Traumatic Posterior Fossa Epidural Hematomas in Children : Experience with 48 Cases and a Review of the Literature

  • Objective : Epidural haematoma (EDH) most commonly occurs in the supratentorial area, particularly in the temporal region, of the brain. Posterior fossa epidural haematoma (PFEDH) is less frequently observed, accounting for only 1.2% to 12.9% of all EDH cases. Because of the non-specific symptoms and the potential for rapid and fatal deterioration in children, an early computed tomography (CT) scanning is necessary for all suspicious cases. The aim of the present study was to share the experience of 48 cases and review the literature concerning PFEDH. Methods : A retrospective analysis was conducted for 48 paediatric cases diagnosed with PFEDH and admitted to Yuying Children's Hospital of Wenzhou Medical University from January 2010 to August 2015. The clinical features and outcomes were analyzed and compared with previous literature. Results : Seventeen patients were surgically treated in this series and 31 patients received non-operative treatment. The outcomes were good in 46 patients, evaluated using the Glasgow outcome score (GOS), while mild disability was observed in one patient, and only one case showed severe disability. There were no cases of mortality in this series. Conclusion : Posterior fossa epidural haematoma is relatively rare compared with supratentorial epidural haematoma. Early and serial CT scans should be performed for all suspicious cases. The criteria for the surgical treatment of paediatric patients with PFEDH were concluded. The overall prognosis was excellent in paediatric patients.

  • 최근 많은 연구에서, 동일한 영상그룹들로부터 추출된 저수준의 특징들을 이용해서 고수준의 정보를 분석한 뒤, 이를 이용해서 영상을 분류하는 방법들을 소개하고 있다. 이러한 연구는 CBIR의 인덱싱에서 저수준의 특징만을 사용할 때 발생하는 의미적인 차이(semantic gap)문제를 해결하여, 검색의 효율을 높일 수 있게 한다. 하지만 이들 연구는 대부분 전경(scenery)영상만을 대상으로 하고 있다. 한편 영상을 객체 단위로 다루는 것은 CBIR의 성능을 크게 향상 시킬 수 있는 요인이 된다. 왜냐하면 대부분의 사용자는 관심있는 객체가 포함된 영상을 검색하기 원하기 때문이다. 본 논문에서는 영상의 객체를 인공객체와 자연객체로 분류하는 방법을 제안한다. 인공객체의 경우 자연객체에 비해 상대적으로 직선형태의 에지가 많이 발견되며 객체를 구성하는 패턴이 규칙적이고 방향성을 가진다. 또한 인공객체는 자연객체에 비해 객체영역의 경계가 직선에 의한 단순한 형태로 나타난다. 이러한 특징들을 EDH(edge Direction Histogram)의 에너지, EDAS(Energy Difference of Adjacent Sector)와 가버 필터를 통해 추출하여 분류에 이용한다. 실험을 통하여 각 특징들을 개별적으로 사용해서 76%에서 84% 사이의 분류 정확성을 얻었으며, 제안한 머징 방법을 이용하여 최종적으로 약 90%의 정확성으로 분류하였다.

Diverse Mechanisms of Relaxin's Action in the Regulation of Smooth Muscles and Extracellular Matrix of Vasculature and Fibrosis (혈관과 섬유증의 평활근 및 세포외기질 조절에 대한 릴랙신의 다양한 작용기전)

  • Relaxin has been demonstrated to have regulatory functions on both the smooth muscle and extracellular matrix (ECM) of blood vessels and fibrotic organs. The diverse mechanisms by which relaxin acts on small resistance arteries and fibrotic organs, including the bladder, are reviewed here. Relaxin induces vasodilation by inhibiting the contractility of vascular smooth muscles and by increasing the passive compliance of vessel walls through the reduction of ECM components, such as collagen. The primary cellular mechanism whereby relaxin induces arterial vasodilation is mediated by the endothelium-dependent production of nitric oxide (NO) through the activation of RXFP1/PI3K, Akt phosphorylation, and eNOS. In addition, relaxin triggers different alternative pathways to enhance the vasodilation of renal and mesenteric arteries. In small renal arteries, relaxin stimulates the activation of the endothelial MMPs and EtB receptors and the production of VEGF and PlGF to inhibit myogenic contractility and collagen deposition, thereby bringing about vasodilation. Conversely, in small mesenteric arteries, relaxin augments bradykinin (BK)-evoked relaxation in a time-dependent manner. Whereas the rapid enhancement of the BK-mediated relaxation is dependent on IKCa channels and subsequent EDH induction, the sustained relaxation due to BK depends on COX activation and PGI2. The anti-fibrotic effects of relaxin are mediated by inhibiting the invasion of inflammatory immune cells, the endothelial-to-mesenchymal transition (EndMT), and the differentiation and activation of myofibroblasts. Relaxin also activates the NOS/NO/cGMP/PKG-1 pathways in myofibroblasts to suppress the TGF-β1-induced activation of ERK1/2 and Smad2/3 signaling and deposition of ECM collagen.

