In inflammatory bowel disease (IBD), chronic inflammation leads to unfavorable clinical outcomes and increases the risk of developing colorectal neoplasm (CRN); thereby highlighting the importance of endoscopically evaluating disease activity as well as detecting and characterizing CRN in patients with IBD. With recent advances in image-enhanced endoscopic (IEE) technologies, especially virtual chromoendoscopy (VCE) platforms, this review discusses state-of-the-art IEE techniques and their applicability in assessing disease activity and surveillance colonoscopy in patients with IBD. Among various IEE, VCE demonstrated the capacity to identify quiescent disease activity. And endoscopic remission defined by the new scoring system using VCE platform better predicted clinical outcomes, which may benefit the tailoring of therapeutic strategies in patients with IBD. High-definition dye-chromoendoscopy (HD-DCE) is numerically superior to high-definition white light endoscopy (HD-WLE) in detecting CRN in IBD; however, discrepancy is observed in the statistical significance. VCE showed comparable performance in detecting dysplasia to HD-WLE or DCE and potential for optical diagnosis to differentiate neoplastic from nonneoplastic lesions during surveillance colonoscopy. Applying these novel advanced IEE technologies would provide opportunities for personalized medicine in IBD and optimal treatment of CRN in patients with IBD.

Background/Aims: Multiple outbreaks of multidrug-resistant organisms have been reported worldwide due to contaminated duodenoscopes. In 2015, the United States Food and Drug Administration recommended the following supplemental enhanced surveillance and reprocessing techniques (ESRT) to improve duodenoscope disinfection: (1) microbiological culture, (2) ethylene oxide sterilization, (3) liquid chemical sterilant processing system, and (4) double high-level disinfection. A systematic review and meta-analysis was performed to assess the impact of ESRT on the contamination rates. Methods: A thorough and systematic search was performed across several databases and conference proceedings from inception until January 2021, and all studies reporting the effectiveness of various ESRTs were identified. The pooled contamination rates of post-ESRT duodenoscopes were estimated using the random effects model. Results: A total of seven studies using various ESRTs were incorporated in the analysis, which included a total of 9,084 post-ESRT duodenoscope cultures. The pooled contamination rate of the post-ESRT duodenoscope was 5% (95% confidence interval [CI]: 2.3%-10.8%, inconsistency index [I2]=97.97%). Pooled contamination rates for high-risk organisms were 0.8% (95% CI: 0.2%-2.7%, I2=94.96). Conclusions: While ESRT may improve the disinfection process, a post-ESRT contamination rate of 5% is not negligible. Ongoing efforts to mitigate the rate of contamination by improving disinfection techniques and innovations in duodenoscope design to improve safety are warranted.

Objectives: Objectives of this study were: 1) to introduce industrial situation and health hazards of synthetic fiber, 2) to review a case of chronic obstructive pulmonary disease in a worker exposed to synthetic fiber reported to the Korea Occupational Disease Surveillance Center, and 3) to suggest supplementary measures for the occupational health system for workers exposed to synthetic fibers. Methods: Respiratory exposure, health hazards, and exposure standards for synthetic fiber dust in Korea and other countries were reviewed. In addition, a case of chronic obstructive pulmonary disease due to exposure to nylon dust reported to the Korea Occupational Disease Surveillance Center was reviewed and summarized. Results: The worker was a 53-year-old non-smoking male who had been involved in the nylon weaving process for 26 years. He had shortness of breath from three years ago. He was diagnosed with chronic obstructive pulmonary disease. PM1.0, PM2.5, and PM10 were measured at 26.6 ㎍/m3, 48.2 ㎍/m3, and 91.7 ㎍/m3, respectively. Fiber components estimated as nylon fiber were detected in the microscopic examination of a solid sample. Conclusions: For workers exposed to synthetic fiber dust, special health examinations of the respiratory system, regular work environment measurement, and work environment management through workplace health management should be performed. It is necessary to research on health effects of synthetic fibers.
본 논문에서는 우리나라의 전염병 감시체계의 현황 및 문제점을 살펴보고 세계보건기구에서 제시한 전염병 관리방안을 충족하기 위한 보다 발전된 전염병 감시체계를 제시한다. 인터넷 기반의 전자문서교환시스템을 이용한 전염병 감시체계 시스템의 구조와 기능을 알아보고 향후 발전방향을 제시한다.
