A Rare Case of Hyponatremia Caused by Reset Osmostat in a Neonate with Cleft Lip, Cleft Palate, and Imperforate Anus

  • Hyponatremia is defined as a plasma sodium concentration of <135 mEq/L. It is a common electrolyte imbalance in newborns. We report the case of a term neonate with cleft lip, cleft palate, imperforate anus, normal male karyotype, and chronic hyponatremia. On the 4th day of life, he showed hyponatremia (plasma sodium concentration 130 mEq/L) with low serum osmolality (275 mOsm/kg), high urine sodium (116.7 mEq/L), and high urine osmolality (412 mOsm/kg). His thyroid and adrenal functions were normal. Despite intravenous and oral sodium supplementation and hydrocortisone treatment, hyponatremia persisted. Brain magnetic resonance imaging showed normal results. He was diagnosed as having reset osmostat, a rare subtype of the syndrome of inappropriate secretion of antidiuretic hormone characterized by a subnormal threshold for antidiuretic hormone secretion, with hypotonic hyponatremia.

Effect of Skater Exercise on Cardiopulmonary Fitness and Balance in Patients with Mild Chronic Stroke. - A Pilot Study -

  • Purpose: This study was conducted to investigate the effects of skater exercise on cardiopulmonary fitness and balance function in patients with mild chronic stroke. Methods: A total of ten chronic stroke patients with mild neurologic deficits were recruited. The participants were divided into two groups, a skater exercise (n=5) and a control group (n=5). Skater exercise was performed by physical therapists in the experimental group, whereas patients in the control group conducted self-exercise. One session of the intervention was carried out for 30 minutes, three times per week for eight weeks. Cardiopulmonary function, falling index, and Euro-Quality of Life-5 Dimension (EQ-5D) were measured before and after the intervention. Results: No side effects were reported during and after skater exercise intervention. The peak aerobic capacity, falling index, and EQ-5D improved significantly in the experimental group compared with the control group after eight weeks of skater exercise (p=0.002, p=0.010, and p=0.006). Significant relationships were identified between the change in EQ-5D and peak aerobic capacity (R2=0.75, p=0.002)/falling index (R2=0.65, p=0.002). Conclusion: Skater exercise was an effective physical therapy to improve cardiopulmonary fitness and balance function in patients with mild chronic stroke.

Milk, Sodium and Potassium Intaken of Breastfed Infants During Lactation (수유기간별 영아의 모유섭취량 및 Na, K의 섭취량)

  • This study was to investigate the intake of human milk, sodium and potassium of breast-fed infants at 7, 15, 30, 60 and 90 days postpartum. Milk samples were obtained from 20 healthy lactating women living in In-Cheon area. Milk intakes of 20 breastfed infants were determined by the test-weighing method and soldium and potassium contents of human milk were analyzed by Atomic Absorptin Spectrophotometer after Wet-Digestion. Infant milk intakes per day tended to increase during lactation. The mean($\pm$SD) intakes at 7, 15, 30, 60 and 90 day were 460(164), 547(202), 626(199), 718(139) and 688(162)g/day, respectively. The sodium contents decreased significantly from 9.7(1.1)mEq/kg at 7 day to 6.3(1.8)mEq/kg at 90 day(p<0.05). The sodium intakes of infants were 100.8(39.6), 103.3(46.7), 107.2(47.9), 115.5(41.5) and 105.2(41.2)mg/day, respectively and average intake was 107.2(43.9)mg/day. The potassium contents were ranged from 10.5(1.3) to 13.3(2.3) mEq/kg and the potassium intakes of infants were 241.8(11.2), 267.8(98.8), 314.1(98.6), 318.0(66.1) and 276.1(62.6)mg/day, respectively and average intake was 288.3(91.8)mg/day. The mean sodium and potassium intakes of infants between boys and girls were not significantly different at 5% level.

Effects of Potassium Ion Concentrations on the Cardiac Performances in the Turtle Heart Amyda japonica (자라 심장 박출량과 $K^{+}$ 농도)

  • Changes in cardiac performances were observed under variations of the arterial and/or venous pressures with K-loading or K-depletion in turtle heart. Hearts were perfused with turtle Ringer-Locke's solutions containing various levels of potassium ion concentration. When venous pressure increased from 4 to 12 cm $H_2O$, cardiac output increased from $6.2{\pm}0.68$ to $15.7{\pm}0.75\;ml/min$, concomittantly. On the contrary, cardiac output decreased during the elevation of arterial pressure. Stroke work increased more prominently during the arterial pressure elevation than during the elevation of venous pressure. During K-depletion$(1{\sim}2mEq/L)$, cardiac output increased to two times that of normal K-concentration$(3{\sim}6\;mEq/L)$ group. Heart rate increased also, but less markedly. In K-loaded$(7{\sim}8\;mEq/L)$ group, both the cardiac output and heart rate decreased but stroke volume rather increased, because heart rate decrement was disproportionate to that of cardiac output. We concluded that in perfused turtle heart, cardiac output variation was more sensitive to K_depletion whereas heart rate to K-loading.

