Objective : Early descent of the diaphragm sellae (DS) during endoscopic endonasal transsphenoidal surgery (EETS) for pituitary macroadenoma surgery is occasionally a troublesome event by blocking the surgical field. Here we introduce an alternative technique with the new pituitary retractor and present our clinical experiences. Methods : We designed a simple and rigid pituitary retractor with the least space occupation in the nasal cavity to be compatible in EETS. The pituitary retractor was held by external holder system to support the herniated DS stably. We retrospectively reviewed a clinical 22 cases of pituitary macroadenomas underwent EETS using the pituitary retractor. Results : The pituitary retractor stably pushed up the herniated DS in all cases, and the surgeon proceeded the procedure with bimanual maneuver. The pituitary retractor was helpful to remove tumors around the medial cavernous sinus and behind the DS in 16 and seven cases, respectively. In four cases, the meticulous hemostasis was completed with the direct visualization by the DS elevation with this retractor. Gross total tumor resection was performed in 20/22 patients (91%). The impaired visual function and hypopituitarism were improved in 18/20 (90%) and 7/14 (50%) patients after surgery, respectively. There was no complication related with the pituitary retractor. Conclusion : During EETS for pituitary macroadenomas, the novel pituitary retractor reported in this study is a very useful technique when the herniated DS block the surgical field and bimanual maneuver. This pituitary retractor can help to result in the excellent surgical outcomes with minimal morbidity.

Purpose : Several studies have investigated the effects of dynamic stretching (DS) and self-mobilization (SM), however, studies comparing the two interventions are rare. Therefore, the purpose of this study was to compare the effects of DS and SM on ankle strength, dorsiflexion range of motion (DFROM), and balance to determine which is superior. Methods : Thirty-two healthy young adults participated in this study. Participants were randomly assigned to two groups (SM and DS). DS was performed for the purpose of stretching the medial gastrocnemius muscle. For the SM group, ankle joint SM was performed in three ways. For all participants, the following measurements were performed as pre- and post-tests: isometric strength of dorsiflexor and plantar flexor, weight-bearing lunge test (WBLT) to evaluate DFROM, Tetrax system to evaluate static balance, and y balance test (YBT) to evaluate dynamic balance. Differences before and after the intervention within each group were compared using paired t-test. Also, the variable's variation was compared between groups using an independent t-test. Results : Significant differences were found in ankle dorsiflexor strength, WBLT, YBT, weight distribution index (WDI) (pillow and opened eyes; PO), and stability index (ST) (normal and closed eyes; NC) before and after intervention in the SM group (p<.05). In the DS group, significant differences were found in ankle dorsiflexor and plantar flexor strength, WBLT, YBT anterior, WDI (normal and opened eyes; NO, PO), and ST (NO, NC, PO, pillow and closed eyes) before and after the intervention (p<.05). Ankle plantar flexor strength and WDI (PO) were significantly different between groups. Conclusion : Based on the results of this study, DS or SM can be considered as a possibility for selective use according to variables for improving ankle joint function (DFROM, muscle strength, balance).

BACKGROUND: We analyzed whether C-11 acetate positron emission tomography (PET) can be used for the evaluation of non-infarct-related artery (NIRA) in patients with ST-elevation myocardial infarction (STEMI) and multivessel disease. METHODS: We prospectively enrolled 31 patients with STEMI and at least one NIRA stenosis (diameter stenosis [DS] ≥ 50%). C-11 acetate PET was performed after successful revascularization for the infarct-related artery (IRA). Myocardial blood flow (MBF) and oxidative metabolism (kmono) were measured and compared between NIRA vs. IRA, stenotic (DS ≥ 50%) vs. non-stenotic (DS < 50%) NIRAs, and NIRAs with significant stenosis (DS ≥ 70% or fractional flow reserve [FFR] ≤ 0.80) vs. those without (neither DS ≥ 70% nor FFR ≤ 0.80). The correlations between PET and angiographic parameters were also analyzed. RESULTS: MBF and kmono were significantly higher in NIRAs than those in IRAs. Stenotic NIRAs showed significantly reduced stress MBF, myocardial flow reserve (MFR), relative flow reserve (RFR) (0.72 ± 0.12 vs. 0.82 ± 0.14; p = 0.001), and stress kmono, as compared to those in non-stenotic NIRAs. NIRAs with significant stenosis had significantly lower stress MBF, MFR, and RFR (0.70 ± 0.10 vs. 0.80 ± 0.14; p = 0.001). RFR showed the best, but modest linear correlation with DS of NIRA stenosis (r = -0.429, p = 0.001). RFR > 0.81 could effectively exclude the presence of significant NIRA stenosis. CONCLUSIONS: C-11 acetate PET could be a feasible alternative noninvasive modality in patients with STEMI and multivessel disease, by excluding the presence of significant NIRA stenosis.

