  • The influence of prazosin (0.1 mg/kg i.v.) on the excretion and diuretic action of furosemide (2mg/kg i.v.) in rabbits was studied to investigate an interaction between ${\alpha}-adrenergic$ blocking agent, prazosin and furosemide. The results were as follows; 1) With the combined administration of prazosin and furosemide, the plasma concentration of furosemide was increased, the urinary excretion rate and renal clearance of furosemide were reduced, and tile biological half-life of furosemide was increased. 2) The diuretic action of furosemide was significantly reduced with the combined administration of prazosin: maximal decrease in urine volume, urinary electrolytes, clearance of $Na^+$ and $Cl^-$, and GFR and RPF, as well as maximal increase in $Na^+$ reabsorption rate were noted 10 minutes after administration of furosemide (2mg/kg i.v.)

Changes of Potassium in Open Heart Surgery (개심술시의 포타시움 변화)

  • Alterations in the serum and urine potassium were studied in twenty patients who underwent open heart surgery in National Medical Center during the period from Jan.1978 to June 1978. There were twelve cases congenital heart disease and eight acquired heart disease. Rigg-Kyvsgaad mark IV roller pump and Polystan bubble oxygenator were used in all patients. Hemodilution was carried out by priming the oxygenator with Hartmann`s solution. Measurements were made of the serum electrolyte, gas analysis and twenty four hour urine electrolytes. During the bypass, the serum potassium decreased Significantly from 4.2?.47 to 3.6?. 72 mEq. per liter. [p<0.05] Fifty seven miliequivalant of potassium chloride were added during operation-urine potassium was higher in the diuretic group than in the non diuretic group. Plasma potassium level in the diuretic group was more significantly reduced than nondiuretic group. In this series large amount of urine potassium loss was noted on the day of operation, the first and second post operative day.

The Effects of Organic Acidic Drugs on the Absorption, Excretion and Diuretic Action of Furosemide (유기산성약물이 푸로세미드의 흡수, 배설및 이뇨효과에 미치는 영향)

  • The effects of organic acidic drugs on the absorption, excretion and diuretic action of furosemide were studied. Cefalexin, p-aminohippuric acid (PAH), ibuprofen and p-amino salicylic acid (PAS) were selected as organic acidic drugs. The in situ absorption rate and absorption rate constant of furosemide (30$\mu{M}$) were significantly (p<0.05) decreased by 30$\mu{M}$ of cefalexin, PAH, ibuprofen and PAS in rat small intestine. The plasma concentration of furosemide was significantly (p<0.01) increased by cefalexin, PAH and ibuprofen in rabbits. But the urinary excretion rate, renal clearance and diuretic action of furosemide were significantly (p<0.05) decreased by cefalexin, PAH, ibuprofen and PAS in rabbits.

  • In anesthetized dogs, antidiuretic action of intravenously administered BRL 34915 (10.0~30.0 $\mu$/kg) was blocked by renal denervation, whereas it was not affected by glibenclamide, a selective $K_{ATP}$ blocker, given into renal artery. Diuretic action in ipsilateral kidney produced by intrarenal administration of BRL 34915 was not influenced by renal denervation, but blocked completely by glibenclamide given into the vein. Above results suggest that the antidiuretic action of BRL 34915 is mediated by renal sympathetic nerves and the diuretic action is caused by opening of $K^+$ channel within kidney.

Improvement of Diagnostic Accuracy by Standardization in Diuretic Renal Scan (소아 이뇨 신장스캔에서 검사 표준화에 의한 폐쇄 진단 성능 향상)

