A Study on factors affecting physician's acceptance of Electronic Health Record(EHR) System (의사들의 의료정보 시스템 수용도에 영향을 미치는 요인에 관한 연구)

  • For successful EHR implementation, it is important to understand physicians's acceptance and attitude for EHR. This study aims to provide basic information for the overseas expansion of Korean EHR by studying Saudi Arabia's physicians' acceptance for the Korean EHR exported to Saudi Arabia. Except for physician autonomy and physician-patient relationship, Likert scales of physician involvement, adequate training, ease of use, usefulness, and attitude about EHR usage were over 3.5 points, which are relatively high. The Physicians' experience of Korean EHR may have influenced the EHR acceptance score. Based on the positive research results of this study, we can say that Korean EHR can be competitive in the overseas EHR business.

Design and Development of an EHR Platform Based on Medical Informatics Standards (의료정보 표준에 기반한 EHR 플랫폼의 설계 및 개발)

  • As the ARRA enacted recently in the United States, the interest in EHR systems have been increased in the field of medical industry. The passage of the ARRA presents a program that provides incentives to office-based physicians and hospitals adapting the EHR systems to guarantee interoperability with various medical standards. Thanks to the incentive program, a great number of EHR systems have been developed and lots of office-based physicians and hospitals have adapted the EHR systems certified by CCHIT. Keeping pace with the rapid changes in the market of healthcare, some enterprises try to push in to the United States healthcare market based on the experience acquired by developing EHR systems for hospitals in Korea. However, the developed system must be customized because of the different medical environment between Korea and the United States. In this paper, therefore, we design and develop an integrated EHR platform to guarantee the interoperability between different medical information systems based on medical standard technologies. In the developed platform, an integrated system has been composed by integrating various basic techniques such as data transmission standards and its methods, medical standard terminologies and its usage, and knowledge management for medical decision-making support. Moreover, medical data can be processed electronically by adapting an HL7 interface engine and the terminologies for exchanging medical information and the standardization of medical information. We develop SeniCare, an EHR system for supporting ambulatory care of the office-based physicians, based on the platform, and we verify the usability of the platform by confirming whether SeniCare satisfies the criteria of "meaningful use" issued by CMS or not.

Influence of Extruded Hemp-Rice Flour Addition on the Physical Properties of Wheat Bread

  • Functional foods play an important role in daily diet, human health and the food industry. Hemp was reported to have many advantages for nutritional and medicinal usage. In this study, extruded hemp-rice (EHR) flour, containing 30% hemp, was mixed with the wheat flour to create bread loaves at the concentrations of 5, 10 and 15%. Bread made from 100% wheat flour (with no added EHR flour) was used as a control. The physical parameters, including expansion ratio, specific volume and crust/crumb color were evaluated separately. In addition, changes in hardness of the bread during storage at ambient temperature for 3 days were also studied. The results showed that 10%-EHR bread exhibited the highest hardness value, while 15%-EHR bread presented the lowest. The bread containing EHR flour had lower specific volume and bigger air cells compared to the control. Moreover, the crust and crumb color of EHR-containing bread was significantly darker than those of the control. In this study, the 15%-EHR bread showed higher specific volume, lower hardness and bigger air cell structures.

  • Electronic Health Record (EHR) systems are widely adopted worldwide in hospitals for generating and exchanging records of patient information. Recent developments are moving towards implementing interoperable EHR systems that enable information to be shared seamlessly across healthcare organizations. In this context, this paper explores the factors that cause medical information privacy concerns, identifies how people react to privacy invasion and what their perceptions are towards the acceptance of the EHR system. Interviews were conducted to draw a grounded theory on medical information privacy concerns in the use of EHRs. Medical information privacy concerns are caused by perceived sensitivity of medical information and the weaknesses in security technologies. Trust in medical professionals, medical institutions and technologies plays an important role in determining people's reaction to privacy invasion and their perceptions on the use of EHRs.

  • In medical Institution, Electronic Health Record (EHR) is "must access information" to medical staff considering it as medical information. However, this unnecessary exploration of personal information must be treated confidentially because the information is highly related to other's private concerns. It is necessary that medical workers should be also restricted to their access to EHR depending on their roles and duties. As the result, this article explains that "EHR access control will be executed by differentiating authorized medical staff from non medical-related staff as well as EHR access will be only permitted to authorized medical staff depending on their work status conditions. By using Advanced RBAC model on medical situation, we expect to minimize unnecessary leak of EHR information; especially, emergency medical care is needed, access control is highly required depending on a person in charge of the cases or not, and restricted medical information defined by the patient one-self is only allowed to be accessed.

