Perfusion RRI of the Brain Using Oxygen Inhalation (산소 호흡을 이용한 뇌의 관류 자기공명영상)

  • Purpose : To know the possibility of clinical application of MRI using oxygen inhalation as a perfusion MRI Materials and methods : Two healthy volunteers and three patients of one moyamoya disease, one acute infarction and one meningioma were studied using a 1.5 Tesla MRI unit. Oxygen (15 liters/min) mixed with room air was given using face mask from 8 second to 35 second during the study. Images were acquired 25 times (scan time per study were 1.6 seconds) using susceptibility contrast EPI (echo planar image) sequence. Difference maps were acquired by early (study 12-18), and late (study 19-25) O2 inhalation image groups minus pre-O2 inhalation image group (study 3-9) with a Z-score of 0.7-1.0 using VB31C program of Magneton Vision. The resulting perfusion images were created by superimposition of difference maps on corresponding T1 weighted anatomic images. On moyamoya patient, similar perfusion images were acquired after Gd-DTPA injection, and compared with O2 inhalation perfusion images. Results ; The author can get the perfusion images of the brain by oxygen inhalation with susceptibility contrast EPI sequence at the volunteers, and the patient of moyomoya disease, acute infarction and meningioma. On moyamoya patient, perfusion images with O2 inhalation are similar with perfusion images by Gd-DTPA injection. Conclusion 1 This study has demonstrated that the susceptibility contrast EPI by oxygen inhalation can be used as the clinically useful perfusion MRI technique

호흡기,소화기계 감염환자로부터 전염성 바이러스 분리 및 특성

  • 호흡기계 및 소화기계에 감염된 전염성 바이러스에 대한 역학적 기초자료로 이용하고자 1999년 1월부터 12월까지 부산지역에서 분리된 전염성 바이러스의 특징과 계절적 발생추이, 환자의 성별, 연령별 발생에 대해 조사한 결과는 다음과 같다. 1. 1999년도 바이러스 가검물 2261건에서 분리한 호흡기계감염 바이러스 279건과 소화기계 감염 바이러스 83주를 분리하였으며, 이중 인플루엔자 바이러스 A형이 96주(29.6%), B형이 107주(33.0%)로 대부분을 차지하였다. 2. 1999년의 바이러스 분리의 성별 분포는 총 360명의 환자 중 179명(49.7%)의 남성 및 181명(50.3%)의 여성으로 비슷한 양상을 나타내었다. 이중 호흡기계의 경우 279명의 감염환자 중 남성이 130명(46.6%), 여성이 149명(53.4%)으로 여성의 감염율이 비교적 높았으나, 소화기계의 경우 83명의 감염환자 중 남성이 51명(61.4%), 여성이 32명(38.5%)으로 남성의 감염율이 거의 2배정도 높게 나타났다. 3. 1999년의 연령별 분포는 10세 이하의 어린이가 194명(59.9%)으로 대부분을 차지하였으며, 이중 인플루엔자 바이러스가 99명(30.6%)으로 가장 높은 감염을을 나타내었다 유행성이하선염 바이러스의 감염어린이 중에 $l1{\sim}15$세의 연령층이 15명으로(53.3%)로 가장 높게 나타났다. 4. 1999년 월별 감염율은 호흡기계 감염증 바이러스의 경우 1월부터 4월까지, 그리고 12월에 증가 추세를 보이면서 4월에 가장 높은 감염율을 나타내었다 소화기계 감염증 바이러스의 경우 9, 10, 11월을 제외한 모든 월별에 관찰되었으며, echo와 coxsackie 바이러스는 무균성 수막염 환자에서 하절기에 집중적으로 발생하였다. 동절기에 유행하는 설사 바이러스는 12월에 비교적 높은 양상을 나타내었다. 5. 인플루엔자 바이러스는 MDCK 세포에서, 아데노 바이러스와 유행성 이하선염 바이러스는 HEp-2 세포에서, 파라인플루엔자 바이러스는 Vero 세포에서, 그리고 echo, coxsackie B 바이러스와 장내 바이러스는 HEp-2, Vero, BGM 세포에서 뚜렷한 세포병변 효과를 나타내었다. 6. 분리한 바이러스는 전자현미경으로 관찰한 결과 인플루엔자 바이러스 A 형(HINI, H3N2)은 95nm, B 형은 70nm크기의 구형을 나타내었으며, 바이러스표면의 지질 이중층이 뚜렷하게 관찰되었다. 아데노 바이러스는 외피가 관찰되지 않았으며, nucleocapsid는 symmetry이고 크기는 71nm로서 바이러스 입자 표면에 icosaheral capsomer의 배열이 명확하게 관찰되었고, 파라인플루엔자 바이러스와 유행성 이하선염 바이러스는 외피가 있는 구형의 큰 viron으로180, 170nm 크기이었다. 7. Echo와 coxsackie B group 바이러스는 모두 외피가 없는 isometric 형으로 크기는 $30{\sim}45nm$ 이었고, enteric adeno 바이러스는 84nm 크기로서 외피가 없고, 입자 표면에 capsomer의 배열이 명확하게 관찰되었고, rotavirus는 크기가 70nm이며 외층 capsid 단백질과 내층 capsid 단백질이 두층으로 되어 있는 전형적인 수레바퀴 모양을 나타내었다. 이상의 결과로 보아 호흡기계 및 소화기계에 감염되는 전염성 바이러스는 연중 지속적으로 분리되고 있으며 전염성이 강하여 집단 발생은 일으키는 경우도 많고 최근 들어 유행성 이하선염과 흥역 바이러스의 발생률이 높은 추이를 나타내고 있지만 아직은 특이한 바이러스 치료제가 개발되어 있지 않았으므로 지속적인 대책과 아울러 장기적인 발생 가능에 대한 예방책을 흥보하여야 할 것으로 보이며 계속적인 바이러스성 전염병 유행예측조사 및 역학조사가 적극적으로 이루어져야 할 것으로 사료된다.

