Effect of Hip Flexion and Internal Rotation on the Hip Abductor Muscle Activity During Side-Lying Hip Abduction in Subjects With Gluteus Medius Weakness

  • Background: Many previous studies recommended the side-lying hip abduction (SHA) exercise for targeting the gluteus medius (Gmed) and gluteus maximus (Gmax) muscle activity while the decreasing tensor fasciae latae (TFL) activation. Mischoice of hip position and angle in SHA may increase the risk of lower extremity injuries and undesirable muscle activation. However, information is limited on the effect of composite hip flexion angles and hip rotation on the gluteal muscle activity during SHA. Objects: This study aimed to compare muscle activity (Gmed, TFL, and Gmax) and activity ratios (Gmed/TFL, Gmax/TFL, and Gmed/Gmax) using surface electromyography (EMG) during SHA exercise at three different hip flexion angles either with or without internal rotation (IR) in subjects with Gmed weakness. We hypothesized that applying hip flexion and IR during SHA would increase gluteal muscle activity and decrease TFL activity. Methods: Muscle activity and activity ratios in 20 volunteers with Gmed weakness during 6 different SHA were investigated with surface EMG. One-way repeated-measures analysis of variance was used to determine the statistical significance. Results: Significant differences were found among the six different exercises for Gmed ($F_{2,41}=11.817$, p<.001) and Gmax ($F_{3,52}=5.513$, p=.003) muscle activity, and Gmed/TFL ($F_{3,54}=8.735$, p<.001) and Gmax/TFL ($F_{2,37}=4.019$, p=.028) activity ratios. Conclusion: Applying hip flexion is an effective method for increasing gluteal activity, and it elicits great Gmed/TFL and Gmax/TFL activity ratios during SHA in subjects with Gmed weakness.

  • The purpose of this study was to compare the onset times of muscle activities and the order of muscle firing in erector spinae, multifidus, rectus abdominis and biceps brachii during perturbation between subjects with and without work-related chronic low back pain (LBP). Twenty-nine subjects, 14 with and 15 without LBP, participated in this study. The muscle responses were measured by surface EMG (electromyography) during perturbation in eye opened and eye closed conditions. The EMG onset times of the erector spinae, multifidus, rectus abdominis and biceps brachii were similar between groups in eye closed condition. But the onset times of the erector spinae, multifidus, rectus abdominis were significantly delayed in subjects with LBP in eye opened condition. The results provide an evidence for impaired feed-forward control of the trunk muscles in subjects with LBP. Further studies are needed to identify whether the impaired feed-forward control of the trunk muscles is the contributing factor to LBP.

다양한 스쿼트 자세 수행 시 전신진동자극이 하지 근 활성도에 미치는 영향 (Effect of Muscle Activation Change of Lower Limb According to Whole Body Vibration During Different Squat Exercises)

  • 본 연구에서는 다양한 스쿼트 자세를 수행할 때 전신진동자극이 하지의 근 활성 변화에 미치는 영향에 대하여 평가하고자 한다. 피험자는 20대 여성 10명(나이 $21{\pm}1.3$년, 키 $160{\pm}2.1cm$, 무게 $52{\pm}4.6kg$)으로 구성되었다. 피험자들은 진동판 위에서 하이스쿼트(HS), 로우스쿼트(LS), 외다리 스쿼트(OS), 뒤꿈치 들고 하이 스쿼트(HU&HS)의 4가지 동작을 수행하였다. 근전도는 하지의 근육(대퇴직근, 외측광근, 중간광근, 내측광근)을 측정하였다. 진동자극은 10초 무자극, 10초 진동자극을 제공하였다. 그 결과 모든 운동 자세와 측정 부위에서 진동 자극을 제공하였을 때 근 활성이 유의하게 증가하는 경향을 보였다(HS +24.8%, LS +23.1%, OS +20.1%, HU&HS +17.9%). 또한 OS 자세는 LS자세나 HU&HS자세보다 근 활성이 유의하게 높은 증가를 보였으며, LS자세는 HU&HS보다 유의하게 높아지는 경향을 보였다.

초기 편마비 환자에서 손목 근수축 개시 및 종료의 특성: 중앙주파수 분석 (Characteristics of Initiation and Termination of Muscle Contraction in Early Hemiparetic Wrists: Analysis of Median Frequency)

  • The purposes of this study were to investigate the median frequency (MDF) between initiation and termination of muscle contraction through surface electromyographic (sEMG) analysis and to propose the basis of clinical treatment for movement problems in early hemiparetic upper limbs. Thirteen patients who had stroke with onset less than 3 months prior to the study and seven control subjects participated in the study. The median frequency in initiation and termination of muscle contraction was recorded from wrist flexor and extensor muscles using the sEMG, with 3 second beeper signals, during maximal isometric wrist flexion and extension. Flexion and extension must be done as quickly and forcefully as possible. The results of the study were as follows: 1. The MDF of the onset and offset sections were significantly lower on the paretic than the nonparetic and control sides. 2. The MDF of the offset section significantly decreased on the paretic and nonparetic sides. Consequently, this study showed that the lowering of the MDF was due to the hemiparetic wrist motor impairment and muscle weakness. These results are also related to Fugl-Meyer motor assessment (FMA) scores in hemiparetic upper limbs. This study also suggests that since muscle weakness of early stroke patients affects the functional decrease of upper limbs, further studies must focus on the treatment to improve muscle agility and muscle fiber recruitment efficiency that can induce the functional recovery correlated to motor control.

