Periodontal regeneration with nano-hyroxyapatite-coated silk scaffolds in dogs

  • Purpose: In this study, we investigated the effect of silk scaffolds on one-wall periodontal intrabony defects. We conjugated nano-hydroxyapatite (nHA) onto a silk scaffold and then seeded periodontal ligament cells (PDLCs) or dental pulp cells (DPCs) onto the scaffold. Methods: Five dogs were used in this study. Bilateral 4 mm${\times}$2 mm (depth${\times}$mesiodistal width), one-wall intrabony periodontal defects were surgically created on the distal side of the mandibular second premolar and the mesial side of the mandibular fourth premolar. In each dog, four of the defects were separately and randomly assigned to the following groups: the PDLCcultured scaffold transplantation group (PDLC group), the DPC-cultured scaffold transplantation group (DPC group), the normal saline-soaked scaffold transplantation group, and the control group. The animals were euthanized following an 8-week healing interval for clinical, scanning electron microscopy (SEM), and histologic evaluations. Results: There was no sign of inflammation or other clinical signs of postoperative complications. The examination of cellseeded constructs by SEM provided visual confirmation of the favorable characteristics of nHA-coated silk scaffolds for tissue engineering. The scaffolds exhibited a firm connective porous structure in cross section, and after PDLCs and DPCs were seeded onto the scaffolds and cultured for 3 weeks, the attachment of well-spread cells and the formation of extracellular matrix (ECM) were observed. The histologic analysis revealed that a well-maintained grafted volume was present at all experimental sites for 8 weeks. Small amounts of inflammatory cells were seen within the scaffolds. The PDLC and DPC groups did not have remarkably different histologic appearances. Conclusions: These observations indicate that nHA-coated silk scaffolds can be considered to be potentially useful biomaterials for periodontal regeneration.

Clinical study on the food impaction between implant prostheses and adjacent teeth (임플란트 보철물과 인접치 사이의 식편압입에 관한 임상적 연구)

  • Purpose: The purpose of this study was to investigate the food impaction between implant prostheses and adjacent natural teeth. Materials and methods: For this study, 51 patients with food impaction were selected and investigated mobility, tightness of contact area, gingival index, plaque index, attachment loss, alveolar bone level, proximal caries, marginal ridge distance and occlusal relationships. Results: Food impaction was found in the upper teeth (60.7%) more than the lower teeth (39.2%). Food impaction was occurred on mesial side of implant prostheses (86.2%) more than distal side (13.7%). Food impaction was mostly found in loose or open contact area (94.2%). Food impaction was frequent on stepped relationship between implant and adjacent teeth. Conclusion: Treatment plan should include proper adjacent and antagonistic occlusal plane and occlusal surface, to prevent food impaction, and the plan should include less adjacent tooth mobility with proper tightness between implant prostheses and adjacent teeth.

PHOTOELASTIC ANALYSIS OF STRESS INDUCED BY FIXED PROSTHESES WITH RIGID OF NONRIGID CONNECTION BETWEEN NATURAL TOOTH AND OSSEOINTEGRATED IMPLANT (골육착성 보철 치료시 임플랜트와 자연 지대치와의 연결 방법에 따른 관탄성 응력 분석)

  • The purpose of this study was to analyze the stress distribution at supporting bone according to the types of connection modality between implant and tooth in the superstrcture. This investigation evaluated the stress patterns in a photoelastic model produced by three different types of dental implants such as Branemark, Steri-Oss, IMZ and resin tooth using the techniques of quasi three dimensional photoelasticity. The teeth-supported bridge had a first molar pontic supported by second premolar and second molar as a control group. The implant and toothsupported bridge had a first molar pontic supported by second premolar and implant posterior retainer as an experimental group. Prostheses were mechanically connected to an adjacent second premolar by the rigid of nonrigid connection, Nonrigid connection used an attachment placed between the tooth-supported and fixture-supported component. The female(keyway) of attachment was placed on the distal end of the retainer supported by the tooth ; the male(Key) of attachment connected to the osseointegrated bridge was engaged into the keyway. All prostheses were casted in the same nonprecious alloy and were cemented and screwed on their respective abutments and implants. 16㎏ of vertical loads on central fossae of second premolar, first molar pontic, implant of second molar were applied respectively and 6.5㎏ of inclined load on middle buccal surface of first molar pontic was applied. The results were as follows : 1. Under the vertical load on the central fossa of first mloar pontic, the stress developed at the apex of tooth of implat was more uniformly distributed in the case of nonrigid connection than in the case of rigid connection. 2. Under the vertical load on the central fossa of first molar pontic, the stress developed around the cervical area of tooth of implant was larger in the case of rigid connection than in the case of nonrigid connection because the bending moment was more occured in the case of rigid connection than in the case of nonrigid connection. 3. Stress was more restricted to the loaded side of nonrigid connection than to that of rigid connection 4. Under the inclined load. The set screw loosening of implant was more easily occured in the case of nonrigid connection than in the case of rigid connection due to torque moment. 5. In the case of Branemark implant, the stress concentration in second premolar was larger and the stress developed around the cervical area of implant was lower than any other cases under the vertical load, because Branemark implant with the flexible gold screw was showed in incline toward second premolar by a bending moment. 6. The stress developed around the apex of tooth or implant was more uniformly distributed in the case of Steri-Oss implant with stiff screw than in the case of Branemark implant under the vertical load. But, the stress developed around the cervical area of the Steri-Oss implant was larger than that of any other implants because bending moment was occured by vertical migration of second premolar. 7. The stress distribution in the case of IMZ implant was similar to the case of natural teeth under small vertical load. But, the residual stress around the implant was showed to occurdue to deformation of IMC and sinking of screw under larger vertical load.

