Double blind randomized control trial to evaluate the efficacy of ketoprofen patch to attenuate pain during venous cannulation

  • Background: Venipuncture pain is an uncomfortable suffering to the patient. It creates anxiety, fear and dissatisfaction. The ketoprofen transdermal patch is a proven treatment for musculoskeletal and arthritic pain. We planned this study to evaluate the efficacy of the ketoprofen patch to reduce venipuncture pain. Methods: Two hundred adult patients, aged 18-60 years, of either sex, ASA grade I or II, were enrolled. Presuming that therapy would decrease venipuncture pain by 30%, a power calculation with ${\alpha}=0.05$ and ${\beta}=0.80$ required enrollment of at least 24 patients into each group. However, 100 patients in each group were recruited. Group I (Control) received a placebo patch; Group II (Ketoprofen) received a 20 mg ketoprofen patch. A selected vein on the dorsum of the patient's non-dominant hand was cannulated with 18 g intravenous cannula 1 h after the application of the respective patch. Assessment of pain was done by a 10 cm visual analogue scale (VAS) of 0-10, where 0 depicts "no pain" and 10 is "the worst imaginable pain". The venipuncture site was assessed for the presence of skin erythema, swelling and rashes at 12 h, 24 h and at the time of decannulation. Results: Incidence of pain was 100% (94/94) in the control group as compared to 93% (85/91) in the ketoprofen group. The severity of the venipuncture pain was 6 (2) and 2 (2) for control and ketoprofen groups respectively (P < 0.05). Conclusions: Application of a ketoprofen patch at the proposed site of venipuncture one hour before the attempt is effective and safe for attenuating venipuncture pain.

Feasibility of Cultured Allogenic Keratinocyte Treatment for Third Degree Burns (3도 화상에 대한 동종 상피세포 치료제의 효과 보고)

  • Allogenic keratinocyte application is widely used for treatment of second degree burns. However, there is no significant body of report on application of allogenic keratinocyte to third degree burns. A geriatric patient visited our burn center showing second to third-degree burn on dorsum of her left hand. Considering the surface area and wound depth, surgery was indicated but her medical condition and age made the surgery high risk for a long operation. Therefore, chemical escharolysis, serial bedside debridement, and cultured allogenic keratinocyte (Kaloderm®, Tegoscience, Seoul, Korea) application was done. The wound was completely epithelialized after four rounds of Kaloderm® application. For third-degree burns where definitive surgical reconstruction is precluded due to medical comorbidity of the patient, we investigated the possibility of allogenic keratinocyte treatment which may allow to avoid high-risk anesthesia and surgery.

Prognosis of Full-Thickness Skin Defects in Premature Infants

  • Background In the extremities of premature infants, the skin and subcutaneous tissue are very pliable due to immaturity and have a greater degree of skin laxity and mobility. Thus, we can expect wounds to heal rapidly by wound contraction. This study investigates wound healing of full-thickness defects in premature infant extremities. Methods The study consisted of 13 premature infants who had a total of 14 cases of full-thickness skin defects of the extremities due to extravasation after total parenteral nutrition. The wound was managed with intensive moist dressings with antibiotic and anti-inflammatory agents. After wound closure, moisturization and mild compression were performed. Results Most of the full-thickness defects in the premature infants were closed by wound contraction without granulation tissue formation on the wound bed. The defects resulted in 3 pinpoint scars, 9 linear scars, and 2 round hypertrophic scars. The wounds with less granulation tissue were healed by contraction and resulted in linear scars parallel to the relaxed skin tension line. The wounds with more granulation tissue resulted in round scars. There was mild contracture without functional abnormality in 3 cases with a defect over two thirds of the longitudinal length of the dorsum of the hand or foot. The patients' parents were satisfied with the outcomes in 12 of 14 cases. Conclusions Full-thickness skin defects in premature infants typically heal by wound contraction with minimal granulation tissue and scar formation probably due to excellent skin mobility.

