Background Even though the quality of medical and surgical care has improved remarkably over time, iatrogenic injuries that require surgical treatment including injuries caused by cast and elastic bandage pressure, extravasation, and dopamine-induced ischemia still frequently occur. The goal of this study was to estimate the incidence and analyze the distribution of iatrogenic injuries referred to our department. Methods A retrospective clinical review was performed from April 2006 to November 2010. In total, 196 patients (116 females and 80 males) were referred to the plastic surgery department for the treatment of iatrogenic injuries. We analyzed the types and anatomic locations of iatrogenic complications, along with therapeutic results. Results An extravasation injury (65 cases, 37.4%) was the most common iatrogenic complication in our study sample, followed by splint-induced skin ulceration, dopamine-induced necrosis, prefabricated pneumatic walking brace-related wounds and elastic bandage-induced wounds. Among these, prefabricated pneumatic walking brace-related complication incidence increased the most during the 5-year study period. Conclusions The awareness of the very common iatrogenic complications and its causes may allow physicians to reduce their occurrence and allow for earlier detection and referral to a plastic surgeon. We believe this is the first study to analyze iatrogenic complications referred to a plastic surgery department in a hospital unit.
We fabricated glucose oxidase (GOx)-modified biosensor for detection of glucose by physical immobilization of GOx after electrochemical polymerization of the conductive mixture monomers of the 3-thiophenecarboxylic acid (TCA) and thiophene (Th) onto ITO electrode in this study. We confirmed the successfully fabrication of GOx-modified biosensor via FT-IR spectroscopy, SEM, contact angle, and cyclic voltammetry. The fabricated biosensor has the detection limit of $0.1{\mu}M$, the linearity of 0.001-27 mM, and sensitivity of $38.75mAM^{-1}cm^{-2}$, respectively. The fabricated biosensor exhibits high interference effects to dopamine, ascorbic acid, and L-cysteine, respectively. From these results, the fabricated GOx-modified biosensor with long linearity and high sensitivity could be used as glucose sensor in human blood sample.
도파민은 카테콜아민류의 중요한 신경전달물질로서 부족하면 파킨스병과 정신분열증 등을 야기할 수 있다. 그러므로 조작이 비교적 간단하면서 감도가 우수한 분석법의 개발이 필요하다. 이에, 도파민에 대한 경쟁적인 효소면역분석법이 연구되었다. 경쟁적인 면역분석법의 분석감도는 일반적으로 두가지 요소에 의해 조절된다. 하나는 경쟁자의 특성과 농도이며, 다른 하나는 결합체, 즉 항체의 그것이다. 따라서, 경쟁자인 BSA-DA과 결합체인 항체-avidin 접합체의 최적화가 수행되었다. 두 접합체는 SATA와 SMCC를 이용한 dual heterobifunctional coupling법에 의해 합성되었으며, 최적화 과정을 통해 BSA-DA 접합체의 농도는 $6.66\;{\mu}g/mL$, 항체-avidin 접합체의 농도 $4.17{\times}10^{-10}\;M$로 결정되었다. 도파민에 대한 doseresponse curve와 calibration curve의 결과로써 도파민에 대한 검출 한계는 $2.3{\times}10^{-2}\;{\mu}g/mL$ 이고 검출 영역은 $1.0{\times}10^{-3}\;M\sim1.0{\times}10^{-7}\;M$ 이다. 직선성을 갖는 검출영역에서의 검정선을 얻은 결과 [Absorbance = -0.1098 log[DA]+0.0353 ($R^2$ = 0.9956)] 우수한 직선관계를 얻었다.
In this work, fabrication and electrochemical analysis of an individual multi-walled carbon nanotube (MWNT) electrode are carried out to confirm the applicability of electrochemical sensing. The reactive ion etching (RIE) process is performed to obtain sensitive MWNT electrodes. In order to characterize the electrochemical properties, an individual MWNT is cut by RIE under oxygen atmosphere into two segments with a small gap: one segment is applied to the working electrode and the other is used as a counter electrode. Electrical contacts are provided by nanolithography to the two MWNT electrodes. Dopamine is specially selected as an analytical molecule for electrochemical detection using the MWNT electrode. Using a quasi-Ag/AgCl reference electrode, which was fabricated by us, the nanoelectrodes are subjected to cyclic voltammetry inside a $2{\mu}L$ droplet of dopamine solution. In the experiment, RIE power is found to be a more effective parameter to cut an individual MWNT and to generate "broken" open state, which shows good electrochemical performance, at the end of the MWNT segments. It is found that the pico-molar level concentration of analytical molecules can be determined by an MWNT electrode. We believe that the MWNT electrode fabricated and treated by RIE has the potential for use in high-sensitivity electrochemical measurement and that the proposed scheme can contribute to device miniaturization.
