베트남 라오까이성의 보건의료체계와 건강수준 (Health Care System and the Health Status in Lao Cai, Vietnam)

  • This study was described and analyzed health care system in Lao Cai, Vietnam. We analyze organization and delivery of health care system, health care resources, heath care facilities, heath care finances, and health index in Lao Cai, Vietnam. Lao Cai Province is a mountainous region located on the Chinese border in North-West Vietnam, with numerous ethnic minority groups. Health care organization and delivery system in Lao Cai Province is well formed Province-District-Commune level with Vietnam Government's Socialism. However, health care personnels are concentrated in the major city and is lacking in commune level. Lao Cai province has only two general hospital and is lacking number of beds. Lao Cai province's health care sector is insufficient financial support because the primary goal of the Vietnam government is economic development. Ethnic minority groups in Lao Cai have a dual burden of disease and health. To solve this problem, it is dispatched health care personnel to the commune level taking advantage of the well health care organization and delivery system in Lao Cai. It is also necessary to modernize hospital and improve number of bed. In conclusion, it will be improved the quality of life of residents and be able to achieve fairness among district through the enhancement of the health care system in Lao Cai province.

대만 중.서의 의료이원화 현황에 관한 연구 - 면허제도 및 교육제도 확립을 중심으로 - (Study on Dual Medical System of Traditional Chinese Medicine and Western Medicine in Taiwan)

  • Recently, interest in traditional medicine has increased steadily. Nations having traditional medicine system have been attempting to change it institutionally for the purpose of public application boost in use of traditional medicine. But there are not too many countries which have established the modern system of education and licensing system for traditional medicine with it maintained as a part of a national health care system. The best known examples of nations utilizing traditional medicine are the People's Republic of China, Republic of Korea, Japan, and Taiwan. These countries follow different patterns in the relationship with western medicine according to different social and historical backgrounds. Taiwan has dual medical system as Korean. In this study, we looked through history and the current state of affairs of national health care system in Taiwan, and also found out the licensing system, the educational system, and the curriculum in several universities. thoroughly. Furthermore, we looked into the direction of the policy of Taiwanese health care system which has been becoming an integrated medical system between traditional Chinese medicine and western medicine. With findings based on this study, we deduced implications of a future policy line about the integrated medical system in Korea to minimize conflicts between the concerned parties.

  • This study examined the knowledge and understanding of integrative medicine in Korea, specifically conflicts between western and oriental medicine within Parallel (Dual) health care systems. Qualitative methodology using grounded theory guided semi-structured, in-depth interviews with Western Medical Doctors (W.M.D., n = 6), Oriental Medical Doctors (O.M.D., n = 5) and Traditional Chinese Medicine Practitioners (T.C.M.P., n = 4). Thematic analysis was used to determine broad themes from the interviews. 15 professionals (W.M.D. (40%), O.M.D. (33%), T.C.M.P. (27%), 10 males (67%) and 5 females (33%), mean age 45) were interviewed, recorded, and transcribed. Thematic analysis revealed three key themes: systematic conflicts, integration and future directions. Subthemes of systematic conflicts included: credibility of Oriental Medicine, commercial imperatives, maintaining social standing of O.M.D., professional qualifications and lack structures supporting collaborative practice. Integration subthemes included lack of academic linkage and clarity for appropriate triage, opposing medical paradigms and limited social imperative. Future directions should include: social justification, guarantee of oriental medicine legitimacy, role of government and understanding of scientific evidence. To successfully integrate dual medical systems there is a need to address differences in social-environmental factors and perceptions of scientific understanding, as well as developing strong academic links in clinical practice.

