후두전적출술 후 장기적인 삶의 질에 대한 평가 (Long Term Assessment of Quality of Life in Laryngectomee)

  • Objectives: Despite various attempts to preserve the normal voice in advanced laryngeal cancer, it is inevitable for many advanced laryngeal cancer patients to undergo total laryngectomy and thus making a trade off between quality and quantity of life. Laryngectomees are faced with voice loss, change in physical appearance and health deterioration which hinder their efforts to rehabilitate back into their family and employment leading to poor quality of life. The objectives of this study were to evaluate the quality of life in long term surviving laryngectomees to, define the factors which are most important in determining their present quality of life and to propose a theoretical model for quality of life after laryngectomy in Korea. Materials and Methods: From 1986 to 1995, 120 laryngectomees with no evidence of disease were followed up for at least 3years were evaluated. Each of 15 quality of life domains with a total of ten points were given for a quality of life score. Results: There was no significant difference in the physical-mental adaptation index such as general health and mental health between the laryngectomees and control group(p>0.05). However, social adaptation index such as social activity, occupational status and economic status for laryngectomees were significantly lower with 2.3, 3.5, and 4.4 points compared to control group with 8.5, 7.6, and 7.1 points respectively(p<0.05). Conclusion : Vocational and social rehabilitation should be emphasized with highest priority for improving their economic status and thus improving their quality of life.

The Effort of Dietary Lipids on CVD Risk Factors in Ovariectomized Rats

  • This experiment was performed to investigate the effect of different dietary lipids on the risk factors of coronary vascular disease(CVD) in ovariectomized rats. female rats of Sprague-Dawley stram were divided into sham-operated(sham) and ovariectomized(ovx) groups and then each group was divided into a beef fallow group, a soy bean oil group and a fish oil group. After 16 weeks of feeding on experimental diets, animals were sacrificed and blood, liver, kidney and perirenal fat pad were obtained. Food intake and weight gain of fish oil group were significantly lower than other dietary lipid groups. food intake and weight gain tended to be higher in ovx groups than in sham groups. The weight Index(g/100g body weight) of liver and kidney was higher in the fish oil group than the other groups and weight index was lower in ovx groups compared to sham groups. The weight of the perirenal fat pad was the highest in the beef tallow group and the lowest in the fish oil group. The fish oil group showed the lowest total cholesterol(TC) and triglyceride (TG) levels in serum. Serum TG levels were lower in all ovx groups than in sham groups, but serum TC levels were not influenced by ovariectomy. fatty acid composition of serum reflects the recent dietary Intake of fat. Linoleic acid content was tile highest in soy bean oil group and eicosapentaenoic acid(EPA) and docosahexaenoic acid(DHA) contents were the highest in fish oil group. fatty acid composition of adipose tissue, especially EPA and DHA contents in perirenal fat pad, was highest in the fish oil group. Saturated fatty acid(SFA) and monounsaturated fatty acid(MUFA) in serum and adipose tissue did not reflect fatty acid intake. The activities of glucose-6-phosphate dehydrogenase, a lipogenic enzyme, in the blood of the beef tallow and soybean oil groups showed the tendency to be high and that of the fish oil group to be low in ovx. Carnitine acetyltransferase, a lipolytic enzyme, showed the highest activity in the liver of the fish oil group and was least active in the soy bean oil group.

Morin alleviates fructose-induced metabolic syndrome in rats via ameliorating oxidative stress, inflammatory and fibrotic markers

  • Metabolic syndrome (MBS) is a widespread disease that has strongly related to unhealthy diet and low physical activity, which initiate more serious conditions such as obesity, cardiovascular diseases and type 2 diabetes mellitus. This study aimed to examine the therapeutic effects of morin, as one of the flavonoids constituents, which widely exists in many herbs and fruits, against some metabolic and hepatic manifestations observed in MBS rats and the feasible related mechanisms. MBS was induced in rats by high fructose diet feeding for 12 weeks. Morin (30 mg/kg) was administered orally to both normal and MBS rats for 4 weeks. Liver tissues were used for determination of liver index, hepatic expression of glucose transporter 2 (GLUT2) as well as both inflammatory and fibrotic markers. The fat/muscle ratio, metabolic parameters, systolic blood pressure, and oxidative stress markers were also determined. Our data confirmed that the administration of morin in fructose diet rats significantly reduced the elevated systolic blood pressure. The altered levels of metabolic parameters such as blood glucose, serum insulin, serum lipid profile, and oxidative stress markers were also reversed approximately to the normal values. In addition, morin treatment decreased liver index, serum liver enzyme activities, and fat/muscle ratio. Furthermore, morin relatively up-regulated GLUT2 expression, however, down-regulated NF-κB, TNF-α, and TGF-β expressions in the hepatic tissues. Here, we revealed that morin has an exquisite effect against metabolic disorders in the experimental model through, at least in part, antioxidant, anti-inflammatory, and anti-fibrotic mechanisms.

