ranged from -0.84 to 0.86 for the EQ-5D and -1.07 to 1.06 for the WHOQOL-BREF. Conclusion: The study provides some supports for the concurrent validity of the two Korean versions of HRQOL instrument, with evidences of weak to strong correlations between the EQ-5D and four domains of the WHOQOL-BREF applied to various cancer survivors. Additionally, the cancer survivors appeared to have more of a tendency to view the EQ-5D items as being slightly more challenging than the WHOQOL-BREF items.

Sex differences in health-related quality of life among older Korean adults with type 2 diabetes mellitus: a cross-sectional study

  • Objectives: This cross-sectional study examined sex differences in Health-Related Quality of Life (HRQoL) among seniors with Type 2 Diabetes Mellitus (T2DM). Methods: Data from the Korea National Health and Nutrition Examination Survey (2008-2020) were analyzed. The EuroQol-5 Dimensions (EQ-5D), a measure of HRQoL, was used. It comprises five dimensions: mobility, self-care, usual activity, pain/discomfort, and anxiety/depression, each with three levels. Results: Analysis of 3,826 older adults with T2DM showed a significant increasing trend in the EQ-5D Index from the 4th survey phase onwards (P for trend < 0.001 for both men and women). Men consistently reported higher EQ-5D levels than women across all survey years. Women's EQ-5D levels remained lower than men's, maintaining a decade-old disparity (P < 0.05). Men scored significantly higher (P < 0.05) in most EQ-5D domains, except for self-care and anxiety/depression, resulting in a higher total EQ-5D Index (P = 0.001). Increased comorbidities were strongly associated with lower EQ-5D levels in both sexes. Additionally, there was a negative correlation between the EQ-5D Index and refined grain intake for both sexes (P for trend < 0.001), with high-EQ-5D groups consuming fewer refined grains. Women in the high-EQ-5D group consumed more nuts, vegetables, and meat compared to men (P for trend < 0.05). Conclusions: Our study highlights the sex disparities in HRQoL among older adults with T2DM. The findings suggest the need for tailored treatment guidelines aimed at improving the HRQoL of elderly T2DM patients, with a focus on their sex-specific characteristics. Implementing these tailored guidelines could enhance the HRQoL of older women with T2DM and promote more equitable healthcare outcomes. This underscores the importance of considering sex differences to comprehensively improve the well-being of this population.

EQ-5D를 이용한 강박장애 환자의 삶의 질과 증상 중증도의 연관성 연구 (Association between Quality of Life and Symptom Severity in Obsessive-Compulsive Disorder Patients Using EQ-5D)

  • Objectives Obsessive-compulsive disorder (OCD) is a disabling psychiatric disorder, and more attention is recently paid on the quality of life (QoL) in OCD patients. The Euro-QoL-5D (EQ-5D) is a widely used self-report to calculate a single score which represents 'health utilities'. The aim of this study was to assess the health-related QoL for patients with OCD using the EQ-5D and to examine the relationship between health-related QoL and symptom severity. Methods Seventy-three patients with a Diagnostic and Statistical Manual of Mental Disorders (DSM-5) diagnosis of OCD were recruited from the outpatient clinic in Seoul National University Hospital. Symptom severity was assessed using the Yale-Brown Obsessive-Compulsive Scale (YBOCS), and QoL was assessed with the EQ-5D-5L questionnaire. Using Korean valuation study, a single score of QoL was calculated. Results Most of the OCD patients were relatively young (< 45 years) with the mean YBOCS total score of 19.33. The mean EQ-5D score was 0.71 and significantly correlated with symptom severity (r = -0.483, p < 0.001). 25% of the EQ-5D score was predicted by the YBOCS total score (b = -0.011, p < 0.001) by regression analysis. Conclusions OCD patients suffer from lower health-related QoL and QoL significantly decreased as symptom severity increased. The results of the EQ-5D would enable further studies on QoL comparison across medical disease and mental disorders.

  • 건강관련 삶의 질(EQ-5D)이 구강건강 문제에 미치는 영향을 규명하여 향후 노인 건강증진 프로그램 및 정책방향을 설정하기 위한 기초자료를 제공하고자 한다. 국민건강영양조사 제7기 원시자료를 이용하여 만 65세 이상 노인대상으로 계통추출법으로 진행 하였다. EQ-5D와 씹기 문제는 운동능력이 없는 그룹 통증불편이 있는 그룹, 불안우울이 있는 그룹에서 씹기 문제 오즈비(OR)가 증가하였다 (p<0.05). EQ-5D와 말하기 문제는 운동능력이 없는 그룹, 통증불편이 있는 그룹, 불안우울이 있는 그룹에서 말하기 문제 오즈비(OR)이 증가하였다 (p<0.05). 본 연구결과 건강관련 삶의 질 (EQ-5D)는 노인의 구강건강과 직결된다고 볼 수 있다. 따라서 노인의 EQ-5D을 증가시키기 위해서는 지역사회에서 구강보건교육 및 계속구강건강관리제도가 요구되어진다.