사업장 보건관리사업의 형태별 수행성과 분석 -실적, 수혜도, 영향요인을 중심으로- (Performance of Occupational Health Services by Type of Service)

  • Occupational health services in Korea have been operated as dual types: one is operated by occupational health care manager and the other is health care agency without their own personnel. The performance of occupational health service should be different due to the variety of characteristics of health care manager and workplace, qualification of health care manager. This study is to analyze performance of occupational health care services with a particular consideration of job, based on comparing those two types of health care management to show on the basic data for the settlement of more qualitative. health care management system at workplace. For this study, total 391 places in Seoul and Inchon city area: 154 places (39.4%) managed by designated. health care manager and 237 places (60.6%) by the agency with their commission are selected as research samples. Tools for data collection are questionnares have been investigated during the period of 20 September 1993-20 December 1993. Those data are compared with percentiles, mean, standard deviation due to the characterstics of each variable and analyzed for impacting factors with relation to the using multiple regression analysis using SPSS PC program, especially using t -test method in order to compare each type of health care management. Conclusions observed from the tests and each comparison could be summerized as follows : 1. Occupational health care have been accomplished at workplaces with designated people than with agencies people, and coverage rate of the occupational health care services has differences, due to management types. The reason of these results is due to visit only one or two times monthly by the agencies, while their own health care manager obsess, at the workplaces all the times. 2. The common sickness management is the most accomplished item in health care area of occupational health care services, while the preventive care and control for the workers who have serious health problems are insufficient in workers health care area. 3. An insufficient accomplishment of overall health education has been shown because it is difficult to perform health education due to almost no chance of the direct introduction at workplaces. Therefore a strong support system for making and supplying the media is necessary in order to activate indirect health education by means of media. 4. Because health care managers and the agencies managers where take the workplaces for this study are almost nurses who have been comparatively high work site rounding rate about an environmental management at the workplaces, that non-profession can also do it, the activities about the professional area not enough. Therefore, an appropriate referral system should be established in order to complement professional area. 5. Two factors which have an effect on the coverage rate of occupational health care services are : one is those from the workplaces such as type of services, the number of workers, the number of harzadous factors and safety & health committee, the other from health care organization about whether there is its own manager or not.

  • The purpose of this study is to design and develop the circuit of the dual mode syringe pump. Syringe pump is used in intensive care unit, delivery room, pediatric room, operating theater and other fields of hospital at present. Normally the syringe pump delivers one medicine in one case, but in case of intensive care unit, it is necessary to deliver more than two kinds of medicines at a time. Therefore we have designed dual mode syringe pump. We used RISC type microcontroller, PIC17C44 as master controller, and PIC16C73 as slave cpu using for the low power consumption. The performance of system is evaluated by analysis of the linearity and accuracy which is the most important factors in application. While the proposed system shows a acceptable linearity and accuracy, a further research about reducing the errorr should be done.

의료기관 아웃소싱업체 도급직 직원의 이중몰입과 업무성과 (Dual Commitment and Job Performance of Outsourced Employees Working at Hospitals)

  • Objectives : The objective of this study was to examine preceding variables that affect the dual commitment of outsourced employees working at hospitals and to analyze the influence of these variables on job performance. Methods : Data were collected from 461 outsourced employees, working at 7 general hospitals, which had introduced the outsourcing system, using a structured, self-administered questionnaires. Frequency, validity/reliability, correlation and path analyses were done for data analysis. Results : The results of the path analyses showed that both commitment to the hiring company and commitment to the client company (hospital) had statistically significant positive effects on job performance. Additionally, when the 'single measurement' approach was used, dual commitment had a larger positive effect, compared with the 'parallel approach.' Among the preceding variables, 'satisfaction for the job itself' was found to be the most important variable affecting dual commitment and job performance. Conclusions : In conclusion, to enhance the job performance of outsourced employees, it is important for management to examine and improve the various factors related to job satisfaction. Additonally, for outsourced employees to have organizational commitments to the hiring and client companies simultaneously, management should emphasize a sense of unity and share organizational values.

The Changes of Health Care Utilization and Cause of Outpatients at an Oriental Medicine Hospital

  • Objectives : The aim of this study was to investigate the changes of attitude toward the oriental medicine for 10 years. Methods : A questionnaire was done about general characteristic of patient, the decision-maker of use and the reason of choice or alteration for oriental medicine hospital. Results : The results of this survey are as follows: For 10 years, sex ratio of outpatients is similar, but age distribution is even at all ages. The choice for oriental medicine is more determined by oneself than by others. The recommenders are more variable increasingly. The most common cause of moving from western medicine is to get the more effective treatment. The more the concomitant users are increasing, the more the patients that did not give notice to the western doctor about the dual therapies is increasing, also. The most frequent reason without a notice is "being afraid of western doctor's bias". Conclusions : The appropriate transfer system between oriental and western clinic must be constructed and the proper recognition about dual therapies will be needed among patients and especially dual doctors.

