Treatment outcomes of extended-field radiation therapy for thoracic superficial esophageal cancer

  • Purpose: To evaluate the efficacy and safety of extended-field radiation therapy for patients with thoracic superficial esophageal cancer (SEC). Materials and Methods: From May 2007 to October 2016, a total of 24 patients with thoracic SEC (T1a and T1b) who underwent definitive radiotherapy and were analyzed retrospectively. The median total radiotherapy dose was 64 Gy (range, 54 to 66 Gy) in conventional fractionation. All 24 patients received radiotherapy to whole thoracic esophagus and 23 patients received elective nodal irradiation. The supraclavicular lymph nodes, the celiac lymph nodes, and both of those nodal areas were included in 11, 3, and 9 patients, respectively. Results: The median follow-up duration was 28.7 months (range 7.9 to 108.0 months). The 3-year overall survival, local control, and progression-free survival rates were 95.2%, 89.7%, and 78.7%, respectively. There were 5 patients (20.8%) with progression of disease, 2 local failures (8.3%) and 3 (12.5%) regional failures. Three patients also experienced distant metastasis and had died of disease progression. There were no treatment-related toxicities of grade 3 or higher. Conclusion: Definitive extended-field radiotherapy for thoracic SEC showed durable disease control rates in medically inoperable and endoscopically unfit patients. Even extended-field radiotherapy with elective nodal irradiation was safe without grade 3 or 4 toxicities.

방사선치료 후 재발한 골반암에서 토모테라피를 이용한 고식적 재치료 (Palliative Irradiation Using Helical Tomotherapy in Recurrent Pelvic Tumors with Prior Radiotherapy)

  • 목 적: 방사선 치료 후에 방사선조사범위내에서 재발한 골반암 환자들을 대상으로 나선형 토모테라피를 이용한 재 치료를 시행한 후 그 효과 및 안전성에 대하여 후향적 분석을 시도하였다. 대상 및 방법: 총 14명의 환자를 대상으로 하였으며, 남녀 비는 1 : 1이었고, 평균연령은 51세였다. 직장암이 8명, 자궁경부암이 5명, 원발부위를 알 수 없는 경우가 1명이었다. 13명의 환자들이 재발 전에 수술을 받은 적이 있고, 환자들의 골반부위에 조사된 방사선량의 중앙값은 50.4 Gy였다. 재발 당시 종양크기의 중앙값은 3.5 cm (2.0~7.5cm)였으며, 재발이 발견된 후 항암화학요법을 먼저 받은 경우가 9명, 고강도초음파치료를 받은 경우가 1명이었다. 나머지 4명은 재발 후에 다른 치료는 시행하지 않은 상태에서 토모테라피를 이용한 재치료를 받았다. 재치료 당시 종양으로 인한 통증을 호소한 환자는 7명이었으며, 종양 표지자가 증가해있던 경우는 8명이었다. 최초 방사선 치료와 재치료의 간격은 중앙값 19.9개월(5.0~75.4개월)이었다. 토모테라피를 이용한 재치료시 육안적종양체적, 임상적표적체적 및 계획용표적체적에 총4~5주간 20~25회에 걸쳐, 각각 중앙값 50 Gy, 47.8 Gy 그리고 45 Gy의 방사선이 조사되었다. 치료 전 매일 초고압전산화단층촬영(megavoltage computed tomography)을 통하여 X, Y, Z축 및 회전각도에 대한 online correction을 시행하였다. 치료 후 반응평가는 Response Evaluation Criteria in Solid Tumors ver. 1.0을 이용하였으며, 국소종양조절기간은 치료 종료 후 Kaplan-Meyer법으로 계산하였고, 치료 후 합병증의 평가는 Common Terminology Criteria for Adverse Event ver. 3.0을 이용하였다. 결 과: 추적기간의 중앙값은 17.3개월(3.0~38.3개월)이었으며, 영상학적반응율은 64.3%(완전관해, 7.1%; 부분관해, 57.2%)였다. 치료 전 통증이 있었던 7명의 환자 중 6명에서 통증이 완화되어 85.7%의 통증완화율을 보였으며, 종양 표지자가 증가되어 있던 8명 중 5명(62.5%)에서 감소가 일어났다. 국소종양조절기간은 중앙값 25.8개월(95% 신뢰구간, 6.12~45.5개월)이었으며, 치료 후 국소재발이 8명(57.1%), 원격전이가 3명(21.4%) 그리고 국소재발과 원격전이가 동시에 일어난 경우가 1명(7.1%)이었다. 합병증으로는 대부분 grade I 혹은 II의 급성 피부염, 직장염, 방광염 등이 나타났지만, 만성 합병증이나 grade IV 이상의 급성 합병증은 나타나지 않았다. 결 론: 방사선치료 후 방사선조사 범위 내에서 재발한 골반암에서 나선형 토모테라피를 이용한 재치료는 비교적 안전하고 효과적인 고식적 치료법이라고 생각된다. 그러나 치료 효과를 좀 더 높이기 위해서 혹은 적은 환자수, 다양한 질환 그리고 후향적 분석이라는 본 연구의 문제점을 극복하기 위해서는 많은 환자들을 대상으로 한 전향적 임상연구가 필요할 것이다.

