The Survey of Adolescent Drug abuse in Seoul (청소년 약물남용 실태에 관한 연구 -서울지역을중심으로-)

  • This study was designed to investigate the situation of adolescent drug use in Seoul, and to com-pare this with the 1991 survey in order to better understand the present situation. The subject for this study were 1000 students in High School and Middle School. The data were collected during the period from september 1, 1992 to December 30, 1992. The guestionnaire developed by Kim So Ya Ja (1991) to survey adolescent drug use was used. The data was analyzed using descriptive statistics, 1-test and ANOVA with the SPSS program. The results of this study can be summarized as follows 1. Prevalence of Substance use : Antihistamines were used by 0.3% of adolescent, Sedatives 0.6%, Stimulants 8.1%, Hallucinogens 0.9%, Inhalants 3.2%, Narcotics 1.7%, and Analgetics 154.3%. 2. Trend in substance use compared to the 1991 surbey : Alcohol use increased from 52.8% to 63.7% and narcotics from 0.6% to 1.7%, while Smeking, Analgetics, Antihistamines, Sedatives, and Inhalants showed a decrease. 3. Smoking and Alcohol use : Twenty five percent of school adolescent had experienced cigarette smoking and 63.7% of school adolescent had experienced alcohol use. 4. Motives for drug use : The highest was avoidence of sleep at 49.4% and the next highest was adventure seeking at 27.7%. As to feeling after drug use, 34.3% felt apathy, 22.8% had feelings of sleepiness and unconsciousness. 5. Places were dreg were purchased : The most frequent was the drug store (78.3%) and 84.4% of the respondents answered that drug purchase was easy and 86.7% that drug use was mainly at home. 6. Related Variables : There was a statistically significent high score for drug and alcohol use by adolescents whose fathers used drugs. (PC.05) In conclusion, adolescents in Seoul showed in decrease in the tendency to use drugs compared to the 1991 survey, however drugs which are habit-forming and lead to dependency are still being abused. Therefore, counter-plans and preventive stratiges are important.

  • Drug development began with the finding of biologically active compounds which are obtained by chemical synthesis or from natural sources. The advent of Combinatorial Chemistry is recognized as a strategy which has a potential to change the methodology of research and development(R&D) of new drugs. Drug development has been carried out with diverse strategies. In the past several decades a variety of new methodology have been introduced in R&D. Random screening of accumulated synthetic samples which had been synthesized for development of other drugs led to the discovery of new drugs. The typical examples are anti-asthma drug trimethoquinol and calcium antagonist diltiazem. (herbesser). In particular the latter drug has been used as a calcium antagonist worldwide, however it was first synthesized to find new tranquilizer and this is the reason why diltiazem has benzodiazepam skeleton. The random screening contributed in the finding of new drugs were carried out with whole animal test and it is a standard methodology in R&D of new drugs. Aspirin is the first synthetic non-steroidal antiinflammatory drug(NSAID) and has been used for more than one hundred years. It is the first example of drug developed from natural product. Salicin is the main constituent of willow bark which had been used in Europe for a long time to treat arthritis and aspirin was developed from salicin. Most of NSAID used clinically were developed from the structure of aspirin, however it took 70 years to clarify why aspirin exhibits its antiinflammatory, analgesic and antipyretic activities. The target of aspirin is cyclooxygenase(COX)which is the first enzyme involved in arachidonate cascade leading to the production of prostaglandins(PG) and thromboxan(TX). Side effect of aspirin causing ulcer in stomach is rather serious problem, since aspirin is so popular drug easily obtained in drug store(OTP). This problem is now going to be solved by a new finding on COX, which have two different types, one is constitutionally expressed COX 1 in almost all organs and the other is inducible COX 2. COX 2 is the responsible enzyme in inflammation etc and now the search of COX 2 specific inhibitors is the target of R&D of next generation NSAID.

Recognition and attitude to functional division between physicians and pharmacists of practising physicians and pharmacists in Taegu city (대구시 개원의사와 개국약사의 의약분업에 대한 인식과 태도)