  • 환자의 빠른 회복과 일상생활의 복귀를 위해 다양한 재활치료가 시행되고 있으며 게임을 활용한 방법이 치료의 효율성을 높이기 위해 점차 사용되고 있는 추세이다. 그러나 기존의 게임을 이용한 방법들은 위치운동에 집중되어 환자의 상태를 파악하고 전문적인 의료목적으로 사용할 수 있는 정보를 얻기 힘들다. 본 논문에서는 이러한 부분을 보완하기 위하여 근전도 및 자이로스코프 센서를 활용하여 외상성 충격으로 인한 환자 또는 노인성 질환을 가진 환자 등을 대상으로 적용될 수 있는 상지재활치료용 기능성 게임을 개발하였다. 게임을 통해 효율적인 운동을 유도하고 환자의 상태를 파악할 수 있는 방법에 대한 연구를 진행하였고 게임으로 인한 운동량 증가와 가능성을 확인할 수 있는 실험을 진행하였다.

총의치의 교합면 형태에 따른 저작 효율 및 기능에 관한 연구 (A STUDY ON MASTICATORY PERFORMANCE AND FUNCTION BY POSTERIOR OCCLUSAL SCHEMES IN COMPLETE DENTURE)

  • This investigation was designed to determine the effectiveness of the posterior occlusal schemes on masticatory activity during mastication in complete denture. Twelve edentulous subjects were selected for this study. All subjects had no past history and no functional abnormality on masticatory system and TMjoint. And, they had residual ridge of favorable morphology, firm mucosa and Class I skeletal jaw relationship, Twelve experimental denture with interchangeable occlusions(0-degree teeth, 30-degree teeth, Levin teeth and S-A teeth) were constructed for this study. The masticatory performance was analyzed by means of standard sieve(10, 16, 20, 30sieve), and the electrical activity from selected muscles(Temporalis and Masseter muscle) was recorded simultaneously with electromyography(Bio-Pak system) as the subject masticated test foods (rice, peanut and gum) with four different occlusal schemes. Mandibular movement was, also, measured with Sirognathography(Bio-Pak system). These recordings were performed in immediately, after 1 week and after 2 weeks of insertion of complete denture. The results were as follows; 1. The average masticatory performance of 0-degree artificial teeth was higher than any other artificial teeth. 2. Masticatory performance in denture wearer was affected preferentially by food and artificial occlusal schemes. 3. During chewing, there was a statistical difference of EMG activity between masseter and temporal muscle(p<0.01). Especially, EMG activity of working masticatory muscle was highly affected by food rather than by artificial occlusal schemes. 4. In denture wearer, the velocity of opening was not affected by food, whereas, the velocity of closing was faster in soft food chewing than in hard food chewing, and the amount of vertical displacement was grater in chewing of soft and large bolus than in chewing of hard and small bolus. However, the amount of lateral displacement showed conversely (p<0.05). 5. It was considered that masticatory performance in denture wearer is not affected by the condition of residual ridge, the history of denture wear, the preference, the adaptation to artificial teeth and the total mesiodistal length of artificial posterior teeth.

흰쥐의 급성 대장염에 대한 주파수에 따른 전침 효과 연구 (Frequency Dependent Electroacupuncture Modulates the Visceromotor Responses to Colonic Distension in TNBS-Induced Colonic Inflammatory Rats)

  • Objectives : The transient inflammation has been demonstrated to alter visceral motor response (VMR) and acute mucosal inflammation may precede the manifestation of visceral hyperalgesia in animal models. The purpose of our study is to compare effects of the different frequencies applied electroacupuncture (EA) on acupoints in acute colitis induced by trinitrobenzenesulphonic acid (TNBS). Methods : In Male Sprague-Dawley rats, weighing 250~400g, a single colorectal administration of TNBS (5mg/kg) was made and electrode for electromyography (EMG) recording were stitched into the external oblique musculature. EA of either ST25 or ST36 were applied and stimulation parameter was modulated as follows: 2, 10, or 100 Hz with intensity of 2 mA and 1 ms pulse duration for 30 min. The balloon was inserted intra-anally and VMR to colorectal distension (CRD) was quantified with an EMG recording system. Results : The VMR increased significantly 3 days after TNBS intra-rectal colonic injection in rats. Both 2 Hz and 10 Hz EA on ST36 suppressed VMR to CRD in the acute colitis model but not 100 Hz. Only 10 Hz EA on ST25 suppressed VMR to CRD in the acute colitis. Conclusions : These data show that 10 Hz EA potently inhibits hypersensitivity of colorectum after TNBS induced colitis.

