Giant Mesenteric Lipoma - a Case Report - (거대 장간막 지방종 1 예)

  • Visceral lipoma originating from the mesentery is very rare in childhood. A 29-month-old male presented with painless abdominal distension. Abdominal ultrasonography and CT revealed a huge multilobulated hypodense mass in the peritoneal cavity. Exploratory laparotomy showed a $26{\times}25{\times}5cm$ sized encapsulated, lobulated, homogenous mass, which originated from the transverse mesocolon. Histologic examination revealed a lipoma. The postoperative course was uneventful.

  • We experienced a case of infiltrating lobular carcinoma of the breast in the ascitic fluid. The patient was a 49 year-old woman who presented with a palable mass of 3 months duration in the left upper quadrant of the abdomen and abdominal distension. Five years ago, she had undergone left radical mastectomy with chemotherapy and radiotherapy for infiltrating lobular carcinoma of the breast. The ascitic fluid smears revealed singly scattered small round to elliptical cells, many of which were arranged in characteristic indian-file patterns consisting of 3 to 5 cells. The tumor cells had scanty cytoplasm, rare cytoplasmic vacuoles and marked chromatin clumping with small, but prominent nucleoli.

A Study of Nutritional Assessment and Dietary intake after Gastrectomy of Gastric Cancer patients (위암환자의 위절제술 후 영양상태 평가 및 식사섭취도에 관한 연구)

  • The purpose of this study is to investigate the nutritional status and dietary intake of gastrectomized cancer patients. For this study, from 1993. 1 to 1993. 8, 50 postoperative gastric cancer patients were selected to examine anthropometric and laboratory data(Body Weight, Body Fat, serum Albumin, Total Lympocyte count), and dietary intake related symptoms. The results were 1) All anthropometric and laboratory data were significantly deteriorated by gastrectomy(s-Albumin, TLC. Body Fat : p<0.001). Weight loss of gastrectomized patients was 8.23$\pm$3.72% from admission to discharge. 2) In many gastrectomized cancer patients, preoperative dietary intake was decreased by abdominal discomfort, indigestion, early satiety, and anorexia. 3) Postoperative energy intake was 602$\pm$158㎉, and it is correspond to 31.18$\pm$.90% of daily energy requirement(1918$\pm$236㎉). The cause of poor oral intake is mostly fear, abdominal distension and fullness, and early satiety. In consideration of the fact that an inadequate energy intake was the main cause of the decreasing nutritional status, a careful nutritional care and dietary education is necessry after gastrectomy.

A T-cell type multicentric Lymphoma affecting central nervous system in a Cocker Spaniel dog

  • A 4-year-old female Cocker spaniel was presented with respiratory distress and abdominal distension. Pleural effusion, ascites, hepatosplenomegaly, and superficial lymphadenopathy were observed and multicentric lymphoma was diagnosed by cytological examination. Immunophenotyping of lymph node and bone marrow using polymerase chain reaction for antigen receptor rearrangement identified a stage V lymphoma originating from T-cell. Despite of systemic chemotherapy using L-asparagenase, vincristine, cyclophoaphamide and prednisolone, neurologic deficits came out and progressed. Cerebrospinal fluid analysis revealed neoplastic lymphocytic pleocytosis indicating central nervous system involvement of lymphoma. The postmortem diagnosis was confirmed based on the histology and imunohistochemistry.

A Case of Neonatal Hydrocolpos due to Vaginal Atresia (질 폐쇄증에 의한 신생아 수질증 1예)

  • A 37-week gestation female neonatal infant presented with lower abdominal distension. Ultrasonography showed a hydrocolpos, measuring $8.3cm{\times}6.9cm{\times}6.1cm$ in size and on perineal examination, vaginal atresia was noticed. On a follow-up ultrasonography performed 41 days after aspiration, the hydrocolpos was enlarged to $10cm{\times}8cm{\times}7cm$ in size, and compressed adjacent small bowel significantly with concomitant bilateral hydronephrosis. Temporary tubed vaginostomy was carried out with the provision of excellent drainage and easy access for contrast studies to outline the pathologic anatomy. We are planning to perform vaginal reconstructive surgery on her age around 2 years, when her vaginal structure might grow sufficiently for reconstructive surgery.