Risk perceptions of a population living near a municipal waste incinerator and associated factors with the prevalence of environmental disease

  • Background: This study investigated the risk perceptions, prevalence of environmental diseases (EDs) and associated factors with the prevalence of environmental disease among the population living near an incinerator Methods: Study area were divided into 3 local areas near the incinerator by distance (A, B, C) and control area (D) by distance and geographic isolation. A Questionnaire was conducted with 1,380 in local residents (A, B, C) and 390 in control area (D), gathered information of demographic characteristics, lifestyle, perception of damage by incinerators, experience of EDs (atopic dermatitis, allergic rhinitis, asthma) diagnosed by physician. Analysis of variance, χ2 test, and Kruskal Wallis test was applied to determine the difference by area. Logistic regression analysis was performed to identify factors associated with the prevalence of allergic rhinitis. Results: Residents residing closer to the incinerator had negative perception in most items in questionnaire compared with control. The prevalence of allergic rhinitis was higher as they lived nearby the incinerator (p = 0.008). The associated factors with the prevalence of allergic rhinitis were carpet (odds ratio [OR]: 1.79, p = 0.001), residential area (marginally significant), duration of residence (OR: 1.09, p < 0.001). The perception of environmental pollution around the residential area was inversely associated with the prevalence of allergic rhinitis: perceived as very dissatisfied (OR: 4.21, p = 0.02) compared with very satisfied. Conclusions: As closer to the incinerator, the risk perception tend to negative and prevalence of EDs were increased. Carpet, residential area, duration of residence and perception of environmental air pollution around the residential area were associated with prevalence of allergic rhinitis. These results may be useful for the communication with residents to discuss the environmental problems caused by the incinerator.

Local radiotherapy for palliation in multiple myeloma patients with symptomatic bone lesions

  • Purpose: To evaluate the clinical outcomes of symptomatic bone lesions in patients with multiple myeloma (MM) who received local radiotherapy (LRT). Materials and Methods: Fifty-one patients with 87 symptomatic bone lesions treated via LRT were analyzed. LRT was delivered at a median total dose of 21 Gy (range, 12 to 40 Gy) in a median of 7 fractions (range, 4 to 20 fractions). The clinical outcomes of LRT and the factors affecting treatment response were assessed. Results: After a median follow-up time of 66.7 weeks, symptom relief was achieved for 85 of 87 lesions (97.7%). The median time to symptom relief was 7 days from the start of LRT (range, 1 to 67 days). The duration of in-field failure-free survival ranged from 1.1 to 450.9 weeks (median, 66.7 weeks). The radiation dose or use of previous and concurrent chemotherapy was not significantly associated with in-field failure for LRT (p = 0.354, 0.758, and 0.758, respectively). Conclusion: Symptomatic bone lesions in patients with MM can be successfully treated with LRT. A higher radiation dose or the use of concurrent chemotherapy may not influence the in-field disease control. A relatively low radiation dose could achieve remission of symptoms in patients with MM.

Hypofractionated three-dimensional conformal radiotherapy for medically inoperable early stage non-small-cell lung cancer

