This is a case report of a 57-year-old patient diagnosed with bradycardia and atrioventricular (AV) block. The chief complaint of the patient was palpitation and dizziness. An electrocardiogram (ECG) showed a second degree AV block with 2:1 AV conduction. The patient was treated with Korean medicine, including herbal medicine, for 35 days. After 2 weeks of treatment, the heart rate increased from 33 to 56 and the follow-up electrocardiogram showed normal findings. The symptoms of palpitation and dizziness were also improved. The effect on heart rate, ECG, and symptoms lasted until the end of the treatment. This case report demonstrated that Korean medicine could be effective for treatment of symptomatic bradycardia with AV block.
Purpose: Pediatric chest pain is considered to be idiopathic or caused by benign diseases. This study was to find out how much upper gastrointestinal (UGI) diseases are major causes of chest pain in pediatric patients. Methods: The records of 75 children (42 boys and 33 girls, aged 3-17 years old) who have presented with mainly chest pain from January 1995 to March 2015 were retrospectively reviewed. Chest X-ray and electrocardiography (ECG) were performed in all aptients. Further cardiologic and gastrointestinal (GI) evaluations were performed in indicated patients. Results: Chest pain was most common in the children of 6 and 9 to 14 years old. Esopha-gogastric diseases were unexpectedly the most common direct causes of the chest pain, the next are idiopathic, cardiac diseases, chest trauma, respiratory disease, and psychosomatic disease. Even though 21 showed abnormal ECG findings and 7 showed abnormalities on echocardiography, cardiac diseases were determined to be the direct causes only in 9. UGI endoscopy was performed in 57 cases, and esophago-gastric diseases which thereafter were thought to be causative diseases were 48 cases. The mean age of the children with esophago-gastric diseases were different with marginal significance from that of the other children with chest pain not related with esophago-gastric diseases. All the 48 children diagnosed with treated with GI medicines based on the diagnosis, and 37 cases (77.1%) subsequently showed clinical improvement. Conclusion: Diagnostic approaches to find out esophageal and gastric diseases in children with chest pain are important as well as cardiac and respiratory investigations.
The clip-type pulsimeter equipped with a Hall sensor has a permanent magnet attached in the "Chwan" position to the center of a radial artery. The clip-type pulsimeter is composed of a hardware system measuring voltage signals. These electrical bio-signals display pulse rate, non-invasive blood pressure, respiratory rate, pulse wave velocity (PWV), and spatial pulse wave velocity (SPWV) simultaneously measured by using the radial artery pulsimeter, the electrocardiograph (ECG), and the photoplethysmograph (PPG). The findings of this research may be useful for developing a oriental-western biomedical signal storage device, that is, the new and fusion patient monitor, for a U-health-care system.

Monitoring for the physiological state of a solider is essential to the realization of individual combat system. Despite all efforts over the last decades, there is no report to point out the optimal location of the wearable biosensors considering both monitoring accuracy and operational robustness. In response, we quantitatively measure body temperature and heartrate from 34 body parts using 2 kinds of biosensor arrays, each of which consists of a thermocouple(TC) sensor and either a photoplethysmography(PPG) sensor or an electrocardiography(ECG) sensor. The optimal location is determined by scoring each body part in terms of signal intensity, convenience in use, placement durability, and activity impedance. The measurement leads to finding the optimal location of wearable biosensor arrays. Thumb and chest are identified as best body parts for TC/PPG sensors and TC/ECG sensors, respectively. The findings will contribute to the successful development of individual combat system.
This study was carried out to evaluate the effects of doxapram after ketamine treatment. Twelve healthy dogs were anesthetized with ketamine(15mg/kg IM) and then twenty minutes after the injection of ketamine six dogs received doxapram(2mg/kg IV)and six dogs received saline(5$m\ell$ IV)as a control group. Recovery time, respiratory rate, heart rate and electrocardiogram findings(ECG)were recorded. Recovery time was significantly decreased(p<0.05)by doxapram. Respiratory rate showed a maximal increase immediately after the administration of doxapram. Thereafter respiratory rate gradually decreased and revealed normal levels 10 minutes after the injection of doxapram. Ketamine increased significantly (p<0.05) heart rate. Heart rate showed slight increase immediately after the administration of doxapram. Thereafter heart rate gradually decreased, and revealed normal levels 20 minutes after the injection of doxapram.
Purpose: The aim of this study was to investigate the effects of manual lymph drainage (MLD) on the activity of sympathetic nervous system, anxiety, pain and pressure pain threshold in subjects with psychological stress. Methods: Twenty-nine subjects with psychological stress were randomly assigned to experimental (MLD) and control (rest) groups. This study was performed as a controlled, randomized study using spectral analysis of electrocardiographic (ECG) activities, the State-Trait Anxiety Inventory (STAI), Visual Analogue Scale (VAS) and Pressure Pain Threshold (PPT). Results: Heart rate variability differed significantly between the experimental and control groups (p<0.05). Anxiety and pain showed positive change in both group but it were not showed statically differences. The pressure pain threshold in the upper trapezius muscle was increased in the experimental group (p<0.05). Conclusion: These findings indicate that the application of MLD was effective in reducing the activity of the sympathetic nervous system, anxiety, pain, and increasing the PPT.
Sixty-four patients with paradoxical ventricular wall motion noticed both in angiocardiography or 2-dimensional echocardiography were assessed by ECG gated blood pool scan (GBPS). Endless cine loop image, phase and amplitude images and paradox image obtained by visual inspection of each cardiac beat or Fourier transformation of acquired raw data were investigated to determine the incremental value of GBPS with these processing methods for identification of paradoxical ventricular wall motion. The results were as follows: 1) Paradoxical wall motions were observed on interventricular septum in 34 cases, left ventricular free wall in 26 and right ventricular wall in 24. Underlying heart diseases were ischemic (23 cases) valvular(9), congenital heart disease (12), cardiomyopathy (5). pericardial effusion(5), post cardiac surgery(3), col pulmonale (2), endocarditis(1) and right ventricular tumor(1). 2) Left ventricular ejection fractions of patients with paradoxical left ventricular wall motion were significantly lower than those with paradoxical septal motion(p<0.005). 3) The sensitivity of each processing methods for detecting paradoxical wall motion was 76.9% by phase analysis, 74.6% by endless cine loop mapping and 68.4% by paradox image manipultion respectively. Paradoxial motions visualized only in phase, paradox or both images were appeared as hypokinesia or akinesia in cine loop image. 4) All events could be identified by at least one of above three processing methods, however only 34 cases (48.4%) showed the paradoxical molies in all of the three images. By these findings, we concluded that simultaneous inspection of all above three processing methods-endless cine loop, phase analysis and paradox image-is necessary for accurate identification and assessment of paradoxical ventricular wall motion when performing GBPS.