Projection of future hydrometeorological change scenarios over Republic of Korea using a dynamical downscaling technique (역학적 상세화 기법을 활용한 우리나라 미래 수문기상변화 시나리오 전망)

  • 지역기후모델 RegCM3 이용하여 역학적 상세화 이중둥지격자체계를 구축하고 관측, ECHO-G/S의 20C3M 및 SRES A2 시나리오를 이용하여 동아시아(60km 분해능)와 한반도(20km 분해능)에 대한 현재 및 미래 (1971-2100, 130년)의 기후변화 시나리오 자료를 생산하여 구축하였다. 현재 1971-2000년 기간 동안 상세화된 기온은 관측에 대해 저온 편의와 여름 강수는 건조 편의가 나타나는 계통오차가 있으나, 상세화된 자료는 한반도의 지형적 특성이 잘 반영되었고 관측의 월별, 계절별 변동성을 유사하게 모의하는 등 재분석 자료를 성공적으로 상세화한 것으로 판단된다. 미래 100년(2001-2100년)에 대해 전반기(2021-2050) 및 후반기(2070-2099)의 시나리오기후변동을 분석한 결과, 상세화된 지역별, 계절별, 연도별 기온 상승의 시 공간적 분포를 잘 보여주며, 기온상승(전반기: 동아시아지역~$1.8^{\circ}C$, 남한~$1.6^{\circ}C$, 후반기: 동아시아지역~$4.7^{\circ}C$, 남한~$4.6^{\circ}C$)에 의한 대기 중 수증기 함유량 증가와 여름 몬순의 강화로 전계절에 대해 강수량(전반기: 동아시아~10.5%, 남한~6.7%, 후반기: 동아시아~20.1%, 남한~31.9%)이 증가할 것으로 전망되었다. 수문기상 변화를 살펴보면, 미래 후반기에 남한은 $4.6^{\circ}C$가 상승하여 적설깊이는 5.3mm(-92.3%)가 감소할 것이고, 강수량의 연변동성을 크나 전체적으로 증가할 것이며, 토양수분, 증발산 또한 강수량 증가와 연관되어 증가할 것으로 전망되었다. 이렇게 ECHO-G/S SRES A2 시나리오를 기반으로 하여 역학적으로 상세화된 시나리오는 통계적으로 상세화된 시나리오 결과와 비교 검증함으로써 다중모델기법에 의해 불확실성을 제시함으로써 수문기상변화 예측을 위한 신뢰성 있는 자료로 활용될 수 있을 것으로 판단된다.

