A Study on Small Area Variations of Hospital Services Utilization in Some Acute diseases -Focused on Gastric Diseases and Acute Appendicitis (일부 급성질환의 지역간 입원의료이용 변이에 관한 연구 -위장질환과 충수염질환을 중심으로-)

  • This study aims to provide an important base resource for the effective medical supply system construction by figuring out the characteristics and changes of admission medical use of gastritis in 160 middle treatment areas classified by 2008 patient examination resources, and by figuring out factors affecting on changes. As a result, in terms of admission rate according to sex and age standardization per small area, there were differences as EQ was 15.1 and CV was 0.4. Top 10 variations were mainly in rural areas. It showed significant positive relations between admission and the number of bed, doctor and special medical equipment per 10,000 population. It is recommended that the government set a priority in the management of regional bed supply and the distribution of efficient medical resources.

A Case Report of Patients Diagnosed with ThoracoLumbar Junction Syndrome Treated by Megadose Pharmacopuncture with Korean Medicine Treatment (대용량 약침치료 및 한의학적 치료를 적용한 흉요추 연접부 증후군 증례 보고)

  • The purpose of this research is to find out the effect of Megadose pharmacopuncture with Korean Medicine Treatment on patients who were diagnosed as ThoracoLumbar Junction Syndrome. Megadose pharmacopuncture was proceeded on 4 patients who received treatment at ${\bigcirc}{\bigcirc}$ Korean Medicine Hospital from November, 2016 to Februrary, 2017. In addition, We also kept a record of NRS, ODI, and EQ-5D on the day of admission and discharge to find out the differences between initial and final figures. As a result, Megadose parmacopuncture showed a significant effect on alleviating pain on patients with ThoracoLumbar Junction Syndrome. There was a significant decrease in NRS, ODI scores. Not only did the scores of NRS and ODI decrease significantly,but also the figures of EQ-5D increased significantly as well. To be specific, the mean of NRS and ODI dcreased from 10 to 1.75 and 61.87 to 12.78 while the mean of EQ-5D increased from 0.258 to 0.850. This research suggests Megadose pharmacopuncture with Korean Medicine Treatment could be a remedy for ThoracoLumbar Junction Syndrome.

Reaction Path Modelling on Geochemical Evolution of Groundwater and Formation of Secondary Minerals in Water-Gneiss Reaction System (편마암-물 반응계에서 지하수의 지화학적 진화 및 이차광물 생성에 관한 반응경로 모델링)

  • The reaction path of water-gneiss in 200m borehole at the Soorichi site of Yugu Myeon, Chungnam was simulated by the EQ3NR/EQ6 program. Mineral composition of borehole core and fracture-filling minerals, and chemical composition of groundwater was published by authors. In this study, chemical evolution of groundwater and formation of secondary minerals in water-gneiss system was modelled on the basis of published results. The surface water was used as a starting solution for reaction. Input parameters for modelling such as mineral assemblage and their volume percent, chemical composition of mineral phases, water/rock ratio reactive surface area, dissolution rates of mineral phases were determined by experimental measurement and model fit. EQ6 modelling of the reaction path in water-gneiss system has been carried out by a flow-centered flow through open system which can be considered as a suitable option for fracture flow of groundwater. The modelling results show that reaction time of 133 years is required to reach equilibrium state in water-gneiss system, and evolution of present groundwater will continue to pH 9.45 and higher na ion concentration. The secondary minerals formed from equeous phase are kaolinite, smectite, saponite, muscovite, mesolite, celadonite, microcline and calcite with uincreasing time. Modeling results are comparatively well fitted to pH and chemical composition of borehole groudwater, secondary minerals identified and tritium age of groundwater. The EQ6 modelling results are dependent on reliability of input parameters: water-rock ratio, effective reaction surface area and dissolution rates of mineral phases, which are difficult parameters to be measured.

