The success of radioation therapy depends on exact treatment of the tumor with significant high dose for maximizing local control and excluding the normal tissues for minimizing unwanted complications. To achieve these goals, correct estimation of target volume in three dimension, exact dose distribution in tumor and normal critical structures and correction of tissue inhomogeneity are required. The effect of therapy oriented CT (plannng CT) were compared with conventional simulation method in necessity of planning change, set dose, and proper distribution of tumor dose. Of 365 new patients examined, planning CT was performed in 104 patients $(28\%)$. Treatment planning was changed in $47\%$ of head and neck tumor, $79\%$ of intrathoracic tumor and $63\%$ of abdmonial tumor. in breast cancer and musculoskeletal tumors, planning CT was recommended for selection of adequate energy and calculation of exact dose to critical structures such as kidney or spinal cord. The average difference of tumor doses between CT planning and conventional simulation was $10\%$ in intrathoracic and intra-abdominal tumors but $20\%$ in head and neck tumors which suggested that tumor dose may be overestimated in conventional simulation Although some limitations and disadvantages including the cost and irradiation during CT are still criticizing, our study showed that CT Planning is very helpful in radiotherapy Planning.

Purpose: Even if the wedge filter is widely used for the radiation therapy to modify the photon beam intensity, the wedged photon beam dose calculation is not so easy. Radiation therapy planning systems (RTPS) have been used the empirical or semi-analytical methods such as attenuation method using wedge filter parameters or wedge filter factor obtained from measurement. However, these methods can cause serious error in penumbra region as well as in edge region. In this study, we propose the dose calculation algorithm for wedged field to minimize the error especially in the outer beam region. Materials and Method: Modified intensity by wedge filter was calculated using tissue-maximum ratio (TMR) and scatter-maximum ratio (SMR) of wedged field. Profiles of wedged and non-wedged direction was also used. The result of new dose calculation was compared with measurement and the result from attenuation method. Results: Proposed algorithm showed the good agreement with measurement in the high dose-gradient region as well as in the inner beam region. The error was decreased comparing to attenuation method. Conclusion: Although necessary beam data for the RTPS commissioning was increased, new algorithm would guarantee the improved dose calculation accuracy for wedged field. In future, this algorithm could be adopted in RTPS.
Purpose : The objective of this study is to introduce our installation of a non-commercial 3D Planning system, Plunc and confirm it's clinical applicability in various treatment situations. Materials and Methods : We obtained source codes of Plunc, offered by University of North Carolina and installed them on a Pentium Pro 200MHz (128MB RAM, Millenium VGA) with Linux operating system. To examine accuracy of dose distributions calculated by Plunc, we input beam data of 6MV Photon of our linear accelerator(Siemens MXE 6740) including tissue-maximum ratio, scatter-maximum ratio, attenuation coefficients and shapes of wedge filters. After then, we compared values of dose distributions(Percent depth dose; PDD, dose profiles with and without wedge filters, oblique incident beam, and dose distributions under air-gap) calculated by Plunc with measured values. Results : Plunc operated in almost real time except spending about 10 seconds in full volume dose distribution and dose-volume histogram(DVH) on the PC described above. As compared with measurements for irradiations of 90-cm 550 and 10-cm depth isocenter, the PDD curves calculated by Plunc did not exceed $1\%$ of inaccuracies except buildup region. For dose profiles with and without wedge filter, the calculated ones are accurate within $2\%$ except low-dose region outside irradiations where Plunc showed $5\%$ of dose reduction. For the oblique incident beam, it showed a good agreement except low dose region below $30\%$ of isocenter dose. In the case of dose distribution under air-gap, there was $5\%$ errors of the central-axis dose. Conclusion : By comparing photon dose calculations using the Plunc with measurements, we confirmed that Plunc showed acceptable accuracies about $2-5\%$ in typical treatment situations which was comparable to commercial planning systems using correction-based a1gorithms. Plunc does not have a function for electron beam planning up to the present. However, it is possible to implement electron dose calculation modules or more accurate photon dose calculation into the Plunc system. Plunc is shown to be useful to clear many limitations of 2D planning systems in clinics where a commercial 3D planning system is not available.
