Dynamic Fracture Toughness of Chevron-notch Ceramic Specimens measured in Split Hopkinson Pressure Bar

  • Measuring dynamic fracture toughness of brittle and small ceramic specimen is very difficult in a SHPB (Split Hopkinson Pressure Bar). As a countermeasure to this difficulty, a dynamic fracture toughness measuring method by the Chevron-notch ceramic specimen was proposed. Tested chevron specimens were of Chevron notch angles of 90$^{\circ}$, 100$^{\circ}$ and 110$^{\circ}$. Through finite element analysis, shape parameters of the Chevron-notch specimens according to notch angles were calculated. And the static fracture tough1ess of the Chevron-notch alumina specimen was measured as 3.8MPa√m similar to that of CT specimen with a precrack. Dynamic fracture toughness was 4.5MPa√m slightly higher than the static one. It was shown in this study that the proposed Chevron-notch specimens are valid to measure dynamic fracture toughness of extremely brittle materials such as ceramic.

Solitary Necrotic Nodules of the Liver Mimicking Hepatic Metastasis: Report of Two Cases

  • We present two cases of solitary necrotic nodules of the liver which on radiologic images mimicked hepatic metastasis. Solitary necrotic nodule of the liver is a rare but benign entity which histopathologically consists of an outer fibrotic capsule with inflammatory cells and a central core of amorphous necrotic material. The lesion was seen on contrast-enhanced CT as an ovoid-shaped hypoattenuating nodule; on CT during hepatic arteriography as enhancing nodule; on intraoperative US as a target-appearing hypoechoic nodule; on T2WI as a hyperintensity nodule, and on dynamic MR as a subtle peripheral enhancing nodule. Although the radiologic features are not specific, solitary necrotic nodule of the liver should be included in the differential diagnosis of hepatic metastasis.

NMR-CT에서 원통좌표계를 구현하는 경사자계의 고안 (Design of Cylindrical Magnetic Gradient field for NMR-CT)

  • 핵자기공명 단층촬영장치에서 원통좌표계를 사용할 수 있게 하는 경사자계를 고안하였다. 고안된 경사계는 자기장방향이 축방향을 향하면서 방사방향으로는 단조증가하고 방사방향과 축방향으로의 평균적인 기울기의 비가 경사자계 코일의 중심에서 10배가 넘도록 했다. 방사방향과 축방향으로의 기울기의 비를 솔레 노이드의 길이와 중심에 감는 역방향전류의 감은 횟수와 전류량의 함수로 계산하여 최대치를 이루는 조건을 찾아내고 이에 근거하여 실제 코일을 제작하였다. 제작된 코일을 사용하여 실험한 결과 이론적인 예측과 10% 정도의 오차를 갖는 영상을 이차원영상 방법보다 훨씬 짧은 시간에 얻을 수 있어 훨씬 빨리 변화하는 대상의 영상화를 가능케함을 보였다.

Dynamic Contrast-Enhanced Ultrasound of Gastric Cancer: Correlation with Perfusion CT and Histopathology

  • Objective: To assess the relationship between contrast-enhanced ultrasound (CEUS) parameters and perfusion CT (PCT) parameters of gastric cancers and their correlation with histologic features. Materials and Methods: This prospective study was approved by our Institutional Review Board. We included 43 patients with pathologically-proven gastric cancers undergoing CEUS using SonoVue® (Bracco) and PCT on the same day. Correlation between the CEUS parameters (peak intensity [PI], area under the curve [AUC], rise time [RT] from 10% to 90% of PI, time to peak [TTPUS], and mean transit time [MTTUS]) and PCT parameters (blood flow, blood volume, TTPCT, MTTCT, and permeability surface product) of gastric cancers were analyzed using Spearman's rank correlation test. In cases of surgical resection, the CEUS and PCT parameters were compared according to histologic features using Mann-Whitney test. Results: CEUS studies were of diagnostic quality in 88.4% (38/43) of patients. Among the CEUS parameters of gastric cancers, RT and TTPUS showed significant positive correlations with TTPCT (rho = 0.327 and 0.374, p = 0.045 and 0.021, respectively); PI and AUC were significantly higher in well-differentiated or moderately-differentiated tumors (n = 4) than poorly-differentiated tumors (n = 18) (p = 0.026 and 0.033, respectively), whereas MTTCT showed significant differences according to histologic types (poorly cohesive carcinoma [PCC] vs. non-PCC), T-staging (≤ T2 vs. ≥ T3), N-staging (N0 vs. N-positive), and epidermal growth factor receptor expression (≤ faint vs. ≥ moderate staining) (p values < 0.05). Conclusion: In patients with gastric cancers, CEUS is technically feasible for the quantification of tumor perfusion and may provide correlative and complementary information to that of PCT, which may allow prediction of histologic features.

