Prevalence and Characteristics of Antimicrobial-Resistant Staphylococcus aureus and Methicillin-Resistant Staphylococcus aureus from Retail Meat in Korea

  • This study was to investigate the prevalence and characteristics of antimicrobial-resistant Staphylococcus aureus and methicillin-resistant S. aureus (MRSA) from 4,264 retail meat samples including beef, pork, and chicken in Korea between 2013 and 2018. A broth microdilution antimicrobial susceptibility testing was performed for S. aureus. Molecular typing by multilocus sequence typing (MLST), spa typing, and pulsed-field gel electrophoresis (PFGE), was performed on mecA-positive S. aureus strain. S. aureus was isolated at a rate of 18.2% (777/4,264), of which MRSA comprised 0.7% (29 strains). MLST analysis showed that 11 out of the 29 MRSA isolates were predominantly sequence type (ST) 398 (37.9%). In addition, ST72, ST692, ST188, ST9, and ST630 were identified in the MRSA isolates. The spa typing results were classified into 11 types and showed a high correlation with MLST. The antimicrobial resistance assays revealed that MRSA showed 100% resistance to cefoxitin and penicillin. In addition, resistance to tetracycline (62.1%), clindamycin (55.2%), and erythromycin (55.2%) was relatively high; 27 of the 29 MRSA isolates exhibited multidrug resistance. PFGE analysis of the 18 strains excluding the 11 ST398 strains exhibited a maximum of 100% homology and a minimum of 64.0% homology. Among these, three pairs of isolates showed 100% homology in PFGE; these results were consistent with the MLST and spa typing results. Identification of MRSA at the final consumption stage has potential risks, suggesting that continuous monitoring of retail meat products is required.

호흡기질환 환자에서 분리한 Mycoplasma pneumoniae의 Quinolone계와 Macrolide계 항생물질에 대한 다제 저항성 (Multi-drug Resistance of Mycoplasma pneumoniae Isolates from Patients with Respiratory Diseases against Quinolone and Macrolide)

  • 호흡기질환 환자의 상기도 도말물로 부터 분리 동정된 M. pneumoniae 116균주의 moxifloxacin, levofloxacin, sparfloxacin, ofloxacin, ciprofloxacin, clarithromycin minocycline, erythromycin, josamycin, and tetracycline에 대한 감수성 검사를 실시하였다. 각 항생물질에 대한 저항성 균주의 기준은 $MIC{\pm}1.0$ ${\mu}g/ml$ 로 하였으며, 초기 MIC와 최종 MIC를 구분하여 다제 저항성 균주의 분포를 분석하였다. 초기 MIC의 결과로 분리된 균주의 단일 약제에 대한 저항성은 ciprofloxacin이 79.3%, ofloxacin이 53.5%, clarithromycin이 10.3%, erythromycin이 7.8%이었으며, 2가지 약제에 저항성은 ofloxacin과 ciprofloxacin이 42.2%, ciprofloracin과 clarithromycin이 9.5%이었으며, 3가지 약제에 저항성은 erythromycin, ofloxacin과 ciprofloxacin이 6.9%, ofloxacin, ciprofloxacin과 clarithromycin이 6.0%이었다. 최종 MIC의 결과로 분리균의 단일 약제에 대한 저항성은 tetracycline, ciprofloxacin, ofloxacin이 각각 91.4%, minocycline이 89.7%, erythromycin이 68.1%, josamycin이 52.6%, clarithromycin이 28.5%, sparfloxacin이 11.2%이었으며, 2가지 약제에 저항성인 균의 분포는 $20.7{\sim}91.4%$, 3가지약제에 저항성인 규의 분포는 &28.5{\sim}89.7%$, 4가지 약제에 저항성인 균의 분포는 2.6%, 5가지 약제에 저항성인 균은 $2.6{\sim}21.6%$, 6가지 약제에 저항성인 균은 $0.9{\sim}24.1%$이었으며, 7가지 약제에 저항성인 균은 $0.9{\sim}2.6%$이었으며, 8종류 약제에 저항성인 균도 1.7%있었다. 이상의 결과로 국내에서 분리된 M. pneumoniae 균주는 적게는 1-4 종류의 항생제에, 많게는 5-8 종류의 항생제에 저항성인 균주가 있으므로 마이코플라스마폐렴 환자를 치료할때는 macrolide계나 quinolone계의 항생제 선택에 신중을 기하여야 하며, 가급적이면 항생제 감수성 검사를 실시하여 적절한 항생제를 선택함으로써 저항성균의 출현율을 줄일 수 있고 효율적인 치료도 할 수 있도록 하여야 할 것으로 생각된다.