One of the most challenging safety precautions for workers in dynamic, radioactive environments is avoiding radiation sources and sustaining low exposure. This paper presents a sampling-based algorithm, DL-RRT*, for minimum dose walk-path re-planning in radioactive environments, expedient for occupational workers in nuclear facilities to avoid unnecessary radiation exposure. The method combines the principle of random tree star ($RRT^*$) and $D^*$ Lite, and uses the expansion strength of grid search strategy from $D^*$ Lite to quickly find a high-quality initial path to accelerate convergence rate in $RRT^*$. The algorithm inherits probabilistic completeness and asymptotic optimality from $RRT^*$ to refine the existing paths continually by sampling the search-graph obtained from the grid search process. It can not only be applied to continuous cost spaces, but also make full use of the last planning information to avoid global re-planning, so as to improve the efficiency of path planning in frequently changing environments. The effectiveness and superiority of the proposed method was verified by simulating radiation field under varying obstacles and radioactive environments, and the results were compared with $RRT^*$ algorithm output.

The treatment planning and dosimetry of small fields for stereotactic radiosurgery with 10 MV x-ray isocentrically mounted linear accelerator is presented. Special consideration in this study was given to the variation of absorbed dose with field size, the central axis percent depth doses and the combined moving beam dose distribution. The collimator scatter correction factors of small fields $(1\times1\~3\times3cm^2)$ were measured with ion chamber at a target chamber distance of 300cm where the projected fields were larger than the polystyrene buildup caps and it was calibrated with the tissue equivalent solid state detectors of small size (TLD, PLD, ESR and semiconductors). The central axis percent depth doses for $1\timesl\;and\;3\times3cm^2$ fields could be derived with the same acuracy by interpolating between measured values for larger fields and calculated zero area data, and it was also calibrated with semiconductor detectors. The agreement between experimental and calculated data was found to be under $2\%$ within the fields. The three dimensional dose planning of stereotactic focusing irradiation on small size tumor regions was performed with dose planning computer system (Therac 2300) and was verified with film dosimetry. The more the number of strips and the wider the angle of arc rotation, the larger were the dose delivered on tumor and the less the dose to surrounding the normal tissues. The circular cone, we designed, improves the alignment, minimizes the penumbra of the beam and formats ball shape of treatment area without stellate patterns. These dosimetric techniques can provide adequate physics background for stereotactic radiosurgery with small radiation fields and 10MV x-ray beam.
The homogeneous dose planning is one of the most important roles in radiation therapy. But, it is not easy to obtain a homogeneous dose to paranasal sinus region including the ethmoidal sinus with conventional irradiation techniques. In this experimental study, the authors tried to get a homogeneous dose at PNS region, but the nasal cartirage does not exceed the tolerance dose, with anterior-posterior beam and two both lateral wedged beams. Used three fields were shielded with full thickness of blocks to preserve the eye-balls and with blocks of one half value layer to create a homogeneous dose at the whole treatment volume. The dose computations are based on the three dimensonal structure with modified scatter contributions of partial shielders and attenuated beams in 6 MV photon beams. The dose distributions of mid-plane is examined with Kodak verification films and teflon-embedded TLD rod (1 mm diameter and 6 mm length) to confirm the computed dose. In our study, the whole PNS regions have shown within $85{\%}$ of the resultant isodose curves with relatively homogeneous dose distribution. The results of dose computation and measurements are agree well within $5{\%}$ uncertainties.

Brachytherapy has a long history in the treatment of cancer. However, the treatment planning technique for brachytherapy has lagged somewhat behind the corresponding developments for external beam therapy as far as the imaging technique is concerned. Currently, the orthogonal-film-based treatment planning is performed at most institutions even though the CT-based planning is available. The aim of this study is to evaluate the CT-based vs. the orthogonal-film-based treatment planning in cervix cancer. The doses to point A, point B, rectum and bladder points according to ICRU 38 were calculated for the two methods above. In addition, the volumetric studies such as 3D dose computation and DVH were obtained for the CT-based planning. For the bulky tumor, the isodose lines of point A prescription were not fairly covered for the CTV. The CT -based dose planning can overestimate the maximum dose delivered to bladder and rectum by 30%. The CT-based planning has several advantages over the orthogonal-film-based such as 3D dose display, DVH, and more accurate target delineation. It is suggested that the prescription point in cervix cancer be revised especially for the bulky tumor.
