GEANT4-based Monte Carlo Simulation of Beam Quality Correction Factors for the Leksell Gamma Knife® PerfexionTM

  • With the publication of TRS-483 in late 2017 the IAEA has established an international code of practice for reference dosimetry in small and non-standard fields based on a formalism first suggested by Alfonso et al. in 2008. However, data on beam quality correction factors ($k^{f_{msr},f_{ref}}_{Q_{msr},Q_0}$) for the Leksell Gamma $Knife^{(R)}$ $Perfexion^{TM}$ is scarce and what little data is available was obtained under conditions not necessarily in accordance with the IAEA's recommendations. This study constitutes the first systematic attempt to calculate those correction factors by applying the new code of practice to Monte Carlo simulation using the GEANT4 toolkit. $k^{f_{msr},f_{ref}}_{Q_{msr},Q_0}$ values were determined for three common ionization chamber detectors and five different phantom materials, with results indicating that in most phantom materials, all chambers were well suited for reference dosimetry with the Gamma $Knife^{(R)}$. Similarities and differences between the results of this study and previous ones were also analyzed and it was found that the results obtained herein were generally in good agreement with earlier PENELOPE and EGSnrc studies.

Comparison of Air Kerma and Absorbed Dose to Water Based Protocols for High Energy Photon Beams: Theoretical and Experimental Study

  • New types of protocols have been recently in development, all based on an absorbed dose-to-water with the aim of improving the accuracy of measurements of absorbed dose to water. IAEA TRS-277, the air-kerma standard-based present protocol, and IAEA TRS-398 and AAPM TG-51, the absorbed dose-to-water standard-based new one, were studied and compared theoretically and experimentally for photon beams of 6, 10, and 15 MV. NE 2571 and 3 Farmer types of ionization chambers in widely commercial use were used to determine an absorbed dose to water at the reference depth in water. Two different kinds of calibration factors were given respectively for every chamber calibrated in $\^$60/CO gamma ray beams from a Korean Secondary Standard Dosimetry Laboratory (KFDA). This work shows that there is around 1 % of difference of absorbed doses measured between two different types of calibration systems owing to different physical parameters and reference conditions used. We hope this work to help form the basis on development of new type of protocol in Korea.

  • I. Objective and Importance of the Project We have been using MC-50 cyclotron and NT-50 neutron therapy machine for treating cancer patients since 1986 at Korea Cancer Center Hospital. It is mandatory to measure accurately the dose distribution and the total absorbed dose of fast neutron for putting it to the clinical use. At present the methods of measurement of fast neutron are proposed largely by American Associations of Physicists in Medicine (Task Group 18), European Clinical Neutron Dosimetry Group, and International Commission on Radiation Units and Measurements. The complexity of measurement, however, induce the methodological differences between them. In our study, therefore, we tried to establish a unique technique of measurement by means of measuring the emitted doses and the dose distribution of fast neutron beam from neutron therapy machine, and to invent a standard method of measurement adequate to our situation. II. Scope and Contents of the Project For establishing a unique technique of measurement and inventing a standard method of measurement of fast neutron beam, 1. to grasp the physical characteristics of neutron therapy machine 2. to study the principles for measrement of fast neutron beam 3. to get the dose distribution (dose rate, percent-depth dose, flatness etc) throught the actual measurement 4. to compare our data with those being cited world-widely.

An innovative idea for developing a new gamma-ray dosimetry system based on optical colorimetry techniques

  • Obtaining knowledge of the absorbed dose up-taken by a certain material when it is exposed to a specific ionizing radiation field is a very important task. Even though there are a plenitude of methods for determining the absorbed dose, each one has its own strong points and also drawbacks. In this article, an innovative idea for the development of a new gamma-ray dosimetry system is proposed. The method described in this article is based on optical colorimetry techniques. A color standard is fixed to the back of a BK-7 glass plate and then placed in a point in space where the absorbed dose needs to be determined. Gamma-ray-induced defects (color centers) in the glass plate start occurring, leading to a degree of saturation of the standard color, which is proportional, on a certain interval, to the absorbed dose. After the exposure, a high-quality digital image of the sample is taken, which is then processed (MATLAB), and its equivalent $I_{RGB}$ intensity value is determined. After a prior corroboration between various well-known absorbed dose values and their corresponding $I_{RGB}$ values, a calibration function is obtained. By using this calibration function, an "unknown" up-taken dose value can be determined.

Stereotactic Radiosurgery

  • Stereotactic radiosurgery is one of the most sophisticated forms of modern advanced radiation therapy. Unlike conventional fractionated radiotherapy, stereotactic radiosurgery uses a high dose of radiation with steep gradient precisely delivered to target lesions. Lars Leksell presented the principle of radiosurgery in 1951. Gamma Knife® (GK) is the first radiosurgery device used in clinics, and the first patient was treated in the winter of 1967. The first GK unit had 179 cobalt 60 sources distributed on a hemispherical surface. A patient could move only in a single direction. Treatment planning was performed manually and took more than a day. The latest model, Gamma Knife® IconTM, shares the same principle but has many new dazzling characteristics. In this article, first, a brief history of radiosurgery was described. Then, the physical properties of modern radiosurgery machines and physicists' endeavors to assure the quality of radiosurgery were described. Intrinsic characteristics of modern radiosurgery devices such as small fields, steep dose distribution producing sharp penumbra, and multi-directionality of the beam were reviewed together with the techniques to assess the accuracy of these devices. The reference conditions and principles of GK dosimetry given in the most recent international standard protocol, International Atomic Energy Agency TRS 483, were shortly reviewed, and several points needing careful revisions were highlighted. Understanding the principles and physics of radiosurgery will be helpful for modern medical physicists.