Epidural hematoma treated by aspiration of accompanying cephalhematoma in a newborn infant (신생아에서 동반된 두혈종 흡인으로 치료된 경막외출혈 1례)

  • Epidural hematoma is relatively rare in newborn infants and frequently associated with instrumental deliveries or other complications during labor and delivery. Although surgical evacuation has been the most common therapy, many other procedures have been described. Although many epidural hematomas require surgical evacuation rather than nonsurgical management, conservative or aspiration of hematoma have been attempted. In the case of EDH associated with cephalhematoma, aspiration of cephalhematoma could be attempted because frequent features of these combination were communication between these hematoma. We report a case of successful nonsurgical management of epidural hematoma through the aspiration of accompanying cephalhematoma in a 5-day-old newborn infant.

  • Recently Standards Developments Organizations (SDO$\_S$) in the field of Information and Communication recognize that "More new and more complex standards should be developed in shorter time". To cope with this challenge they try to construct Standards Information Cooperation Network (SICN) or Electronic Document Handling (EDH) systems for efficient standards development process. This paper presents the design and implementation of an Extranet based Web system dedicated to effective on-line standards making environments. The system, which is called SICN (Standards Information Cooperation Network), is a workflow-based network application created with a view to fostering faster standards development with functionalities such as an electronic signature mechanism, electronic voting, comment gathering and dynamic links for ready retrieval of standards information stored in a database. This paper also describes the concept of a VSDO (Virtual Standards Development Organization) that supports all the features needed by the relevant standards making bodies to carry out their activities in dynamic on-line environments.ironments.

  • In an era of very rapid technological change, standardization organizations have been confronted with the problem that they should develop more new and more complex standards as strategic tools for enforcement of competitive power in shorter time according to the reduction of Life Cycle of products and the increase in the number of standards and their complexity. To cope with this challenge, we introduce the functionalities and development directions for implementing standardization information system. Also this paper describes the architecture of the next generation system for standard information that enabling information sharing and exchanging using XML. Finally, we suggest implementation framework of Standardization Vortal Site for the One-stop service of integrated standards information which provide users with standards development roadmap and give brief introduction to SOL(Standards On-Line) site which is operated by ETRI/PEC.

  • Nonsurgical management of four cases of the parasagittal epidural hematoma were experienced. Patients were mildly symptomatic or minimal neurological disturbances on admission. Patients were treated conservatively because of stable neurologic sign. All patients had who diastatic fracture and/or suture have become a complete neurological recovery with satisfactory absorption of EDH over a period of 5 to 12 weeks.

Clinical Features of the Patients with Fracture on the Frontal Bone (전두골 골절손상 환자의 임상 고찰)

