Systematic Review of Cupping Including Bloodletting Therapy for Musculoskeletal Diseases in Korea

  • To evaluate the effectiveness of cupping and bloodletting therapy in the treatment of musculoskeletal diseases. Systematic searches were conducted on KSI, KISTI, DB Pia, KIOM Database, and Koreamed until January 2007 Hand-searches included conference proceedings and our own files. There were no restrictions regarding the language of journals published in Korea. Controlled trials of dry cupping, wet cupping, or blood letting for patients with musculoskeletal disease were considered for inclusion. Trials testing other forms of dry cupping therapy were included. Methodological quality was assessed by two doctors. 20 possibly relevant studies were identified and 5 studies were included. One trial tested wet cupping for ankle sprain and reported positive result. Two trials tested blood letting for low back pain, one was positive and the other one was neutral. One trial tested the types of dry cupping for low back pain, and Ki-gong cupping therapy was superior to other two types of cupping. One trial compared wet cupping with dry cupping for low back pain and the result was negative. The effectiveness of bloodletting plus acupuncture for treating patients with low back pain is superior to acupuncture in spite of low quality. One trial of wet cupping for ankle sprain had effects in reducing pain. However, I suggest that the rigorous RCTs of cupping and blood letting therapy will be conducted in well designed features.

건부항요법이 간호사의 견통과 피로도에 미치는 영향 (The Effects of Dry Cupping Therapy on the Shoulder Pain and Fatigue of Nurses)

  • Objective : This research was done to identify the effects of dry cupping therapy on the shoulder pain and fatigue of nurses. Methods : The research design was time series design. The participants were 27 nurses with shoulder pain at P University Hospital in Busan metropolitan city from July 6, 2009 to August 3, 2009. The dry cupping therapy was done 4 times, twice a week for 2 weeks. Before intervention, the severity and frequency of pain and fatigue were measured three times at one week intervals, and then those were measured after one week and two weeks of experimental treatment. The results were analyzed using descriptive statistics, paired t-test and repeated measure ANOVA with the SPSS program. Results : There were statistically significant difference in severity and frequency of shoulder pain, and fatigue after dry cupping therapy. Dry cupping therapy was effective for the management of shoulder pain and fatigue among nurses in this study. Conclusion : Therefore dry cupping therapy can be considered an effective nursing intervention that relieves shoulder pain and fatigue of nurses.

불면장애에 대한 부항요법의 체계적 문헌고찰 (A Systematic Review of Cupping Therapy for Insomnia Disorder)

  • Objectives: To examine the efficacy and safety of cupping therapy for insomnia disorder and provide clinical evidence that could contribute to further research. Methods: We searched randomized controlled trials (RCTs) that verified effects of cupping therapy for insomnia disorder from 11 domestic and foreign databases. Included studies were evaluated using Risk of Bias (RoB). Results: Nineteen RCTs were selected. Wet cupping was more frequently used than dry cupping. Moving cupping was mostly used among dry cupping methods. Dry cupping was mainly performed in Bladder Meridian on the back. Wet cupping was mainly performed on BL18, LR14, and BL15. The most common treatment period was 4 weeks. The average number of treatments per week was about 2.4 times for all types of cupping, about 3.2 times for dry cupping and about 1.9 times for wet cupping. All selected studies showed a significant sleep improvement compared to the control group. As a result of the meta-analysis, in terms of effective rate, SAS score, and SDS score, the wet cupping in combination with acupuncture was significantly more effective than acupuncture alone, although the quality of selected RCTs was low. Conclusions: Cupping therapy is effective for insomnia disorder. Based on results of this study, it is reasonable to use wet cupping two times per week on BL18, LR14, and BL15 or use moving cupping three times per week in Bladder Meridian on the back for 4 weeks to treat insomnia disorder.

Effect of traditional dry cupping therapy on heavy menstrual bleeding in menorrhagia: A preliminary study

  • Menorrhagia (kasrate tams) is the most common gynecologic complaint in contemporary gynecology, affecting 10 to 15% of the adult female population. It can occur at any age. In the Unani system of medicine, since antiquity, dry cupping therapy (hijamat bila shurt) has been used to treat menorrhagia. An effort was made to evaluate the usefulness of dry cupping on excessive menstrual blood loss in menorrhagia with a well validated menstrual pictogram. This study was conducted on 15 patients at the National Institute of Unani Medicine, Hospital Bangalore, from January 2010 and July 2010. Unmarried or married patients aged 17 - 47 year with a history of excessive or prolonged bleeding per vaginum were included. Two large cups (with 5.7 cm internal diameter) were applied below each breast for 15 min, only once during the menstrual period when the patient reported to the outpatient department. If the menstrual flow was not reduced, the next day again dry cupping was repeated. The outcome was to assess the efficacy of dry cupping on the reduction in the amount of menstrual blood loss. The data was analyzed by the Wilcoxon matched test. The mean scores of menstrual blood loss before and after the treatment was 400.26 (277.95) and 48.4 (32.082) ml respectively, p = 0.002, considered statistically significant. A dry cupping therapy is a useful treatment modality in decreasing the amount of menstrual blood flow in menorrhagia. Further randomized controlled clinical trials and validation are needed in a large population.