Histological characteristics of newly formed cementum in surgically created one-wall intrabony defects in a canine model

  • Purpose: Periodontal regenerative therapies for defects created by severe periodontitis are mainly focused on bone regeneration. Although cementum regeneration needs to be better understood, it is believed to play an important role in periodontal regeneration. The first step toward a full understanding of cementum regeneration is to compare repaired cementum to pristine cementum. This study, which used histological techniques, was designed to focus on cementum regeneration and to compare pristine cementum to repaired cementum after surgical procedures with 8 and 24 week healing periods in a canine model. Methods: Buccal and lingual mucoperiosteal flaps of 10 beagle dogs were surgically reflected to create critical-sized defects. Intrabony one-wall defects, of which dimension is 4 mm width and 5 mm depth, were made at the distal aspect of mandibular second premolars and the mesial aspect of mandibular fourth premolars in the right and left jaw quadrants. Animals were sacrificed after 8 and 24 weeks post-surgery for histological specimen preparation and histometric analysis. Results: The repaired cementum was composed mostly of acellular cementum and cellular mixed fiber cementum and was thicker in the apical area than in the coronal area. The acellular cementum of the supracrestal area appeared to be amorphous. The newly formed cellular cementum was partially detached from the underlying circumpulpal dentin, which implied a weak attachment between new cementum and dentin, and this split was observed to a lesser extent in the 24 week group than in the 8 week group. The vertical height of the repaired cementum was greater in the 24 week group than in the 8 week group. Conclusions: Within the limitations of this study, we can conclude that repaired cementum after root planing was mainly acellular cementum and cementum tissue that matured to a shape similar to pristine cementum as the healing progressed from 8 to 24 weeks.

The Effects of calcium sulfate on healing of 1-wall intrabony defects in dogs (성견의 1면 골내낭에 calcium sulfate 이식이 치주조직 치유에 미치는 영향)

  • The main goal of periodontal therapy is the regeneration of periodontal tissue which has been lost due to destructive periodontal diseases. Although conventional forms of periodontal therapy show sound clinical results, the healing results in long junctional epithelium. There have been numerous materials and surgical techniques developed for new attachment and bone regeneration. Bone grafts can be catagorized into: autografts, allografts, xenografts and bone substitutes. Synthetic bone substitute materials include hydroxyapatite, tricalcium phosphate, calcium carbonate, and Plaster of Paris. Calcium sulfate has found its use in dental practice for the last 30 years. Recent animal studies suggest that periodontal regeneration in 3 wall intrabony defect may be enhanced by the presence of calcium sulfate. And it is well known that 2 wall & 1 wall defect have less osteogenic potential, So we need to study the effect of calcium sulfate in 1 wall intrabony defect in dogs. The present study evaluates the effects of calcium sulfate on the epithelial migration, alveolar bone regeneration and cementum formation in intrabony defects of dogs. Four millimeter-deep one-wall intrabony defects were surgically created in the mesial aspect of anterior teeth and mesial & distal aspects of premolars. The test group received calcium sulfate grafts with a flap procedure. The control underwent flap procedure only. Histologic analysis following 8 weeks of healing revealed the following results: 1. The lengths of junctional epithelium were: 2.52mm in the control, and 1.89mm in the test group. There was no statistical significance between the two groups. 2. Alveolar bone formation were: 0.61mm in the control, and 1.88mm in the test group. There was a statistically significant difference between the two groups (p<0.05). 3. Cementum formations were: l.lmm in the control, and 2.46mm in the test group. There was a statistically significant difference between the two groups (p<0.05). 4. The length of CT adhesion were: O.97mm in the control, and 0.17mm in the test group. There was no statistically significant differences between the two groups These results suggest that the use of calcium sulfate in intrabony defects has little effect on junctional epithelium migration, but has significant effects on new bone and new cementum formations.