  • Mail questionnaire was administrated to 370 practising physicians and 388 pharmacists in Taegu city selected by systematic sampling to examine utilization states and opinion of pharmacy under medical care insurance programme and the attitude to the functional division between physicians and pharmacists from April to May 1992. Regarding the opinion on the outcome of drug-store under medical insurance, 71.2 percent of practicing physician answered faliure but 13.4 percent of practicing pharmacists answered failure in contrast. Fifty percent of practicing physician asserted introducing functional division between physician and pharmacist while 66.9 percent of practicing pharmacist answered drug-store under medical insurance itself is sucessful programme. Average daily numbers of preparation of medicine was 32.2 case. Percentage of utilization of drug-store under medical issurance to average daily cases of preparing of medicine was 20 percent, percentage of utilization with physician's prescription was 0.7 percent. And 58.7 percent of practicing physician experienced outside the institute prescription. Regarding the opinion on the pros and cons of enforcing functional division between physician and pharmacist, 59.2 percent of practicing physician prefered pros and 17.7 percent cons, but 38 percent of practicing pharmacist prefered pros and 45.5 percent cons. And pharmacist knew better the content of functional division between physician and pharmacist than physician. As a reason for pros of enforcing functional division between physician and pharmacist, practicing physician emphasized to prevent misuse or abuse of medicine but practicing pharmacist emphasized to display physician and pharmacist's professional ability. And as an opinion on implementation style of functional division between physician and pharmacist in pros respondents, practicing physician favored mandatory enforcement (52.3%), while practicing pharmacist favored partial incomplete functional division (81.7%). As the method of prescription if functional division between physician and pharmacist will be enforced, both practicing physician and pharmacist prefered generic name (44.0%, 89%) mostly, but physician prefered brand name (35.3%) secondly. Regarding the reason for not implementing functional division between physician and pharmacist up to date, both physician and pharmacist answered problem of business right between physician and pharmacist, followed by lack of recognition, and interest of people and lack of the govermental willness. Regarding the opinion on prior decision of condition for enforcing functional division between physician and pharmacist, practicing physician and pharmacist named uneven distribution of medical facilities and drug-store between rural and urban, inequality of physician and pharmacist manpower and the problem of manpower demand and supply mostly, and practicing physician pointed out establishing attitude of acceptance on the part of pharmacist and practicing pharmacist favored establishing attitude of acceptance on the part of physician, which was different attitudes between physician and pharmacist. Following conclusion was reached ; 1. Current drug-store under medical insurance program yield insufficient outcome, so we should consider program conversion from drug-store under medical insurance program to functional division between physician and pharmacist. 2. There were problem of business right and conflicts between physician and pharmacist at enforcing functional division between physician and pharmacist, so the goverment should search for formulating plan to resolve the problem and have neutral willness for the protection of the national health.

  • To maximize the services of phatmacists, we studied the best national liscensing examination for improvement of pharmacist. Among 650 Pharmacist who are posed the questions, 282 pharmacist such as professor in college of pharmacy(46), pharmacists in drug store and hospital (136), administrators in The Ministry of The Health and Welfare(48) and researchers and phamacists in pharmaceutical company (52) replied it. By analyzing these questionares, the results were as follows: In the Korean national liscensing examination for medial care team, pharmacist has been examed over two times the subjects as compared with another national examinations for medical and public health liseneses and 6-year curriculum of medical school. Now, the national examination for pharmacy liscence have been tested the basic subjects such as pharmaceutical analytical chem-istry, medicinal chemistry, biochemistry, etc., but almost demander, pharmacist who has been engaged about over 80% in the drug stores and hospital pharmacy, want to have exam pharmacy practics which are clinical pharmacy, pathohysiology and pharma-cotherapy of diseases like The United State of America, but not basic chemisty and biology. If the Korean liscensing examination for pharmacy will be changed like America, pharmacy education in the college of phar-macy will be progressed in detail. There are no clerkships and internships in the pharmaceutical education program in Korea, and so most of pharmacist want to have 6-year curriculum for pharmacy education and to test the pharmacy practice for Doctor of Pharmacy(Pharm. D.) but not basic science.

Monitoring of Tar Color Content in Children's snack and Its Exposure Assessment (어린이 기호식품 중 타르색소 모니터링 및 노출량 분석)

  • This survey was conducted to develop an appropriate management for safety of children snacks. In this study, monitorings of food additives such as nine kinds of tar colors (tartrazine, sunset yellow FCF, brilliant blue FCF, indigo carmine, new coccine, amaranth, erythrosine, allura red and fast green FCF) which are sold at stationary store around the school, were performed. Eighty two samples (3 snacks, 71 candies, 4 chocolates and 4 beverages) were analyzed for tar colors. Results of risk assessment for tar colors were expressed as EDI (Estimated Daily Intake) comparing with ADI (Acceptable Daily Intake). The ratio of high risk group for tar color intake (95th) were 0-3.56%. The consumptions of tar colors from domestic and imported products for nine kinds of tar colors in candies were not significantly different. The results of this study indicated that each ED! of nine kinds of tar colors sold at stationary store around the school is much lower than each ADI in general. Consequently, the children snacks are thought to be safe for consumption.