Comparison of the effects of muscle stretching exercises and cupping therapy on pain thresholds, cervical range of motion and angle: a cross-over study

  • Objective: Computers and smartphones have become a necessity for modern people, and the use of these things in an inappropriate position has increased the number of people who complain about neck problems. The purpose of this study was to compare the changes of cervical angle, range of motion (ROM) and pain threshold according to the McKenzie stretching and dry cupping therapy. Design: Cross-over design. Methods: We included 12 male and 6 female college students in their twenties, and conducted a pre- and post-test to evaluate the changes of each variable after the application of the McKenzie stretching and dry cupping therapy. Results: Neither the cervical spine angle nor the turtle neck angle showed any change in both the McKenzie stretching and the dry cupping treatment. In the McKenzie stretching, the pain threshold decreased, and the ROM of the cervical spine increased in all directions but there was no significant difference. The pain threshold was increased in the dry cupping treatment, and the ROM of the cervical spine was significantly increased in all directions (p<0.05). Comparisons of the McKenzie stretching and cupping treatment showed that the cupping treatment produced significantly greater pain thresholds and improvements in ROM of the cervical spine than the McKenzie stretching technique (p<0.05). Conclusions: Cupping treatment is more effective in improving ROM of the cervical spine and pain thresholds than the McKenzie stretching technique. In the future, cupping treatment will be one of the treatment options for pain and ROM impairments of the cervical spine.

흉배부(胸背部)에 시행한 부항요법(附缸療法)이 자율신경계에 미치는 영향 - 심박변이도 측정을 통한 연구 - (The Effect of Dry Cupping Treatment Applied to Back-shu Points on the Autonomic Nervous System through HRV)

  • Objectives : This study was performed to evaluate the effect of dry cupping treatment applied to back-shu points on the autonomic nervous system. Methods : Two groups of sympathicotonia and normal with each 30 volunteers were set up for this experiment. The sympathicotonia group was selected by the criterion for sympathicotonia by the questionnaire composed of 11 items. After 10 minutes for environmental adaptation, the first HRV(heart rate variability) test was conducted, and then, dry cupping therapy was applied to back-shu points for 5 minutes to stimulate sympathetic ganglia lying along the spine. The second HRV test was carried out just after the cupping therapy under the same condition and then, the third test was repeated after two hours based on the first test time. Results : 1. In sympathicotonia group, SDNN(standard deviation of all normal R-R intervals), RMSSD(the square root of the mean of the sum of the squares of differences between adjacent normal R-R intervals), Ln(HF)(high frequency power), nmHF(normalized high frequency power) increased and mHR(mean heart rate), nmLF(normalized low frequency power) decreased significantly right after dry cupping therapy which means dry cupping affects on autonomic nervous system. The effect lasts in these items of nmLF, Ln(HF), nmHF, rLHF(rate ratio of LF/HF). 2. In normal group, SDNN, RMSSD increased and mHR decreased significantly right after dry cupping therapy, too. But, Ln(LF)(low frequency power), nmLF, rLHF unexpectedly increased and nmLF, rLHF stay increased up to 2 hours. Conclusions : The results suggest that the dry cupping therapy has effect on the autonomic nervous system. It is effective to stabilize hyper-sympathetic tone of people diagnosed as Sympathicotonia and activate parasympathetic tone.

