A Study on Stressful Life Events, Self-esteem and Perceived Health Status of the Elderly in a Rural Community (노인의 스트레스 생활사건, 자아존중감, 지각된 건강상태에 관한 연구)

  • This study was performed to estimate stressful life events, self-esteem and perceived health status in the elderly persons and to compare degree of self-esteem and perceived health status according to experience of stressful life events and general characteristics. The data was collected from 179 elders in a rural community. Data collection was done from October 20 to December 6, 1997. A comparison of self-esteem and perceived health status by experience of stressful life events and general characteristics was summarized as follows : 1) Eighty three persons, $46.4\%$ of the surveyed, have experienced stressful life events, including disease(54 persons) and death of family member(l2 persons). 2) Thirty eight percents of the surveyed persons evaluated they are not healthy. The average score of perceived health status of the elderly was 2.88± .92 for the scale of 5, which represents the healthiest status. 3) There were significant differences on the self-esteem score in the elderly according to having a spouse or not(t=3.51. p=.00l), having family members living together or not(t=2.98. p=.003) and socioeconomic status(F=7.08. p=.00l). 4) There were significant differences on the perceived health status in the elderly according to experience of stressful life events(t=3.51. p=.00l), having family members living together or not(t=2.09, p= .038) and socioeconomic status(F=6.56, p=.002). 5) Positive correlation was observed between self-esteem and perceived health(r= .5037, p=.000). The above results imply that support of family and society should be reinforced to improve self-respect and health of aged persons, and that it is desirable to build up social and economic environment promoting health status through daily life.

The Effect of a Community-Based Self-Management Program for Patients at Thyroid Cancer-Diagnosis Stage : a Pilot Study (재가 갑상선암 환자를 위한 지역암센터 자가관리프로그램 중재 효과에 대한 예비연구)

  • Purpose: This study was conducted to examine the effectiveness of a self-management program on patients with thyroid cancer, particularly during the time of waiting for surgery after cancer diagnosis. Psychological distress, biological responses of immune cell counts, and quality of life were the variables of this study. Methods: One group pre-post test design was used with the nature of a pilot study. Ten newly diagnosed thyroid cancer patients were recruited through physicians' referrals. After drop out of 4 participants, final data were collected from six participants. Small group technique, a one and half hour-session per week for one month (total 4 sessions, 6 hours) was used. Relaxation techniques, meditation training, and strategies to reduce distress were provided by researchers. Standardized questionnaires and an established bio-assay were used for collection of data. Results: Participants showed significant lowering of psychological distress (p<.05) and improvement in global quality of life (p<.05). Biological responses of immune cell counts did not show statistical significance. Conclusion: The self-management program may reduce psychological distress and improve quality of life of patients with thyroid cancer between the time of diagnosis and surgery. The self-management program would be a valuable approach for patients with an unexpected cancer diagnosis to prepare for their disease experience in a community setting.

Elderly Sleep Pattern and Disturbing Factors Before and After Hospitalization (노인환자의 입원 전ㆍ후 수면양상과 수면장애요인에 관한 연구)

  • Sleep is a necessity for survival. Disruption of sleep leads to numerous adverse physiological and psychological consequences. These could be particularly undesirable for older patients, who are subject to many additional factors. But there is limited research related to hospitalized elderly in Korea. The purpose of the study is to explore sleep patterns and disturbing factors of before and after hospitalization, in order to present basic information regarding elderly sleep to develop nursing intervention. The sample consisted of 32 elderly men and women between the ages of n and 87 years. Data collection was done from September to November 1997. Measures of sleep patterns and related factors were obtained from self-reported sleep questionnaires. Analysis of data was done by use of t-test, paired t-test, ANOVA, and Pearson Correlation Coefficient. The results of this study were summarized as follows : 1. In comparision between before and after admission of their sleep pattern, “sleep onset” tends to be delayed and nocturnal sleep time was significantly reduced. So, hospitalized elderly reported less total sleep time than before admission. 2. Regarding the sleep disturbing factors, medication(hypnotics ; 37.5%), physiological factor (snoring ; 59.4%) environmental factor (pillow ; 78.1%), emotional factor(anxiety related to disease ; 37.5%), and illness factor(fatigue ; 34.7%) were reported. 3. Significant differences in gender were found. Men had more disturbances in sleep than women owing to difficulty in falling a sleep and lack of nocturnal sleep. Women consumed more sleep inducing drugs. Significant increase was reported in napping during the day with increasing age. 4. Significant differences between good sleepers and poor sleepers were found for the following variables : nocturnal sleep time, total sleep time, bed time, sleep onset latency time, sleep latency time after nocturnal awakening, time spent in bed upon arousal, environmental factors, and emotional factors. In conclusion, it was found that the quantity and quality of sleep were significantly altered in hospitalized elderly, but adequate strategies for better sleep were not practiced. Further research is needed to develop Intervention strategies to promote sleep and to prevent sleep problems.

