Prevalence of Psychiatric Disorders and Psychosocial Adjustment in Patients with Acquired Immune Deficiency Syndrome(AIDS) (후천성면역결핍증후군(Acquired Immune Deficiency Syndrome ; AIDS) 환자의 정신질환 유병률과 심리사회적 적응)

  • Objectives : We examined quality of life, psychosocial adjustments to illness, changes in sexual functioning, and prevalence of psychiatric disorders in AIDS patients compared to patients with chronic hepatitis B virus infection (CHB). Methods : Thirty-one men with AIDS and 50 men with CHB were enrolled. The Short-Form 12-Item Health Survey (SF-12), the Psychosocial Adjustment to Illness Scale (PAIS), and the Changes in Sexual Functioning Questionnaire short form (CSFQ-14) were administered. Results on these assessments were compared between the 31 AIDS patients and 50 CHB patients. The Structured Clinical Interview for DSMIV (SCID) was administered to determine the psychiatric diagnosis only for the AIDS patients. Results : The Physical Component Summary score (PCS) was lower in AIDS patients than in CHB patients (p<0.001). In the section examining sexual relationships, AIDS patients exhibited a lower level of adjustment (p<0.05) and had more changes in sexual function (p<0.05) than did CHB patients. Administration of the SCID to AIDS patients indicated that the lifetime prevalence of any psychiatric disorder was 56.7% ; 43.3% for mood disorders, 33.3% for alcohol use disorders, 26.7% for anxiety disorders, and 20% for adjustment disorder. Patients who had experienced any psychiatric disorder had more severe psychosocial distress (p=0.004) and evidenced a lower level of overall psychosocial adjustment (p=0.030) than patients who had not. Conclusion : We showed that AIDS patients have a high prevalence of psychiatric disorders, and that AIDS patients with psychiatric disorders were particularly low in levels of psychosocial adjustment. Thus, careful attention should be given to psychiatric aspects of AIDS patients emphasizing the early diagnosis and treatment of psychiatric disorders.

  • Objective : The aim of this study was to categorize tic disorders and introduce the treatments of tic disorders in oriental medicine. This article includes the report of successful treatment with oriental medicine. Methods : The author classifided the terms of oriental medicine in associated with tic disorders based on DSM-IV and then studied symptoms, etiological causes, and treatment through ${\ll}$Dong yui bo gam Book${\gg}$ . Results and Conclusions : 1. The concept of Tic disorders is similar to Mok-cha (目箚), Gun-chuck-youk-soon, Soon-dong, Mae-hack-gi (梅核氣) and etc, in oriental medical term. 2. In oriental medicine, tic disorders can be classified Gan-poong-nae-dong-zung (肝風內動證) type, Dam-hwa-yo-sin-zung (淡火擾神證) type, Bee-her-gan-wang-zung (脾虛肝旺證) type and Gan-sin-um-her-zung (肝腎陰虛證) type. 3. In oriental medicine, tic disorders are treated by much(so) various therapy such as herb medication, acupuncture and moxibustion, oriental psychotherapy and dirigation (Gi-Gong, 氣功) etc. 4. Tourette’s disorder is not easy to cure. When the treatment of occidental medicine and that of oriental medicine are mutually cooperated, more effective medical care might be expected. Therefore, it is necessary for further study to treat tic disorders by mutually cooperated therapy.

Associations between Physical Disorders and Suicidal Ideation in Elders (노인의 신체질환과 자살사고 간의 연관성)

  • Objectives : This study aimed to investigate the associations between physical disorders and prevalent/incident suicidal ideation in a community dwelling older population aged 65 years or over. Methods : 1204 people aged 65 years or over evaluated at baseline. Suicidal ideation was identified using the questions from the community version of the Geriatric Mental State Schedule ("GMS B3"). Reported physical disorders covering 11 common and generally chronic health problems were ascertained. Covariates included were depression, age, gender, years of education, accommodation status, past occupation, and current occupation. Of 1066 without suicidal ideation at baseline, 805 (76%) were followed 2 years later, and incident suicidal ideation was evaluated. Results : Prevalent suicidal ideation was significantly associated with 4 of 11 physical disorders: eyesight problems, persistent cough, heart disease and paralysis or weakness in one leg or arm. Incident suicidal ideation was associated with 3 physical disorders: asthma, high blood pressure and paralysis or weakness in one leg or arm. Both prevalent and incident suicidal ideation were significantly associated with increased number of physical disorders. Conclusions : Certain physical disorders were comorbid and precipitating factors of suicidal ideation in elders. And appropriate intervention and treatment of physical disorders might prevent suicidal ideation in elderly.