  • 1981년 5월${\sim}$9월까지 부산시 3개 순환여과식 양식잉어와 1982년 5월과 7월${\sim}$9월까지 부산수산대학의 양어지와 낙동강에 서식하고 있는 잉어 및 물을 시료로 해서 약제내성균을 조사했다. 약제내성균의 검출률은 외국에 비하면 적은 숫자이기는 하지만 증가경향을 띄고 있으며 또 내성정도도 강해지고 있었다. 따라서 우리 나라에서도 약제내성균의 증가를 방지하기 위해 좋은 환경에서 어류를 사육하고 질병의 정확한 진단과 처방 및 투약에 의해서 치료함은 물론, 근본적으로 약제남용을 하지 않는 것이 최선의 길이 될 것이다.

Concise Clinical Review of Hematologic Toxicity of Linezolid in Multidrug-Resistant and Extensively Drug-Resistant Tuberculosis: Role of Mitochondria

  • Multidrug-resistant tuberculosis (MDR-TB) is caused by an organism that is resistant to both rifampicin and isoniazid. Extensively drug-resistant TB, a rare type of MDR-TB, is caused by an organism that is resistant to quinolone and one of group A TB drugs (i.e., linezolid and bedaquiline). In 2018, the World Health Organization revised the groupings of TB medicines and reclassified linezolid as a group A drug for the treatment of MDR-TB. Linezolid is a synthetic antimicrobial agent in the oxazolidinone class. Although linezolid has a good efficacy, it can cause substantial adverse events, especially hematologic toxicity. In both TB infection and linezolid mechanism of action, mitochondrial dysfunction plays an important role. In this concise review, characteristics of linezolid as an anti-TB drug are summarized, including its efficacy, pathogenesis of hematologic toxicity highlighting mitochondrial dysfunction, and the monitoring and management of hematologic toxicity.

  • The emergence of antibiotic-resistant pathogens is a Serious clinical problem in the treatment of infectious diseases that increase mortality, morbidity, hospitalization length, and the cost of healthcare. In particular, $Streptococcus$ $pneumoniae$ is a major etiologic pathogen of pneumonia, sinusitis, otitis media, and meningitis. As the definition of penicillin resistance to $S.$ $pneumoniae$ was recently changed, macrolide-resistant $S.$ $pneumoniae$ is a major resistant pathogen in the community. Infections caused by antibiotic-resistant strains are associated with incorrect use of antibiotics and critical clinical outcomes. For the appropriate use of antibiotics to treat infections, physicians always should have up-to-date information on the current epidemiologic status of antibiotic resistance for common pathogens and their susceptibility to antimicrobials. Appropriate selection of antimicrobials, strict control of infection, vaccination, and development of a feasible national policy of infection control are important strategies for the control of antimicrobial resistance. This review article focuses on the current status of antibiotic-resistant $S.$ $pneumoniae$ in community-acquired pneumonia in Korea.

다제 내성 암세포에서의 Polyamine 특성 (Polyamines in Multi-drug Resistant Cancer Cells)

  • Since the advent of chemotherapy, certain types of cancer have been particularly resistant to chemotherapeutic treatment. One of the most well-studied types of resistance is resistance to multiple struc-turally dissimialr hydrophobic chemotherapeutic agents, or multidrug resistance (MDR). We found that MDR cells (KBV20C, KB7D) being highly resistant to colchicine, etoposide, and vincristine were found to have very low level of putrescine and low level of spermidine than the drug sensitive parental cells (KB) but they had almost same level of spermine as the drug sensitive cells. Although both MDR and drug sensitive cells had almost same rate of polyamine uptake, MDR cells were much more sensitive to an inhibitor of polyamine synthesis, methylglyoxal-bis guanylhydrazone (MGBG), suggesting that MDR cells might be defective in polyamine synthesis. These results also suggest that HGBG can be used for treatment of MDR in vivo.

