IMRT optimization on multiple slice using gradient based algorithm (Gradient based algorithm을 이용한 multiple slice IMRT optimization)

  • IMRT optimization method on multiple slice has been developed by using gradient based algorithm. On about 10-30 CT slices including treatment region of a patient, dose optimization has been performed slice by slice to meet the condition that each organ should be exposed below maximum tolerable doses and that the tumor dose within the range of 100$\pm$5 %. Field size was limited to 8$\times$8 cm$^2$ and in this condition, beam divergence was not taken into account to calculate dose distribution. Total dose distribution was calculated by superposing each beamlet whose dose distribution had been precalculated. In order to investigate beam number dependency, dose optimization was performed for one, three, five, seven, and nine coplanar beams and then each optimization index was evaluated. It is found that optimization time was proportional to number of slices to be optimized, and the most efficient plan was obtained from the case of three-to-seven incident beams with respect to calculation time and optimization index. In conclusion, dose optimization of multiple slice was able to be obtained by repeating dose optimization of single slice under condition that the beam size is not too large to ignore beam divergence. And it turns out that result of dose optimization was so sensitive to the position of isocenter that some method to optimize isocenter position is needed to improve it.

Analysis of Physical Properties for Various Compositions of Reusable LMG and LCV Micelle Gel

  • In this study, we evaluated the reusable leuco malachite green (LMG) micelle gel properties dependent on various components of chemical concentration and compared with leuco crystal violet (LCV). The gels were delivered to 10, 20, 30, 40 and 50 Gy at 6 MV photon beam from linear accelerator and analyzed using spectrophotometry. We confirmed that the reusable LMG and LVC absorbance wavelength peak were made up at 630 nm and 600 nm respectively. The transparency of reusable LMG decreased with higher amount of trichloroacetic acid (TCAA) and lower reusable LMG dyes. 1 mM reusable LMG was the lowest transparency. The sensitivity was increased depending on lower trichloroacetic acid (TCAA) concentrations and the amount of suitable surfactant (Triton X-100), which was found to be 7 mM. However, we were not able to investigate sensitivity effects factor from reusable LMG dyes. The gel dosimeter containing 16 mM TCAA, 7 mM Triton X-100 gel dosimeter showed the highest sensitivity at $0.0021{\pm}0.0001cm^{-1}.Gy^{-1}$. The sensitivity of LCV was found to be higher than reusable LMG at $0.0037{\pm}0.0005cm^{-1}.Gy^{-1}$. The reusable LMG and LCV dose responses were shown to be $R^2=0.997$, $R^2=0.999$ respectively, as stable measurement results. Future research is necessary to improve dose sensitivity, dose rate dependency and gel fading with extensive chemical formulations.

Efficacy and Safety of Cyclosporine Therapy in Children with Nephrotic Syndrome (소아 신증후군에서의 Cycplosporine의 치료효과 및 안전성)

  • Although most children with idiopathic nephrotic syndrome respond to corticosteroid therapy, many responders show steroid dependency and frequent relapse. In these children, one of the major problems is the serious side effects resulting from continuous steroid therapy. Thus, this study was conducted to assess the therapeutic efficacy and safety of six-month cyclosporine treatment with the low-dose deflazacort therapy in children with nephrotic syndrome. Thirty children with steroid dependence (SD), frequent relapse (FR) and steroid resistance (SR) were enrolled in this study. They were treated with 6-month oral cyclosporine $(Cypol-N^{(R)})$ plus the low-dose deflazacort $(Calcort^{(R)})$ therapy at Samsung Medical Center from September 2002. The dosage of cyclosporine was started at 5 mg/kg/day and was monthly adjusted to maintain clinical remission and/or a trough blood level, while deflazacort dosage was reduced gradually. Clinical evaluation and monitoring of cyclosporine toxicity were performed every $2\sim4$ weeks. Outcomes were compared to the latest sir-month period of steroid only therapy before cyclosporine treatment. Student's t-test and ANOVA were used for statistical analysis. Out of 28 children with SD and FR, 23 $(82.1\%)$ sustained remission, and 5 $(17.9\%)$ experienced 1 or 2 relapses during therapy. Out of 2 children with SR, 1 child sustained remission, and 1 child showed no response. The mean duration of remission and occurrence of relapse were significantly improved (p <.0001). In addition, the mean dosage of steroid was significantly reduced (p=.003). Although a number of adverse effects occurred in this study, they were not so serious as to necessitate discontinuation of the therapy. No nephrotoxicity was observed. Twenty out of the 28 children who had been in remission relapsed after withdrawal of cyclosporine. Fifteen of these children showed relapse within a month. These results demonstrated that the combination of cyclosporine with the low-dose deflazacort was efficient and safe in children with SD and FR during the six-month treatment. However, further studies are necessary in order to resolve the problem of high relapse rate after discontinuation of cyclosporine.

