Oriental medical treatment in Meniere's disease (메니에르병의 현훈에 대한 한방치험 례)

  • Meniere's disease is not a life-threatening but life-altering inner ear process characterized by episodic vertigo, fluctuating hearing loss, tinnitus, and ear fullness. The etiology and pathophysiology of the symptoms is still disputed. Thereby the treatment of Meniere's disease is empirical. As yet, no treatment has prospectively modified the clinical course of the condition and thereby prevented the progressive hearing loss. We experienced two cases of Meniere's disease treated with herbal medication and acupuncture. 62 year-old male and 64 year-old female patients had definite Meniere's disease. Banhabaeckchulchunma-tang was administered in both cases. After treatment Vertigo attacks were completely controlled in both, whereas Hearing level remained unchanged. Tinnitus was improved in one patient and unchanged in the other. Therefore, Banhabaeckchulchunma-tang and acupuncture could be a safe and simple therapeutic modality that is effective in controlling the vertigo ; however further studies must be done on hearing preservation and tinnitus.

Burden and Quality of Life in Caregivers of Patients with Rare and Incurable Disease (재가 희귀.난치성질환 돌봄 제공자의 부담감과 삶의 질)

  • Purpose: The purpose of the study was to investigate burden and the qualify of life in caregivers who are taking care of home-based rare and incurable disease patients. Methods: The subjects of this study were 300 caregivers of rare and incurable disease patients registered at five health centers in Seoul. A survey was conducted by mail and visit in person during the period from the 25th of March to the 12th of May 2005. Collected data were analyzed through t-test, ANOVA, Pearson's correlation coefficient. Result: The mean burden of caregivers was 3.42, and the mean qualify of life of caregivers was 2.71. Burden and QoL showed significant differences according to caregivers' characteristics such as sender, age, relation to the patient, academic qualification, religion, occupation, monthly household income and perceived health condition. Caregivers' burden was in an inverse correlation with their quality of life. Conclusions: According to the results of this study, rare and incurable disease caregivers' burden and their quality of life were in a significant correlation with each other. In order to improve caregivers' quality of life by reducing their burden, we need to reestablish comprehensive policies for rare and incurable disease management including nursing intervention strategies for caregivers.

Noncommunicable Diseases: Current Status of Major Modifiable Risk Factors in Korea

  • A noncommunicable disease (NCD) is a medical condition or disease that is by definition non-infectious and non-transmissible among people. Currently, NCDs are the leading causes of death and disease burden worldwide. The four main types of NCDs, including cardiovascular disease, cancer, chronic lung disease, and diabetes, result in more than 30 million deaths annually. To reduce the burden of NCDs on global health, current public health actions stress the importance of preventing, detecting, and correcting modifiable risk factors; controlling major modifiable risk factors has been shown to effectively reduce NCD mortality. The World Health Organization's World Health Report 2002 identified tobacco use, alcohol consumption, overweight, physical inactivity, high blood pressure, and high cholesterol as the most important risk factors for NCDs. Accordingly, the present report set out to review the prevalence and trends of these modifiable risk factors in the Korean population. Over the past few decades, we observed significant risk factor modifications of improved blood pressure control and decreased smoking rate. However, hypertension and cigarette smoking remained the most contributable factors of NCDs in the Korean population. Moreover, other major modifiable risk factors show no improvement or even worsened. The current status and trends in major modifiable risk factors reinforce the importance of prevention, detection, and treatment of risk factors in reducing the burden of NCDs on individuals and society.

A diagnostic dilemma in pediatric osteomyelitis: a case report

  • Infantile osteomyelitis is a rare disease that is infective in nature and may rapidly turn fatal, as the disease is often misdiagnosed due to its varied presenting signs. Early diagnosis may help in avoiding systemic involvement and permanent deformity. The disease presents with signs of orbital involvement, nasal congestion, and emesis, as well as other standard hallmarks of infection. Furthermore, the maxilla is a highly vascular and porous bone and the occurrence of osteomyelitis in an infant maxilla is highly uncommon. In addition, routine blood work is not suggestive of the presence of this disease. Thus, prompt diagnosis of this condition poses a challenge to surgeons due to the confusing array of symptoms combined with the rarity of the disease. One such case of osteomyelitis of the maxilla in a young child is presented. The dilemma encountered by the surgeon during the diagnosis and treatment of the disease is discussed.