ICNP를 적용한 한국 가족의 현상 및 특성 분석 (Analysis of Phenomena and Characteristics of Family Nursing in Korea by ICNP)

  • The purpose of this study was to analyze phenomena and characteristics of family nursing in Korea based on the study by Yun et al. (1999) by ICNP. The subjects of this study were 115 reports on family nursing care in Korea National Open University. The results of this study were as follows; 1. In the phenomenon of family nursing, 'Inappropriate family coping' comprised of 17.0% and 'Inadequate care for a sick member' comprised of 16.8%. 2. It was found that 'A few close neighbors was 62.5%, 'Lack of communication between married couple' was 60.0%, 'Neglet of general child rearing' was 40.9%, 'Closed communication pattern of the head of a family' was 36.0%, and 'Inadequate distribution of each family member's role' was 27.2%. 3. 'Lack of family interaction in community' and 'Social isolation' were combined with 'Lack of family interaction in community', and 'Disturbance in family communication' and 'Lack of family intimacy' were combined with 'Disturbance in family communication'. 4. 'Dual-loading with office work and housework', 'Overload of caring one's elders because of the status as a dual-income family' and 'Lack of family support system' were added into 'Inappropriate family coping'.

Occupational health care have been accomplished at workplaces with designated people than with agencies people, and coverage rate of the occupational health care services has differences, due to management types. The reason of these results is due to visit only one or two times monthly by the agencies, while their own health care manager obsess, at the workplaces all the times. 2. Most of the expense for environmental control of all health care services expenditures shows that there is almost no fundamental improvement because more expenses are needed for procuring personal protective equipment and measuring work environment instead of environmental improvement. 3. It is investigated how much the cost of occupational health care services needs per worker, and calculated how much the cost needs per service hour per worker. The results from this show that the cost of occupational health services at workplaces with their own managers used less than the cost of health care agencies, eventually the former gives better services with less cost than the latter. 4. Benefit/Cost ratio is also produced by total benefit/total cost. The result from the above way reads 4.57 as a whole, while their own manager having workplaces reads 4.82 and the agencies do l.56. Even if their own manager performing workplaces spent more cost, this system produces more benefit than the agencies management. 5. The B/C ratio for medical organization such as local clinic, health care center and pharmacy shows more than or equal to at the workplaces controlled by the agencies. It is inferred that benefit would be much less than the cost used, with so being inefficient. 6. It is assumed that the efficiency ratio of health education is equal to reduction rate of workers medical organization visit. Estimated reduction rate 5%, 10%, 15%, show that the efficiency ratio of health education have an effect on producing benefits. It is estimated that more benefit can be produced if more qualitative education will be provided for enhancing health care efficiency. 7. Results of this study cannot be generalized because there are large scale of deviation in case of workplaces with less than 300 full time workers, but B/C ratio reads 2.69 as a whole and 3.25 at workplaces with their own health care manager are higher than 1.63 at the workplaces manged by the agencies. Finally, all the benefit concerning health care services could not be quantified, measured and shown on the value of money. This is a reason that a considerable part of benefits are so underestimated. This is also thought that measurement tools should be developed for measuring benefits of health care services with a comprehensive quantification. in the future. It is also expected that efficiency of occupational health care services should be investigated using cost-effectiveness analysis.

  • Essential oils aims to rejuvenate and beautify medical skin care the body through the use of essential oils, and generally to improve our outward appearance. Much of this section fouses on the different methods by which essential oils can be used to improve the complexion or to treat specific medical care skin conditions. Beauty, however, is not just skin-deep. The condition of the medical care express the overall health of an individual. Essential oils effect the mature/sun damaged skin for some red or couperose areas, loose saggy skin, exposure to a lot of ultraviolet light. Essential oils effect the medical care nomal skin good elasticity, healthy color(good circulation), smooth terture.