  • Objective : The fracture on the frontal bone in head-injured patients may be commonly encountered in the clinical situations. Biomechanical studies demonstrate that the anterior wall of the frontal sinus is intermediate in its ability to resist fracture on direct impact. If the frontal sinus is large and the anterior table is able to disperse the force of the impact over a greater area, the posterior table and intracranial contents usually can be spared. We analyzed the clinical features of the patients who presented with frontal skull fracture due to frontal blows. Patients and Methods : From January, 1992 to December, 1997, 172 patients with frontal skull fracture were selected among 1911 patients with head injury who were admitted to department of neurosurgery. Clinical records and radiological studies of all patients were reviewed and evaluated retrospectively. Results : The neurobehavioral changes was seen in 34 cases(19.8%) and showed statistical significances in case of facial bone fractures, acute subdural hematoma(SDH), and positive frontal lobe releasing sign(p<0.05). The good glasgow outcome score group(GOS, good recovery & moderate disability) at discharge was revealed in 77.3% of total patient population. The poor GOS group(severe disability & vegetative state & death) at discharge was revealed in 22.7%. The poor GOS group at discharge have statistical significances with acute epidural hematoma(EDH), traumatic intraventricular hemorrhage(t-IVH), traumatic intracranial lesion, poor initial glasgow coma scale(GCS) scores & Revised Trauma Score(RTS)(p<0.05). Conclusion : Because of their anatomical relationships and neurobehavioral patterns due to vulnerability of the frontal lobe, the frontal injury should be considered as complicated facial injuries. Therefore, these patients are more likely to have a cosmetic or neuropsychiatric problems.

Analysis of Clinical and Radiological Outcomes in Microsurgical and Endovascular Treatment of Basilar Apex Aneurysms

  • Objective : We aimed to analyze clinical and radiological outcomes retrospectively in patients with basilar apex aneurysms treated by coiling or clipping. Methods : Outcomes of basilar bifurcation aneurysms were assessed retrospectively in 77 consecutive patients (61 women, 16 men), ranging in age from 25 to 79 years (mean, 53.7 years) from 1999 to 2007. Results : Forty-nine patients out of 77 patients (63.6%) presented with subarachnoid hemorrhages of the 49 patients treated with coiling, 27 (55.1 %) showed complete occlusion of the aneurysm sac. Of these, 13 patients (26.5%) developed coil compaction on angiographic or MRI follow-up, with recoiling required in 9 patients (18.4%). Procedural complications of coiling were acute infarction in nine patients and the bleeding of the aneurysms in six patients. The remaining 28 patients underwent microsurgery : twenty-six of these (92.9%) with microsurgery followed up with conventional angiography. Complete occlusion of the aneurysm sac was achieved in 19 patients (73.1%). Operation-related complications of microsurgery were thalamoperforating artery injuries in three patients, retraction venous injury in two, postoperative epidural hemorrhage (EDH) in one, and transient partial or complete occulomotor palsy in 14 patients. Glasgow Outcome Scores (GOS) were 4 or 5 in 21 of 28 (75%) patients treated with microsurgery at discharge, and at 6 month follow-up, 20 of 28 (70.9%) maintained the same GOS. In comparison, GOS of four or 5 was observed in 36 of 49 (73.5%) patients treated with coiling at discharge and at 6 month follow-up, 33 of 49 patients (67.3%) maintained the GOS from discharge. Conclusion : Basilar top aneurysms were still challenging lesions based on our series. Endovascular or microsurgery endowed with its inborn risks and procedural complications for the treatment of basilar apex aneurysms individually. Microsurgery provided better outcome in some specific basilar apex aneurysms. For reaching the most favorable outcome, endovascular modality as well as microsurgery was inevitably considered for each specific basilar apex aneurysm.

ANS responses in Negative Emotions Induced by Audio-visual Film Clips (시청각 동영상에 의해 유발된 부정적 감성에 따른 자율신경계 반응)

  • Negative emotions play an important function as to human's existence. In this research, we employed the audio-visual film clips to induce negative emotions and examined the classified responses in the autonomic nervous system(ANS) due to each negative emotion.30 adults(22.6 years $old{\pm}1.24$, 15 males and 15 females) took part in this experiment. Through the preliminary experiment, 2 minutes film's stimuli were selected as the emotion-induced stimuli. During the period when participants were viewing and listening to the selected movie, EDA and ECG were examined as soon as one stimulus was displayed, participants were tested by completing the psychological appraisals of their experienced emotion due to each emotional stimulus. With regard to the result of analyzing the psychological responses, each negative emotion appropriately and effectively induced its target emotion. While concerning the result of analyzing ANS responses, each negative emotion induced its respective activation in ANS. What is more, compared with other types of negative emotional stimuli, the scaring stimulus induced higher activation of the sympathetic nervour system(SNS) as to the indexes in EDh and ECG. This research made segmentation of ANS responses to each negative emotion, which has its significance.