Correction of Syndactyly using Pentagonal Flap with Minimal Skin Graft (오각형피판과 최소한의 피부이식을 이용한 합지증의 교정)

  • Purpose: The key of treatment in syndactyly is to separate the fused digits safely, and to create a normal web space with enough cutaneous coverage. Despite many techniques have described the correction of syndactyly, skin graft still remains the annoying one. We designed the pentagonal flap from hand dorsum to reconstruct the web space reliably and try to minimize the need for skin graft. Methods: Between July 2003 and August 2005, six cases of syndactyly were corrected at UCLA Medical Center and Hallym University Sacred Heart Hospital using dorsal pentagonal flap for web space reconstruction and straight incisions for the sides of digits to minimize the need for skin graft. The proximal edge of the pentagonal flap was designed in V shape to allow for easy closure of the donor site after advancement. The pentagonal flap was advanced volarly with the underlying dermofat tissues to form a digital web. In some cases, skin defects were unavoidable and covered with full thickness skin graft from the inguinal area. Results: Syndactyly were seen in 4 cases of Apert syndrome, 1 postburn scar webbing with PIP joint contracture and 1 recurrence after the incomplete reconstruction. In all Apert syndrome, straight line incision was used along the sides of the fingers and skin graft was needed. But, in 2 cases of incomplete type, we could save the need for skin graft only for the correction of syndactyly. We could get a good looking web space without any complications such as flap or graft loss. Conclusion: As a modification of Sherif's V-Y dorsal metacarpal flap, we believe pentagonal flap could be one of the easiest and safest way to reconstruct the web space of syndactyly in functional and cosmetic standpoint.

Clinical Analysis on the 219 cases of Dyshidrotic Eczema (한포진 219례에 대한 임상적 고찰)

  • Objectives : The purpose of this study was to investigate the clinical manifestations and influences of occupation, concomitant allergic diseases, various aggravating factors and seasonal changes for patients with dyshidrotic eczema. Methods : A total of 219 patients who showed clinical manifestations of dyshidrotic eczema were included in this study. We retrospectively reviewed the clinical characteristics of all the patients. Results : 1. Among 219 patients, there were 76 male patients (34.7%), 143 female patients (65.3%), male to female ratio was 1: 1.88. The average age of the patients at admission was $31.9{\pm}9.8$ years, had a lot of 20s and 30s the distribution. 2. The average of duration of the admission to the clinic after the onset was $4.01{\pm}4.42$ years. 3. Common accompaning diseases dyshidrotic eczema patients have were allergic rhinitis(87 cases), and atopic dermatitis(55 cases), sweating(45 cases), and asthma(10 cases) in order. 4. The most commonly occurring sites for dyshidrotic eczema patients are fingers(60.3%), palm(53.4%), dorsum of hand(19.2%) and nails(5.5%) in order. 5. The most commonly contacted materials for dyshidrotic eczema patients are water(130 cases), cosmetics(40 cases) and rubber gloves(34 cases) in order. The aggravating factors of dyshidrotic eczema patients are emotional stress(110 cases), fatty food(22 cases) and alcohol(22 cases) in order. 6. The number of patients showing seasonal changes in symptoms was 111. The season with the most severe symptom was summer with 76 cases, which was statistically significant. Conclusions : This study demonstrated the various clinical characteristics of Korean patients with dyshidrotic eczema.

Primary Cutaneous Anaplastic Large Cell Lymphoma: A Case Report (수부 및 상지의 피부에 발생한 원발성 피부 무형성 대세포 림프종 증례보고 1례)

  • Purpose: Anaplastic large cell lymphoma, has the following three characteristics of a malignant lymphoma; 1) An irregular large nucleus, called pathologic atypical cells, 2) Eosinophilic cytoplasm, 3) Immunologically positive for Ki-1. Anaplastic large cell lymphoma occurs mostly in the lymph nodes, but about 40% has been observed to occur in other tissues. Skin is the one of the main sources of origin and it is called 'primary cutaneous anaplastic large cell lymphoma'. Methods: A 69-year-old male patient with an erythematous nodule, sized $1.5{\times}1.7cm$ on his right hand dorsum was excised under local anesthesia and on biopsy was diagnosed as 'Dermatofibrosarcoma Protuberans'. Three months after the local excision and biopsy, same natured mass reoccurred in the same region, and then spontaneous regressed after three weeks. However, metastatic large mass of $4.0{\times}5.0cm$, of same nature was observed on the elbow. The large mass was operated with wide excision and biopsy. Results: On final diagnosis, with an immunofluorescent stain with CD30(Ki-1), 'Primary cutaneous large cell lymphoma' was made. After follow up for three years, we did not observed recurrence and metastasis. Conclusion: We have reported that we have diagnosed primary cutaneous large cell lymphoma and treated without recurrence and metastasis.

  • We report a case of an accidental extravasation of contrast material. A large-volume extravasation occurred in an adult during spiral contrast-enhanced CT. The amount of contrast material extravasated was 47 ml. The patient had a swelling of the dorsum right hand. The extravasation injury site was determined by CT scanning. The extavasation case was examined using five separate display techniques: axial, multi planar reformation (MPR), maximum intensity projection (MIP), volume rendering, and shaded-surfaced display (SSD). This paper introduces extravasation with the CT and the three-dimensional appearance.