Purpose : This study was conducted to estabilish the prevalence, clinical features and relationship between ECG findings and echocardiographic findings of Wolff-Parkinsion-White(WPW) syndrome in asymptomatic preschool children. Methods : An electrocardiographic screening study was performed on 77,824 preschool children in Jeonbuk province from April, 1999 to August, 2001. Patients with WPW syndrome underwent echocardiographic study. Results : Twenty three patients with WPW syndrome were discovered by electrocardiographic screening of preschool children. The prevalence rate was 2.9 per 10,000 preschool children and there was no significant sexual difference. Two patients had a history of symptoms related to tachyarrythmia. According to the ECG classification of Rosenbaum et al., five patients were type A and 18 were type B. Utilizing the criteria of Gallagher et al, right anterior, 12 patients; right anteiror paraseptal, four patients; left anteiror, three patients. Nineteen of 23 patients underwent echocardiographic study. Four of five patients with type A WPW syndrome had abnormal early systolic anterior motion of left ventricular posterior wall. Twelve of 14 patients with type B had abnormal interventricular septal motion characterized by early sytolic posterior motion immediately after inscription of the delta wave. Conclusion : The prevalence rate of preschool children in Jeonbuk province was 2.9/10,000. By the classification according to the electrocardiographic findings, the accessory pathway location was dominant right side than left side. In the echocardiographic study, type A WPW syndrome showed abnormal left ventricular posterior wall motion and type B WPW showed abnormal interventricular septal motion.

Purpose: This study was designed to analyze the contributing factors, as well as the incidence and nature of the cardiac toxicity, in patients presenting with diphenhydramine overdose. Methods: We retrospectively reviewed the medical records of the intoxicated patients who presented to the ED of Korea University Anam Hospital from January 2008 to December 2010. Those patients who visited due to a diphenhydramine overdose were selected and the following features were recorded for analysis: the general characteristics, vital signs, the amount of ingested diphenhydramine, the time interval from ingestion to presentation, the coingested drugs (if any), the toxicities and the ECG findings. Cardiac toxicity, while defined mainly in terms of the temporary ECG changes such as QTc prolongation, right axis deviation, QRS widening, high degree AV block and ischemic changes, also encompassed cardiogenic shock, which is a clinical finding. Results: A total of eighteen patients were enrolled. Of the eighteen patients, eight had ingested diphenhydramine only, while ten had ingested other drugs in addition to diphenhydramine. The most commonly observed toxicity following diphenhydramine overdose included cardiac toxicity (78%). Cardiac toxicity was observed in all the patients who presented to the emergency department 2 hours after ingestion. The patients with QTc prolongation turned out to have ingested significantly larger amounts of diphenhydramine. Conclusion: QTc prolongation and right axis deviation were common findings for the patients with a diphenhydramine overdose. QTc prolongation was more likely to occur with ingesting larger amounts of diphenhydramine. Close monitoring is mandatory for patients who have ingested large amounts of diphenhydramine to prevent such potentially lethal cardiac toxicity.
Purpose: The aim of this research is to examine the abnormal liver function complicated with Kawasaki disease (KD) and to analyze its clinical characteristics and complications. Methods: Ninty eight cases were diagnosed as having KD, among which thirty four cases had abnormal alanine aminotransferase (ALT) (>30 IU/L). These abnormal ALT cases were evaluated in terms of age and sex distribution, major symptoms, complications, laboratory and histopathologic findings. Results: Male to female ratio was 2.4:1 and most patients (91.2%) were under 5 years of age. Cervical lymphadenopathy was observed in 3 cases (8.8%); and recurred cases were two (5.8%). Average fever duration was 8 days; average length of hospitalization was 9 days; and average recovery period was 13 days. Immediately after admission, positive CRP was observed in 31 cases (91.2%), leukocytosis (> $10,000/mm^3$) in 26 cases (76.0%), thrombocytosis (> $450,000/mm^3$) in 7 cases (20.6%), and anemia (<10 gm/dl) in 7 cases (20.6%), respectively. GB hydrops or cholecystitis were noted in 3 cases (8.8%), abnormal ECG finding in 1 case (2.9%), coronary dilatation or aneurysm in 2 cases (5.9%). Liver biopsy was done in four cases and revealed mild infiltration of lymphocytes on the portal area and mild bile duct proliferations. Conclusion: The abnormal liver function was noted in 34.7% of KD patients, and subsided all within one month. But the liver function test should be checked closely in patients of the abnormal liver function test complicated with KD despite of its good prognosis.