We fabricated an electrochemical detector (ECD) to catalyze redox reaction efficiently by electrodepositing Prussian blue (PB) on the indium tin oxide (ITO) electrode. Capillary electrophoresis (CE) and amperometric method were used. We investigated the PB surface properties by topography from atomic force microscopy (AFM). Also PB film thickness calibration with respect to deposition time and voltage was used to get better PB surFace. The PB thin film of dense and smooth surface could catalyze redox reaction efficiently. Comparing with CE-ECD microchip using bare-lTO electrode, proposed CE-ECD microchip using PB deposited electrode has shown better sensitivity by determining the detected peak current from the electropherograms while the concentration of tested analyzes was maintained the same. It is verified that detection limit can be lowered for 0.01 mM of dopamine and catechol respectively.
전기화학검출기를 이용한 HPLC법으로 흰쥐의 뇌 부위별 신경전달물질의 함량을 측정하였고 이것을 이용하여 뇌기능 개선에 빈용되는 원지, 육두구, 산조인, 석창포, 상기생, 맥문동, 몰약의 메탄올 추출물이 에탄올 투여 흰쥐의 뇌 부위별 신경전달 물질의 함량변화에 미치는 영향을 검토하였다. 에탄올 투여에 의해 전피질의 dopamine (DA), 3,4-dihydroxyphenyl acetic acid (DOPAC) 및 serotonin (5-HT) 함량이, 해마의 5-HT 함량이 각각 생리식염수 투여군에 비해 증가하였고, 전피질의 ${\gamma}$-aminobutyric acid(GABA) 함량은 생리식염수 투여군에 비해 감소하였다. 에탄올 투여 흰쥐의 선조체와 전피질의 DA 함량은 생약 추출물의 투여로 증가되는 경향을 보였다. 특히, 몰약 및 상기생은 에탄을 투여 휜쥐의 전피질에서 DA함량은 유의적으로 증가시키는 반면, 5-HT 함량은 유의적으로 감소시키는 것으로 나타났다. 몰약, 육두구, 상기생은 에탄올 투여 흰쥐의 선조체에서 GABA 함량을 유의적으로 감소시켰다. 이러한 결과는 실험에 사용된 생약 추출물의 작용이 에탄올 투여 흰쥐에서 뇌 부위 및 신경전달물질에 대해 선택적으로 나타나고 있음을 보여준다.
흰쥐 뇌의 선조체로부터 catecholamine, indoleamine 및 대사산물을 동시분석할 수 있는 간편한 방법을 제시하였다. 기기는 ECD를 장착한 HPLC를 사용하였고, 고정상인 $C_{18}$ 컬럼에서, 이 동상은 10mM citric acid, 0.13mM $Na_4EDTA$, 0.58mM SOS를 함유하는 35mM sodium acetate buffer(pH 3.4) : MeOH=85:15가 최적이었다. 이 때 컬럼온도는 $30^{\circ}C$였다. 이 조건으로 dopamine(DA), 3, 4-dihydroxyphenyl acetic acid(DOPAC), homovanilic acid(HVA), 5-hydroxyindole acetic acid(5-HIAA), serotonin(5-HT) 및 noradrenaline(NA)을 미량까지 분석할 수 있었고, 검출한계는 각 성분에 대해 1회 주입당 2~10pg이었다. 이 방법으로 흰쥐의 주령 및 성별에 따른 선조 체내 생체아민과 대사산물의 함량을 비교하였다. 7주령 쥐에서 암컷은 수컷보다 DA와 5-HT의 함량이 유의성 있게 높은 것으로 나타났으며, 주령이 증가함에 따라 암컷에서 DOPAC의 함량이 유의적으로 증가하였다.