시간교환법을 이용한 일부 EQ-5D 건강상태의 질 가중치 측정 (General Population Time Trade-off Values for 42 EQ-5D Health States in South Korea)

  • Objectives : This study was conducted to elicit quality weights for 42 EQ-5D health states with the time trade-off (TTO) method from the general population of South Korea. Methods : We selected the same EQ-5D health states as those in the UK MVH study. The Korean version of EQ-5D questionnaire and TTO method were used for the valuation process. We interviewed 500 people as a representative sample of the general population in Seoul and Gyeonggido. The result was compared with those from UK, Japan, and USA by Spearman's rank correlation and t-test. Results : TTO values for 42 EQ-5D health states and 'unconscious' state were obtained from the general South Korean population. The best one was '11112' state and the worst one was 'unconscious' state. The states worse than death were '33323', '33333', and 'unconscious' states, which had negative TTO values. There was a strong correlation between TTO values of the EQ-5D health states and those of their corresponding states from UK, Japan, and USA (Spearman's correlation coefficient: 0.885, 0.882, and 0.944, respectively, p <0.001). However, absolute TTO values of most EQ-5D health states were significantly different from those of their corresponding states in other foreign studies (UK : 41/42, USA : 32/42, Japan : 15/17). Conclusions : We found that the Korean general population TTO values for EQ-5D health states were different from those of other foreign studies, suggesting that a specific Korean valuation set should be developed and used for economic evaluation studies in South Korea.

복합표본분석을 적용한 한국 대사증후군자들의 건강관련 삶의 질(EQ-5D) (Health-Related Quality of Life (EQ-5D) among Korean Adults with Metabolic Syndrome using Complex Samples Analysis)

  • 본 연구의 목적은 대사증후군을 가진 사람의 성별, 연령에 따른 건강관련 삶의 질(EQ-5D)의 차이를 확인하는 것이다. 국민건강영양조사(KNHANES VI-2)에 응답한 5,021명의 자료를 이용하여 대사증후군 판정을 하였으며, IBM SPSS STATISTICS 25.0의 복합표본분석을 적용한 빈도분석, 교차분석과 Complex Samples GLM과 Complex Samples t-test를 사용하였다. 연구결과 EQ-5D 지수에 있어서 남성이 여성보다 다소 높았다(p<.001). 연령별로는 청장년(19~39세), 중년(40~65세), 노년(65세 이상)의 순서로 높게 확인되었다(p<.001). 대사증후군 여부로는 대사증후군에 속하지 않는 경우가 대사증후군을 가진 경우보다 추정치가 높게 나타났으나 통계적으로 유의하지는 않았다. 대사증후군 여부로는 대사증후군이 있는 경우에 그렇지 않은 경우보다 건강관련 삶의 질이 추정치에서 낮음으로 나타났으나 통계적으로 유의하지 않았다(p>.05). 한편, 전체 모집단(population)에서 성인을 대상으로 복합표본을 적용한 독립표본 t 검정(Complex Samples Independent t-test)을 통해 성별 간 대사증후군 유병여부에 따른 EQ-5D는 여성의 경우 대사증후군에 속하지 않은 경우가 대사증후군에 속하는 경우 보다 높은 지수가 확인되었으며 통계적으로 유의한 차이가 확인되었다(p<.01).

EuroQol 도구로 측정한 성인 공황장애 환자의 건강관련 삶의 질 수준 (Assessment of Health-Related Quality of Life among Patients with Panic Disorder Using EuroQol in Korea)

  • Objectives : The purpose of this study was to assess the health-related quality of life (HRQoL) for patients with panic disorder using EQ-5D and EQ-VAS, and to examine the relationship between health-related quality of life and clinical outcomes following treatment. Methods : 29 patients with panic disorder were recruited from the Seoul Metropolitan area and 20 patients were followed up after two months of outpatient treatment. The Panic Disorder Severity Scale (PDSS) was used to assess the severity of the panic disorder and the changes in symptoms. HRQoL was assessed with EQ-5D and EQ-VAS at baseline and at two months of treatment. Results : All enrolled panic disorder patients showed significantly impairment of HRQoL in view of the subscales of EQ-5D, EQ-VAS index scores. The severity of PDSS was correlated the HRQoL in the panic patients. After treatment, the EQ-5D, EQ-VAS index scores showed significant improvement. Conclusion : Panic disorder patients suffer from lower HRQoL as well as from symptoms of the disorder. They showed clinical improvement and a restored HRQoL with treatment. These outcomes suggest using the EQ-5D, EQ-VAS such as the HRQoL in the assessment of patients with panic disorder is essential.