In this paper, we suggest an architecture of DS/CDMA tranceiver composed of one pilot channel used as reference and multiple traffic channels. The pilot channel-an unmodulated PN code-is used as the reference signal for synchronization of PN code and data demondulation. The coherent demodulation architecture is also exploited for the reverse link as well as for the forward link. Here are the characteristics of the suggested DS/CDMA system. First, we suggest an interlaced quadrature spreading(IQS) method. In this method, the PN coe for I-phase 1st channel is used for Q-phase 2nd channels and the PN code for Q-phase 1st channel is used for I-phase 2nd channel, and so on-which is quite different from the eisting spreading schemes of DS/CDMA systems, such as IS-95 digital CDMA cellular or W-CDMA for PCS. By doing IQS spreading, we can drastically reduce the zero crossing rate of the RF signals. Second, we introduce an adaptive threshold setting for the synchronization of PN code, an initial acquistion method that uses a single PN code generator and reduces the acquistion time by a half compared the existing ones, and exploit the state machines to reduce the reacquistion time Third, various kinds of functions, such as automatic frequency control(AFC), automatic level control(ALC), bit-error-rate(BER) estimator, and spectral shaping for reducing the adjacent channel interference, are introduced to improve the system performance. Fourth, we designed and implemented the DS/CDMA MODEM to be used for variable transmission rate applications-from 16Kbps to 1.024Mbps. We developed and confirmed the DS/CDMA MODEM architecture through mathematical analysis and various kind of simulations. The ASIC design was done using VHDL coding and synthesis. To cope with several different kinds of applications, we developed transmitter and receiver ASICs separately. While a single transmitter or receiver ASC contains three channels (one for the pilot and the others for the traffic channels), by combining several transmitter ASICs, we can expand the number of channels up to 64. The ASICs are now under use for implementing a line-of-sight (LOS) radio equipment.
To find out the optimum application method of slow-releasing fertilizer(SRF) and conventional fertilizer(CF) with different fertilization rate under two culture methods[l0-day old seedling machine transplanting(MT) and direct-sowing on dry paddy(DS)] in the south-western region(clay loam soil) of Korea, used were Chosun slow-releasing fertilizer(silicate latex coated fertilizer: N-P$_2$O$_{5}$-K$_2$O =18-12-13) and conventional fertilizer. Plant height and number of tillers with different two culture methods were higher at MT than DS in early growth. The ratio of dry weight in heading stage was higher at CF than SRF in MT than DS and especially, SRF 80% + CF 20% than SRF 100% or CF 100%. Leaf area index (LAI) in heading stage was higher at CF in MT but higher at SRF in DS than their counterparts. Chlorophyll content was higher at SRF than in CF expect for heading stage(HS), especially in DS. It was highest at HS in CF without its difference during maximum tillering stage(MTS) and panicle formation stage(PFS), while highest at PFS in SRF with tendency of gradual increase and decrease before and after PFS, respectively. Heading was delayed 2~3 days at SRF in two cultrue methods and 4~5 days at SRF in DS in comparison with CF in MT with delay of 2 days at DS compared with MT. Culm length was longer at CF in MT and at SRF in DS than their counterparts. Panicle number per m was more at SRF and in DS. Filled grain ratio was higher at CF and in MT. Yield was obtained 101 and 100% at 100% and 80% level of SRF in DS respectively, and 96% at 80% level of CF in MT, compared with conventional application method (516kg /l0a), and increased 2~4% at DS and 0~3% at MT in SRF. Yield was high in order of 100%(SRF) =80%(SRF) + 20%(CF) > 100%(SRF) + 20%(CF) > 80%(SRF) at MT and 80%(SRF) + 20%(CF) =100%(SRF) > 80%(SRF) =100%(SRF) + 20%(CF) at DS.
Objectives : The distressed personality (type-D) pattern, consisting of negative affectivity and social inhibition, has been shown by Denollet et al. For measuring the Type D characteristics, Denollet has devised the 14 item Type D scale (DS14). In the present study, this instrument was translated into Korean. The reliability and validity of the Korean DS14 was pilot tested. Methods : Preliminary version of the total 17-item DS14 scale was translated into Korean. 372 controls that did not have any coronary heart diseases (CHD) were randomly sampled in Seoul and Gyeonggi province. Preliminary Korean version was applied to controls and 14 items were finally selected to maximize reliability of the scale. We compared the distribution of type-D personality among the normal controls, hypertensive patients, and the CHD patients. Results : 7 of 10 items in social inhibition were selected and final 14-item version was made. The internal consistency of negative affectivity (0.817) and social inhibition (0.797) were high. In addition, the prevalence of type-D personality in the CHD patients group was significantly higher than normal controls. Conclusion : These findings suggest that Korean version of DS14 is a valid tool for measuring the type D characteristics. Type-D characteristics can be suggested to predict adverse prognosis in patients with CHD.

