Stability analysis of slopes under groundwater seepage and application of charts for optimization of drainage design

  • Due to the seepage of groundwater, the resisting force of slopes decreases and the sliding force increases, resulting in significantly reduced slope stability. The instability of most natural slopes is closely related to the influence of groundwater. Therefore, it is important to study slope stability under groundwater seepage conditions. Thus, using a simplified seepage model of groundwater combined with the analysis of stresses on the slip surface, the limit equilibrium (LE) analytical solutions for two- and three-dimensional slope stability under groundwater seepage are deduced in this work. Meanwhile, the general nonlinear Mohr-Coulomb (M-C) strength criterion is adopted to describe the shear failure of a slope. By comparing the results with the traditional LE methods on slope examples, the feasibility of the proposed method is verified. In contrast to traditional LE methods, the proposed method is more suitable for analyzing slope stability under complex conditions. In addition, to facilitate the optimization of drainage design in the slope, stability charts are drawn for slopes with different groundwater tables. Furthermore, the study concluded that: (1) when the hydraulic gradient of groundwater is small, the effect on slope stability is also small for a change in the groundwater table; and (2) compared with a slope without a groundwater table, a slope with a groundwater table has a larger failure range under groundwater seepage.

  • Objective : This paper presents data from a retrospective study of endoscopic third ventriculostomy (ETV) in patients with shunt malfunction and proposes a simple and reasonable post-operative protocol that can detect ETV failure. Methods : We enrolled 19 consecutive hydrocephalus patients (11 male and 8 female) who were treated with ETV between April 2001 and July 2010 after failure of previously placed shunts. We evaluated for correlations between the success rate of ETV and the following parameters : age at the time of surgery, etiology of hydrocephalus, number of shunt revisions, interval between the initial diagnosis of hydrocephalus or the last shunt placement and ETV, and the indwelling time of external ventricular drainage. Results : At the time of ETV after shunt failure, 14 of the 19 patients were in the pediatric age group and 5 were adults, with ages ranging from 14 months to 42 years (median age, 12 years). The patients had initially been diagnosed with hydrocephalus between the ages of 1 month 24 days and 32 years (median age, 6 years 3 months). The etiology of hydrocephalus was neoplasm in 7 patients; infection in 5; malformation, such as aqueductal stenosis or megacisterna magna in 3; trauma in 1; and unknown in 3. The overall success rate during the median follow-up duration of 1.4 years (9 days to 8.7 years) after secondary ETV was 68.4%. None of the possible contributing factors for successful ETV, including age (p=0.97) and the etiology of hydrocephalus (p=0.79), were statistically correlated with outcomes in our series. Conclusion: The use of ETV in patients with shunt malfunction resulted in shunt independence in 68.4% of cases. Age, etiology of hydrocephalus, and other contributing factors were not statistically correlated with ETV success. External ventricular drainage management during the immediate post-ETV period is a good means of detecting ETV failure.

  • Burr hole drainage has been widely used to treat chronic subdural hematomas (SDH), and most of them are easily treated by simple trephination and drainage. However, various complications, such as, hematoma recurrence, infection, seizure, cerebral edema, tension pneumocephalus and failure of the brain to expand due to cerebro-cranial disproportion may develop after chronic SDH drainage. Among them, intracerebral hemorrhage after evacuation of a recurrent chronic SDH is very rare. Here, we report a fatal case of delayed intracerebral hemorrhage caused by coagulopathy following evacuation of a chronic SDH. Possible pathogenic mechanisms of this unfavorable complication are discussed and a review of pertinent literature is included.

Clinical Evaluation of Thoracic Empyema (농흉에 대한 임상적 고찰 - 109례 -)

  • One hundred and nine Patients with thoracic empyema were treated at the Chosun university hospital from Jul. 1983 to Sep. 1989. Seventy-nine[72.5%] of the empyemas were adults and 30[27.5%] patients were under fifteen-year children. 29 patients[26.6%] were associated with pulmonary tuberculosis, 23[21.1%] occurred as pneumonia, and 13[11.9%] were unknown. The cardinal symptoms were dyspnea, chest pain, fever, coughing. When used as the initial mode of drainage, repeat thoracentesis was successful in only 46 of 93 cases[49. 5%]. Rib resection, however, provided cure or controlled in 7 cases[100%]. And decortication showed high cure rate in 19 of 24 cases[79.2%] Eventual control or cure of empyema was achieved in 90 patients[89.6%], whereas 7 patients[6.4%] died [3 from their empyema and 4 with empyema as an active problem at the time of death]. of all empyema-caused deaths occurred in patients who underwent chest tube drainage as the most invasive treatment modality. Chest tube drainage was often inadequate and more aggressive management was likely to result in fewer treatment failure and fewer total procedure. Early rib resection was recommended.