  • VIAGRATM, an oral therapeutic agent for erectile dysfunction, is the citrate salt of sildenafil. VIAGRATM is formulated as blue, film-coated rounded-dia-mond-shaped tablets, equivalent to 25mg, 50mg and 100mg of sildenafil for oral administration. ViagraTM has been allowed to be sold at the drug store in Korea officially, but it is still increased to sell or use counterfeits or smuggled goods, because of its high price or strict restriction on both sale and purchase. Discrimination and analysis of 13 cases of VIAGRATM tablets for verification of genuineness or counterfeit were requested to our institute on the period, from January 1999 to July 2003. (omitted)

Developing and Evaluating a Drug Information Leaflet of Antidiabetics for Senior Citizens; Employing Performance-based User-testing (노인용 당뇨치료제 안전사용설명서 개발 및 평가: 수행능력 기반 사용자 평가 활용)

  • Objective: The study purpose was to develop a drug information leaflet for the elderly and to evaluate it with performance-based user-testing. Methods: We performed a stratified randomized controlled trial. We recruited 62 elderly patients with age of 65 or above who were taking antidiabetic medications at the point of participating and excluded those who suffered illiteracy. We randomly allocated them into the intervention group with a leaflet for the elderly and the control group with a leaflet for the general public. Main outcome measures were to 'be able to find information' and to 'be able to understand information.' We measured outcome variables by employing performance-based user-testing and analyzed data to find any differences between two groups with t-tests, chi-squared tests or Fisher's exact tests accordingly. Results: More participants in the intervention group understood how to store their medications than those in the control group (intervention group 93% vs. control group 70%; p=0.02). There were no significant differences in other information items between two groups. Mostly 'being able to understand information' was lower than 'being able to find information.' The gaps between two outcome variables were about 10% in the intervention group and about 18% in the control group. The lowest understanding was observed in information relating to drug names and their potential adverse events. Conclusion: Without providing personalized drug information, it might be hard for the elderly to improve their drug knowledge even with leaflets that were developed specifically for the elderly.

Monitoring of Food Additives as an Artificial Sweetener on Favorite Foods of Children (어린이 기호식품의 인공감미료 함량 모니터링)

  • In this study, monitoring of food additives as an artificial sweetener on favorite foods of children, which are deal at retail store and stationery store around eliminatory school, was performed. We analyzed aspartame, potassium acesulfame, sodium saccharin, and cyclamate from candys, biscuits, chocolates and others. Total 604 items as targeted food were collected from the other country; 285 items of candys, 131 items of biscuits, 74 items of chocolates., 114 items of others. Targeted foods were classified by manufacture nation; 308 samples from domestic products, 211 from China, 26 from Indonesia, and 59 items from other nations. Artificial sweeteners were detected from 75 cases of food stuff which were 38 native product, 25 China, 9 Indonesia, and 3 others. The percentage of detected artificial sweeteners was aspartame 7.8% (47 cases), potassium acesulfame 3.0% (18 cases), sodium saccharin 1.8% (11 cases), and cyclamate 2.6% (16 cases). The detected concentration were followed [average(minimum-maximum) mg/kg]; aspartame 817 (21-4,988), potassium acesulfame 192 (24-1,136), sodium saccharin 91 (5-326), and cyclamate 926 (8-4,680). Aspartame was detected mainly on Korea foods, and cyclamate and sodium saccharin were detected from Indonesia food, artificial sweeteners were detected Chinese food, broadly. As a result, 17 cases were violated against regulatory standard of cyclamate, and sodium saccharin. Considering average body weight (36.9 kg) and daily intake of biscuits (15.6 g) for elementary student, ratio of estimated daily intake and acceptable daily intake was 0.86% for aspartame, 0.54% for potassium acesulfame, 0.77% for sodium saccharin, and 3.56% for cyclamate.

Illegal and Harmful Information Detection Technique Using Combination of Search Words (단어 조합 검색을 이용한 불법·유해정보 탐지 기법)

  • Illegal and harmful contents on the Internet has been an issue and been increased in Korea. They are often posted on the billboard and website of small enterprise and government office. Those illegal and harmful contents can relate to crime and suspicious activity, so, we need a detection system. However, to date the detection itself has been conducted manually by a person. In this paper, we develop an automated URL detection scheme for detecting a drug trafficking by using Google. This system works by analyzing the frequently used keywords in a drug trafficking and generate a keyword dictionary to store words for future search. The suspected drug trafficking URL are automatically collected based on the keyword dictionary by using Google search engine. The suspicious URL can be detected by classifying and numbering each domain from the collection of the suspected URL. This proposed automated URL detection can be an effective solution for detecting a drug trafficking, also reducing time and effort consumed by human-based URL detection.