A mixed infection of epitheliocystis, Microcotyle sebstes and unknown aetiology in cultured rockfish (Sebastes schlegeli) from farms in Huksan Island (양식 조피볼락(Sebastes schlegeli)의 Epitheliocystis, Microcotyle sebastes 및 미동정 생물체의 혼합감염)

  • Fifteen diseased rockfish (Sebastes schlegeli) were collected from 4 net-cage farms in Huksan island. Some round transparent nodules were observed on gill filaments in fish. Bacteriological and histopathological examinations on the diseased fish were performed. Any pathogenic bacteria could not be isolated from the internal tissues and histopathological alterations were not found in any of the tissues except the gills. Rock fish were infected with epitheliocystis, Microcotyle sebastes and unknown aetiology on the gills. Cysts of epitheliocystis measuring 44.1-68.9 $\mu{m}$ in diameter were mainly found in the distal portion of the gill filaments and sometimes in the middle portion. But host responses were moderate in spite of formation of the epithelial cysts. Some Microcotyle sebastes captured the secondary filaments with clamps on the opisthohaptor. Attachment of the parasites provoked epithelial proliferation and resulted in gill lamellar fusion.Cysts of unknown origins measuring 26-425 $\mu{m}$ in diameter were found in the cartilage of gill filaments. These cysts contained eosinophilic materials and proliferation of cartilage cells was confirmed around the cysts. But there was no significant pathological changes associated with the presence of the cysts except enlargement of the gill filament with the cysts.

The effects of Acellular dermal matrix on the healing of 1 wall intrabony defects in dogs (성견에서 Acelluar dermal matrix가 1면 골내낭 결손부의 치주조직 재생에 미치는 영향)

  • Although the main purpose of periodontal treatment to regenerate is the complete regeneration of periodontal tissue due to periodontal disease, most of the treatment cannot meet such purpose because healing by long epithelial junction. Therefore, diverse materials of resorbable and non-resorbable have been used to regenerate the periodontal tissue. Due to high risk of exposure and necessity of secondary surgical procedure when using non-resorbable membrane, guided tissue regeneration using the resorbable membrane has gain popularity, recently. However, present resorbable membrane has the disadvantage of not having sufficient time to regenerate date to the difference of resorption rate according to surgical site. Meanwhile, other than the structure stability and facile manipulation, acellular dermal matrix has been reported to be a possible scaffold for cellular proliferation due to rapid revascularization and favorable physical properties for cellular attachment and proliferation. The purpose of this study is to estimate the influence of acellular dermal matrix on periodontal ligament, cementum and alveolar bone when acellular dermal matrix is implanted to 1-wall alveolar bone defect. 4 dogs of 12 to 16 month old irrelevant to sex , which below 15Kg of body weight, has been used in this study. ADM has been used for the material of guided tissue regeneration. The 3rd premolar of the lower jaw was extracted bilaterally and awaited for self-healing. subsequently buccal and lingual flap was elevated to form one wall intrabony defect with the depth and width of 4mm on the distal surface of 2nd premolar and the mesial surface of 4th premolar. After the removal of periodontal ligament by root planing. notch was formed on the basal position. Following the root surface treatment, while the control group had the flap sutured without any treatment on surgically induced intrabony defect. Following the root surface treatment, the flap of intrabony defect was sutured with the ADM inserted while the control group sutured without any insertion. The histologic specimen was observed after 4 and 8 weeks of treatment. The control group was partially regenerated by periodontal ligament, new cementum and new alveolar bone. the level of regeneration is not reached on the previous formed notch. but, experimental group was fully regenerated by functionally oriented periodontal ligament fiber. new cementum and new alveolar bone. In conclusion, we think that ADM seems to be used by scaffold for periodontal ligament cells and the matrix is expected to use on guided tissue regeneration.