Appendiceal Perforation in the Neonate (신생아에서 발생한 충수돌기 천공)

  • Appendiceal perforation is uncommon in the neonate. Diess reported the first case in 1908. Approximately 111 additional cases have been reported since that time. However, with exclusion of neonatal appendicitis associated with inguinal or umbilical hernias, necrotizing enterocolitis, meconium plug, and Hirschsprung's disease, there are only 36 cases of primary neonatal appendicitis. We treated a 12 days old boy with perforation of the appendix. The infant was 3000 g at birth and had a normal spontaneous vaginal delivary at 35 weeks of gestation. The mother was 31-year-old and had premature rupture of membrane. After normal feeding for the first 5 days of life, the infant had emesis of undigested milk, decreased activity and jaundice. The baby was admitted to the Pediatrics. Progressive abdominal distension, fever, decreased activity, and vomitting developed over the next six days. Erect abdominal radiography showed pneumoperitoneum. At exploratory laparotomy, a $0.8{\times}0.6$ cm sized perforation was noted at antime-senteric border of midportion of the appendix. Trasmural inflammation and the presence of ganglion cells were noticed on histology.

Spontaneous Perforation of Meckel's Diverticulum in a Neonate (멕켈 게실 천공에 의한 신생아 복막염 1례)

  • Meckel's diverticulum is the most common malformation of gastrointestinal tract. Symptoms of Meckel's diverticulum usually arise within 2 years of life. The most common presentations include hemorrhage, perforation, diverticulitis and intestinal obstruction. Perforation of the Meckel's diverticulum in a neonate is uncommon but life threatening condition. We describe the first case of perforation of Meckel's diverticulum in a 1-day-old neonate who presented with abdominal distension without hemorrhage.

  • Agastachis Herba has been used for the treatment of poor appetite, nausea diarrhea and distension of chest. Pogostemonis Herba has been used for the treatment of fever caused by heart stroke, poor appetite, nausea, vomiting and diarrhea. Pattern recognition for the analysis of Agastachis Herba and Pogostemonis Herba was conducted using PHLC method. Rosmarinic acid from Agastachis Herba was detected at retention time of 14.996 min and quantitative analysis of rosmarinic acid showed average range of $0.183{\pm}0.183%$ (n=4). Rosmarinic acid was not detected from Pogostemonis Herba and we distinguished two medicinal plants by HPLC method.

Gastric Teratoma in a Newborn Infant; A Case Report (신생아 위 기형종 1례)

  • Gastric teratoma is an extremely rare neoplasm in children which account for less than one percent of all teratomas. Unlike other teratomas, most reported gastric teratomas was benign except one case and occurred predominantly in boys. Additionally, gastric teratomas present in the first year of life as a palpable mass, abdominal distension, vomiting, hematemesis or respiratory distress. Patients with gastric teratomas have an excellent prognosis after complete excision of the tumor. We report a case of immature gastric teratoma in an one-day-old boy, which was diagnosed by abdominal sonography and CT scan, and confirmed by surgery.

고양이에서 발생한 Perinephric pseudocysts와 선천성 Peritoneo-pericardial Kiaphragmatic Hernia in Cat

  • An 8-year old castrated domestic long-hair cat was presented with a two week history of abdominal distension. Physical examination revealed a non-painful, fluctuant, palpable mass in the right craniodorsal abdomen, and unilaterally muffled heart sounds on the right thorax. Routine clinico-pathological values were unremarkable apart from mild azotemia with a concurrent urine specific gravity of 1.031, which reflect a degree of renal dysfunction. Radiographic and ultrasound examinations of the thorax revealed the cardiac enlargement to be due to the congenital peritoneo-pericardial diaphragmatic hernia with liver occupying the right half of the pericardial sac. There was also a mild gypertrophy of the heart. Radiography and ultrasonography of the abdomen showed the mass to be composed of a large fluid filled cystic structures surrounding the right and left kidneys, and the kidneys themselves were of increased echogenecity. A diagnosis of perinephric pseudocysts was made. The patient responded well to the surgical procedures. Perinephric pseudocysts and peritoneo-pericardial diaphragmatic hernia in the cat are rare, and a case is described and the literature is reviewed in this report.
