Relationships Between Rounded Shoulder Posture and Biceps Brachii Muscle Length, Elbow Joint Angle, Pectoralis Muscle Length, Humeral Head Anterior Translation, and Glenohumeral Range of Motion

  • Background: Rounded shoulder posture (RSP), a postural abnormality, might cause shoulder pain and pathologic conditions. Although most previous research has investigated RSP focusing on the proximal structures of the shoulder, such as the scapula and pectoralis muscles, the relationship between RSP and anterior distal structures of the upper extremity, such as the biceps brachii muscle and elbow joint, is not clearly understood. Objects: This study aimed to investigate the correlations between RSP and the biceps brachii length, elbow joint angle (EJA), pectoralis minor length, general pectoralis major length, humeral head anterior translation (HHAT), glenohumeral internal rotation (IR), external rotation (ER), and horizontal adduction (HAD). Methods: Twelve subjects with RSP (6 male, 6 female) were recruited. All subjects fulfilled the RSP criteria indicated by a distance ${\geq}2.5cm$ from the posterior aspect of the acromion to the table in the supine position. The examiner measured each of the following parameters twice: RSP, biceps brachii length, EJA, pectoralis minor length, pectoralis major length, HHAT, glenohumeral IR, ER, and HAD. Pearson's correlation coefficient(r) was used to assess the correlation between RSP and all the variables. Results: There was a significant moderate positive correlation between RSP and biceps brachii length (r=.55, p=.032), moderate negative correlation between RSP and pectoralis minor length (r=-.62, p=.015), and moderate positive correlation between RSP and HHAT (r=.53, p=.038). Conclusion: The biceps brachii length, pectoralis minor length, and HHAT could be used to evaluate patients with RSP. Better understanding of the correlation between these factors and RSP could help in the development of effective methods to treat patients with this condition in clinical management.

이두박근 원위부 파열의 임상적 치료 결과 (Clinical Results of Treatment of Distal Biceps Rupture)

  • 목적: 원위부 이두박근 파열의 치료 결과를 보고하고자 한다. 대상 및 방법: 1987년 2월부터 2004년 3월까지 이두박근 원위부 파열로 치료를 받은 16명의 환자를 대상으로 하였다. 전례 남자였으며, 평균 연령은 33.9세였다. 9례(56%)에서는 수술적 치료를 시행하였다. 수술 시기는 수상 후 평균 4.7일이었다. 수술은 모두 파열된 원위부 이두박근을 해부학적으로 복원해주었다. 평가는 술 후 1년째에, 보존적인 치료를 시행 받았던 환자는 전화 인터뷰를 통하여 이두박근 근력, 환자의 만족도, 직업의 복귀 등에 대하여 조사하였다. 결과: 수술적인 치료를 시행 받았던 9례에서는 주관절의 굴곡-신전 운동범위와 회외-회내전 운동 범위는 각각 건측의 85.8%, 86.3%를 보였고, 이두박근 근력은 건측의 75%로 측정되었다. 주관적인 만족도는 8례에서 매우 만족, 1례에서 매우 불만족의 결과를 보였으며, 9례 중 8례는 예전의 직업으로 복귀하였다. 보존적 치료를 시행 받았던 예에서는 이두박근 근력은 수상 전 근력의 65%를 보였고, 환자의 주관적인 만족도는 4례에서 만족, 2례에서 불만족, 1례에서 매우 불만족의 결과를 보였다. 7례 중 3례에서만 기존의 직업으로 복귀할 수 있었다. 결론: 조기 해부학적 복원술은 이두박근의 근력과 주관절의 굴곡, 회외전 범위를 최대한 회복 시 킬 수 있는 효과적인 방법이다. 젊고 활동력이 높은 환자에서는 가급적 보존적 치료 보다는 수술적 치료를 시행해야만 한다.

뇌졸중 환자에서 운동단위의 초기 방전 양상 (Initial Motor Unit Discharge Pattern in Patients with Stroke)