Antimicrobial resistance in Klebsiella pneumoniae: identification of bacterial DNA adenine methyltransferase as a novel drug target from hypothetical proteins using subtractive genomics

  • Klebsiella pneumoniae is a gram-negative bacterium that is known for causing infection in nosocomial settings. As reported by the World Health Organization, carbapenem-resistant Enterobacteriaceae, a category that includes K. pneumoniae, are classified as an urgent threat, and the greatest concern is that these bacterial pathogens may acquire genetic traits that make them resistant towards antibiotics. The last class of antibiotics, carbapenems, are not able to combat these bacterial pathogens, allowing them to clonally expand antibiotic-resistant strains. Most antibiotics target essential pathways of bacterial cells; however, these targets are no longer susceptible to antibiotics. Hence, in our study, we focused on a hypothetical protein in K. pneumoniae that contains a DNA methylation protein domain, suggesting a new potential site as a drug target. DNA methylation regulates the attenuation of bacterial virulence. We integrated computational-aided drug design by using a bioinformatics approach to perform subtractive genomics, virtual screening, and fingerprint similarity search. We identified a new potential drug, koenimbine, which could be a novel antibiotic.

약제 내성 결핵 환자의 역학적 특징 (Epidemiological Characteristics of Patients with Drug-Resistant Tuberculosis)

  • 연구배경 : 전세계적으로 약제 내성 결핵에 대한 관심이 높ㅇ지고 있다. 일차 내성 결핵의 유병률은 수년 간의 국가 결핵 관리 체계를 평가하는 역학적 지표로 사용된다. 저자들은 내성 결핵에서의 사회경제적 인자를 포함한 임상적 특정에 대해 평가하고자 하였다. 방법 : 1995년 3월부터 2000년 2월까지 이대목동병원에서 내성 결핵으로 진단 받은 환자 중 조사가 가능했던 68명을 대상으로 하였다. 결과 : 일차 내성 결핵 환자는 획득 내성 결핵환자보다 평균 연령이 더 젊고(39.6$\pm$16.3yr vs. 48.2$\pm$16.5yr ; p<0.05), 40대 미만 연령층의 구성비가 더 높았으며(62.9% vs. 36.4% ; p<0.05), 고학력자가 더 많았다 (38.9% vs. 11.1% ; p<0.05). 획득 내성 환자는 일차 내성 환자보다 통계학적 의의는 없었으나 가족력의 빈도가 더 높고 자가 주택이 없는 경우가 더 많았다. 획득 내성 환자는 일차 내성 환자보다 침범된 폐엽의 수가 더 많았고(2.0$\pm$0.8 vs. 1.4$\pm$0.7; p<0.01), 총 치료 기간이 더 길었다(18.3$\pm$7.2 months vs. 10.6$\pm$6.3 months ; p<0.05). 획득 내성 환자가 일차 내성 환자보다 통계학적 의의는 없었으나 내성 약제의 수가 더 많고 입원률이 더 낮았으며 임의로 투약을 중단하는 경우가 더 많았다. 일차 내성 환자가 획득 내성 환자보다 단일 약제 내성률이 더 높은 반면, 복수 약제 내성률과 다제 내성률은 더 낮았다. 약제별 내성률에서 isoniazid에 대한 내성률이 가장 높았고, 획득 내성 환자에서 일차 내성 환자보다 isoniazid에 대한 내성률이 유의하게 높았다(90.9% vs. 71.4% ; p<0.05). 결론 : 획득 내성 환자가 낮은 사회경제적 계층의 비율이 높은 반면, 일차 내성 환자는 젊고 활동적인 집단인 것으로 보인다. 약제 내성 결핵의 전파와 재발을 막기 위해서는 내성 환자의 적절한 격리와 적극적인 추적 관찰을 통한 치료의 종결이 중요하다.