An Anatomic Study of the Extensor Tendons of the Human Hand (수배부의 신전건에 대한 해부학적 연구)

  • Purpose: Hands are the chief organs for physically manipulating the environment, using anywhere from the roughest motor skills to the finest, and since the fingertips contain some of the densest areas of nerve endings on the human body, they are continuously used organ with complex functions, and therefore, often gets injured. To prevent any functional loss, a detailed anatomical knowledge is required to have a perfect surgical treatment. Also it is necessary to have a thorough understanding of arrangements of the human extensor tendons and intertendinous connections when tenoplasty or tendon transfer is required. We performed a study of the arrangements of the human extensor tendons and the configuration of the intertendinous connections over the dorsum of the wrist and hand. Methods: A total of 58 hands from Korean cadavers were dissected. The arrangements of extensor indicis proprius, extensor digitorum communis, and extensor digiti minimi tendons and intertendinous connections were studied. Results: The most common distribution patterns of the extensor tendons of the fingers were as follows: a single extensor indicis proprius (EIP) tendon which inserted ulnar to the extensor digitorum-index (EDC-index); a single EDC-index; a single EDC-middle; a double EDC-ring; an absent EDC-little; a double extensor digiti minimi (EDM), a single EDC-index (98.3%), a single EDC-middle (62%), a double EDC-ring (50%), and an absent (65.5%) or a single (32.8%) EDC-little. A double (70.6%) EDM tendons were seen. Intertendinous connections were classified into 3 types: type 1 with thin filamentous type, type 2 with a thick filamentous type, and type 3 with a tendinous type subdivided to r shaped 3r type and y shaped 3y type. The most common patterns were type 1 in the 2nd intermetacarpal space, type 2 in the 3rd intermetacarpal space, and type 3r in the 4th intermetacarpal space. And in the present study, we observed one case of the extensor digitorum brevis manus (EDBM) on the boht side. Conclusion: A knowledge of both the usual and possible variations of the extensor tendon and the intertendinous connection is useful in the identification and repair of these structures.

Analysis of Skin Micro-relief on Healthy Korean Youth According to Sex (건강한 한국인 청년에서 성별에 따른 피부표면 미세지형 분석)

  • The skin is the largest organ of the integument system whose surface is closely related to many physiological and pathological conditions. The skin surface is closely related to functional status of the epidermis and dermis. The outermost surface of the skin shows characteristic pattern defined by small furrows are called micro-relief. The micro-relief change is according to physiological and pathological conditions of the skin. But, there is no data on health Korean skin micro-relief yet. This study presents some aspect of health Korean skin micro-relief from the twenties. The silicon replicas obtained from forearm, dorsum hand and finger were analyzed by profiles were accessed by stereoscopy. We measured density of furrow, width of furrow, and depth of furrow in micro-relief with sectioned silicon replica sample using stereoscope. Stereoscopic analysis showed that male group has a larger density of furrow and depth furrow in finger significantly. The densities of furrow were shown different not only between forearm and finger but also between finger and hand in male with female group. In conclusion, there were differences results between sex different and regional difference in skin replica profile. These quantitative data can be used for basis of further skin research for Korean.

Usefulness of Ultrasonography in Diagnosis of Small Foreign Bodies (체내 소형 이물질 진단에서 초음파 검사의 유용성)

  • Purpose: The purpose of this study is to evaluate the usefulness of ultrasonographic examination for diagnosis and removal of small sized foreign bodies, which invaded extremity but difficulty to find on physical examination and/or plane radiographs. Materials and Methods: Since March, 2009 to February, 2012, we performed preoperatively ultrasonography and operation in 9 cases of foreign bodies of hand or foot. Mean symptom duration periods was 32 months. The location were fingers in 5, palms in 2, hand dorsum in 1, heel in 1 case. In 2 cases, foreign bodies were seen on plane radiographs. In the remaining 7 cases, foreign bodies could be diagnosed by ultrasonography only. Using high resolution stick probe, we performed evaluation on size, location, character of foreign bodies and compared preoperative ultrasonographic findings to intraoperative ones. Results: The site of foreign bodies in preoperative ultrasonography corresponded well with intraoperative findings. Foreign bodies were glass particles in 5, plant thorns in 4, pencil lead in 1. Mean size was 3.9 mm (2~7 mm). Conclusion: Ultrasonography is usefull evaluation tool for diagnosis and treatment in whom residual foreign bodies were suspected.