A Comparative Study on the Effect of Cupping Therapy Combined with Korean Medicine Treatment in Peripheral Facial Paralysis

  • Background: The aim of this study was to compare the effects of different cupping therapy methods combined with Korean medicine treatments for peripheral facial paralysis. Methods: 105 patients treated for peripheral facial paralysis with cupping therapy and other Korean medicine treatment at Gil Korean medicine hospital, Gachon University between May 19, 2014 and June 30, 2018 were selected and their medical charts retrospectively analyzed. 48 patients who met the inclusion criteria were divided into 2 groups: wet cupping (WC) therapy, or dry cupping (DC) therapy combined with Korean medicine treatment. The duration of treatment ranged from 2 weeks for inpatient treatment to 2 months for outpatient treatment. Both WC and DC therapy were performed on TE13, 3 times per week during the treatment period. The effect of cupping therapy was evaluated by using the Gross Grading System of the House-Brackmann (HB score) and the Yanagihara's Unweighted Grading System (Y score). Results: For both WC and DC treatment of symptoms related to peripheral facial paralysis, HB scores showed a significant decrease and Y scores showed a significant increase from baseline to end of treatment, indicating a beneficial improvement in patient symptoms for both WC and DC. Conclusion: In this study, both DC and WC treatment had significant improvements on peripheral facial paralysis symptoms, with WC having significantly greater beneficial effects than DC.

부항요법에 대한 문헌고찰 및 부항시술 현황 조사 (Literature Investigation Regarding Cupping Therapy and Analysis of Current Professional's Cupping Treatment)

  • Objectives : This study was performed to report the present situation of the cupping treatment to make standardization of cupping treatment in Korea. Methods : We searched relevant case reports, surveys, and review articles using a databases of online bibliography. And we had research to oriental medical doctor with questionnaire about the cupping treatment. Results : 1. Cupping treatment is used for diagnoisis, protection and treatment for many kinds of diseases such as musculoskeletal diseases, internal diseases, sequela of cerebral attacks and so on in Korea. 2. Adequate cupping area is the area of lesion. 3. Cupping time and pressure are various. 4. Adequate amount of venesection is 10cc. 5. Adequate dry cupping term is 1 time/day and adequate wet cupping term is 1 time/2~3days. 6. Cognition of adverse reaction of cupping treatment is different among the doctors. 7. Method of disinfection of cup is different among the doctors. Conclusions : The result of this study will help to make the a guideline of cupping treatment. And we have to go ahead studying to make standardization of cupping treatment.

부항 시술에 의해 형성된 수포에 관한 고찰 (The Study of Blister Caused by Cupping Therapy)

  • Objectives : The aims of this study were to evaluate a blister caused by cupping. Methods : We searched relevant case reports, survey, and review articles using databases of online bibliography. Results : 1. The fluid in the blister caused by cupping therapy is normal substance by laboratory analysis. The fluid has no signs of infection in the culture, Gram stain, or tissue biopsy 2. In histological finding, the blister caused by cupping therapy is made by dermo-epidermal seperation at subcellular level. Suction blistering was neither inflammatory nor autolysis activation of lysosomal hydrolases. 3. Blistering times directly, related to suction pressure. Suction blister formation time is accelerated in older subjects compared with younger individuals and higher temperature was more susceptable to the blister compared with lower temperature. The flexor aspect of forearm is a easy site for suction blister formation compared with leg and abdominal site. 4. Blister caused by cupping therapy is treated by regular and judicious changes of sterile dressing over several weeks. The vesicles healed well and left no visible scar. Conclusions : Blister caused by cupping therapy is artificially controlled by doctor's therapeutic purpose. Blister is not histologically injurious to health and the blister is a natural concomitant after cupping therapy.

청기산(淸肌散) 변방(變方)을 이용한 아토피 피부염 치험 1례 (A clinical study on a patient with atopic dermatitis)

  • Objectives : Atopic dermatitis is a skin problem that causes dry skin, intense itching, and then a red, raised rash. It may be a long-lasting (chronic) skin problem that requires more than one treatment. The cause of atopic dermatitis isn't clear, but it affects the skin's ability to hold moisture. The skin becomes dry, itchy, and easily irritated. The purpose of this study was to report the clinical effects of herbal medicine on atopic dermatitis. Methods : We employed oriental medical treatments; herbal-medication (Cheonggisanbyeonbang), acupuncture, moxibustion and cupping therapy. At the same time, the patient sweated out the skin poison. We treated him two times a week with oriental therapy method. He took medicine three times a day after a meal. During taking medicine, we let him avoid fatty food, flour based food, and alcohol. We took a picture the patient's body parts and compared the symptom with previous symptom. With the picture, we evaluated the patient's condition. Results : After taking treatment - 17 times' acupuncture, moxibustion and cupping therapy during 4 months- and taking herbal-medicine, the skin condition of the patient was much improved. At first, he was troubled with itching and hot feeling at reddish area, time after time the itching and hot feeling was weakened. The reddish area was changed to dry condition and changed to normal state. In advance, the symptom of arms was improved, and later the symptom of legs was started to improve. Conclusion : Herbal medicine (Cheonggisanbyeonbang) was an effective in the treatment of atopic dermatitis and it helped to improve regenerating the skin in the body.