  • Purpose: The purpose of this study was to assess the clinical outcomes of hypofractionated radiotherapy (HFRT) with three-dimensional conformal technique for medically inoperable patients with early stage non-small-cell lung cancer (NSCLC) and to evaluate prognostic factors. Materials and Methods: We performed a retrospective review of 26 patients who underwent HFRT for early stage NSCLC between September 2005 and August 2011. Only clinical stage T1-3N0 was included. The median RT dose was 70 Gy (range, 60 to 72 Gy) and the median biologically equivalent dose (BED) was 94.5 Gy (range, 78.0 to 100.8 Gy). In 84.6% of patients, 4 Gy per fraction was used. Neoadjuvant chemotherapy with paclitaxel and cisplatin was given to 2 of 26 patients. Results: The median follow-up time for surviving patients was 21 months (range, 13 to 49 months). The overall response rate was 53.9%, and the initial local control rate was 100%. The median survival duration was 27.8 months. Rates of 2-year overall survival, progression-free survival (PFS), local control (LC), and locoregional-free survival (LRFS) were 54.3%, 61.1%, 74.6%, and 61.9%, respectively. Multivariate analysis showed that BED (>90 vs. ${\leq}90$ Gy) was an independent prognostic factor influencing PFS, LC, and LRFS. Severe toxicities over grade 3 were not observed. Conclusion: Radical HFRT can yield satisfactory disease control with acceptable rates of toxicities in medically inoperable patients with early stage NSCLC. HFRT is a viable alternative for clinics and patients ineligible for stereotactic ablative radiotherapy. BED over 90 Gy and 4 Gy per fraction might be appropriate for HFRT.

I, II기 악성 림프종 환자의 방사선 치료 성적 (Result of Radiation Therapy for Stage I, II Non-Hodgkin's Lymphoma)

  • A retrospective analysis was done for 69 patients with Stage I and II non-Hodgkin's lymphoma who were treated from May 1981 to December 1990, in the Department of Radiadtion Oncology, Korea University Hospital. We used Ann Arbor Staging system and Working Formulation for histological classification. Forty-three patients (43/69, $62.3{\%}$) were Stage I and 26 patients (26/69, $37.7{\%}$) were Stage II, and B symptom was found in $10.1{\%}$ (7/69). Nodal lymphoma was $21.7{\%}$ (15/69); 14 patients with supradiaphragmatic disease and 1 patient with infradiaphragmatic disease. Extranodal lymphoma was $78.3{\%}$ (54/69): $64.8{\%}$ (35/54) for head and neck, $25.9{\%}$ (14/54) for gastrointestinal tract. Histologically, low grade consists of $8.7{\%}$ (6/69), intermediate grade $84.2{\%}$ (56/69), high grade $10.1{\%}$ (7/69), and diffuse large cell type was the most frequent form with 36 patients (36/69, $52.2{\%}$). Eighteen patients ($26.1{\%}$) were treated with radiation therapy alone,20 patients ($29.0{\%}$) with radiation therapy combined with chemotherapy, 15 patients ($21.7{\%}$) with radiation therapy combined with surgery and chemotherapy, Median survival duration was 28 months, and the range of survival time was from 1 month to 134 months. Overall five-year survival rate for Stage I and II disease was $54.2{\%}$, with $64.5{\%}$ for Stage I and $37.1{\%}$ for Stage II. For nodal lymphoma,5-year survival rate was $45.9{\%}$, and $56.5{\%}$ for extranodal lymphoma; $60.6{\%}$ for head and neck, $52.9{\%}$ for GI tract primary disease. Local control rate for all patients was $88.4{\%}$ (61/69), with $80{\%}$ (12/15) for nodal lymphoma and $90.7{\%}$ (49/54) for extranodal lymphoma. The total failure rate was $34.8{\%}$ (24/69). Five of 24 ($20.8{\%}$) patients who were failed developed local failure only, $12.5{\%}$ (3/24) local failure with distant failure, and distant failure only were found in $66.7{\%}$ (16/24). Between nodal lymphoma and extranodal lymphoma, there was no significant survival difference, but extranodal lymphoma showed higher incidence.

Effectiveness of radiotherapy for head and neck skin cancers: a single-institution study

  • Purpose: External beam radiotherapy (EBRT) is a useful option to treat head and neck skin cancer patients who are not indicated for surgery. In this study, we evaluated the treatment outcomes of EBRT in an Asian population. Materials and Methods: The records from 19 head and neck skin cancer patients (10 with squamous cell carcinoma and 9 with basal cell carcinoma) who were treated with definitive or adjuvant EBRT from 2009 to 2017 were retrospectively reviewed. The radiotherapy doses administered ranged from 50 to 66 Gy (median, 55 Gy) with 2.0-2.75 Gy per daily fraction (median, 2.5 Gy). The T stage at presentation was as follows: Tis (1 patient), T1 (11 patients), T2 (6 patients), and T3 (1 patient). None had regional lymph node disease or distant metastasis at presentation. The local failure-free survival (LFFS) rates, toxicity, and cosmetic results were analyzed. Results: The median age was 75.5 years (range, 52.6 to 92.5 years). The median follow-up duration from the completion of radiotherapy was 44.9 months (range, 5.8 to 82.6 months). One local failure occurred in a patient with a 2.1-cm posterior neck squamous cell carcinoma at 32.5 months after radiotherapy (1/19, 5.3%). The 3-year LFFS rate was 91.7%. No patients died from skin cancer during follow-up, and no grade 3 complications occurred. The cosmetic outcomes were excellent for 16 (84.2%) and good for 3 (15.8%) of the 19 patients. Conclusion: EBRT offers good local control and cosmetic outcomes in patients with head and neck skin cancer, with no grade 3 complications.

