Otolaryngologic Management related with Cleft Lip & Palate (구순구개열 환자의 이비인후과적 관리)

  • Team approach for the management of cleft lip & palate patients is very important. Plastic surgeon, oral-maxillofacial surgeon, orthodontist, otolaryngologist, and speech therapist should be included in the team. Main role of the ENT surgeon may be variable and is up to the team characteristics. Main topics of ENT surgeons' interesting fields are evaluation and management of hearing impairment due to SOM, voice disorder, and velopharyngeal incompetency due to submucous cleft palate & still remained VPI after curative palatoplasty. Basic review of anatomy & physiology related with otolaryngologic aspect of velopharyngeal system was done. Diseases related with hyponasality as well as hypernasality were discussed. Diagnostic and therapeutic methods were discussed. Proper management of hearing impairment and speech disorders are important.

Development of Surgical Two-wavelegth Nd:YAG Laser (수술용 2파장 펄스형 Nd:YAG 레이저 개발)

  • The development of a compact two-wavelength Nd:YAG laser for dental and ENT applications is presented. The Nd:YAG resonator generates either 1.06$\mu$m or 1.32$\mu$m. The wavelength selection is made at the control panel. The Nd:YAG laser parameters at 1.06$\mu$m are ; the maximum pulse duration of 150$\mu$s, repetition rates of I-100Hz, and the maximum average power of 25W. At 1.32$\mu$m, the pulse duration is the same where the repetition rates and the maximum average power are I-30Hz and lOW respectively. High voltage power supply consists of a simmer module and two identical high voltage DC converters. In order to make a complete medical laser system, an optical fiber delivery unit, foot pedal and water spray handpiece are also developed. The wavelength selection is reliable since no movement of optical or mechanical components is required. The high voltage power supply is compact, easy to be maintained and applicable for other laser systems due to its modular design.

Enantiomeric Compounds with Antileishimanial Activities from a Sponge, Plakortis sp

  • As part of a program to discover bioactive natural products from marine organisms, two new enantiomers, ent-3,6-Epidioxy-4,6,8,10-tetraethyltetradeca-7,11-dienoic acid and ent-[3,5-Diethyl-5-(2-ethyl-hex-3-enyl)-5H-furan-2-ylidene]-acetic acid methyl ether, were isolated from a sponge Plakortis sp. These compounds showed strong in vitro antiproliferative effects on promastigotes of Leishmania mexicania, flagellate protozoan that causes leishmaniasis. Structures were assumed by interpretation of NMR spectroscopic data and optical rotation. Both compounds exhibited significant antileishmanial activities in vitro with $IC_{50}$ of 1.0-23.0 ${\mu}g/ml$.

  • In this paper an effort is made to develop an innovative spatial arrangement concept pertaining to ship building industry. The spatial scheduling is the problem that concentrates on effective planning of available space and arrangements of blocks and in a priority manner. In order to create an effective spatial scheduling. a database providing the priority has to be available to make the erection sequence. Such a system works hand in hand with erection sequence generator program The erection sequence program works on the conventional network analysis method which uses a typical parent-children idea for the calculation of the ENT(possible earliest network start time) and LNT(possible latest network start time). This program works in a cyclic manner taking turns by calculating the ENT in upward trace and LNT on the return trace thereby generating the entire erection sequence diagram for the requisite problem The generated database serves as an input data for spatial scheduling problem. When the system works it takes into consideration the entire system based on heuristic concepts as mentioned. There system uses the spatial aspects such as the available area of the P. E area and plan area of the corresponding blocks and its priority of erection from the erection sequence generator program develops the spatial scheduling arrangement. In this paper using all these concepts an innovative spatial schedule development system developed.

Prediction of changes in distribution area of Scopura laminate in response to climate changes of the Odaesan National Park of South Korea

  • This study was performed to provide important basic data for the preservation and management of Scopura laminata, a species endemic to Korea, by elucidating the spatial characteristics of its present, potential, and future distribution areas. Currently, this species is found in the Odaesan National Park area of South Korea and has been known to be restricted in its habitat due to its poor mobility, as even fully grown insects do not have wings. Utilizing the MaxEnt model, 20 collection points around Odaesan National Park were assessed to analyze and predict spatial distribution characteristics. The precision of the MaxEnt model was excellent, with an AUC value of 0.833. Variables affecting the potential distribution area of S. laminata by more than 10% included the range of annual temperature, seasonality of precipitation, and precipitation of the driest quarter, in order of greatest to least impact. Compared to the current potential distribution area, no significant difference in the overall habitable area was predicted for the 2050s or 2070s. It was, however, demonstrated that the potential habitable area would be reduced in the 2050s by up to 270.3 km from the current area of 403.9 km; further, no potential habitable area was anticipated by the 2070s according to our predictive model. Taken together, it is anticipated that this endemic species could be significantly affected by climate changes, and hence effective countermeasures are strongly warranted for the preservation of habitats and species management.