Acoustic-phonetic characteristics of fricatives distortion in functional articulation disorders (기능적 조음음운장애아동의 치조 마찰음 왜곡의 음향음성학적 특성)

  • This study aims to explain the difficulties children with articulation and phonological disorders have in producing alveolar fricative sounds. The study will perform a comparative analysis revealing how ordinary children produce alveolar fricative sounds through five different acoustic variables, and consequently identifying objective differences, compared to children with articulation and phonological disorders. Therefore, this study compared and analyzed the differences between 10 children with articulation and phonological disorders and 10 ordinary children according to a phonation type of alveolar fricative sounds (/s/ and /$s^*$), a type of vowel (/i/, /ε/, /u/, /o/, /ɯ/, /ʌ/, /ɑ/), and a structure of syllables (CV, VCV) through acoustic variables including a central moment, skewness, kurtosis, a center of gravity and variance. That is, children with articulation and phonological disorders, when compared to ordinary children, have difficulties with concentrating an agile and momentary friction with strength when articulating alveolar fricative sounds, which uses strong energy and accompany tension. Furthermore, the values of alveolar fricative sounds of children with articulation and phonological disorders appeared to spread evenly over the average range, which means that the range of overall the standard deviation values for children with functional phonological disorders is wider than that of ordinary children. For a future study, if the mispronounced sounds relating to omission, substitution, and addition can be compared and analyzed for various target groups, it could be used effectively to help children with functional phonological disorders.

A Review on Effects of Non-Invasive Brain Stimulation in the Treatment of Sleep Disorders (수면장애에서 비침습적 뇌자극술의 치료 효과 고찰: 경두개자기자극술과 경두개직류전기자극술을 중심으로)

  • Sleep disorders, increasingly prevalent in the general population, induce impairment in daytime functioning and other clinical problems. As changes in cortical excitability have been reported as potential pathophysiological mechanisms underlying sleep disorders, multiple studies have explored clinical effects of modulating cortical excitability through non-invasive brain stimulation in treating sleep disorders. In this study, we critically reviewed clinical studies using non-invasive brain stimulation, particularly transcranial magnetic stimulation (TMS) and transcranial direct current stimulation (tDCS), for treatment of sleep disorders. Previous studies have reported inconsistent therapeutic effects of TMS and tDCS for various kinds of sleep disorders. Specifically, low-frequency repetitive TMS (rTMS) and cathodal tDCS, both of which exert an inhibitory effect on cortical excitability, have shown inconsistent therapeutic effects for insomnia. On the other hand, high-frequency rTMS and anodal tDCS, both of which facilitate cortical excitability, have improved the symptoms of hypersomnia. In studies of restless legs syndrome, high-frequency rTMS and anodal tDCS induced inconsistent therapeutic effects. Single TMS and rTMS have shown differential therapeutic effects for obstructive sleep apnea. These inconsistent findings indicate that the distinctive characteristics of each non-invasive brain stimulation method and specific pathophysiological mechanisms underlying particular sleep disorders should be considered in an integrated manner for treatment of various sleep disorders. Future studies are needed to provide optimized TMS and tDCS protocols for each sleep disorder, considering distinctive effects of non-invasive brain stimulation and pathophysiology of each sleep disorder.

A Survey on Public Perception of Korean Medical Treatment for the Development of Korean Medicine Clinical Practice Guideline and Critical Pathway for Growth Disorders (성장장애 한의표준임상진료지침 및 한의표준임상경로 개발을 위한 일반인의 한의치료에 대한 인식 조사)

  • Objectives The purpose of this study was to understand the public's perception of Korean medical treatment for growth disorders to develop Korean medicine clinical practice guidelines and critical pathways for growth disorders in children and adolescents. Methods A survey was conducted using an online platform targeting 252 adults aged 19 years from May 16, 2021 to May 17, 2021. The questionnaire consisted of questions about the demographic characteristics of respondents; overall perception, experience, and satisfaction with Korean medical treatment for growth disorders; willingness to use or recommend Korean medical treatment for growth disorders; and points for improvement. Results The overall perception of treatment for growth disorders was 3.30 ± 0.892 on a 5-point scale. Concerning the negative reasons, 54.4% of the respondents were concerned about safety; regarding the positive reason, expectations for overall health as well as height growth were the highest at 46.5%. Additionally, there was a high demand for information, such as providing safety information on treatment, presenting evidence for the efficacy of treatment, and standardized clinical process, as points requiring improvement. Conclusions To raise public perception of Korean medical treatment for growth disorders, it is necessary to satisfy the opinions of the public identified through this survey. Therefore, the development and utilization of Korean medicine clinical practice guidelines and critical pathways for growth disorders would play an important role.