  • The purpose of this study was to investigate the effects of increased saddle height on the length and activity pattern of vastus lateralis (VL) and biceps femoris (BF) muscle. To compare the effects of increased saddle height, Preferred (self-selected height of subject) and High saddle height (approximately 5% higher saddle height than self-selected) were used. Seven elite cyclists (career: $16.1{\pm}8.5years$) participated in 3 min. sub-maximal pedaling tests under the same cadence (90 RPM) and pedaling power (150 W). Hip and knee joint angles, and the length and activity of VL and BF were compared by measuring 3D motion and electromyography (EMG) data. Results showed that there were significant differences in peak extension timing of the hip joint angle and the range of motion of the hip and knee joint between different saddle heights. Although there were significant differences in muscle length of both muscles with increasing saddle height, the timing and amount of muscle activity differed only at the BF. These findings suggest that the timing and amount of bi-articular muscle activity (i.e. BF) can be altered by changing the saddle height. For practically applying these results, further study is necessary to evaluate the effects of various cadence and the pedaling power with various saddle heights.

흰쥐의 TNBS 유발 대장염에서 전침(電鍼)의 대장(大腸) 흥분성(興奮性)에 미치는 효과(效果) (Effects of Electroacupuncture on the Visceromotor Response to Colonic Distension in TNBS-Induced Colonic Inflammation in Rats)

  • Objectives: Single colorectal instillation of trinitrobenzenesulphonic acid (TNBS) causes a dose-dependent increase of visceral motor response (VMR) and severity of inflammation. In this study we compared the effects of electroacupuncture in the different acupoints in the acute colitis induced by TNBS intracolonic injection in rats. Methods: In Male Sprague-Dawley rats, weighing $250{\sim}400g$, a single colorectal administration of TNBS 5mg/kg and 50% ethanol under isoflurane anaesthesia after an overnight fast. Electrodes for electromyography (EMG) recording were stitched into the external oblique musculature under general anesthesia. Acupoints of LI4, ST25, or ST36 were stimulated by electroacupuncture, respectively. The balloon was inserted intra-anally and visceral motor response (VMR) to colorectal distensioin (CRD) was quantified with an EMG recording system. Results: At an observation of the visceral hyperalgesia in the day-time series, the visceromotor response increased significantly 3 days after TNBS intra-rectalcolonic injection in rats. Electroacupuncture on either ST25 or ST36 suppressed the visceromotor response to colorectal distension, but not LI4, at 3 days after TNBS injection. Pretreatment of naltrexone (10 mg/kg, i.p.), opioids antagonist, inhibited the VMR suppress of 10Hz EA to ST36 but not phentolamine (5 mg/kg, i.p.). Pretreatment of either naltrexone or phentolamine inhibited effects of 10Hz EA to ST25. Conclusions: Data show that EA at either ST25 or ST36 potently inhibits hypersensitivity of colorectum after TNBS induced colitis and is differently mediated through the endogenous opioid system and adrenergic system.

Changes in Muscle Activity of the Serratus Anterior According to Surface Tilt Angle During Push-up Plus Exercise in Subjects With Winged Scapula

  • Background: The serratus anterior is one of the most important muscle for maintaining good scapular alignment in the shoulder joint. The pectoralis major and upper trapezius may also compensate for weak serratus anterior muscles. The push-up plus exercise has been identified as the optimal exercise for maximum activation of the serratus anterior. Objects: The purpose of this study was to examine differences in surface electromyography (EMG) activity of upper trapezius, pectoralis major, and serratus anterior muscles during push-up plus exercises on variously angled surfaces in subjects with winged scapula. Methods: Sixteen subjects with winged scapula (male=5, female=11) volunteered for this study. The subjects performed push-up plus exercise on four different tilt angles, namely $0^{\circ}$, $30^{\circ}$, $60^{\circ}$, and $90^{\circ}$. EMG activities in the serratus anterior, upper trapezius, and pectoralis major muscles during performance of push-up plus exercise were measured in all subjects. Data were processed from repeated measures one-way analysis of variance. Results: There was significant difference in the muscle activity of the serratus anterior on the different surface angles (p<.05). The results of the post-hoc analysis showed significantly greater serratus anterior muscle activity on a surface at a $0^{\circ}$ angle than at others tilt angles (p<.05). There was also significant difference in the ratio of serratus anterior to upper trapezius and serratus anterior to pectoralis major across the four surfaces (p<.05), and post-hoc analysis showed significantly greater values on the $0^{\circ}$ surface than on other tilts (p<.05). Conclusion: This study found that performing push-up plus exercises on a flat surface with $0^{\circ}$ and $30^{\circ}$ tilt angle achieves high activation of the serratus anterior muscle for selective strengthening. It can also take into account the sequential application, which is first performed at a $30^{\circ}$ and at a $0^{\circ}$ tilt angle for and effective but not excessive muscle activation.