The Effects of COVID-19 Pandemic on the Recovery of Hip Fracture Patients

  • Purpose: To figure out how complete control of family visits to prevent infection of coronavirus disease 2019 (COVID-19) affected the activity recovery of hip fracture patients admitted to nursing hospitals. Materials and Methods: Eighty-one patients with hip surgery in the two years prior to COVID-19 pandemic were classified as Group A, and 103 patients in the next two years were designated as Group B. The subjects' walking ability was evaluated by using the modified Koval index (MKI). In order to analyze the impact of the family visit control to the subjects, each group was classified into two different groups: (1) inpatients group who admitted to nursing hospitals and (2) home-treated patients. Additionally, statistical elements were processed in consideration of other factors that may affect the results of the experiment. Results: The MKI evaluated at 6 months postoperative was 3.31±1.79 in Group A and 2.77±1.91 in Group B, and it was meaningfully low after the pandemic (P=0.04). There was significantly low among both of Group A 2.74±1.76 and Group B 1.93±1.81 after the pandemic (P=0.03) among those treated at the nursing hospital. The rate of deterioration of the MKI was 35 (43.2%) in Group A and 57 (55.3%) in Group B, which increased by 12.1% after the pandemic. Conclusion: The pandemic had a negative effect on the recovery of postoperative activities of elderly hip fracture patients who admitted to nursing hospitals when family access was completely restricted to prevent infection.

한방병원 입원환자의 양생(養生)과 양생(養生) 영향요인 (Health Promotion and Related Factors in Oriental Medicine Hospital Inpatients)

  • The purpose of this study was to examine health promotion level and related of patients hospitalized. The participants for this study 225 of oriental medicine hospital inpatients. Health promotion, sleep, anxiety and discomfort are measured structured questionnaires each. Data were collected using structured questionnaires and analyzed using t-test, ANOVA and multiple regression. The general health promotion score is 98.12, mortality health promotion 20.31, mental health promotion 12.89, dietary health promotion 15.28, activity and rest health promotion 12.84, exercise health promotion 7.23, sleepy health promotion 15.42, seasonal health promotion 7.60 and sexual health promotion 9.31. There were significant differences in health promotion level according to spouse (p=0.003), sleep (p<0.001), anxiety (p<0.001) and discomfort (p<0.001). The significant predictors of health promotion were spouse (p<0.001), religious (p<0.001), drinking (p=0.036), sleep (p<0.001) and discomfort (p<0.001), explaining 33.7% of the variance in health promotion. Health promotion influenced not only the physical condition but also the psychological condition. The result suggest the health promotion level and predictors of health promotion is a useful index for improvement of health and prevention of disease in oriental medicine.

미국이민 중년 여성의 비만 관련요인과 만성질환에 미치는 영향 (Factors Related to Obesity of Mid-year Korean American Women and Their Prevalence of Chronic Diseases)

  • Purpose: The purpose of this study was to investigate factors related to obesity of mid-age women and to examine the relationship between obesity and chronic diseases including diabetes, hypertension, and heart disease. Methods: Using data from Korean American adults living in California, we analyzed the health behavior (diet, exercise, smoking, and binge drinking) and psychological stress of obese women with body mass index ${\geq}25$ by using chi-square test. Logistic regression was used to investigate independent effects of obesity on chronic diseases, after controlling for risk factors. Results: The prevalence of obesity appeared about 16.9%. The Obese group was less likely than the non-obese group to eat vegetables and more likely feel psychological stress. There were no significant differences in smoking, alcohol intake, and physical exercise between the two groups. Obesity was strongly related to chronic diseases (OR=4.642, 95% CI=1.328-16.222). Conclusion: This study suggests that obesity of mid-age women could be reduced by encouraging health behavior such as eating vegetables daily, performing physical exercise regularly, and taking care of emotional stress. Diet and physical activity interventions and emotional supports should be developed for weight loss and prevention of weight gain in mid-age women.