  • Opuntia ficus-indca(Op) extract has been claimed to have several therapeutic properties in oriental medicine including anti-inflammatory and anti-rheumatoid arthritis effects. Little is known of its effect on the activation of immune cells such as T cells and macrophages. To evaluate the functional effect of Op extract on immune cells, we examined whether Op extract stimulates the proliferation of T cells and the secretion of cytokines including IL-1 beta, IL-6 and tumor necrosis factor-alpha in THP-1 cell lines by RT-PCR. Op extract significantly enhanced the proliferation of T cell clone(D10S). Transcription of cytokines including IL-1 beta, IL-6, and TNF-alpha peaked 6 hrs after exposure to Op extract(100g/ml) in the THP-1 cell line and declined and declined thereafter. In an experiment to test the dose dependency of transcription of cytokines, transcription increased at a dose of 10 g/ml and the maximum expression was obtained at 100 g/ml, 6 hrs after exposure to Op extract. These findings suggest that Op extract is a potent stimulant of immune cells including T cells and macrophages, which acts by stimulating T cell proliferation and upregulating cytokines. These phenomena imply that some edible plants may be beneficial to living animals through the activation of immune functions.

Sedative Effect of Sublingual Midazolam after Oral Chloral Hydrate or Hydroxyzine in Children (소아 진정시 Chloral Hydrate, Hydroxyzine복용 후 Midazolam의 점막하 투여 용량에 따른 진정효과에 관한 연구)

  • Recently combining regimen of minimal doses of chloral hydrate, hydroxyzine and midazolam is good in sedation of children. Midazoalm, water soluble benzdiazepine, has rapid onset and relatively short duration of action. And midazolam has prospective amnesic effect. With these advantages midazolam is popular in conscious sedation for children. This study was to reveal the dose-dependency of behavior and physiologic effects of sublingual midazolam. Sedation records were surveyed retrospectively, of which the patients admitted from April, 2005 to July, 2007. we assigned three groups according the dose of midazolam, 0.1 mg/kg, 0.2 mg/kg and 0.3 mg/kg, respectively and the behavioral evaluation was analyzed with Houpt scale statistically. Combined sublingual midazolam increased the success rate in sedation and the vital signs were stable during sedation.

Inducing Effects of Rubus coreanus on Cell Death and Apoptotic Gene Expressions in Human Breast Cancer Cells (복분자의 유방암 세포 사멸 및 사멸 유전자 발현 유도 효과)

  • We investigated the inducing effects of Rubus coreanus extract (RCE) on apoptosis and its related gene expressions in human breast cancer cells. MDA-MB-231 cells were cultured in the presence of 0, 200, 300, and $400{\mu}g/mL$ RCE for 24h. MTT assay demonstrated that relative cell viability measured a decrease in a dose-dependent manner (p<0.05). This dependency was also found in the increasing levels of cell death by a dual staining with Hoechst 33322 and propidium iodide (p<0.05). These close associations was also observed by different stages of apoptotic processes, as shown by an Apoptosis Detection Kit. To determine whether the alterations in such cell activities obtained above cause the induction of apoptotic genes, PT-PCR was performed expressions of both Bcl-2 and Bax mRNAs. The Bcl-2/Bax ratio which is an important indicator of apoptosis, was found to have significantly decreased dose dependence (p<0.05). Western blot analysis also demonstrated that Caspase-3 significantly increases in a dose-dependent manner (p<0.05) in addition to similar alterations of other proteins examined. Taken these results together, the ethanolic RCE used induces a reduction in cell viability along with increased membrane permeability. This leads to a precautious apoptotic process and, subsequently, cell death through the apoptotic pathway involving Bax and Caspase-3 in human breast cancer MDA-MB-231 cells.