  • Conventional removable partial dentures (RPDs) with distal extensions are associated with some problems, including lack of stability that calls for frequent relining, and cantilever actions of claps that can produce excessive loading to abutment teeth, and the need for unesthetic retentive arm clasps. Therefore, IARPDs (Implant-assisted RPD) that use implants to support or retain RPDs has been reported to improve stability, esthetics and masticatory performance of RPDs. Also, an IARPD that has zirconia occlusal table can prevent the incongruity of occlusal plane and the extrusion of antagonistic tooth. In this case of partially edentulous patient with crossed occlusion, each edentulous area was restored with implant fixed prosthesis and implant retained partial denture to suit each situation. Through the procedure, satisfactory outcomes were achieved both in functional and esthetic aspects.

THE EFFECTS OF PASTE TYPE CALCIUM SULFATE ON THE PERIODONTAL HEALING OF 3-WALL INTRABONY DEFECTS IN DOGS (성견 3면 골내낭에서 Paste형 Calcium Sulfate 가 치주조직 치유에 미치는 영향)

  • There are numerous kind of materials and techniques to regenerate the periodontal tissue which has been lost due to destructive periodontal disease, including bone graft material. Many bone graft materials have been reported and among these materials, synthetic material has been developed fin the long time because of its sufficient supply economically. Calcium sulfate which was evaluated as including much calcium, has been used in the clinical field. In the dental field calcium sulfate has been used as bone graft material and Kim reported that improved bone formation and more amount of new attachment after grafting calcium sulfate. but, because calcium sulfate has the problem that it generates the heat in setting period and resolves fast, we need to evaluate the effect of the improved calcium sulfate on periodontal tissue. The present study evaluates the effect of paste type calcium sulfate on the epithelial migration, alveolar bone regeneration, cementum formation and gingival connective tissue attachment in intrabony defect in dogs. Four millimeter deep and four millimeter wide 3-wall defects were surgically created in the mesial or distal aspects of premolars or molars. the test group received paste-type calcium sulfate with a flap procedure and the control group underwent flap procedure only. Histologic analysis after 8 weeks of healing revealed the following results : 1. The length of epithelial growth(the distance from CEJ to the apical end of JE) was 0.52${\pm}$0.26mm in the control and 0.56${\pm}$0.25mm in the test group. there was no statistically significant difference between the two groups. 2. The length of connective tissue adhesion was 1.74${\pm}$1.06mm in the control and 1.28${\pm}$0.57mm in the test group. there was no statistically significant difference between the two groups. 3. The length of new bone was 2.01${\pm}$0.95mm in the control and 2.62${\pm}$0.81mm in the test group. there was no statistically significant difference between the two groups. 4. The length of new cementum was 1.86${\pm}$0.80mm in the control and 2.77${\pm}$ 0.86mm in the test group. there was a statistically significant difference between the two groups.(P<0.01) These results suggest that the use of paste type calcium sulfate in 3-wall intrabony defects has significant effect on new cementum formation , but doesn't have any significant effect on the prevention of junctional epithelium migration and new bone formation. Finally, the paste type calcium sulfate that is used in this study is suggested to be the material that can have a significant effect on the periodontal healing, if its biocompatibility is improved.

Periodontal Response According to the Timing of Orthodontic Force Application alter Bone Graft into Angular Bony Defect in the Dog (성견에서 수직적 골결손부에 골이식후 교정력 적용시기에 따른 치주조직의 반응)

  • The aim of this study was to investigate the periodontal response according to the timing of orthodontic force application after bone graft into the angular bony defect. Nine dogs were divided into three groups, 2, 4, and 6 weeks, according to the timing of orthodontic force application after bone graft. Periodontal angular bony defects were created surgically at the distal aspect of both maxillary third incisors. Two weeks later, flap operation was performed to eliminate inflammation and reference notch was made on the root surface at the level of the bottom of each defect. Demineralized freeze-dried bone was implanted on the left side whereas only debridement was done on the other side. Experimental tooth movement was executed during 8 weeks on both graft and non-graft sides. After 2 weeks of retention period, animals were sacrificed for histologic specimens. The results were obtained as follows 1 New bone formation was more pronounced in the graft side than in the non-grad side in all experimental animals. 2. In the 6-week group, new bone and cementum formation was observed in more than half from the notch to the cemento-enamel junction, and the zone of connective tissue attachment was found without apical migration of junctional epithelium. 3. In the 4-week group, the amount of new bone formation was smaller than in the 6-week group whereas the overall remodeling pattern was similar. 4. New bone formation was confined to around the notch and the junctional epithelium migrated apically to the level of the notch with no connective tissue attachment and cementum formation in the 2-week group. The results of the present study suggest that periodontal response may be influenced by the timing of orthodontic force application after bone graft into angular bony defect.