도파민과 세로토닌은 신경전달물질 및 호르몬으로서 미량이지만 체내에서 중요한 기능을 한다. 검체는 혈청과 24시간 뇨를 쓰며, 주로 HPLC-MS를 이용하여 검사한다. 본 연구에서는 항원-항체반응을 이용한 경쟁적 ELISA를 통해 도파민과 세로토닌을 검출하고자 하였다. $5{\mu}g/mL$ BSA conjugate를 96 well 표면에 고정시킨 뒤 각각 농도를 다르게 한 신경전달물질과 1차 항체를 넣어 반응시키고 HRP가 결합된 2차 항체와 TMB를 처리하여 흡광도를 측정하였다. 측정 결과를 토대로 회귀식과 $R^2$값을 구하여 신경전달물질에 대한 항체의 민감도와 신경전달물질 농도와 흡광도 사이의 상관관계를 판단하였다. 도파민은 1차 항체를 6,000배, 7,000배로 희석했을 때 $R^2=0.91$로 신경전달물질 농도와 흡광도의 상관관계가 가장 높게 나타났고, 세로토닌은 3,000배와 6,000배를 제외한 모든 희석배수에서 $R^2{\geq}0.90$의 높은 상관관계를 나타냈다. 도파민과 세로토닌 모두 $1.0{\times}10^{-7}M$ 이하의 저농도에서는 효과적으로 검출되지 않았기 때문에 참고치가 이보다 낮은 혈청 도파민 검출은 HPLC-MS 이용이 필요해 보이지만, 24시간 뇨 도파민과 혈청 및 24시간 뇨 세로토닌의 검출에는 competitive ELISA가 효과적일 것이라 사료된다. 보다 낮은 농도의 신경전달물질도 정확하게 검출하기 위해서는 추가 연구가 필요하다.
모세관 전기영동 및 전기화학적 검출 시스템을 마이크로 시스템에 적용하여 ITO 유리기판과 polydimethylsiloxane (PDMS)로 제작하였다. 제작된 모세관 전기영동 및 전기화학적 검출 시스템은 일회용으로 사용가능하며 전기화학적 검출에 아주 적합한 특성을 보인다. 모세관 전기영동 및 전기화학적 검출 시스템은 주입과 분리 채널 (80 ${\mu}m$ 폭 ${\sim}$ 40 ${\mu}m$ 깊이)을 가진 PDMS 층과 유리기판 위에 검출 전극으로 사용되는 ITO가 형성된 층으로 구성된다. PDMS 층과 ITO 유리 기판은 UV-$O_3$ cleaner를 사용하여 접합하였다. 완충용액은 10 mM 2-(N-morpholino)ethanesulfonic acid (MES)를 사용하였고 분석물질은 1 mM 농도의 dopamine과 1 mM 농도의 catechol을 사용하였다. 60 V/cm 전계로 주입 및 분리를 하였으며 작업전극과 기준전극 간의 전위는 +600 mV로 유지하며 분석물질의 농도에 비례하는 전류량법으로 측정하였다. 전기화학적 검출 회로는 천기영동 전계의 간섭으로부터 분리하였다. 10 mM MES 완충용액에서 바탕 전류의 크기가 ${\sim}$10 pA 일 때 측정전류 값은 10 nA이다. 측정된 피크 값은 기존의 Au 전극과 비교하여 선택성, 감도, 분해능이 유사한 특성을 보여준다.
The placebo effect, a response observed during the placebo arm of a clinical trial, is produced by the psychobiological action of the placebo as well as by other potential contributors to symptom amelioration such as spontaneous improvement, regression to the mean, biases, concurrent treatments, and study design. From a psychological viewpoint, there are many mechanisms that contribute to placebo effects, including expectations, conditioning, learning, and anxiety reduction. Placebo responses are also mediated by opioid and non-opioid mechanisms including dopamine, serotonin, cholecystokinin, and immune mediators. During recent years, a trend towards increased placebo effects in clinical trials of neuropsychiatric drugs has been noted. Indeed, the placebo effects observed in clinical trials constitute an increasing problem and interfere with signal-detection analyses of potential treatments. Several potential factors including protocol/study design and conduct related factors may account for the placebo effect observed in clinical trials. This paper reviews key issues related to this problem and aims to identify potential solutions.