Analysis of Signal Intensity in Choroid Plexuses by Diffusion Weighted Imaging (확산강조영상의 검사기법에 따른 맥락얼기의 신호강도 분석)

  • This report aims at picturing out the clinical usefulness by analyzing the signal intensity in choroid plexuses which produce cerebrospinal fluids by diffusion weighted imaging. At first, subjects were chosen among the patients who showed high in signal intensity by diffusion weighted imaging. The subjects were taken another test by fluid attenuated inversion recovery diffusion weighted echo planer image(FLAIR-DW-EPI) the signals of fluid attenuation. And it was found that there are differences between the signal intensities of the two methods, which showed that the signal intensity in FLAIR-DW-EPI is equal to or low than, that in the Brain. By this, it is felt that it is helpful to diagnose the disease in choroid plexus by testing another more with FLAIR-DW-EPI methods the patients who showed high in signal intensity in choroid plexus by $T2^*$ diffusion weighted echo planer image($T2^*$-DW-EPI).

  • T2 FLAIR sequence of MRI in Acute Ischemic Stroke patients is meaningful to those who have the diagnosis of cerebral hemorrhage and neurological deficiency, including the detection of cerebral infarction around Cerebrospinal fluid. However, because of the long acquisition time and mutative motion artifacts caused by movement, It is difficult to take the MRI image acquisition for non-cooperative patients who need to apply a rapid image acquisition. In this study, we applied the EPI-FLAIR sequence, which is one of the fastest pulse sequences in use, which is a combination of EPI sequence and T2 FLAIR sequence, to patients with acute ischemic cerebral infarction. Based on the qualitative and quantitative evaluation results of the EPI-FLAIR and T2 FLAIR image, we will evaluate the diagnostic usefulness of the EPI-FLAIR sequence.

Robust Audio Watermarking Algorithm with Less Deteriorated Sound (음질 열화를 줄이고 공격에 강인한 오디오 워터마킹 알고리듬)

  • This paper proposes a robust audio watermarking algorithm for copyright protection and improvement of sound quality after embedding a watermark into an original sound. The proposed method computes the FFT (fast Fourier transform) of the original sound signal and divides the spectrum into a subbands. Then, it is necessary to calculate the energy of each subband and sort n subbands in descending order corresponding to its power. After calculating the energy we choose k subbands in sorted order and find p peaks in each selected subbands, and then embed a length m watermark around the p peaks. When the listeners hear the watermarked sound, they do not recognize any distortions. Furthermore, the proposed method is robust as much as Cox's method to MP3 compression, cropping, FFT echo attacks. In addition to this, the experimental results show that the proposed method is generally 10 dB higher than Cox's method in SNR (signal-to-noise ratio) aspect.

