The effect of synovium graft on conjunctiva in rabbit model of dry eye

  • This study was performed to identify the effect of synovium graft on conjunctiva in rabbits after dry eye induction. Six New Zealand White rabbits were used as dry eye models. Both eyes were divided to two groups as control and synovium graft group. The synovium graft was performed in fourth week after dry eye modeling. Quantitative change of tears through Schirmer tear test (STT), qualitative change of tear film through tear film break up time (TFBUT), and damage of cornea through fluorescein staining were observed for 10 weeks at intervals of two weeks. Histological examination was performed to evaluate cornea and conjunctiva at tenth week. In both groups, STT and TFBUT were significantly decreased in 4 weeks after modeling compared to 0 weeks (p < 0.05) . After synovium graft, there were increases in STT value at 4 weeks and TFBUT at 4 and 6 weeks in graft group (p < 0.05). Corneal fluorescein staining showed no significant difference between the two groups. In histopathological examination, grafted synovium was detected as round to ovoid ingression folds, well attached to grafted regions with 0.11 ± 0.04 mm2 (range, 0.05-0.16 mm2). These results indicated that the synovium graft on the conjunctiva had an effect on the qualitative and quantitative improvement of the tear film even though there was no histological change.

A study of Conjunctival Cellular Changes in Dry Eye Patients by Impression Cytology (Impression cytology를 이용한 건성안의 결막 세포변화에 관한 연구)

  • Impression cytology refers to application of cellulose acetate filter material to the ocular surface to remove the superficial layers of the conjunctival epithelium. The technique is non-invasive, is easy to perform, causes minimal discomfort to the patient, and can be used to follow changes in the conjunctival ocular surface over time. With this method, the morphology of the conjunctival ocular surface can be studied and the degree of squmaous metaplasia assessed. This study was performed to evaluate the conjunctival surface by impression cytology in dry eye patients. A total of 70 students with no contact lens wearing history were recruited. Subjects were required to fill in a McMonnies dry eye symptom questionnaire. The non-invasive tear thinning time(TIT) test of each subject was measured, followed by Schirmer tear test(STI), tear film break-up time(TBUT) tests and Rose-bengal staining were performed as a baseline. Conjunctival epithelial cells from the inferior bulbar conjunctiva were harvested by the impression cytology technique. The specimens collected were labelled and stained with PAS(Periodic Acid Schift)-haematoxylin. The goblet cells and conjunctival epithelial cells were observed under a light microscope of 400x magnification. The specimens were classified according to the Nelson Grading scale which was based on the degree of squamous metaplasia such as changes of goblet cells density, size/form, N:C(nucleus : cytoplasm) ratio. Dry eye patients were observed morphological changes of the epithelial cells, different nuclear alterations, decrease of the goblet cells density. The degree of cytological changes was related to severity of dry eye conditions. When the epithelial cell morphology was graded according to the system described by Nelson, specimens from the control group revealed 91.43% of the eyes to be grade 0 and 8.57% to be grade 1, whereas of the dry eye patients, 20% were grade 0, 42.86% grade 1, 34.29% grade 2 and 2,86% grade 3. Impression cytology represents a non- or minimally invasive biopsy of the ocular surface epithelium with no side effects or contraindications. It has demonstrated to be a useful diagnostic aid for a wide variety of processes involving the ocular surface. This technique is a safe, simple method and may help increase understanding of various ocular surface alterations in dry eye patients.

Reference values for selected dry eye tests in normal Beagle dogs: a pilot study

  • Background: Dry eye disease (DED) cannot be diagnosed by a single test because it is a multifactorial disorder of the ocular surface. Although studies on various dry eye tests (DETs) in dogs have been reported, standard criteria have not been established except for the Schirmer tear test 1 (STT-1). Objectives: To establish reference values for DETs in dogs with normal STT-1 values (≥ 15 mm/min) and to analyze the correlation between DETs. Methods: The STT-1 was performed in 10 healthy Beagle dogs (20 eyes). After 20 min, interferometry (IF) for estimating the lipid layer thickness (LLT) of the tear film, tear meniscus height (TMH), non-invasive tear breakup time (NIBUT), and meibomian gland loss rate of the upper eyelid (MGLRU) and lower eyelid (MGLRL), were measured using an ocular surface analyzer. Results: Mean values for STT-1, TMH, and NIBUT were as follows: 21.7 ± 3.4 mm/min, 0.41 ± 0.21 mm, and 19.1 ± 9.5 sec, respectively. The most frequent LLT range, measured by IF, was 30-80 nm (11 eyes), followed by 80 nm (5 eyes) and 80-140 nm (4 eyes). MGLRU ≤ 25% was measured in 11 eyes and 26%-50% in 9 eyes; MGLRL ≤ 25% in 8 eyes and 26%-50% in 12 eyes. Besides positive correlation between TMH and NIBUT (P = 0.038), there were no significant associations between DETs. Conclusions: Data obtained in this study provided normative references that could be useful for diagnosing DED and for further research into correlation between DETs in dogs with DED.