  • 최근 인공지능과 빅데이터 등 최첨단 기술이 빠른 속도로 의료 정보시스템에 도입됨에 따라 환자정보를 포함한 민감한 개인정보에 대한 사이버 공격이 급증하고 있다. 다양한 개인정보 비식별화에 대한 표준이 제안되었지만, 데이터의 범주에 따른 기법 적용에 대한 연구가 미비하다. 본 논문에서는 EHR 데이터를 위한 심근경색을 대상으로 하는 원격 의료 시스템을 위한 개인정보들에 대한 민감도를 4단계로 분류하고 이에 따른 비식별화 기법에 대해 제안한다. 본 논문에서 제안한 EHR 데이터에 대한 분류 및 비식별화 기법은 다양한 의료 정보 서비스를 위한 프라이버시 보호에 활용될 수 있다.

The Case and Implications of Terminology Mapping for Development of Dankook University Hospital EHR-Based MOA CDM (단국대학교병원 EHR 기반 MOA CDM 구축을 위한 용어 매핑 사례와 시사점)

  • Purposes: The Common Data Model(CDM) is very important for multi-institutional research. There are various domestic and international CDM construction cases to actively utilize it. In order to construct a CDM, different terms from each institution must be mapped to standard terms. Therefore, we intend to derive the importance and major issues of terminology mapping and propose a solution in CDM construction. Methodology/Approach: This study conducted terminology mapping between Electronic Health Record(EHR) and MOA CDM for constructing Medical Record Observation & Assessment for Drug Safety(MOA) CDM at Dankook University Hospital in 2022. In the process of terminology mapping, a CDM standard terminology process and method were developed and terminology mapping was performed by applying this. The constructions of CDM mapping terms proceeded in the order of diagnosis, drug, measurement, and treatment_procedure. Findings: We developed mapping guideline for CDM construction and used this for mapping. A total of 670,993 EHR data from Dankook University Hospital(January 1, 2013 to December 31, 2021) were mapped. In the case of diagnosis terminology, 19,413 were completely mapped. Drug terminology mapped 92.1% of 2,795. Measurement terminology mapped 94.5% of 7,254 cases. Treatment and procedure were mapped to 2,181 cases, which are the number of mapping targets. Practical Implications: This study found the importance of constructing MOA CDM for drug side effect monitoring and developed terminology mapping guideline. Our results would be useful for all future researchers who are conducting terminology mapping when constructing CDM.

  • In this paper we developed a CCM Manager which is a management system to support efficient development, publication and promotion of Clinical Contents Model(CCM)'s outcomes. We derived the functions of manager and user from the analysis of work process from development of CCM to management and publication, designed the database based on CCM architecture, and developed web-based system for each user to easily access and search the contents. CCM Manager supports to share the results between model developers well, distribute and promote the outcomes of model type to general users without manager's manual packaging and publishing it. This paper shows superiority compared with other clinical information model management system in aspects of providing multiple search functions, visual model browsing methods and management functions. The proposed CCM Manager is currently used in process of CCM development, and the analysis of its efficiency with the results will be made in the future.

  • Smart healthcare environments which merge medical and IT technology are getting ready for the third generation centering EHR from current second generation. As a basic technology for the introduction and activation of EHR systems it requires heterogeneous network interworking techniques between various wired and wireless medical devices. Interworking technology for heterogeneous network among various medical devices is needed to introduce EHR system. The heterogeneous network interworking technology is needed for construction of a reliable data system to convert each of unstructured data into structured data. Therefore, in this paper, we identify the domestic and international trends of smart medical field and analyze the characteristics of wired and wireless communication technology that is used in a heterogeneous network. and also suggest requirements needed for interworking technology and provide interworking technology based on them. we expect that proposed method which is designed for smart healthcare environments would provide a basic architecture needed for third smart medical technology generation.

  • HITSP(Healthcare Information Technology Standards Panel)은 헬스 케어 관련 산업의 상호 운용성을 위해 일반적으로 수용되고 유용한 표준들을 선별하여 표준 세트를 제공하는 것을 목적으로 한다. HITSP에서는 평생전자건강진료정보(EHR, Electronic Health Record)의 활성화를 위해 첫 번째 해결해야 할 영역으로 검사실 결과 정보 교류를 정하였다. 이에 본 논문에서는 검사실 결과 정보 교류를 위한 방법으로 HITSP에서 제시하는 HL7 버전 2.x 메시지와 CDA 방법 중 인증(authentication) 처리가 가능하고 영속성(persistence)이 있는 CDA 방법을 선택하였다. 또한 CDA를 작성하고 처리하는 방법을 제시하고, 더 나아가 평생전자건강진료정보(EHR)를 위해 CDA를 적용하여 검사실 결과 정보를 교류하여 보았다. 이에 병원과 EHR 시스템의 상호 운용성이 높아져 진료 과정의 효율성을 높일 수 있었고 환자와 의료진에게 양질의 검사 결과 정보를 제공할 수 있었다.