The Relationship between Yangsaeng and General Health Status : Based on 2008-2010 Korean National Health and Nutrition Examination Survey (2008-2010년 국민건강영양조사 자료를 활용한 한의학 양생법과 전반적인 건강수준과의 관계)

  • Objective : The aim of this study is to investigate the relationship between Yangsaeng and general health status. Methods : In this study, we used data from the combined 2008-2010 Korean National Health and Nutrition Examination Survey(KNHANES). Mental Yangsaeng was measured by stress recognition and depression. Food Yangsaeng was measured by breakfast skipping and mixed grain diet. Life style Yangsaeng was measured by sleeping time and working pattern. Exercise Yangsaeng was measured by walking and exercise. We defined general health status as EQ-5Dindex, EQ-5Dvas, frequency and day of lying in a sickbed in last one month and activity limitation. Results : Mental Yangsaeng, food Yangsaeng, life style Yangsaeng and exercise Yangsaeng have 8 factors. Most of factors significant correlation with EQ-5Dindex, EQ-5Dvas, frequency and day of lying in a sickbed in last one month and activity limitation. Conclusion : Yangsaeng and general health status have positive correlation.

  • In this study, eco efficiency analysis is performed to analyze Neodymium (Nd) containing permanent magnet recycling process. Life cycle assessment (LCA) and life cycle costing (LCC) are used to apply eco efficiency analysis. In the environmental aspects, global warming potential (GWP) of 1kg permanent magnet is 1.25E + 00 kg $CO_2$ eq. and abiotic resource depletion potential (ADP) is 1.10E - 02 Sb eq. This recycling process costs about 2130 KWR. Environmental efficiency of GWP is at 6.43 and ADP is at 5.32 when compared with vigin metal. Economic efficiency is at 6.74. This study confirms that Nd containing permanent magnet recycling process is sustainable system because of environmental and economical improvement.

Efficacy of Korean Medicine Combination Treatments for Recurrent Back Pain after Medical Procedures: A Retrospective Study

  • Background: The purpose of this study was to investigate the clinical efficacy of Korean medicine combination treatments for recurring back pain after medical procedures. Methods: This was a retrospective study performed on 311 patients admitted to Daejoen Jaseng hospital who were diagnosed with lumbar spine herniated intervertebral discs. The patients were divided into 2 groups according to whether or not they had undergone at least one medical procedure on the lumbar spine. They were assessed with Numeric Rating Scale (NRS), Oswestry Disability Index (ODI) and EuroQol-5 Dimensions Index (EQ-5D) before and after treatment. Results: Patients who experienced a medical procedure on the lumbar spine in the prehospital phase (Group A), statistically significantly improved EQ-5D and ODI scores. The NRS scores also decreased however it was not statistically significant. Patients who had not undergone a medical procedure on the lumbar spine in the prehospital phase (Group B) had statistically significantly improved ODI and NRS scores. The average EQ-5D score decreased however, there was no statistically significant difference before and after scores in Group B. There was no statistically significant difference in variation in EQ-5D, ODI, and NRS scores before and after treatment between the groups. Conclusion: The results of this study indicated that even after a bilateral procedure and surgery, when pain in patients with lumbar spine herniated intervertebral disc did not decrease, (as in failed back surgery syndrome), Korean medicine combination treatment of postoperative pain was helpful and there was improvement in daily life.

An Analysis of Long-Term Scenarios for The GHG Emissions Projections Considering Economic Growth and Industrial Structure Change (경제성장과 산업구조 변화에 따른 장기 온실가스 배출량 전망 시나리오 분석)

  • Both economic growth and industrial structure have great influence on energy consumption and GHG emissions. This study analyzed long-term scenarios for GHG emissions projections considering economic growth and industry value added change. In consideration of 3 GDP and 3 industry value added outlook, total 9 scenarios were set; 'Assembly Industry Baseline(AI)', 'Assembly KEIT industry(AK)', 'Assembly Advanced Country industry(AA)', 'KDI Industry Baseline(KI)', 'KDI KEIT industry(KK)', 'KDI Advanced Country industry(KA)', 'OECD Industry Baseline(OI)', 'OECD KEIT industry(OK)', and 'OECD Advanced Country industry(OA)' scenarios. In consideration of the GDP increase rate and industry value added outlook, it is estimated that AI scenario's GHG emissions would be 777 million tons of $CO_2eq$ in 2030. On the other hand, in the case of OA scenario, GHG emissions would be 560.2 million tons of $CO_2eq$ in 2030. Differences between AI scenario's and OA scenario's were 216.8 million tons of $CO_2eq$. It can be identified by that GDP and industry value added change have great influence on GHG emissions. In view of the fact that Korea's amount of GHG emission reduction targets in 2030 were 218.6 million tons of $CO_2eq$ that the result of this research could give us valuable insight.