Association of serum 25-hydroxyvitamin D concentrations with Schirmer tear test 1 and tear film breakup time in dogs

  • Background: The association between vitamin D and canine keratoconjunctivitis sicca (KCS) has not been investigated in dogs. Objectives: To investigate the association of serum 25-hydroxyvitamin D [25(OH)D] concentrations with Schirmer tear test 1 (STT-1) and tear film breakup time (TFBUT) in dogs. Methods: Sixty-one clinically healthy, client-owned dogs were enrolled. STT-1 and TFBUT were measured in 122 (61dogs) and 82 (41 dogs out of total 61 dogs) eyes, respectively. Serum 25(OH)D concentrations were evaluated by quantitative chemiluminescent immunoassay. The dogs were classified into 6 groups according to the evaluations (STT-1: group 1, normal [≥ 15 mm/min] in both eyes; group 2, normal in one eye and abnormal [< 15 mm/min] in the fellow eye; group 3, abnormal in both eyes; TFBUT: group 4, normal [≥ 20 sec] in both eyes; group 5, normal in one eye and abnormal [< 20 sec] in the fellow eye; group 6, abnormal in both eyes). Results: STT-1 was positively correlated with TFBUT (p < 0.001). Among the STT-1 groups, the mean serum 25(OH)D concentration in group 1 was significantly higher than in groups 2 and 3 with positive correlation (p < 0.001). However, there were no significant differences among the TFBUT groups 4, 5, and 6. Conclusions: In dogs, it was found that serum 25(OH)D concentrations had a greater effect on quantitative KCS than qualitative KCS. Therefore, it is considered that measurement of serum 25(OH)D concentration could be included in the diagnostic tests in canine quantitative KCS patients.

A Comparative Clinical Study on an Electronic Moxibustion with Acupuncture for Dry Eye Syndrome (건성안 증후군에 대한 전자뜸과 침의 효과 비교 임상연구)

  • Objectives : Recently, the number of patients with dry eye syndrome has been steadily increasing. However, established treatment for it does not yet exist. This study compared electronic moxibustion with acupuncture for patients with dry eye in terms of efficacy and safety. Methods : This trial was designed as an investigator-initiated, single-blinded, comparative, randomized controlled trial. Thirty patients with dry eye were randomized to the electronic moxibustion treatment group(EMG) or the acupuncture treatment group(AG) in equal proportion. The participants who assigned to the EMG were treated with electronic moxibustion to ten acupoints including both sides of BL2, GB14, TE23, and LI4 for 12 minutes. The participants who were assigned to the AG were treated with acupuncture to the same acupoints for 15 minutes. Over 4 weeks, each intervention was carried out twelve times in total. The primary outcome was the ocular surface disease index(OSDI). The secondary outcomes were the subjective symptoms visual analog scale(VAS), quality of life(QoL), Schirmer I test(SIT), and general assessment. Adverse events and vital signs were also investigated for safety assessment. Results : In intragroup comparisons, both the EMG and the AG significantly improved the OSDI scores, the subjective symptoms VAS scores, the QoL scores, and the SIT results after 4 weeks of the trial. However, there were no statistical differences in intergroup comparisons between the two groups after 4 weeks. The safety of electronic moxibustion and acupuncture was confirmed by no occurrence of serious adverse events. Conclusions : Both electronic moxibustion and acupuncture were effective for dry eye syndrome, and they were safe. Electronic moxibustion and acupuncture can be used for dry eye syndrome as equivalent treatments.

The Effects of Self-acupressure on the Visual Health of College Students (자가 경혈지압이 대학생의 눈 건강에 미치는 효과)

  • Objectives : The purpose of this study was to identify the effects of self-acupressure on the visual health of college students. Methods : A nonequivalent control group pretest-posttest design was used. Participants were determined by OSDI(ocular surface disease index) and the Schirmer test, they were assigned to an experimental group of 22 and control group of 22. The experimental group participated in self-acupressure, which were practiced 2 times a day, 5 days a week for 4 weeks. Results : Scores of ocular surface disease index, Schirmer test and ocular fatigue were significantly better in the experimental group after the intervention compared to the control group. Conclusions : The result indicate that self-acupressure is an effective nursing intervention for improving the dry eye syndrome and decreasing ocular fatigue in college students.