This study is to make the liquid crystalline structure using sucrose distearate (Sucro-DS) emulsifier to create the hydrophilic type oil-in-water (O/W) emulsion, the droplets of the emulsion having a structure of a multi-lamellar structure. We have studied the physicochemical properties of Sucro-DS using those techniques. And it has been studied in the emulsion performance. In order to form the liquid crystalline structure applying 3 wt% of Sucro-DS, 5 wt% of glycerin, 5 wt% of squalane, 5 wt% of capric/caprylic triglyceride, 3wt% of cetostearyl alcohol, 1wt% of glyceryl mono-stearate, 78 wt% of pure water in mixture having the lamellar structure of stable multi-layer system was found to formed. By applying them, they were described how to create an unstable active material encapsulated cream. Further, the moisturizing cream was studied using this technique. It reported the results to the skin improvement effect by the human clinical trials. The pH range to produce a stable liquid crystal phase using a Sucro-DS was maintained in 5.2~7.5. In order to increase the stability of the liquid crystal, it was when behenyl alcohol containing 3 wt%, the hardness at this time was 13 kg/mm,min. Viscosity of the same amount was 25,000mPas/min. After a test for the effects of the emulsions, the concentration of 6 wt% Sucro-DS is that was appropriate, the particle size of the liquid crystal was 4~6mm. It was observed through a microscope analysis, reliability of the liquid crystal changes for 3 months was found to get stable at each $4^{\circ}C$, $25^{\circ}C$ and $45^{\circ}C$. In clinical trial test, before applying a moisturizing effect it was $13.4{\pm}7%$. Moisturizing cream liquid crystal was not formed in $14.5{\pm}5%$. Therefore, applying than ever before could see the moisture about 8.2% was improved. On the other hand, it was the moisturizing effect of the liquid cream is $19.2{\pm}7%$. The results showed that 43.3% improvement than that previously used. Applications fields, Sucro-DS emulsifier used liquid cream, lotion, eye cream and a variety of formulations can be developed, as well as the cosmetics industry is expected to be wide fields in the application of the external preparation for skin emulsion technology in the pharmaceutical industry and pharmaceutical industry.
The purpose of this study was to investigate muscle activity and vertical ground reaction force(F) in children with Down syndrome(DS) during vertical jump. Six DS and one healthy child performed vertical jump. Four muscles(Biceps femoris, Rectus femoris, Tibialis anterior & Gastrocnemius) and F were analyzed. Gastrocnemius in DS showed lower muscle activity in a propulsive phase. Impulse during 0.3sec before toe-off in DS displayed lower value than that in the healthy child. The second peak of F in DS occurred later than that in the healthy child, so DS performed landing with their knee more flexed. The first and second peak of F and loading rate to the second peak of F in DS showed lower value than those in the healthy child. Therefore, DS might have lower ability to absorb the force while landing from a vertical jump.

This paper presents an analysis of a hybrid direct-sequence/slow frequency hopped code division multiple access(DS/SFH-CDMA) system employing noncoherent M-ary frequency shift keying(MFSK) modulation in a multiple m-distribution fading environment. Multipath interfer- ence(MPI) and multiuser interference(MUI) is taken into accout and the spectral efficiency is calculated for uncoded as well as simple channel coding systems. The predetection multipath CCI canceller in conjunction with convolution coding is employed for improving the bit error rate(BER) performance. The BER of noncoherent hybrid system is obtained using a Gaussian interference approximation. From the results, we know that the error performance more deteriorates as the depth of fading becomes deeper. The DS part of the modulation combats the multipath interference, whereas the FH part is a predetection against large multiuser interference. It is shown that, for the con- sidered types of a channel coding, the use of a predetection coding is still essential for obtained a satisfactory bit error performance. The results show that the capacity of the DS/SFG-CDMA MFSK communication system increases in proportion to the length of PN code sequence in the presence of AWGN and MUI. In m-distribution fading environment the capacity increases in proportion to the fading index. The capacity is increased and error performance is improved when the CCI Canceller and Convolution code technique are adopted, respectively. From the results, it is known that the error performance of $4\times10^{-2}$ by adopting Canceller technique. Also convolutional coding technique is the improvement of error performance attains about $10^{-5}$ in code rate 1/2.

The increasing number of users of mobile communication systems and the corresponding need for increased system capacity require the use of a modulation scheme which is both power and spectrally efficient. In this paper, we numerically calculate the BER performance of DS/CDMA-BPSK system in different fading channel (Rayleigh, Rician, Shadow Rician). Also, we calculate BER performance and the channel capacity of DS/CDMA-BPSK system which is constant or nearly constant envelopes in 3-state fading channel model. The Shadow Rician fading model described in this paper apply the parameters of the Canadian Mobile Satellite (MSAT). And we assume that the 3-state fading channel model is consist of Rayleigh fading state, Rician fading state, and shadow Rician fading state. This model can be used as a basis for the simulation of the land mobile satellite channel. The dynamic 3-state fading channel model is considered corresponding to different environments and the transitions between these environments. From the numerically calculate results, the DS/CDMA-BPSK system with MUI-20, PG-511 can not achieve the BER performance ($P_b\leq10^{-5}$). And the channel capacity did not meet the system requirement. Also, we know that the BER performance is depend m the occupancy probability of radio channel and the degree of shadow. From the results, we how that during shadowed time intervals it is necessary to use some form of error control coding and receiver diversity in order to support reliable data communication.
