  • A 42-year-old Marfan female had a chronic dissection of the descending aorta[Type B and the aneurysmal expansion of the descending aorta caused compression of the trachea resulting in respiratory distress which mimicked bronchial asthma. The patient has been successfully managed by resection and replacement of the aneurysm in the descending aorta. The operation could be done without the aid of the partial cardiopumonary bypass. As the patient had been prepared by unilateral axillo-bifemoral by-pass using prosthetic graft 8mm in diameter 10days prior to the main operation.

A Case Study for the Estimation of Remaining Lives of Asphalt Pavements (아스팔트포장 잔존수명 예측 사례 연구)

  • This study presents a case study of condition evaluation of various asphalt pavement sections to estimate performance lives. The pavement surface conditions including cracking and rutting are first evaluated using a automatic pavement analyzer, ARAN. HPCI(Highway Pavement Condition Index) values are estimated using the pavement surface distress data. It is observed from the pavement distress survey that the major distress type of the sections is top-down cracking. The modulus value of each pavement layer is back-calculated from the defection data obtained from a FWD(Falling Weight Deflectometer) and compared with the laboratory measured dynamic modulus values. Remaining lives of the various pavement sections are estimated based on a mechanistic-empirical approach and AAHTO 1993 design guide. The structural capacities of the all pavement sections based on the two approaches are strong enough to maintain the pavement sections for the rest of design life. Since the major distress type is top-down cracking, the remaining lives of the pavement sections are estimated based on HPCI and existing performance database of highway pavements. To evaluate the causes of premature pavement distress, various material properties, such as air void, asphalt binder content, aggregate gradation, dynamic modulus and fatigue resistance, are measured from the field cores. It is impossible to accurately estimate the binder contents of field samples using the ignition method. It is concluded from the laboratory tests that the premature top down cracking is mainly due to insufficient compaction and inadequate aggregate gradation.

Patterns of commitment to Work Role and Fathering and the Level of Life Satisfaction among Married Men (기혼 남성의 아버지역할과 직업역할 몰입 유형화와 생활만족도)

  • The main purpose of this study is to examine the relation between different patterns of commitment to work role and fathering and the level of life satisfaction. 272 fathers, whose oldest child is under 12 years old with professional, managerial and clerical jobs, were selected. The major findings of this study were as follow: 1) Married men with the oldest child under 12 years old were more likely to be committed to fathering than to work role. 2) The significant variables that affected the level of commitment to fathering were the age of the oldest child, the frequency of participation in the meeting after work, parental satisfaction, and how they perceive the spousal support. And the significant variables that affected the level of commitment to work role were type of job, parental distress, work satisfaction, work distress. 3) Patterns of commitment to fathering and work role were categorized into four groups. The names were given as positively balanced commitment pattern, positive fathering commitment pattern, negative fathering commitment pattern, and totally negative commitment pattern. 4) The significant variables important in classifying the patterns of commitment to work role and fathering appeared to be the age of the oldest child, work distress, the frequency of participation in the meeting after work, parental satisfaction, how they perceive the spousal support, work satisfaction and weekend total working hour. 5) The balanced positive commitment to work and fathering was associated with the highest level of life satisfaction.

Study on the Validation of Sasang Urination Inventory (SUI) for Analyzing Pathophysiological Symptoms and Diagnosing Sasang Types (배뇨 소증 분석과 사상체질 진단을 위한 사상소변기능검사(SUI)의 타당화 연구)