물 흡수선량 표준에 기반한 선량교정 프로그램 개발 (Development of a Dose Calibration Program Based on an Absorbed Dose-to-Water Standard)

  • 최근 선량측정의 정확성을 향상시키기 위하여 기존의 공기커마 교정인수에 기반한 표준측정법에서 물 흡수선량 교정인수에 기반한 표준측정법으로 변환하고 있는 추세이다. 본 연구에서는 미국의학물리학회의 AAPM TG-51과 국제원자력기구의 IAEA TRS-398 표준측정법에 대한 선량교정 프로그램을 개발하였다. 이 프로그램은 윈도우 환경에서 사용이 용이하도록 비쥬얼 C++언어를 사용하여 각각의 표준측정법에서 권고하고 있는 방법 및 절차에 따라 사용자의 편의성을 고려하여 개발하였다. 이 프로그램은 이온함에 대한 정보와 물리적인 자료에 대한 표와 그래프 값들을 수식화하여 데이터베이스화함으로써 수작업으로 인해 발생할 수 있는 실수 및 오차를 줄일 수 있다. 개발된 프로그램의 활용을 통하여 국내 실정에 적합한 물 흡수선량 표준에 기반한 표준측정법 개발에 토대를 마련하는데 있어 기여할 것으로 사료된다.

A feasibility study of using a 3D-printed tumor model scintillator to verify the energy absorbed to a tumor

  • The authors developed a volumetric dosimetry detector system using in-house 3D-printable plastic scintillator resins. Three tumor model scintillators (TMSs) were developed using magnetic resonance images of a tumor. The detector system consisted of a TMS, an optical fiber, a photomultiplier tube, and an electrometer. The background signal, including the Cherenkov lights generated in the optical fiber, was subtracted from the output signal. The system showed 2.1% instability when the TMS was reassembled. The system efficiencies in collecting lights for a given absorbed energy were determined by calibration at a secondary standard dosimetry laboratory (kSSDL) or by calibration using Monte Carlo simulations (ksim). The TMSs were irradiated in a Gamma Knife® IconTM (Elekta AB, Stockholm, Sweden) following a treatment plan. The energies absorbed to the TMSs were measured and compared with a calculated value. While the measured energy determined with kSSDL was (5.84 ± 3.56) % lower than the calculated value, the energy with ksim was (2.00 ± 0.76) % higher. Although the TMS detector system worked reasonably well in measuring the absorbed energy to a tumor, further improvements in the calibration procedure and system stability are needed for the system to be accepted as a quality assurance tool.

  • Purpose: The objective of this study was to estimate the radiation absorbed doses in certain critical organs in the head and neck region with 16 imaging programs available on the Orthopos/sup (R)/ panoramic machine. Materials and Methods: A Rando phantom and LiF TLD chips were used for dosimetry. The absorbed doses were measured at the thyroid gland, the submandibular gland, the parotid gland, the mouth floor. the maxillary sinus, the brain, the mandibular body, the mandibular ramus. the 2nd cervical spine and the skin over TMJ area. Results : The overall absorbed doses with imaging programs available on the Orthopos/sup (R)/ panoramic machine were much less than that of standard program (program 1) except program 8, 11, and 16. Generally, the absorbed doses to the bone marrow of the mandibular ramus and the parotid gland were high, but the absorbed doses to the bone marrow in the mandibular body, brain, maxillary sinus, and, especially, the thyroid gland were very low. Conclusion : The modified imaging programs available on the Orthopos/sup (R)/ panoramic machine can be effectively used in aspect of radiation protection.

Numerical Method for Exposure Assessment of Wireless Power Transmission under Low-Frequency Band

  • In this paper, an effective numerical analysis method is proposed for calculating dosimetry of the wireless power transfer system operating low-frequency ranges. The finite-difference time-domain (FDTD) method is widely used to analyze bio-electromagnetic field problems, which require high resolution, such as a heterogeneous whole-body voxel human model. However, applying the standard method in the low-frequency band incurs an inordinate number of time steps. We overcome this problem by proposing a modified finite-difference time-domain method which utilizes a quasi-static approximation with the surface equivalence theorem. The analysis results of the simple model by using proposed method are in good agreement with those from a commercial electromagnetic simulator. A simulation of the induced electric fields in a human head voxel model exposed to a wireless power transmission system provides a realistic example of an application of the proposed method. The simulation results of the realistic human model with the proposed method are verified by comparing it with the conventional FDTD method.