Changes in Cardiovascular and Renal Functions by Temperature and Epinephrine in the Freshwater Turtle, Amyda Japonica (수온변화와 Epinephrine에 의한 자라의 심맥관계 및 신장기능의 변화)

  • 수온변화에 따른 심맥관계 및 신장기능의 변화와 생체실험을 통한 온도에 의한 adrenoceptor의 변형을 알아보기 위해, 무마취 자라에서 $18^{\circ}C$에서 $25^{\circ}C$로 수온을 증가시 나타나는 혈압, 심박동수 및 신장기능의 변화를 관찰하고, epinephrine 1 ug/kg과 10 ug/kg을 상이한 온도에 노출된 자라의 정맥내 투여하여 나타나는 효과를 비교하였다. 1) $18^{\circ}C$에서 $25^{\circ}C$로 수온을 증가시킴에 따라 심박동수는 현저히 증가하여 일정하게 유지되었으나, 혈압 및 혈장 renin 활성도는 변화하지 않았다. 온도증가에 의해 뇨량, 사구체여과율 및 전해질 배설량의 현저한 증가를 보였으나 90분부터는 서서히 감소하기 시작하였다. 2) 수온 $18^{\circ}C$에 노출된 자라에서 epinephrine은 dose-dependent한 양상으로 혈압 및 심박동수를 증가시켰으며, 다량의 epinephrine 투여시 작용시간은 현저히 연장되어 있었다. $25^{\circ}C$에 노출된 자라에서는 epinephrine에 의한 혈압상승 효과 및 심박동수 증가는 나타났으나, dose dependency나 작용시간의 차이는 발견할 수 없었다. 3) 동량의 epinephrine에 의한 혈압 및 심박동수의 증가효과는 $18^{\circ}C$$25^{\circ}C$에 노출된 자라에서 유의한 차이를 발견할 수 없었으나, $18^{\circ}C$에 노출된 자라에서 epinephrine의 작용시간 및 반감기가 현저히 연장되어 있었다. 4) Epinephrine 투여에 의해 뇨량, 사구체여과율 및 전해질 배설량의 증가를 관찰하였으며, 이는 dose-dependent 양상이었다. 그러나, 신장효과의 유의한 차이는 상이한 온도에 노출된 두 군에서 발견할 수 없었다. 이상의 결과로, 온도증가에 의한 이뇨 및 sodium 배설효과는 혈관이완에 의한 사구체여과율의 증가에 기인한 것으로 사료되며, 상이한 온도에 노출된 자라에서 epinephrine 효과의 차이를 발견할 수 없었던 것은 본 실험에서 가한 좁은 범위의 온도의 변화 내에서는 adrenoceptor의 변형이 나타나지 않을 것이라고 추론하였다. 그러나 저온에서의 epinephrine의 작용시간의 연장은 아마도 epinephrine의 파괴 효소의 활성도의 감소인 것으로 사료된다.

Toxicity of Methylcyclohexane and Its Effect on the Reproductive System in SD Rats

  • Objectives: There is limited data regarding the toxicity of methylcyclohexane, despite its wide use in rubber adhesives, paint diluents, and cleansing agents. This study aimed to verify the toxicity and influence on the reproductive system of methylcyclohexane after its repeated injection in Sprague Dawley (SD) rats. Methods: Methylcyclohexane was injected subcutaneously into male and female SD rats once a day, five times a week, for 13 weeks at different doses (0, 10, 100, and 1,000 mg/kg/day) for each group. The toxicity of testing material was verified by observing the change in body and organ weight, hematological change, pathological findings, and effect on the reproductive system at each different concentration. Results: In the 1,000 mg/kg/day group, there were cases of animal deaths. In animals that survived, hematological changes, including a decrease in the red blood cell count, were observed. A considerable weight gain or loss and pathological abnormalities in the liver, kidney, and other organs were found. However, the 10 and 100 mg/kg/day groups did not cause deaths or other specific abnormalities. In terms of reproductive toxicity, there were changes in hormone levels, including a significant decrease in hormones such as estradiol and progesterone (p < 0.001) in male animals. Menstrual cycle change for female animals did not show concentration dependency. Conclusion: When injected repeatedly for 13 weeks, methylcyclohexane proved to be toxic for the liver, heart, and kidney at a high dose. The absolute toxic dose was 1,000 mg/kg/day, while the no observed adverse effect level was less than 100 mg/kg/day. The substance exerted little influence on the reproductive system.