  • Macular degeneration and glaucoma are representative age-related retinal diseases that rank second and third in the prevalence of retinal diseases, and are a kind of degenerative neurological disease. Irreversible visual acuity and visual field damage may occur, and the number of patients is rapidly increasing as the population ages. Since this retinal disease is a chronic disease, continuous drug treatment is required. There are various drug delivery methods for treatment, but direct injection of the drug into the intravitreal is the most effective for continuous delivery of the drug over a long period of time. In order to solidify Dexamethasone, a retinal disease treatment, and insert it into the primary intravitreal, it is important to develop a technology to miniaturize the treatment and an applicator to deliver the treatment. In this study, a mold technology was developed to integrate the cannula and hub, which are one part of applicator. In addition, surface treatment was performed on the outside of the cannula to improve the bonding strength between the cannula and the hub, and the bonding strength according to each condition was analyzed through a tensile test.

Endovascular Revascularization for Aortoiliac Occlusive Disease (대동맥-장골동맥 폐쇄성 질환의 혈관 내 재개통술)

  • Peripheral arterial disease is an occlusive condition commonly involving the lower extremity vessels. When the aortoiliac region is affected by this disease, conventional management involves surgical bypass and endovascular treatment has been mainly recommended for patients with focal and simple lesions. It has been common strategy to perform endovascular treatment for selected patients with high surgical risk due to its minimally invasive nature. However, recent advances in the devices and techniques for endovascular treatment have resulted in its utilization for treating patients with various disease status and its clinical outcomes are comparable to those of conventional surgery. This review discusses the current diagnostic strategies for peripheral artery disease in the aortoiliac region, followed by the introduction of techniques and devices, and the role of endovascular treatment.

Review on Current Status of Use and Measurement Condition of Heart Rate Variability in Clinical Study of Korean Medicine (한의학 임상연구에서의 심박변이도 검사 활용현황과 측정조건에 대한 고찰)

  • Objectives : The purpose of this study is to review on current status of use and measurement condition of heart rate variability(HRV) by reviewing clinical studies published in journal of Korean medicine registered Korea citraion index(KCI). Methods : One hundred twenty articles were found by searching the representative Korean databases. At first analysis was performed by current status of use(journal, academic field of corresponing author, purpose of study, disease of subject, HRV measurement devices, intervention methods and tests compared to HRV). At second analysis was performed by measurement condition(uncontrolled study, relaxation before measurement, measurement position, measurement place, measurement time period and restriction of smoking, alcohol, caffeine, meal). Results : The analysis showed that department of acupuncture & moxibustion, department of oriental neuropsychiatry are major part in journal & academic field. The purpose of study were classified under studies to verify therapeutic effect on normal subject(39%), to verify therapeutic effect on the patients(8%), to investigate characteristics of the patients(34%) and to be compared with other test(19%). Disease of subject & measurement devices in article were so various. Major intervention methods were acupuncture(61.4%), mind-body intervention(19.2%). Studies that measurement condition were not controlled were relax time(24%), measurement position(19%), measurement place(26%), measurement time period(91%) and restriction of smoking, alcohol, caffeine, meal(49%). Conclusions : This study showed that current status of use of HRV is widespread in Korean medicine but more controlled measurement conditon are needed to get more reliable result from clinical studies and to improve diagnosis and treatment of Korean medicine.