  • The anti-fungal activity of plant extractives was investigated and the lacquer extractives were selected. The lacquer extractives were coated on anti-fungal paper and confirmed its effect. Water repellency, tensile index and brightness of anti-fungal paper treated with natural extractives were similar to commercial pesticide-treated paper. The incidence of Fusicaladium leuieri MAGNUS of commercial pesticide-treated paper and anti-fungal paper treated with lacquer were similar but, the incidence of Botryosphaeria dothidea (Moug.) Ces & De Not and Gloeodes pomigena of those were increased 10-60 % than commercial pesticide-treated paper.

치매노인의 낙상위험요인에 관한 메타분석 (A Meta-analysis of the Risk Factors related to Falls among Elderly Patients with Dementia)

  • Purpose: The purpose of this study was to provide data about the risk factors related to falls among elderly patients with dementia using meta-analysis. Methods: Key words used for search through electronic database (CINAHL, PubMed, Ovid-MEDLINE, RISS, KISS, DBPIA, National Assembly Library) included 'dementia', 'Alzheimer', 'fall'. Twenty studies met the inclusion criteria for the meta-analysis and 'R' version 3.2.2 was used to analyze the correlated effect size. Results: Study results showed that risk factors related to falls were identified as the demographic (age, gender, education), dementia-related (disease duration, cognition), physical (body mass index, walking, balance, activity of daily living, use of walking aids, number of medications including psychotropic drugs, musculoskeletal problems, parkinsonism, comorbidity), psychological (neuropsychiatric symptom, depression), environmental (Physical environment), and fall-related (fall history, high risk group of fall) factors. The effect size of risk factors such as high risk group of fall (r=.35), use of walking aids (r=.33), depression (r=.31), psychotropic drugs (r=.27), Musculoskeletal problems (r=.25) were higher than the other risk factors. Conclusion: Based on the findings of this study, strategies to improve elderly patient's depression, intensive care for high risk group of fall, and adequate training with walking aids are needed for prevention of falls in elderly patients with dementia.

Administration of Aqueous Extract of Schizandra chinensis Fruit Inhibits the Experimental Colitis in Mice

  • Schizandra chinensis fruits (SC) have been used as a traditional Oriental medicine for treatments of many stress-induced diseases. In the present study, we investigated the protective effect of SC aqueous extract(SC-Ex) in the inflammatory diseases of intestine using a mouse model of ulcerative colitis. An experimental colitis was induced by daily treatment with 5% dextran sulfate sodium (DSS). SC-Ex was orally administered from day 2 of DSS treatment in a dose-dependent manner. Administration of SC-Ex reduced significantly clinic signs of DSS-induced colitis, including body weight loss, shorten colon length, increased disease activity index, and histological colon injury. Moreover, SC-Ex suppressed significantly not only the activities of myeloperoxidase (MPO) and chymase, but also the expressions of $TNF-{\acute{a}}$ and COX-2 in DSS-treated colon tissues. Inhibitory effect of SC-Ex was effective at a dose over 20 mg/kg. Our results indicate that SC-Ex may possess therapeutic effect on the development of DSS-induced colitis.

The Health Examinees (HEXA) Study: Rationale, Study Design and Baseline Characteristics

  • Background: Korea has experienced rapid economic development in a very short period of time. A mixture of traditional and modern risk factors coexists and the rapid change in non-genetic factors interacts with genetic constituents. With consideration of these unique aspects of Korean society, a large-scale genomic cohort study-the Health Examinees (HEXA) Study-has been conducted to investigate epidemiologic characteristics, genomic features, and gene-environment interactions of major chronic diseases including cancer in the Korean population. Materials and Methods: Following a standardized study protocol, the subjects were prospectively recruited from 38 health examination centers and training hospitals throughout the country. An interview-based questionnaire survey was conducted to collect information on socio-demographic characteristics, medical history, medication usage, family history, lifestyle factors, diet, physical activity, and reproductive factors for women. Various biological specimens (i.e., plasma, serum, buffy coat, blood cells, genomic DNA, and urine) were collected for biorepository according to the standardized protocol. Skilled medical staff also performed physical examinations. Results: Between 2004 and 2013, a total of 167,169 subjects aged 40-69 years were recruited for the HEXA study. Participants are being followed up utilizing active and passive methods. The first wave of active follow-up began in 2012 and it will be continued until 2015. The principal purpose of passive follow-up is based on data linkages with the National Death Certificate, the National Cancer Registry, and the National Health Insurance Claim data. Conclusions: The HEXA study will render an opportunity to investigate biomarkers of early health index and the chronological changes associated with chronic diseases.