The Study of Satisfaction of Voice Therapy in Patients with Voice Disorders (음성장애환자의 음성치료 후 만족도 연구)

  • Background and Objectives : Voice therapy is proven to be effective, but patients are reluctant to receive the therapy when physicians recommend it. This phenomenon may due to patients'lack of trust in or low satisfaction level of the therapy. This study aimed to evaluate patients'satisfaction level after voice therapy and identify factors that could increase the satisfaction. Materials and Methods : First, the fraction of patients who were recommended voice therapy ahead and completed it was studied, and survey was conducted on those who finished ten voice therapy sessions. The patients'1) satisfaction level during the therapy, 2) satisfaction level about physician's explanation about the therapy, 3) willingness to recommend, 4) satisfaction level about the results were assessed, and was correlated with overall satisfaction level. In each category, patients' gender-, age-, and disease type-related differences were analyzed. Results : Patients under 19 years old were most satisfied during the voice therapy; patients above 40 years old showed statistically significantly higher satisfaction level regarding satisfaction with physician explanation about treatments and with treatment results compared to other ages groups. Patients above 40 also showed the highest willingness to recommend. 26.5% of patients either refused to or discontinued voice therapy. 84.3% were satisfied with treatment results. Considering variabilities among factors, patients'satisfaction with physician explanation about treatment and willingness to recommend had positive correlation. Conclusion : Though 26.3% of patients received voice therapy when physicians recommended, patients who completed the therapy were overall satisfied.

Changes in Aporia crataegi's potential habitats in accordance with climate changes in the northeast Asia

  • This study was conducted in an effort to provide important clues pertaining to the conservation and restoration of Aporia crataegi by identifying the spatial distribution characteristics of the current habitats, prospective habitats, and future habitats of A. crataegi in accordance with climate changes. To determine the distribution of A. crataegi, data from a total of 36 collecting points throughout South Korea, North Korea, China, Japan, Mongolia, and Russia are used. The spatial distributions of the data were examined through MaxEnt modeling. The distribution probability rates exceeded 75% at 18 locations among the 36 species occurrence locations, with Gangwon province showing the highest distribution probability in South Korea. The precision of the MaxEnt model was remarkably high, with an AUC value of 0.982. The variables that affect the potential distribution of A. crataegi by more than 10% are the degree of temperature seasonality, the amount of precipitation in the warmest quarter, the annual mean temperature, and the amount of precipitation in the driest month, in that order of importance. It was found that the future potential distribution area of A. crataegi continuously moves northward over time up to 2070s. In addition, the area of the potential distribution showing a habitable probability rate that exceeds 75% in northeast Asia was $28,492km^2$, where the area of potential distribution in the north part of Korean peninsula was $20.404km^2$ in size. Thus, it is anticipated that the most important future habitats of A. crataegi in the northeast Asia will be North and South Hamgyeong provinces and Ryanggang province near Mt. Baekdoosan in the northern area of the Korean peninsula.

Prediction of potential habitats and distribution of the marine invasive sea squirt, Herdmania momus

  • The influx of marine exotic and alien species is disrupting marine ecosystems and aquaculture. Herdmania momus, reported as an invasive species, is distributed all along the coast of Jeju Island and has been confirmed to be distributed and spread to Busan. The potential habitats and distribution of H. momus were estimated using the maximum entropy (MaxEnt) model, quantum geographic information system (QGIS), and Bio-ocean rasters for analysis of climate and environment(Bio-ORACLE), which can predict the distribution and spread based only on species occurrence data using species distribution model (SDM). Temperature and salinity were selected as environmental variables based on previous literature. Additionally, two different representative concentration pathway (RCP) scenarios (RCP 4.5 and RCP 8.5) were set up to estimate future and potential habitats owing to climate change. The prediction of potential habitats and distribution for H. momus using MaxEnt confirmed maximum temperature as the highest contributor(77.1%), and mean salinity, the lowest (0%). And the potential habitats and distribution of H. momus were the highest on Jeju Island, and no potential habitat or distribution was seen in the Yellow Sea. Different RCP scenarios showed that at RCP 4.5, H. momus would be distributed along the coast of Jeju Island in the year 2050 and that the distribution would expand to parts of the Korea Strait by the year 2100. RCP 8.5, the distribution in 2050 is predicted to be similar to that at RCP 4.5; however, by 2100, the distribution is predicted to expand to parts of the Korea Strait and the East Sea. This study can be utilized as basic data to effectively control the ecological injuries by H. momus by predicting its spread and distribution both at present and in the future.

Radiation Treatment and Survival of Maxillary Sinus Carcinoma (상악동암의 방사선치료와 생존율)

  • Irradiated 88 patients of Maxillary Sinus Carcinoma at Yonsei Cancer Center for 10years between 1971 and 1980 were retrospectively analysed. The majority of patients had very advanced disease(87.5% of $T_3\;and\;T_4$) and 17% of cervical lymph node involvement. 80.6% of all patients were epidermoid type. In 44 cases(50%), irradiation alone was performed. 28 cases(32%) of postoperative radiation after incomplete surgery and the remaining 16 cases(18%) of postoperative radiation after radical surgery were done. The majority of patients except 6 cases had irradiation a dose between 60Gy and 80Gy in 30-40 fractions over 6-8 weeks. The actuarial overall 3 and 5 year survival rate were 362% and 26%, respectively. The actuarial 5 year survival rate for 44 cases of radiation alone group was 14.1%, The actuarial 5 year survival rate for 28 cases of incomplete surgery and postoperative radiation group and 16 cases of radical surgery and postoperative radiation group were 312% and 67.4% respectively. In the actuarial 5 year survival rate according to the stage, stage II, III and IV were 79.5%, 20.9% and 0%. In recent, for the improvement of survival rate of advanced Maxillary Sinus Carcinoma at Yonsei Cancer Center, combined multidisplinary or trimodal treatment modality have been applied and in near time the more excellent results expect to be analyse.