With the recent development of static and dynamic modulated brachytherapy methods in brachytherapy, which use radiation shielding to modulate the dose distribution to deliver the dose, the amount of parameters and data required for dose calculation in inverse treatment planning and treatment plan optimization algorithms suitable for new directional beam intensity modulated brachytherapy is increasing. Although intensity-modulated brachytherapy enables accurate dose delivery of radiation, the increased amount of parameters and data increases the elapsed time required for dose calculation. In this study, a GPU-based CUDA-accelerated dose calculation algorithm was constructed to reduce the increase in dose calculation elapsed time. The acceleration of the calculation process was achieved by parallelizing the calculation of the system matrix of the volume of interest and the dose calculation. The developed algorithms were all performed in the same computing environment with an Intel (3.7 GHz, 6-core) CPU and a single NVIDIA GTX 1080ti graphics card, and the dose calculation time was evaluated by measuring only the dose calculation time, excluding the additional time required for loading data from disk and preprocessing operations. The results showed that the accelerated algorithm reduced the dose calculation time by about 30 times compared to the CPU-only calculation. The accelerated dose calculation algorithm can be expected to speed up treatment planning when new treatment plans need to be created to account for daily variations in applicator movement, such as in adaptive radiotherapy, or when dose calculation needs to account for changing parameters, such as in dynamically modulated brachytherapy.
Background: Radiation therapy is a key part of the combined modality treatment for Hodgkin's lymphoma (HL) and non-Hodgkin's lymphoma (NHL), which can achieve locoregional control of disease. The 3D-conformal radiation oncology can be extended-field (EFRT), involved-field (IFRT) and involved node (INRT). New techniques have resulted in a smaller radiation field and lower dose for critical organs such as lung heart and breast. Materials and Methods: In our research, we made a virtual simulation for one patient who was treated in four different radiotherapeutic techniques: mantle field (MFRT), EFRT, IFRT and INRT. After delineatiion we compared dose-volume histograms for each technique. The fusion of CT for planning radiotherapy with the initial PET/CT was made using Softver Xio 4.6 in the Focal program. The dose for all four techniques was 36Gy. Results: Our results support the use of PET/CT in radiation therapy planning. With IFRT and INRT, the burden on the organs at risk is less than with MFRT and EFRT. On the other hand, the dose distribution in the target volume is much better with the latter. Conclusions: The aim of modern radiotherapy of HL and NHL is to reduce the intensity of treatment and therefore PET/CT should be used to reduce and not increase the amount of tissue receiving radiation.
This paper presents a hybrid algorithm to solve the multi-objective path planning (MOPP) problem for mobile robots in a static nuclear accident environment. The proposed algorithm mimics a real nuclear accident site by modeling the environment with a two-layer cost grid map based on geometric modeling and Monte Carlo calculations. The proposed algorithm consists of two steps. The first step optimizes a path by the hybridization of improved ant colony optimization algorithm-modified A* (IACO-A*) that minimizes path length, cumulative radiation dose and energy consumption. The second module is the high radiation dose rate avoidance strategy integrated with the IACO-A* algorithm, which will work when the mobile robots sense the lethal radiation dose rate, avoiding radioactive sources with high dose levels. Simulations have been performed under environments of different complexity to evaluate the efficiency of the proposed algorithm, and the results show that IACO-A* has better path quality than ACO and IACO. In addition, a study comparing the proposed IACO-A* algorithm and recent path planning (PP) methods in three scenarios has been performed. The simulation results show that the proposed IACO-A* IACO-A* algorithm is obviously superior in terms of stability and minimization the total cost of MOPP.