안구건조증의 침 치료에 대한 최근 임상 연구 동향 분석 - 2010년 이후 Pubmed, Cochrane Library 검색 중심으로

  • Objectives : The purpose of this review is to validate the effectiveness of acupuncture treatment on dry eye syndrome. Methods : Through foreign online databases(Pubmed, Cochrane Library) and domestic online databases(Science On, OASIS) we only searched for Randomized controlled trials(RCTs) that performed acupuncture for dry eye syndrome from January 2010 to December 2022. Results : The most commonly used acupoints were 風池(GB20), 贊竹(BL2) and the most commonly used meridian was the gall bladder meridian of foot-shaoyang. The most commonly used evaluation criteria was Tear Break Up Time(TBUT) followed by Schirmer Test and Ocular Surface Disease Index(OSDI). In most of 8 studies, acupuncture treatment showed significant therapeutic effects. Conclusions : According to this study results, acupuncture can be suggested to effective way for treating dry eye syndrome. More clinical studies are needed to prove the effectiveness of the acupuncture on dry eye syndrome.

Influencing factors of self-reported dry mouth in the employees in social welfare facilities (일부 지역 사회복지시설 종사자의 주관적 구강건조증에 영향 요인)

  • Objectives: The purpose of the study was to examine the influencing factors of self-reported dry mouth in the employees in social welfare facilities. Methods: A self-reported questionnaire was completed by 260 employees in social welfare facilities from January 5 to 30, 2015 by convenience sampling method. Except 25 incomplete answers, 215 data were analyzed by t test, one way ANOVA, Pearson's correlation coefficient analysis, and multiple linear regression analysis using SPSS 180. program. The questionnaire consisted of general characteristics of the subjects, health-related characteristics, whole body dryness and self-reported dry mouth. The oral health-related quality of life was measured by five point Likert scale, and a higher score indicated a lower quality of life. Results: The self-reported dry mouth in the employees in the social welfare facilities varied by the general health status, stress, oral health status and oral malodor. The self-reported dry mouth was closely related to the quality of life and the four subfactors including dryness of skin, eye, lip and nasal mucosa. The quality of life had the influence on the self-reported dry mouth, nasal mucosa dryness, eye dryness, and oral malodor in order. Conclusions: The self-reported dry mouth was closely related to whole body dryness and the quality of life. It is necessary to develop the quality of life improvement programs that prevent and manage the dry mouth and whole body dryness in the employees in the social welfare facilities.

Dry Eyes and its Associated Factors in the elderly living in the community (지역사회 거주노인의 안구건조증 유병률과 관련요인)

  • This study aimed to identify the prevalence of dry eye, sleep disorder, depression and factors related to dry eye among the elderly living in the community. Study subjects were 149 elderly, 65 years of age or older from nearby people, acquaintances and community elderly-related centers that researchers could access. Data collection period was from June 1 to August 31, 2021. Data were analyzed by frequency, percentage, mean, standard deviation, ANOVA, χ2-test, and multiple logistic regression. The results showed that dry eyes in elderly people were statistically significant according to age (χ2=4.50, p=.034), occupation (χ2=7.14, p=.008), chronic diseases (χ2=6.59, p=.010), drugs (χ2=4.86, p=.027), eye surgery (χ2=6.02, p=.014), and sleep disorders (χ2=7.29, p=.007). Multiple logistic regression revealed that dry eyes were associated with sleep disorders (OR=2.45, 95% CI=1.17-5.13). Therefore, for the management and prevention of dry eyes in the elderly, there is a need for nursing intervention and strategy that can find the causes of sleep disorders and improve the quality of sleep.

Development of Cellulose Strip for Dry Eye Inspection (건성안검사용 셀룰로즈 스트립 개발)

  • Schirmer test is one of the most used methods for the diagnosis of dry eye. We attempted to develop a tear level measurement tool to replace unreliable Schirmer test with inaccurate results. Absorbency tests for various absorbents were carried out. As a result, ${\alpha}$ cellulose pulp was selected as the absorbent. Prototypes were produced and evaluated twice. Finally a tear level measurement tool in the form of a strip of ${\alpha}$ cellulose pulp adhered to a polyurethane was prepared. Usability evaluation of prepared tear level measurement tool was performed. As a result, it was confirmed that it has a significant correlation with SM tube developed oversea recently. In addition, it was judged to be useful as an alternative to the Schirmer test in terms of measurement time and accuracy.