  • Background : Changes in firing pattern and in the recruitment order of single motor unit(MU) have been claimed to be characteristic of central motor lesions, and a reduced firing rate was found in upper motor neuron lesions. But these findings have been rarely studied before in Korea, so we studied initial MU recruitment pattern in stroke patients with hemiparesis. Methods : We studied six patients(3 men and 3 women) whose mean age was $60.6{\pm}7.4$ years. A mean $20.6{\pm}16.2$ months had elapsed since the stroke. To compare the initial MU activation patterns in proximal and distal segments of paretic limb with their contalateral unaffected counterparts, we studied the onset and recruitment intervals in biceps brachii(BB) and first dorsal interossei(FDI) muscles in paretic and healthy arms. In a single muscle we examined from 5 to 10 individual MUs. And in a single motor unit, both the onset interval and the recruitment interval was examined. Results : The mean onset interval in paretic limb was significantly(p<0.05) longer than unaffected limb at proximal and distal location: BB $118.5{\pm}17.8$ msec vs $96.1{\pm}8.3$ msec(n=58); FDI $125.8{\pm}16.7$ msec vs $101.5{\pm}17.2$ msec(n=38). The mean recruitment interval in paretic limb was also significantly(p<0.05) longer than unaffected limb: BB $87.7{\pm}14.9$ msec vs $73.4{\pm}11.5$ msec(n=53); FDI $96.3{\pm}16.4$ msec vs $87.7{\pm}14.1$ msec(n=38). Conclusion : The first recruited MU had a lower baseline firing rate and the second recruited motor unit potential appeared earlier in paretic than in healthy muscles. And these findings may explain one of the reasons for paresis in patients with stroke.

재발성 내측 어깨 탈구를 보이는 개에서 본 앵커를 이용한 외과적 치료 증례 (Stabilization of Recurrent Medial Shoulder Luxation by Use of Three Bone Anchors in a Dog)

  • 수컷, 12년령, 2 kg의 푸들견이 오른쪽 앞다리 파행 평가를 위해 내원하였다. 병력 검사에서 교통사고 후 1년 동안 재발성 양측 어깨 관절 탈구를 보이다가 최근 들어 왼쪽 어깨는 유지가 잘 되고 오른쪽 어깨는 탈구가 심해진 것을 확인 하였다. 신체 검사에서 오른쪽 앞다리 파행을 확인 하였고, 방사선 검사에서 상완골 내측 탈구와 상완골과 견갑골의 겹침 현상을 확인 하였다. 수술 중 근위축, 상완 두갈래 근육 톤 감소, 어깨 관절 내측 인대 파열, 어깨 아래근 정지부 파열을 확인 하였다. 상완 두갈래근 변위술을 사용하였으나 수술 7일 후 재탈구를 확인 하였다. 견갑골 어깨 관절 부위에 장착한 두 개의 $2mm{\times}6mm$ 피질용 본 앵커와 상완골 어깨 관절 부위에 장착한 한 개의 $4mm{\times}6mm$ 해면질용 본 앵커를 이용 어깨 관절 내측 인대 복원술을 실시 하였다. 수술 직후 및 6주 후 방사선 검사에서 어깨 관절 탈구를 확인 할 수 없었다. 수술 후 6개월 신체 검사에서 파행은 더 이상 관찰 되지 않았다.

진구성 상완 신경총 마비에 대한 유리박근이식술 (Gracilis Muscle Transplantation in Neglected Brachial Plexus Palsy)

  • Complete denervation after severe brachial plexus injury make significant muscle atrophy with loss of proper function. It is much helpful to reconstruct the essential function of the elbow flexion movement in patient with total loss of elbow flexion motion after brachial plexus lesion which was not recovered with nerve surgery or long term conservative treatment from onset. In whole arm type brachial plexus injury, if there were no response to neurotization or neglected from injury, the volume of the denervated muscle is significantely reduced month by month. About 18 months most of the muscle fibers change to fibrous tissues and markedly atrophied irreversibly, further waiting is no more meaningful from that period. Authors performed 14 cases of functioning gracilis muscle transfer from 1981 to 1995 with microneurovascular technique, neuromusculocutaneous free flaps were performed for reconstruction of lost elbow flexion function. Average follow-up period was 5 years and 6 months. We used couple of intercostal nerves as a recipient nerve which were anastomosed to muscular nerve from obturator nerve in all cases. Recipient vessels were three deep brachial artery and eleven brachial artery which were anastomosed to medial femoral circumflex artery with end to end or end to side fashion. Average resting length of the transplanted gracilis were 24 cm. We can get average 54 degree flexion range of elbow with fair muscle power from flail elbow. There were one case of muscle necrosis with lately developed thrombosis of microvascular anastomosed site which comes from insufficient recipient arterial condition, 3 cases of partial marginal necrosis of distal skin of the transplanted part which were not significant problem with spontaneously solved with time goes by gracilis muscle has constant neurovascular pattern with relatively easy harvesting donor with minimal donor morbidity. Especially it has similar length and shape with biceps brachii muscle of upper arm and longer nerve pedicle which can neurorrhaphy with intercostal nerve without nerve graft if sufficient mobilization of the nerves from both sides of gracilis and intercostal region. Authors can propose gracilis muscle transplantation with intercostal nerves neurotization is helpful method with minimal donor morbidity for neglected brachial plexus palsy patients.