Estimation of National Greenhouse Gas Emissions in Agricultural Sector from 1990 to 2013 - Focusing on the Crop Cultivation - (1990년부터 2013년까지 농업 분야 국가 온실가스 배출량 평가 - 경종부문 중심으로 -)

  • The major greenhouse gases (GHGs) in agricultural sector are methane ($CH_4$), nitrous oxide ($N_2O$), carbon dioxide ($CO_2$). GHGs emissions are estimated by pertinent source category in a guideline book from Intergovernmental Panel on Climate Change (IPCC) such as methane from rice paddy, nitrous oxide from agricultural soil and crop residue burning. The methods for estimation GHGs emissions in agricultural sector are based on 1996 and 2006 IPCC guideline, 2000 and 2003 Good Practice Guidance. In general, GHG emissions were calculated by multiplying the activity data by emission factor. The total GHGs emission is $10,863Gg\;CO_2-eq$. from crop cultivation in agricultural sector in 2013. The emission is divided by the ratio of greenhouse gases that methane and nitrous oxide are 64% and 34%, respectively. Each gas emission according to the source categories is $7,000Gg\;CO_2-eq$. from rice paddy field, $3,897Gg\;CO_2-eq$. from agricultural soil, and $21Gg\;CO_2-eq$. from field burning, respectively. The GHGs emission in agricultural sector had been gradually decreased from 1990 to 2013 because of the reduction of cultivation. In order to compare with indirect emissions from agricultural soil, each emission was calculated using IPCC default factors (D) and country specific emission factors (CS). Nitrous oxide emission by CS applied in indirect emission, as nitrogen leaching and run off, was lower about 50% than that by D.

Comparison of Greenhouse Gas Emission from Construction Equipment by Tier 2 and Tier 3 Methodologies (건설기계의 Tier 2와 Tier 3 방법론에 의한 온실가스 배출량 비교)

  • Recently, the operation of construction equipments have increased by many construction project. So a respectable amount of greenhouse gas is expected from construction equipments. But the greenhouse gas emissions from construction equipment have been high uncertainty due investigation of a lack of activity data and emission factors in Korea. In this study, annual greenhouse gas emissions from construction equipment are estimated by IPCC's Tier 2 and Tier 3 method. These methods require emission factors, fuel consumption, average kilowatts and operating hours. As the results, the nationwide emission from construction equipments by Tier 2 and Tier 3 are calculated as $21,784kton-CO_2eq/year$ and $22,811kton-CO_2eq/year$ in 2008.

Development of Greenhouse Gas Emission Factors from Sewage Sludge Incinerator (하수슬러지 소각장의 온실가스 배출계수 개발)

  • In this study, the researchers have developed the greenhouse gas emission coefficients targeted at sewage sludge incineration plants that treat sewage sludge by incineration. Among the gases emitted from the sewage sludge incineration plants, the greenhouse gases showed concentrations of 6.84% for $CO_2$, 4.51 ppm for $CH_4$, and 86.34 ppm for $N_2O$; calculated into greenhouse gas emission coefficients, these gave $276.06kg\;CO_2/ton$, $0.0066kg\;CH_4/ton$, and $0.35kg\;N_2O/ton$. As the result of calculating the greenhouse gas emission quantity in sewage sludge incineration plants using the greenhouse gas emission coefficients, the gross greenhouse gas emission was $84.63ton\;CO_2\;eq./day$, and the net emission was $23.90ton\;CO_2\;eq./day$; this was $37.52ton\;CO_2\;eq./day$ less than the net greenhouse gas emission that was calculated using the standard values of IPCC, which was $61.42ton\;CO_2\;eq./day$. This difference is probably because unlike the standard values of IPCC, the greenhouse gas emission coefficients of this study reflected the special properties of subject facilities. Thus, it is thought that emission coefficient research on the facilities that deviated from the standard values of IPCC should continue to achieve the development of national greenhouse gas coefficient that reflects the special properties of Korea.