  • Introduction The Sasang Urination Defecation Inventory was developed to analyze Sasang-type specific pathophysiological clinical symptoms, and its revised Sasang Urination Inventory (SUI) illustrated its clinical usefulness in Sasang type differentiation. However, the correlation with established clinical measures for its concurrent validity were not provided yet. Methods Total of 48 healthy university students were recruited to response Urogenital Distress Inventory-6 (UDI-6) and Overactive Bladder Symptom Score (OABSS) of Western medicine along with the SUI of traditional Korean medicine. The correlation coefficients between UDI-6 and OABSS and SUI were acquired with Pearson's correlation, and Analysis of Variance (ANOVA) and Profile Analysis were used to analyze significant differences in SUI subscale profiles of each Sasang types. Results The SUI-HSS of urogenital hypersensitivity correlated positively with OABSS (r=0.442, p<0.01), and the SUI-DIS of urinary discomfort positively with UDI-6 (r=0.289, p<0.05). Interestingly, the SUI-total was correlated positively with age (r=0.326), height (r=0.318) and weight (r=0.304). The SUI-DIS for urinary discomfort of So-Yang (9.48±2.11) type was significantly (p<0.05) higher than that of Tae-Eum (7.2±2.90) type. And SUI subscale profiles of each Sasang type were significantly (Wilks' Lambda=0.792, F=2.723, p=0.034) distinctive to each other. Discussion The acceptable convergent validity of SUI was shown using established measures of urogenital symptoms. The SUI might be used as an objective traditional Korean clinical measure for Sasang type differentiation and urological patients.

Application of Veno-venoarterial Extracorporeal Membrane Oxygenation in Multitrauma Patient with ARDS - A case report - (다발성 외상으로 유발된 급성호흡부전증후군 환자에서 정맥-정맥동맥 체외막 산화기(ECMO)의 적용 - 1예 보고 -)

  • Acute respiratory distress syndrome (ARDS) is difficult to treat and it is often fatal. If the medical treatment for ARDS is not effective, then extracorporeal membrane oxygenation (ECMO) can be applied to the patient. A 22-year-old female who suffered multiple traumatic injuries due to a car accident presented with acute respiratory distress syndrome. Veinarterial extracorporeal membrane oxygenation (VA ECMO) was started to treat her respiratory failure. With the VA ECMO, the systemic oxygen saturation remained at only 84%, and so the ECMO system was switched to V-VA ECMO via an additional venous outflow through the right jugular vein to increase both the systemic and pulmonary oxygen saturation. After conversion to the V-VA type ECMO, the systemic oxygen saturation increased to 94% and the partial pressure of oxygen ($PaO_2$) increased to 65 mmHg. We report here on a successful case of ECMO conversion from the VA type to the V-VA type in a patient with severely hypoxic respiratory failure.

Psychosocial Analysis of Cancer Survivors in Rural Australia: Focus on Demographics, Quality of Life and Financial Domains

  • Background: Cancer treatments can have long-term physical, psychological, financial, sexual and cognitive effects that may influence the quality of life. These can vary from urban to rural areas, survival period and according to the type of cancer. We here aimed to describe demographics and psychosocial analysis of cancer survivors three to five years post-treatment in rural Australia and also assess relationships with financial stress and quality of life domains. Materials and Methods: In this cross-sectional study, 65 participants visiting the outpatient oncology clinic were given a self-administered questionnaire. The inclusion criteria included three to five years post-treatment. Three domains were investigated using standardised and validated tools such as the Standard Quality of Life in Adult Cancer Survivors Scale (QLACS) and the Personal and Household Finances (HILDA) survey. Included were demographic parameters, quality of life, treatment information and well-being. Results: There was no evidence of associations between any demographic variable and either financial stress or cancer-specific quality of life domains. Financial stress was however significantly associated with the cancer-specific quality of life domains of appearance-related concerns, family related distress, and distress related to recurrence. Conclusions: This unique study effectively points to psychosocial aspects of cancer survivors in rural regions of Australia. Although the majority of demographic characteristics were not been found to be associated with financial stress, this latter itself is significantly associated with distress related to family and cancer recurrence. This finding may be of assistance in future studies and also considering plans to fulfil unmet needs.