Symptoms of the Musculoskeletal disorders in Delivery Workers (택배종사자의 근골격계질환 증상에 관한 연구)

  • Due to the routine nature of social distancing in accordance with the COVID-19 pandemic, the logistics industry is under rapid development, given that offline demand is focused on online platforms. The number of warehouse workplaces and workers are steadily increasing per annum, and the industrial accident rate of transportation, warehouse, and telecommunication industries to which warehouse employees belong is higher than the total industrial accident rate in Korea. In previous studies, warehouse workers reported exposure to health hazards such as musculoskeletal disorders due to the handling of heavy objects and improper working postures. Accordingly, in this study, a survey was conducted to investigate symptoms of musculoskeletal disorders with focus on parcel delivery workers nationwide. The questionnaire included a musculoskeletal disorder symptom survey table to identify information such as worker occupational history, work type, and signs or symptoms of musculoskeletal disorders. Survey response data from 453 people were obtained to determine the influence of delivery business characteristics on occupational musculoskeletal disorders, and the influencing factors were analyzed. Based on the results, in the analysis of pain with respect to body part, the duration, degree, and frequency of pain were highest in the leg part, and as a result, the average value for the leg part exhibited a significant difference from those of other body parts. In addition, 52.32% of workers exhibited symptoms of musculoskeletal disorders, and a high number of patients with musculoskeletal disorders was observed in the work group with less than three years of service and with ages ranging from 30-39. The results of this study can serve as basic data for the derivation of a management plan that meets the characteristics of musculoskeletal disorders that impact logistics workers overburdened with work due to the rapid increase in parcel delivery volume in accordance with an increase in online consumption.

A Case of Idiopathic Polyneuropathy with Right Upper Limb Weakness (우측 상지 소력감을 주소로 하는 특발성 다발신경병증 남환 치험 1례)

  • A patient with right upper limb weakness was diagnosed with idiopathic polyneuropathy and received a series of Korean Medicine including acupuncture, electroacupuncture, bee venom acupuncture, and administration of herbal medicine BacJung-hwan for 17 days of hospitalization period. The progression of the weakness was measured by Hand grip tester, neuralgia and numbness were measured by the NRS(Numerical Rating Scale) scores. After treatment, the right grip strength improved from 12kg to 35kg and the right upper limb neuralgia of the NRS5 was improved to NRS2. The present case study suggests that the potential effects of Korean Medicine treatment for idiopathic polyneuropathy.

Impact of Eating Psychopathology, Obsessive-Compulsion and Depression on Self-Harm Behavior in Patients with Eating Disorders (섭식장애 환자의 섭식장애 증상, 우울, 강박성이 자해행동에 미치는 영향)

  • Purpose: The purpose of the study was to investigate psychological factors such as eating psychopathology, depression, and obsessive-compulsion that might influence self-harm behavior in patients with eating disorders. Methods: Patients with eating disorders (n=135) who visited "M" clinic for eating disorders participated in the study. Data were collected from March to August 2007 using the Eating Disorder Inventory-2, Beck Depression Inventory, Maudsley Obsessional-Compulsive Inventory, and Self-Harm Inventory (SHI). Results: The participants scored high on self-harm as well as on depression and obsessive-compulsion. On the SHI, a high frequency of self harm behavior such as 'torturing self with self-defeating thoughts', 'abused alcohol', 'hit self', and 'suicide attempt' were found for the participants. There were significant correlations between most eating psychopathology variables, depression, obsessive-compulsion, and self-harm behavior. 'Interoceptive awareness' (eating psychopathology), depression, and 'checking' (obsessive-compulsion) were significant predictors of self-harm behavior. Conclusion: Future interventions for patients with eating disorders should focus on assessing the possibility of self-harm and suicidal attempts, especially in those patients with high levels of eating psychopathology, depression, or obsessive-compulsion. Early intervention for depression and obsessive-compulsion could contribute to preventing self-harm and suicide in patients with eating disorders.

Development of a Quantitative Assessment Method of Finger Force Control Capabilities forMotor Intentional Disorders (손가락 힘 통제 특성에 기반한 운동의지장애의 정량적 평가 방법 개발)

  • Motor intentional disorders have been studied by visual examination methods such as cross-response task and Luria loop. To provide more analytical, quantitative information for motor intentional disorders, the present study developed a method which evaluates force control capabilities by the index finger. The assessment method analyzes the finger force control capabilities at four stages(initiation, development, maintenance, and termination) with NK Pinch-GripTM(force resolution= 0.098 N; temporal resolution=50Hz). By applying the assessment method, a patient(age=66) with callosal legion was compared with 6 healthy males(mean age=65.5, SD=2.8), showing significant but different decreases in force control capabilities depending on hand and response location. The assessment method would be of use to better understand various aspects of motor intentional disorders such as the effects of the disorders to the neurological network of the brain and severity assessment of the disorders.