We aimed to review the current data composition of the Korean Tuberculosis and Post-Tuberculosis Cohort, which was constructed by linking the Korean Tuberculosis Surveillance System (KNTSS; established and operated by the Korean Disease Control and Prevention Agency since 2000) and the National Health Information Database (NHID; established by the National Health Insurance Service in 2012). The following data were linked: KNTSS data pertaining to patients diagnosed with tuberculosis between 2011 and 2018, NHID data of patients with a history of tuberculosis and related diseases between 2006 and 2018, and data (obtained from the Statistics Korea database) on causes of death. Data from 300 117 tuberculosis patients (177 206 men and 122 911 women) were linked. The rate of treatment success for new cases was highest in 2015 (86.7%), with a gradual decrease thereafter. The treatment success rate for previously treated cases showed an increasing trend until 2014 (79.0%) and decreased thereafter. In total, 53 906 deaths were confirmed among tuberculosis patients included in the cohort. The Korean Tuberculosis and Post-Tuberculosis Cohort can be used to analyze different measurement variables in an integrated manner depending on the data source. Therefore, these cohort data can be used in future epidemiological studies and research on policy-effect analysis, treatment outcome analysis, and health-related behaviors such as treatment discontinuation.
The Philippines is still in a tight battle with the coronavirus disease 2019 pandemic since many cases are detected daily. With the continuous spread of another disease worldwide-monkeypox, many Filipinos are alarmed if the country's healthcare system is prepared enough, especially with the detection of its first case. Learning from the unfortunate experiences of the country during the current pandemic is essential in facing another health crisis. With this, recommendations for a robust healthcare system are proposed centered on: a massive digital information campaign about the disease; training healthcare workers to raise awareness about the virus and its transmission, management, and treatment; an intensified surveillance and detection procedure to monitor cases and execute contact tracing properly; and a persistent procurement of vaccines and drugs for treatment, with a well-designed vaccination program.
감염감시를 위한 신뢰성 있고 객관적인 의료관련 감염의 정의 및 자동화 된 프로세스를 개발하는 것이 중요하다. 그러나 자동화 된 감시 시스템으로의 전환은 여전히 어려운 과제이다. 초기의 발생 확인은 대개 비정상적인 사건과 진행중인 질병 감시를 인식하는 임상 검사자들이 기준선 비율을 결정하도록 요구한다. 이 시스템은 잘 정의 된 감시 규칙에 따라 의료 관련 혈류 감염의 후보를 감시하기 위해 매일 검사정보 시스템 데이터를 검사한다. 시스템은 추가 확인을 요구함으로써 전문적인 자율성을 탐지하고 예약한다. 또한 웹 기반 혈류감염 감시 및 분류 시스템은 검사실 정보 시스템에서 얻은 개별 데이터 요소를 사용할 수 있고 검사정보 시스템은 기존의 감염 제어 인력 감시 시스템과 높은 상관관계가 있는 데이터를 제공한다. 이런 시스템은 예방 지침에 따를 경우 적절하고, 수용 가능하며, 유용하고 민감하다. 감시 시스템은 병원에서 광범위한 병원균의 전파가 언제 어디서 발생하는지에 대한 이해를 획기적으로 향상시키기 때문에 유용하다. 국가적 차원의 계획은 의료관련감염 예방, 보건 관련 예방 통제위원회(HAIPCC), 살균 서비스(SS), 미생물학 실험실, 손 위생 차원의 주요 구조를 강화하기 위해 추진되어야하며 해당 지역은 의료관련 감염 예방에 미치는 영향을 고려하여 선정해야 한다.
African swine fever (ASF) is one of the most complex infectious swine diseases and the greatest concern to the pig industry owing to its high mortality and no effective vaccines available to prevent the disease. Since the first outbreak of ASF in pig farms, ASF has been identified in 14 pig farms in four cities/counties in South Korea. The outbreak was resolved in a short period because of the immediate control measures and cooperative efforts. This paper reviews the ASF outbreak and the experience of successfully stopping ASF in pig farms in South Korea through active responses to prevent the spread of ASF. In addition, suitable changes to build a sustainable pig production system and collaborative efforts to overcome the dangerous animal disease, such as ASF, are discussed.