High-dose chemotherapy and autologous peripheral blood stem cell transplantation in the treatment of children and adolescents with Ewing sarcoma family of tumors

  • Purpose: We performed a pilot study to determine the benefit of high-dose chemotherapy and autologous peripheral blood stem cell transplantation (HDCT/autoPBSCT) for patients with Ewing sarcoma family of tumors. Methods: We retrospectively analyzed the data of patients who received HDCT/autoPBSCT at Korea Cancer Center Hospital. Patients with relapsed, metastatic, or centrally located tumors were eligible for the study. Results: A total of 9 patients (3 male, 6 female), with a median age at HDCT/autoPBSCT of 13.4 years (range, 7.1 to 28.2 years), were included in this study. Patients underwent conventional chemotherapy and local control either by surgery or radiation therapy, and had achieved complete response (CR, n=7), partial response (n=1), or stable disease (n=1) prior to HDCT/autoPBSCT. There was no transplant-related mortality. However, the median duration of overall survival and event-free survival after HDCT/autoPBSCT were 13.3 months (range, 5.3 to 44.5 months) and 6.2 months (range, 2.1 to 44.5 months), respectively. At present, 4 patients are alive and 5 patients who experienced adverse events (2 metastasis, 2 local recur, and 1 progressive disease) survived for a median time of 2.8 months (range, 0.1 to 10.7 months). The 2-year survival after HDCT/autoPBSCT was $44.4%{\pm}16.6%$ and disease status at the time of HDCT/autoPBSCT tended to influence survival ($57.1%{\pm}18.7%$ of cases with CR vs. 0% of cases with non-CR, P=0.07). Conclusion: Disease status at HDCT/autoPBSCT tended to influence survival. Further studies are necessary to define the role of HDCT/autoPBSCT and to identify subgroup of patients who might benefit from this investigational treatment.

연부 조직 육종의 수술 후 방사선 치료 결과 (Postoperative Radiation Therapy in the Soft-tissue Sarcoma)

  • 목적 : 최근들어 연부 조직 육종의 치료방법이 광범위 구획절제에서 사지기능을 보존하는 제한적 수술과 방사선/항암화학요법의 다병용치료로 변환되고 있으며 광범위 수술과 유사한 치료성적을 거두고 있다. 저자들은 수술후 방사선 치료를 시행한 연부 조직 육종 환자를 대상으로 치료 결과 및 실패 양상을 알아보고 관련된 예후 인자를 분석하여 수술 후 방사선치료의 역할을 알아보고자 하였다. 대상 및 방법 : 대상환자는 1983년부터 1994년까지 치료한 60명이었고 모두 추적관찰이 가능했으며 평균 추적기간은 50개월이었다. 원발병소는 상$\cdot$하지가 35례(58$\%$)로 가장 많았고 체간 12례(20$\%$), 두경부 7례(12$\%$)였으며 병리학적 유형에 따른 구분은 악성섬유구종 14례(23$\%$), 지방육종이 10례(17$\%$), 악성신경섬유종 7례(12$\%$) 등 이었다. 전체환자중 6례를 제외하고는 조직학적등급의 분석이 가능했고 grade I, II, III가 각각 27례(45$\%$), 3례(5$\%$), 24례(40$\%$)였다. 수술적 절제는 19례(32$\%$)에서 광범위절제, 36례(50$\%$)에서 변연절제, 5례(8$\%$)에서 국소절제를 시행하였다. 방사선치료선량은 28.8-80Gy였고 25례에서 방사선치료와 함께 항암화학 요법을 병용하였다. 결과 : 최종분석 시 실패 양상은 국소재발이 20례(25$\%$), 원격전이 7례(12$\%$), 국소재발과 원격전이를 동반한 경우가 14례(23$\%$)였다. 원격전이한 환자는 구제치료와 상관 없이 모두 사망하였고 국소재발한 환자중 5명이 구제치료에 성공하여 무병생존하였다. 전체환자의 2년 및 5년 국소제어률은 68.0$\%$와 48.7$\%$로 비교적 저조한 결과를 보였다. 국소제어을에 영향을 미친 예후인자는 조직학적 유형, AJCC 병기, 조직학적 등급, 수술의 범위, 수술절연 침범 유무 및 잔존종양 정도, 림프절 전이 유무(p<0.05)였다. 전체 환자의 5년 생존률과 5년 무병생존률은 각각 60.4$\%$, 36.6$\%$였고 평균 생존기간은 89개월이었다 단변량 분석에 의한 생존률에 영향을 미친 예후인자로는 조직학적 유형, AJCC 병기, 림프절 전이 유무, 조직학적 등급, 수술절연 침범 유무와 잔존 종양 정도 였다. 결론 : 결론적으로 본 연구 결과 연부 조직 육종에서 제한적 수술과 수술 후 방사선치료로 비록 저조한 국소제어률을 보였으나 사지 절단 혹은 광범위 구획절제와 비교시 유사한 생존률을 얻었다.