  • Osteoporosis, one of the age-related disease causes vertebra body fracture due to weakening trabecular bone and makes a substantial effect on load sharing among vertebras. Recently, vertebroplasty is one of the most popular treatment, as augmenting PMMA into vertebra. Biomechanical studies about vertebroplasty have been evaluated by several experiments or analysis under static loading but there has been no study on response under dynamic loading. This study included the FE analysis of patients who treated vertebroplasty under dynamic loading. For this study, 3-D FE model of lumbar spine(L1-L2) was modeled from CT scanning data and compared with experimental results in vitro in order to validate this model. Biomechanical behavior about each of normal person, osteoporotic patient and patient treated vertebroplasty for quantitative evaluations of vertebroplasty was compared and investigated.

List Mode에서 PET/CT Scanner의 직선성 평가 (Linearity Estimation of PET/CT Scanner in List Mode Acquisition)

  • Rb-82를 이용한 PET 검사는 심근 관류의 임상적 평가에 중요한 역할을 한다고 알려져 있다. 그러나 PET/CT의 dead time 때문에 count value와 방사능농도 사이의 직선성이 유지되지 않는다면 데이터를 획득할 때 좌심실 입력 함수가 과대평가되어 심근관류가 과대평가될 수 있다. 본 연구에서는 리스트 모드에서 획득한 PET data에서 방사능농도에 따른 count value의 직선성을 평가하였다. Biograph 40 True Point PET/CT를 이용하여 직경 12 cm, 길이 10.5 cm의 cylindrical phantom에 F-18 333 MBq과 물 800 mL를 채우고 7반감기동안 10min frame/bed로 리스트 모드를 이용하여 획득하였다. Raw data는 OSEM (order: 4, subsets: 8)과 FBP (Gaussian filter FWHM 5 mm) 알고리즘을 이용하여 재구성하였다. Sinogram 정보에서 prompt counts, net true counts, random counts를 측정하였다. 재구성 된 phantom 영상에 ROI를 설정하여 총 계수와 background를 측정하고 background correction을 사용하여 count value를 측정하여 직선성을 평가하였다. 리스트 모드를 이용하여 sinogram에서 측정된 prompt counts는 방사능농도에 비례하여 증가하였다. 낮은 방사능농도에서 net true counts와 random counts는 방사능농도에 따라 증가하였다. 높은 방사능농도에서는 net true counts의 증가율이 점차 감소되었고, 반면에 random counts의 증가율은 증가하였다. 그리고 OSEM과 FBP 알고리즘으로 재구성된 영상에서 측정한 count value의 차이는 없었고 방사능농도에 비례하여 count value가 증가하고 직선성이 유지되었다. Biograph 40 True Point PET/CT scanner는 재구성된 영상에서 낮은 방사능농도뿐만 아니라 높은 방사능농도(~416.25 kBq/mL)에서도 측정 된 count value와 방사능농도 사이의 직선성이 유지되는 것을 보여주었다. 따라서 실험에서 사용한 PET/CT scanner는 Rb-82, N-13, O-15, F-18을 이용한 heart dynamic PET study에서데이터의 정량적 분석에 유용할 것으로 판단된다.