  • We evaluated diagnostic accuracy of diuretic renal scan with standardization in 45 childrens(107 hydronephrotic kidneys) with 91 diuretic assessments. Sensitivity was 100%, specificity was 78%, and accuracy was 84% in 49 hydronephrotic kidneys with standardization. Diuretic renal scan without standardization, sensitivity was 100%, specificity was 38%, and accuracy was 57 % in 58 hydronephrotic kidneys. The false-positive results were observed in 25 cases without standardization, and in 8 cases with standardization. In diuretic renal scans without standardization, the causes of false-positive results were 10 early injection of lasix before mixing of radioactivity in pelvocalyceal system, 4 full bladder, 2 markedly dilated pelvocalyceal systems postpyeloplsty, 6 extrarenal pelvis, and 3 immature kidneys of neonates. In diuretic renal scans with standardization the causes of false-positive results were 2 markedly dilated systems postpyeloplsty, 2 extrarenal pelvis, 1 immature kidney of neonate, and 2 severe venal dysfunction, 1 vesicoureteral reflux. In diuretic renal scan without standardization the false-positive results by inadequate study were common, but false-positive results by inadequate study were not found after standardization. The false-positive results by dilated pelvocalyceal systems postpyeloplsty, extrarenal pelvis, and immature kidneys of neonates were not dissolved after standardization. In conclusion, diagnostic accuracy of diuretic renal scan with standardization was useful in children with renal outflow tract obstruction by improving specificity significantly.

Natural History of Unilateral Ureteropelvic Junction Obstruction : Five Cases of High Grade Hydronephrosis (신생아 편측 신우요관이행부 폐색의 자연경과 : 중증 수신증 5례)

  • Purpose: Unilateral ureteropelvic junction obstruction with high grade of hydronephrosis an obstructed diuretic renogram but good differential renal function were studied to evaluate the natural history of untreated obstruction. Materials and Methods: During the last three years, we encountered 18 kidneys with hydronephrosis detected on prenatal ultrasound. In all these patients other urologic abnormalities were excluded by voiding cystourethrography and other radiological studies. Diuretic renography was initially performed at an age ranging from 2 weeks to 4 weeks. Five had high degree of hydronephrosis and obstructed diuretic renogram despite good relative renal function. Periodic urine examination, ultrasonography and diuretic renogram were done at regular intervals. Results: We have followed 5 neonates with unilateral hydronephrosis and suspected ureteropelvic junction obstruction with noroperative treatment for 15 months. During followup, percentage and relative renal function were maintained in all patients and hydronephrosis improved. Obstructive patterns on diuretic renogram were variable. Conclusions: These findings help to define the natural history of untreated good functioning hydronephrotic kidneys. Many newborn kidneys with severe hydronephrosis are not obstructed despite even high grade of hydronephrosis and obstructed diuretic renogram.

Effect of Renal Denervation on Renal Action of Methoxyverapamil in Dogs (Methoxyverapamil의 신장작용에 미치는 신 신경제거의 영향)

  • In dogs, renal denervation did not affect the diuretic action accompanied with renal hemodynamic chanties and inhibition of electrolytes reabsorption rates in renal tubules by methoxyverapamil infused into the vein or into a renal artery, while renal denervation blocked the antidiuretic action due to the decreased free water and osmolar clearances along with the reduced sodium amounts excreted in urine by methoxyverpamil infused into the carotid artery. These experimental results suggest that methoxyverapamil may cause diuresis by direct action in kidney but the antidiuretic action through central function mediated by renal nerves.

  • The water extract of Chenopodium album L. var. centrorubrum had significant diuretic effect when administered orally in rats. The effect of combined administration of the extract with furosemide was slow and prolonged as compared with furosemide.

  • Pharmacological studies have been carried out with the methanol extract of the whole plant of Pseudostellaria palibiniana Ohwi (Caryophyllaceae). The results showed that it had stimulation effect on heart, hypotensive actions depending on extract contents, stimulation effect on respiration, contraction on excised intestines, enhancement of tension on excised uterus, antineoplastic activity on Ehrlich carcinoma, liver protective activity against $CCl_4$ intoxication, writhing and diuretic activities.

  • Ethylacetate fraction from Orobanche crenata, contained two phenylpropanoid glycosides, exhibited some pharmacological properties. It was found to be non-toxic to rats in oral doses up to 500mg/100gm body weight. In large doses, it lowered the arterial blood pressure of anaethetised rats, and produced significant analgesic effect in mice and diuretic effect in rats. It further showed smooth muscle relaxant and antispasmodic effects in the isolated rabbit intestine and guinea-pig ileum respectively.