내과적 문제로 수술이 불가능한 조기 비소세포성 폐암에서의 방사선치료 (Radiotherapy in Medically Inoperable Early Stage Non-small Cell Lung Cancer)

  • 목적 : 조기 비소세포성 폐암의 경우 수술이 최선의 치료법으로 생각 되고있다. 환자가 내과적인 질환으로 수술이 불가능한 경우, 혹은 수술을 거부한 경우 방사선치료가 수술의 대체적 치료로 사용 가능하다. 근치적방사선치료를 시행 받은 환자에서의 치료성적 및 이에 영향을 미치는 요인의 분석을 통하여 향후 치료에 도움을 얻고자 본 연구를 시행하였다. 대상 및 방법 : 서울대학교병원 치료방사선과에서 1987년 6월부터 1997년 6월 사이에 치료를 시행 받은 조직학적으로 진단된 조기 비소세포성 폐암 환자 32명을 대상으로 하였다. 수술이 불가능했던 이유로는 폐질환이 21명으로 가장 많았다. 대상환자의 중간 연령은 68세였으며, 조직학적으로는 편평상피암이 24명으로 가장 많았다. 임상 병기는 T1, T2, T3가 각각 5명, 25명, 2명이었으며, 진단시의 종양의 크기는 3$\~$5 cm가 13명으로 가장 많았다. 방사선치료는 6 MV또는 10 MV 선형가속기를 이용하여, 종양부위에 54.0$\~$68.8 Gy (중앙값; 61.2 Gy)를 조사하였고 12명의 환자에서는 동시분할조사를 시행하였다. 추적관찰기간은 2개월에서 93개월 (중앙값; 23개월)이었고, 생존기간은 치료개시일을 기준으로 산정하였다. 결과 : 전체생존률은 코년, 5년이 각각 44.6$\%$, 24.5$\%$이었으며, 무병생존률은 38.9$\%$, 28.3$\%$, 중앙생존기간은 23개월이었다. 전체환자 32명중 최종 추적관찰 시 25명이 사망하였으며, 이중 7명이 페암이외의 질환으로 사망하였다. 단변량분석 상 종양의 크기는 전체생존률과 무병생존률에 통계적으로 유의한 영향을 주는 요인으로 판정되었고 (p=0.0015, p=0.0022), T 병기는 전체생존률에 의미있는 요인으로 판정되었다(p=0.0395). 다변량분석 상 종양의 크기는 무병생존률에 통계적으로 의미있는 요인으로 판정되었으며(p=0.0317), 전체생존률에 영향을 주는 경향을 보였다 (p=0.0649). 종격동의 방사선조사 여부는 생존률에 영향을 주지 않았다. 결론 : 근치적방사선치료는 조기 비소세포성 폐암 환자로 내과적인 질환으로 수술 불가능한 경우나 환자가 수술을 거부한 경우, 특히 T1 또는 3 cm 이하의 종양에서는 수술적치료를 대치할 수 있는 치료법이다. 그러나 종양의 크기가 5 cm를 넘는 경우에는 방사선치료만으로는 장기생존자가 거의 없었으며, 따라서 이러한 환자의 치료에 있어 과분할조사나 기관지내 추가조사, 방사선감작제의 사용, 입체조형방사선치료, 강도변조방사선치료 등의 이용을 고려해야 할 것이다.