Occurrence of Sclerotinia Rot on Perilla Caused by Sclerotinia sclerotiorum under Structured Cultivation Condition (Sclerotinia sclerotiorum에 의한 시설재배 들깨 균핵병 발생특성)

  • Sclerotinia rot caused by Sclerotinia sclerotiorum frequently causes serious and unpredicable yield losses of the leaves of perilla growing under structured cultivation as vegetable in Korea. Temperature for mycelial growth ranged from 5 to $30^{\circ}C$ with optimum temperature d $20^{\circ}C$. Sclerotia were formed fewer at low temperature, but their dry weight was heavier than that at high temperature. The apothecia were formed from the sclerotia that buried up to 3 cm soil depth at $15^{\circ}C$ in moisture condition. The incidence of perilla Sclerotinia rot caused by S. sclerotiorum was observed throughout the growing season at greenhouse. The occurrence of this disease was especially severe from January to February of low temperature period. The average incidence rates of this disease was up to 15%. The significant occurrence of this disease was showed mainly in the continuous cropping field for more than five years. The incidence of this disease increased according to the increase of continuous cropping year. The incidence rates of this disease reached up to 20% in the continuous cropping field for ten years. Also it was firstly investigated the natural infection caused by S. sclerotiorum on weed plants, Gnaphalium affine and Mazus pumilus in farmer's field. The casual fungus showed pathogenicity on 11 weed plants species tested, and more severe pathogenicity on G. affine, Latuca indica and Ixeris dentata included in the family Compositae. This result suggests that effective crop rotation and weed eradication can be the method for organic control of perilla sclerotinia rot, and sudden outbreaks of this disease on perilla growing under structure after paddy rice call be explained by the presence of weed hosts.

Design of Rough Set Theory Based Disease Monitoring System for Healthcare (헬스 케어를 위한 RDMS 설계)

  • This paper proposes the RDMS(Rough Set Theory based Disease Monitoring System) which efficiently manages diseases in Healthcare System. The RDMS is made up of DCM(Data Collection Module), RDRGM(RST based Disease Rules Generation Module), and HMM(Healthcare Monitoring Module). The DCM collects bio-metric informations from bio sensor of patient and stores it in RDMS DB according to the processing procedure of data. The RDRGM generates disease rules using the core of RST and the support of attributes. The HMM predicts a patient's disease by analyzing not only the risk quotient but also that of complications on the patient's disease by using the collected patient's information by DCM and transfers a visualized patient's information to a patient, a family doctor, etc according to a patient's risk quotient. Also the HMM predicts the patient's disease by comparing and analyzing a patient's medical information, a current patient's health condition, and a patient's family history according to the rules generated by RDRGM and can provide the Patient-Customized Medical Service and the medical information with the prediction result rapidly and reliably.

The Influence of Disease Activity and Uncertainty on Anxiety and Depression in Patients with Ankylosing Spondylitis (강직성 척추염 환자의 질병활성도와 불확실성이 불안과 우울에 미치는 영향)

  • Purpose: The purpose of this study was to understand how disease activity and uncertainty influence anxiety and depression in patients with ankylosing spondylitis Methods: Participants were 125 patients with ankylosing spondylitis who had attended the rheumatology division of a university hospital. A structured questionnaire was used to assess disease activity, uncertainty, anxiety, and depression. The data gathered were analyzed using t-tests, ANOVAs, Pearson correlation coefficients, and a multiple regression. Results: Differences were observed in anxiety based on job status, and in depression based on age, marriage, and job status. Uncertainty and disease activity predicted patients' anxiety and explained 40% of the variance in this measure, and the relative influence of uncertainty (${\beta}=.38$, p<.001) was larger than that of disease activity (${\beta}=.30$, p<.001). Furthermore, uncertainty and disease activity predicted depression and explained 36% of the variance therein, and the relative influence of uncertainty (${\beta}=.27$, p=.002) and disease activity (${\beta}=.27$, p=.003) was similar. Conclusion: This study confirmed that disease activity and uncertainty influenced anxiety and depression in patients with ankylosing spondylitis. Therefore, efforts to decrease anxiety and depression in patients with this condition must take into consideration disease activity when implementing nursing interventions and should include strategies to lower uncertainty.