  • Geosynthetics are widely used in landfill for filtration, drainage, separation and so on. For many landfill failure cases, interfaces of geosynthetics can be potential failure surfaces. Therefore, it is very Important to estimate the interface properties of geosynthetics. In this study, shaking table tests were peformed between smooth geomembrane and geotextile. From the test results, it was found that there is a limited acceleration that can be transmitted from smooth geomembrane to geotextile. And the influence of normal stress and frequencies of excitation were discussed.

  • A-61-year-old COPD patient was hospitalized due to dyspnea and was diagnosed with acute exacerbation of COPD. During the hospital stay, the patient's dyspnea was aggravated by massive spontaneous subcutaneous emphysema. Multiple 16 gauge intravenous catheters were inserted at the midclavicular line for drainage. Although subcutaneous catheter drainage was carried out, respiratory failure developed with an increased in massive subcutaneous emphysema. Continuous suction drainage with wall suction was applied resulting in the rapid resolution of the subcutaneous emphysema. We report a case of the that effective management of massive subcutaneous emphysema using multiple 16 gauge intravenous catheters with continuous suction.

Coil embolization of ruptured intrahepatic pseudoaneurysm through percutaneous transhepatic biliary drainage

  • A 75-year-old man with chronic cholangitis and a common bile duct stone that was not previously identified was admitted for right upper quadrant pain. Acute cholecystitis with cholangitis was suspected on abdominal computed tomography (CT); therefore, endoscopic retrograde cholangiopancreatography with endonasal biliary drainage was performed. On admission day 5, hemobilia with rupture of two intrahepatic artery pseudoaneurysms was observed on follow-up abdominal CT. Coil embolization of the pseudoaneurysms was conducted using percutaneous transhepatic biliary drainage. After several days, intrahepatic artery pseudoaneurysm rupture recurred and coil embolization through a percutaneous transhepatic biliary drainage tract was conducted after failure of embolization via the hepatic artery due to previous coiling. After the second coil embolization, a common bile duct stone was removed, and the patient presented no complications during 4 months of follow-up. We report a case of intrahepatic artery pseudoaneurysm rupture without prior history of intervention involving the hepatobiliary system that was successfully managed using coil embolization through percutaneous transhepatic biliary drainage.

Safety of endoscopic ultrasound-guided hepaticogastrostomy in patients with malignant biliary obstruction and ascites

  • Background/Aims: Endoscopic ultrasound (EUS)-guided hepaticogastrostomy (EUS-HGS) is useful for patients with biliary cannulation failure or inaccessible papillae. However, it can lead to serious complications such as bile peritonitis in patients with ascites; therefore, development of a safe method to perform EUS-HGS is important. Herein, we evaluated the safety of EUS-HGS with continuous ascitic fluid drainage in patients with ascites. Methods: Patients with moderate or severe ascites who underwent continuous ascites drainage, which was initiated before EUS-HGS and terminated after the procedure at our institution between April 2015 and December 2022, were included in the study. We evaluated the technical and clinical success rates, EUS-HGS-related complications, and feasibility of re-intervention. Results: Ten patients underwent continuous ascites drainage, which was initiated before EUS-HGS and terminated after completion of the procedure. Median duration of ascites drainage before and after EUS-HGS was 2 and 4 days, respectively. Technical success with EUS-HGS was achieved in all 10 patients (100%). Clinical success with EUS-HGS was achieved in 9 of the 10 patients (90%). No endoscopic complications such as bile peritonitis were observed. Conclusions: In patients with ascites, continuous ascites drainage, which is initiated before EUS-HGS and terminated after completion of the procedure, may prevent complications and allow safe performance of EUS-HGS.

Clinical Experience of Acute Pericarditis with Effusion (삼출액을 동반한 급성심낭염의 임상적 고찰)

  • Clinical experience of 48 acute pericarditis with effusion was reviewed and presented. There were 28 male and 20 female patients ranging from 3 years to 77 years old. Malignant effusion; Twenty patients had underlying malignancy. These etiologies were lung ca[8 patients, 40%], breast ca[7 patients 35%], lymphoma[2 patients, 10%], esophageal ca[1 patients, 5%], stomach ca[1 patient, 5%], ovarian ca[1 patient, 5%]. Uremic effusion; 15 patients with renal failure required surgical intervention. Traumatic effusion; 7 patients had traumatic pericarditis. These etiologies were stab wound [5 patients, 71.4%] and aspiration[2 patients, 28.6%]. Pyogenic effusion: 6 patients had pyogenic pericarditis. These etiologies were empyema thoracis[3 patients, 50%], liver abscess[2 patients, 33.3%], pneumonia[1 patient, 16.7%]. The patients were treated by pericardiocentesis, subxiphoid tube drainage, pericardiectomy: 4 of them underwent pericardiocentesis; 37, subxiphoid tube drainage; 5, pericardiectomy. We conclude that subxiphoid tube pericardial drainage was effective for treatment of pericardial effusion.