Effect of Cyclic Nucleotides on Phorbol Ester-Induced Contraction in Rabbit Carotid Artery

  • This study was designed to clarify the action of cyclic nucleotides, cyclic AMP and cyclic GMP, on phorbol 12,13-dibutyrate (PDBu)-induced contraction in rings isolated from rabbit carotid artery. Arterial rings, 2 mm in width, were myographied isometrically in an isolated organ bath. PDBu produced slowly developing, sustained contraction in rabbit carotid artery, in a dose dependent manner, which was independent of extracellular $Ca^{2+}$ PDBu-induced contraction was relaxed by staurosporine, which suggests that PDBu-induced contraction is mediated by protein kinase C (PKC). $^{45}Ca^{2+}$ uptake by rabbit carotid artery was increased by PDBu during depolarization, but not in control. Isoproterenol and sodium nitroprusside (SNP) relaxed phenylephrine-induced contraction. However, SNP but not isoproterenol relaxed the contraction induced by PDBu. Acetylcholine relaxed PDBu-induced contraction in the presence of the endothelium. 8-bromo-cyclic AMP, a permeable analogue of cyclic AMP, suppressed phenylephrine-induced contraction but not PDBu-induced contraction. 8-bromo cyclic GMP relaxed both of them with dose dependency. A large dose of forskolin relaxed PDBu-induced contraction. PDBu increased cyclic AMP without considerable change in the level of cyclic GMP. Based on these findings, PDBu-induced contraction of rabbit carotid artery was relaxed by cyclic GMP more effectively than cyclic AMP, and the action of cyclic AMP could be mediated by cyclic GMP dependent protein kinase. Therefore it is suggested that the antagonistic action between protein kinase C and cyclic GMP-dependent protein kinase plays a major role in the regulation of vascular tone.

Lymphoblastosis Inhibition and Plaque-forming Cell Response of Several Anti-inflammatory Steroids in Mice

  • Anti-inflammatory glucocorticoid (GC) derivatives have been clinically used in immune-malfunctional diseases for their immunosuppressive activity. However, there is still a lack of knowledge on the relationship between anti-inflammatory and immunosuppressive activities. In order to compare immunosuppressive activities with the known anti-inflammatory activities of the GC derivatives, eight clinically used GC derivatives including hydrocortisone, prednislone, 6$\alpha$-methyl prednisolone, triamcinolone, dexamethasone, betamethasone, triamcinolone acetonide and fluocinolone acetonide were selected, and lymphoblastosis inhibition and plaque-forming cell (PFC) response in mice were studied as immunological parameters. In Con A-induced lymphoblatosis inhibition invitro, all derivatives showed potent inhibition $IC_{50}$ values of the derivaties except methyl prednisolone and triamcinolone were less than $10^{-7}$M and good dose dependency was obtained. This result was well correlated with that of their anti-inflammatory potencies obtained. This result was well correlated with that of their anti-inflammatory potencies and their receptor binding affinities. However, in PFC response, consistent result were not obtained. Total numbers of PFCs per spleen were decreased by some derivatives, but numbers of PFCs per $10^6$ cells were not decreased by systemic administration of but numbers of PFCs per $10^6$ cells were not decreased by systemic administration of GC at the dose of 0.05 mg/mouse. Furthermore, at the dose of 0.1 mg/mouse, numbers of PFCs per $10^6$ cells were found to be increased, although total PFCs per spleen were decreased.