H2O2 Generating Ability and Multi-Drug Resistance of Lactic Acid Bacteria Required for Long-Term Inpatient Treatment with Antibiotic Resistance

  • Purpose: In our study, in order to find lactic acid bacteria (LAB) with multi-drug resistance to antibiotics, we isolated 140 strains from 15 types of kimchi commercially available in Korea and 20 types of Kimchi made at home from January to December in 2016, and investigated their H2O2 generating ability and multi-drug resistance to antibiotics. Methods: In order to observe the H2O2 generation ability of LAB, we performed the experiment with methods such as Rabe, Hillier, and Kang. To test the antibacterial susceptibility of LAB, we used the disc agar diffusion method using MRS agar (Difco, USA) according to the CLSI and WHO test methods. There are 18 types of antibiotic discs used. Results: Out of the total numbers of 140 strains, 6 strains of Ent. Faecium, 25 strains of L. plantarum, 1 strain of L. rhamnosus, 3 strains of L. sakei, 1 strain of L. acidophilus, 1 strains St. thermophilus, and 7 of unidentified strains generated H2O2. The antibiotic susceptibility of Ent. Faecium indicated SXT, OX, NA, and E; and the antibiotic susceptibility of L. plantarum indicated NA; and the antibiotic susceptibility of St. thermophilus indicated NA, CC, RA, CTT, CM, and P ; and the antibiotic susceptibility of L. rhamnosus indicated SXT, VA, NA and CTT; and the antibiotic susceptibility of 6 strains of L. sakei indicated SXT, OX, NOR, NA, CTT and CIP, all indicating antibiotic resistance. In the case of multi-drug resistance to antibiotics for 53 strains of L. antarum, 8-drug resistance was the most common with 25 strains, followed by 7-drug-resistant strains with 18 strains, 9-drug-resistant strains with 4 strains, 6-drug-resistant strains with 3 strains, 5-drug-resistant strains with 2 strains, and 17-drug-resistant strains with 1 strain. In the case of multi-drug resistance to antibiotics for Ent. Faecium 27 strains, 9-drug resistance was most commonly identified as 9 strains, 8-drug resistance was identified as 6 strains, 7- and 11 drug resistances were identified as 4 strains each, and 4- and 6-drug resistances were identified as 1 strain each. Conclusion: Ent. Faecium, L. plantarum, L. rhamnosus, L. sakei, and St. thermophilus, shown to have anantibacterial activity in previous studies on LAB and shown to have and H2O2 generating ability, antibiotic resistance and multi-drug resistance in this study, are expected to be able to play an excellent role for long-term inpatients to use as an alternative to antibiotics and to cope with emerging antibiotic resistance.

수입산 담수관상어 및 양식 해산어의 장내세균에서 나타나는 내성균 위험성 비교 (Comparative risks of resistant microorganisms in the intestinal track of imported freshwater ornamental fish and cultured marine fish)

  • Various antibiotics, that could induce the appearance of resistant microorganisms, have been used for treatment or prevention of bacterial diseases in marine and ornamental fish. We determined and characterized the level of antibiotic-resistant bacteria and proportion of multi-drug resistant bacteria in intestinal microflora of both marine fish cultured in Korea and imported ornamental freshwater fish. For this the bacterial species and resistance to antibiotics were investigated in intestine of rock bream Oplegnathus fasciatus cultured in Korea and pearl gourami Trichogaster leeri imported from Singapore to characterise. Although the bacterial species were different, proportions of resistant bacteria to single antibiotics or multi-drug were higher in intestinal microflora of pearl gourami Trichogaster leeri imported from Singapore than in rock bream Oplegnathus fasciatus cultured in Korea. These results indicate that various antibiotics have been being used before trading without measures in the market of asian ornamental fishes, providing high risks for the emergence of multi-drug resistant bacteria.