Diffusion-Weighted MR Imaging of Intracerebral Hemorrhage

  • Objective: To document the signal characteristics of intracerebral hemorrhage (ICH) at evolving stages on diffusion-weighted images (DWI) by comparison with conventional MR images. Materials and Methods: In our retrospective study, 38 patients with ICH underwent a set of imaging sequences that included DWI, T1-and T2-weighted imaging, and fluid-attenuated inversion recovery (FLAIR). In 33 and 10 patients, respectively, conventional and echo-planar T2* gradient-echo images were also obtained. According to the time interval between symptom onset and initial MRI, five stages were categorized: hyperacute (n=6); acute (n=7); early subacute (n=7); late subacute (n=10); and chronic (n=8). We investigated the signal intensity and apparent diffusion coefficient (ADC) of ICH and compared the signal intensities of hematomas at DWI and on conventional MR images. Results: DWI showed that hematomas were hyperintense at the hyperacute and late subacute stages, and hypointense at the acute, early subacute and chronic stages. Invariably, focal hypointensity was observed within a hyperacute hematoma. At the hyperacute, acute and early subacute stages, hyperintense rims that corresponded with edema surrounding the hematoma were present. The mean ADC ratio was 0.73 at the hyperacute stage, 0.72 at the acute stage, 0.70 at the early subacute stage, 0.72 at the late subacute stage, and 2.56 at the chronic stage. Conclusion: DWI showed that the signal intensity of an ICH may be related to both its ADC value and the magnetic susceptibility effect. In patients with acute stroke, an understanding of the characteristic features of ICH seen at DWI can be helpful in both the characterization of intracranial hemorrhagic lesions and the differentiation of hemorrhage from ischemia.

The Population Growth of Local Cities and the Stage Migration: A case study of Jeollabuk-do (비수도권 중소도시의 인구성장과 단계적 이동: 전라북도를 사례로)

  • This study seeks to clarify the influence of the stage migration which has proceeded through generations, on the population growth of local cities in the case of Jeollabuk-do. For this, setting to baby boomer as parents generation and echo boomer as their children, this study traces each generation's birth, distribution and the process of selective migration using aggregate level data. And the stage migration is analyzed by O-D, divided into three regions in each; Jeollabuk-do rural areas, Jeollabuk-do cities and other provinces. The major findings of this study are as follows. First, it was estimated that the number of the baby boomer births in Jeollabuk-do rural areas was seven times more numerous than in cities. Second, both rural and urban born baby boomers overwhelmingly moved into other provinces, especially Seoul metropolitan area and metropolis in their selective migration processes. However, there was also migration stream from rural areas to cities in Jeollabuk-do and the amount of this stream was approximately equal to the amount of outflux from cities in Jeollabuk-do to other provinces. Third, due to baby boomers' stage migration, from rural to urban, echo boomers were born in cities more than in rural areas. Fourth, urban born echo boomers still have moved into other provinces just like their parents generation in selective migration process. But comparing with baby boomer, the number of echo boomers influx from rural areas to cities has decreased. Consequentially the population of echo boomer in Jeollabuk-do cities also has decreased. Finally, the stage migration has been a basic cause of the social growth of urban population, and also influenced on the natural growth, closely connected with migrants' life course, such as marriage, childbirth and rearing. Therefore, this study concludes that the stage migration through generations is one of the crucial factor to understand the population growth in local cities.

Review of Recent Advancement of Ultra High Field Magnetic Resonance Imaging: from Anatomy to Tractography

  • Purpose: Advances of magnetic resonance imaging (MRI), especially that of the Ultra-High Field (UHF) MRI will be reviewed. Materials and Methods: Diffusion MRI data was obtained from a healthy adult young male of age 30 using a 7.0T research MRI scanner (Magnetom, Siemens) with 40 mT/m maximum gradient field. The specific imaging parameters used for the data acquisition were a single shot DW echo planar imaging. Results: Three areas of the imaging experiments are focused on for the study, namely the anatomy, angiography, and tractography. Conclusion: It is envisioned that, in near future, there will be more 7.0T MRIs for brain research and explosive clinical application research will also be developed, for example in the area of connectomics in neuroscience and clinical neurology and neurosurgery.

Development of 7-Year-Old Korean Child Model for Computational Dosimetry

  • A whole-body voxel model of a 7-year-old male volunteer was developed from 384 axial magnetic resonance images (MRIs). The MRIs were acquired with intervals of 3 mm for the entire body in a body coil. In order to reduce the MRI acquisition time for the child, the repetition and echo times under T1 weighted image were chosen to be 566 ms and 8 ms, respectively. The MRIs were classified according to 30 types of tissues with known electrical parameters. The developed voxel model was adjusted to the physical average of 7-year-old Korean boys. The body weight of the adjusted model, calculated with the mass tissue densities, is within a 6% difference from the 50th percentile weight.