Sensitivity Study of the Flow-through Dynamic Flux Chamber Technique for the Soil NO Emissions

  • A mathematical sensitivity analysis of the flow-through dynamic flux chamber technique, which has been utilized usually for various trace gas flux measurement from soil and water surface, was performed in an effort to provide physical and mathematical understandings of parameters essential for the NO flux calculation. The mass balance equation including chemical reactions was analytically solved for the soil NO flux under the steady state condition. The equilibrium concentration inside the chamber, $C_{eq}$, was found to be determined mainly by the balance between the soil flux and dilution of the gas concentration inside the chamber by introducing the ambient air. Surface deposition NO occurs inside the chamber when the $C_{eq}$ is greater than the ambient NO concentration ($C_{0}$) introducing to the chamber; NO emission from the soil occurs when the $C_{eq}$ is less than the ambient NO concentration. A sensitivity analysis of the significance of the chemical reactions of NO with the reactive species (i.e. $HO_{2},/CH_{3}O_{2},/O_{3}$) on the NO flux from soils was performed. The result of the analysis suggests that the NO flux calculated in the absence of chemical reactions and wall loss could be in error ranges from 40 to $85\%$ to the total flux.

  • We report the activation energies which is calculated by adding a term being neglected usually, and magnetic relaxation effects for the surface barrier. The activation energies U at initial magnetization m (m$_{en}$ and m$_{ex}$) and equilibrium magnetization m$_{eq}$ are nearly similar to those of Burlachkov, but the m dependence of the activation energy U is remarkably different. The relaxation effects, which were determined by the m dependence of the activation energies U, are nonlinear for vortex entry, but linear at the initial stage and nonlinear at m(Int) ${\simeq}$ m$_{eq}$ for vortex exit. During relaxation process, the vortex entry at m = m$_{en}$ is faster than the vortex exit at m = m$_{ex}$ by about factor 90. The vortex exit at m = m$_{eq}$ is faster than one at m = m$_{ex}$ by about factor 1.3

Formation and Chemical Characteristics of Dewfall in 2005 at Busan (2005년 부산지역 이슬의 생성과 화학적 특성)

  • In order to understand chemical characteristics and formation of dewfall in Busan, we analysed monthly distribution of dewfall, and investigated its chemical composition of dewfall. This study used the modified teflon plate $(1m{\times}1m)$ at Jangyongsil science high school from June 2005 to October 2005. In order to estimate qualitatively water soluble components, IC, ICP and UV methods for water soluble ions are also used respectively. Dewfall amount of sampling periods (26 day) collected 1.29 mm. Distribution of water soluble ions in dewfall founded the highest concentration $(81.3{\mu}eq/{\ell}\;for\;NO_3^-,\;146.6{\mu}eq/{\ell}\;for\;SO_4^{2-},\;and\;114.3{\mu}eq/{\ell}\;for\;nss-SO_4^{2-})$ during the June. pH was the lowest by 5.12 June, and October (pH 6.68) by most high and average pH was 5.46. Monthly equivalent ratio of $[SO_4^{2-}]/[NO_3^-]$ showed the highest value (2.94) during the September, the lowest value (1.77) during the July, and the mean value was 3.45.

Parametric study of the convergence of deep tunnels with long term effects: Abacuses

  • The objective of this paper is to present abacuses obtained from a parametric study of deep-lined tunnels using a numerical finite element model. This numerical model was implemented in software GEOMEC91, which is a two-dimensional axisymmetric model that considers the progress of excavation and the placing of the lining through the activation and deactivation of elements. It is adopted a step of excavation constant (1/3 of radius), constant velocity and circular cross section along the tunnel axis. It is used for rock mass a viscoplastic constitutive law with von-Mises criterion of viscoplasticity without hardening whose deformation rate over time is given by the Bingham model. The lining uses a linear elastic constitutive law. In total are 1716 analysis presented in 60 abacuses that show the value of ultimate convergence ($U_{eq}$) due to tunneling speed. In addition, it is shown an example of the use of the abacuses to determine the ultimate convergence ($U_{eq}$) of the tunnel and pressure ($P_{eq}$) on the lining.

Serum and CSF Mineral Profile of Himalayan Yak (Bas grunniens) in their Natural Habitat

  • Mineral profile of serum and cerebrospinal fluid (CSF) of 15 Himalayan Yak (adult female, n=8; adult male n=4 and young male,n=3) was studied in their natural habitat at an altitude of 3300 meters above mean sea level at Sangla in north western Himalayas. The macro and micro minerals estimated in serum and CSF were; Sodium, Potassium, Calcium and Magnesium and Zinc, Copper and Iron respectively. The values recorded among different Yak groups did not significantly differ from each other except serum iron and haemoglobin which were significantly higher (p<0.05 ) in young male Yaks compared to the adult male and female Yaks. An observation of great significance was considerably higher potassium and lower sodium level in Yaks compared to other bovine species of plains. The serum potassium values in some adult female Yaks were recorded as high as 10.4 mEq/l and the values varied between 6.6 to 9.8 mEq/l in young male Yaks. The serum Sodium values and Na:K ratios in Yak serum ranged between 117.5 to 122.6 mEq/l and 13.7 to 15.3 respectively. The possible relationship of high serum Potassium value with hypoxic conditions and hostile mountain environment has been discussed.

  • Before Smartphone releases, playing music form previous cellphone is very limited, so we need mp3 player instead. However, when Smartphone releases, a function of playing multimedia became one of the important things in Samrtphone including all functions of mp3 player. Even though it is easier to use since you can play music and multimedia in one device, it still has problem issues on the sound quality. This thesis will discuss about functions of EQ from Smartphone music players. After that, we will to achieve high fidelity sound through balance adjustment of Right Mark Audio Analyzer program.