Methodology of Field Investigation and Laboratory Test for Distresses of Old Concrete Pavements (노후 콘크리트 포장 파손에 대한 현장조사 및 실내시험 방법)

  • PURPOSES : The purpose of this study is to suggest a specific investigation guideline to decide priority of repairing old concrete pavements that pile up substantially. METHODS : In this study, a principle of division of homogeneous sections was proposed to reflect the characteristics of the pavement reasonably in the specific investigation results. In addition, a checklist and guideline of field investigation were suggested for the old concrete pavement sections, which require inspection toward their durability and structural performance. Furthermore, the items of laboratory test necessary to the old concrete pavement were suggested based on the existing laboratory test considering characteristics of the old concrete pavement. The present condition of the old concrete pavement could be analyzed by the test results. RESULTS : A method of division of homogeneous sections suitable for the specific investigation of the old concrete pavement was suggested. The proportions of distress severity of pavement sections were compared by distress type to figure out the present state of the old concrete pavement. Scaling, durability cracking (or alkali-silica reaction), and longitudinal spalling were selected as the most severe distress types. The detailed positions of the sections were also suggested. The checklist of the specific investigation was categorized by field survey and laboratory test, and its evaluation criteria were proposed. The three types of the sections of durability cracking (or alkali-silica reaction), bridge connection, and asphalt overlay were selected as the sections of the field survey. The compressive strength, void structure, and chloride penetration depth were suggested as the items of the laboratory test. CONCLUSIONS : A fundamental level of the guideline was suggested in this study to resolve the problem of old concrete pavement. Appropriate guidelines related to the repair of the old concrete pavement should be provided by performing additional research efforts.

Performance Evaluation of Long-Life Asphalt Concrete Overlays Based on Field Survey Monitoring in National Highways (일반국도 현장조사 모니터링을 통한 장수명 아스팔트 덧씌우기 포장의 공용성 분석)

  • PURPOSES : Performance evaluation of four types of asphalt concrete overlays for deteriorated national highways. METHODS : Pavement distress surveys for crack rate and rut depth have been conducted annually using an automated pavement survey vehicle since 2007. Linear and non-linear performance prediction models of the asphalt concrete overlays were developed for 43 sections. The service life of the asphalt overlays was defined as the number of years after which a crack rate of 30% or rut depth of 15mm is observed. RESULTS : The service life of the asphalt overlays was estimated as 17.4 years on an average. In 90.7% of the sections, the service life of the overlays was 15 years or more which is 1.5 times the life of conventional asphalt concrete overlays used in national highways. The performance of the overlays was dependent on the type of asphalt mixture, traffic volume levels, and environmental conditions. CONCLUSIONS : The usage of stone mastic asphalt (SMA) and polymer-modified asphalt (PMA) for the overlays provided good resistance to cracking and rutting development. It is recommended that appropriate asphalt concrete overlays must be applied depending on the type of existing pavement distress.

Congenital Cystic Adenomatoid Malformation of The Lung - A Case Report - (선천성 낭포성 선양기종: 1례 보고)

  • The congenital cystic adenomatoid malformation of the lung is a rare disease, and is one of the most common congenital lung diseases which require prompt surgical intervention. The prognosis depends on its tissue type, prompt diagnosis and surgical intervention. The lesion consists of enlarged, variable sized multiple cyst with overgrowth of terminal bronchioles, like hamartoma. This disease can be associated with other vascular anomalies or other congenital defect especially in type II lesion We recently experienced one case of congenital cystic adenomatoid malformation The patient was 2 months old infant who showed respiratory distress without associated anomaly. After right upper lobe lobectomy, the patient was recovered uneventfully.

  • Extracorporeal life support (ECLS) is a type of cardiopulmonary bypass. It is an artificial means of supplying oxygen and removing $CO_2$ on behalf of damaged lungs while patients are recovering from underlying diseases. Recently, the use of ECLS is rapidly increasing as this machine becomes smaller, less invasive and easier to use. In addition, the improvement of clinicians' technique and outcome is increasing their application to patients with acute respiratory distress. In this regard, the purpose of this review is to introduce the physiological principles, risk factors, and advantages of ECLS, clinical rationale for using ECLS, ventilatory strategy during ECLS, which are still causing different opinions, the weaning from ECLS, and the use of anticoagulant.