  • Single nucleotide polymorphisms (SNPs) are alterations in DNA base that occur most frequently throughout the human genome. The SNPs of DNA repair genes, hMLH1, hMSH2 and ATM, among 100 Korean people were analyzed using Dynamic Allele specific Hybridization (DASH) techniques. Mutation at the position of exon 38 (GA) and exon 10 (CG) of ATM gene, mutation at the position of exon 8 (AG), and exon 1 (AG) of hMLH1 gene and exon 14 (AG) of hMSH2 gene were investigated. No mutation at the selected position of ATM gene and hMSH1 gene was found. However, while there was no mutation at the position of exon of hMSH2 gene, mutation was found at the promotion region (CT) with the frequency of 24% CC, 36% CT and 62% TT genotyes. This results might be used as baseline data for research on SNP of Korean population.

Leak Sign on Dynamic-Susceptibility-Contrast Magnetic Resonance Imaging in Acute Intracerebral Hemorrhage

  • Purpose: A CT angiography spot sign (CTA-spot) is a significant predictor of the early expansion of an intracerebral hemorrhage (ICH-Ex). Dynamic-susceptibility-contrast magnetic resonance imaging (DSC-MRI) can track the real-time leaking of contrast agents. It may be able to indicate active bleeding, like a CTA-spot. Materials and Methods: From September 2014 to February 2017, we did non-contrast CT, CTA, and DSC-MRI examinations of seven patients with acute ICH. We investigated the time from symptom onset to the first contrast-enhanced imaging. We evaluated the time course of the contrast leak within the ICH at the source image of the DSC-MRI and the volume change of ICH between non-contrast CT and DSC-MRI. We compared the number of slices showing CTA-spots and DSC-MRI leaks. Results: The CTA-spot and DSC-MRI leak-sign were present in four patients, and two patients among those showed ICH-Ex. The time from the symptom onset to CTA or DSC-MRI was shorter for those with a DSC-MRI leak or CTA-spot than for three patients without either (70-130 minutes vs. 135-270 minutes). The leak-sign began earlier, lasted longer, and spread to more slices in the patients with ICH-Ex than in those without ICH-Ex. The number of slices of the DSC-MRI leak and the number of the CTA-spot were well correlated. Conclusion: DSC-MRI can demonstrate the leakage of GBCA within hyperacute ICH, showing the good contrast between hematoma and contrast. The DSC-MRI leakage sign could be related to the hematoma expansion in patients with ICH.

Pharmacokinetic Compartment Modeling을 이용한 나선식 CT에서의 간암-간 대조 곡선의 Simulation (Simulation of lesion-to-liver contrast difference curves in Dynamic Hepatic CT with Pharmacokinetic Compartment Modeling)

  • 조영 증강제를 이용한 나선식 CT는 간 질환을 진단하는데 있어 중요한 역할을 하고있음에도 불구하고 진단의 효율을 최적화하는 프로토콜은 명확하게 알려져 있지 않다. 따라서 나선식 CT에서의 간암-간 대조 곡선을 모의 실험(simulation) 하여 다양한 요소들이 시간-조영 곡선에 어떻게 영향을 미치는지 파악하고, 또한 CT 검사 전에 모의 실험을 하여 이론적으로 최적의 스캔을 할 수 있도록 하기 위하여 약동학(pharmacokinetics)에 기초한 compartment model을 구성하였다. 간암, 간, 대동맥 및 간문맥 등을 각 구획(compartment)으로 설정하여 각 구획에서의 미분방정식을 얻은 후 적분하여 Hounsfield unfit 값을 조영제 주입 후 시간의 함수로 얻었으며 각 구획의 시간-조영 곡선을 출력하였다. 구현한 프로그램에서는 간암의 크기 및 종양 혈관의 분포 등과 같은 간암의 성질, 간경화의 정도에 따른 간 혈관 공급의 양상 및 조영제의 부피, 농도, 주입 속도 등의 조영제 주입 방법, 환자의 몸무게, 키 등의 환자의 신체 계수, 그리고 심박출량 등의 환자의 혈역학적 계수 등을 입력 받아 간암을 비롯한 각 구획의 시간-조영 곡선 및 간-간암 대조 곡선을 출력할 수 있도록 하였다. 모델링을 통해 얻은 조영 증강 곡선은 같은 환경하에서 얻은 24명의 환자 데이터와 비교하여 유사한 결과를 얻을 수 있었으며, 조영 증강제 주입 방법의 변화가 간암-간 대조 곡선에 미치는 영향을 비교할 수 있었다.