Normal Walking Versus Toe-walking in Healthy Subjects: An Electromyographic Analysis (정상 보행과 발가락 보행의 하지 근육 근 활성도 비교)

  • This study was designed to identify the effects of walking conditions (normal walking vs. toe-walking) on electromyographic (EMG) activity of gastrocnemius, tibialis anterior, and soleus muscle. Seven healthy adult males participated in this study. The exclusion criteria were orthopedic or neurologic disease, congenital anomaly or acquired deformity, or pain in low back or lower extremities. The maximal voluntary isometric contraction for each muscle was used for the reference contraction, and EMG activity of each muscle during normal walking and toe-walking was expressed as a percentage of reference contraction. The gait cycle was determined with two foot switches, and gait was normalized as 100% gait cycle for each condition. The maximal values of EMG activity in terminal stance (30~50% of gait cycle) of each condition were compared for data analysis. No significant differences were found in EMG activity of the tibialis anterior and soleus (p>.05), whereas significant decrement was found in EMG activity of gastrocnemius during toe-walking compared to normal walking (p<.05). There is a limitation to generalize the results of this study, because small number of subjects participated for this study and only EMG was used for data collection. The treatment methods should be developed to improve gait efficiency by substituting the weakened muscles secondary to upper motor neuron, or by strengthening the distal muscles in lower extremity.

Benefits, barriers, self-efficacy and knowledge regarding healthy foods; perception of African Americans living in eastern North Carolina

  • African Americans in the United States suffer from many health disparities such as obesity, diabetes or hypertension. Lifestyle factors including diet and physical activity play an important role in prevention of these health conditions. The purpose of this research project was to assess beliefs, barriers and self-efficacy of eating a healthy diet and self efficacy of shopping for foods such as whole grains or foods designated as low fat or low sodium. Additionally, the objective was to assess beliefs about healthfulness, appropriate consumption, and protective aspect of specific foods including fruits, vegetables, and whole grains. The assessment was done using a survey instrument developed for this study. Data collection took place at two church locations. Data were obtained from 57 African Americans, mean age 50 years old (SD 12.70) completed the survey. The majority of respondents (58.1%) were females and most (75%) had at least some college education. Generally, benefits of eating healthy foods received considerably higher scores compared to barriers of eating healthy. A belief that healthy foods would help to take care of one's body received the highest mean score while a belief that healthy foods are too expensive had the highest score from all barriers. The results showed high self-efficacy of eating and purchasing healthy foods, high awareness of knowledge regarding foods associated with disease prevention but low awareness of recommendations for fruits and vegetables. The high scores for benefits, self-efficacy and knowledge regarding eating healthy foods did not translate into the perception of intake of such foods. Most participants believed that they do not eat enough of healthy foods. Interventions design to help African Americans make dietary changes should be culturally relevant and should involved working on a community level utilizing messages that are familiar and relevant to African Americans.

Corelation between Nurses' Posttraumatic Stress Disorder, Depression and Social Stigma in Nursing COVID-19 Patients (COVID-19 환자 간호한 간호사들의 외상 후 스트레스 장애, 우울 및 사회적 낙인 간의 관계)

  • Purpose: This study aims to examine the levels of posttraumatic stress disorder (PTSD), depression and social stigma among nurses caring for Corona Virus Disease-19 (COVID-19) patients. Methods: 169 nurses caring for COVID-19 patients participated in this study. Data collection was conducted at university hospitals from March 1 to August 31, 2020. Data analysis was performed for descriptive statistics, t-test, ANOVA, and Pearson correlation coefficients using SPSS/WIN 24.0 program. Results: The mean scores of PTSD, depression and social stigma were 25.16±16.80, 17.26±8.63 and 5.83±2.84, respectively. The PTSD scores were significantly different between the department (F=2.89, p=.037). Depressive scores were significantly different between the marital status (t=2.27, p=.024) and the department (F=4.91, p=.003). Social stigma scores were significantly different between age (F=6.49, p=.002), marital status (t=-3.30, p=.008), having or not having children (t=3.82, p=.001), department (F=5.82, p=.001) and clinical experience (F=7.43, p=.001). Positive correlations were found between PTSD and depression (r=.70, p<.001) and social stigma (r=.22, p<.004). Conclusion: Integrated assessment and management are required to address the psychological and emotional problems faced by nurses caring for COVID-19 patients, and active follow-up measures should be considered.

Correlation between Knowledge, Attitude, and Compliance of Preventive Behaviors regarding Middle East Respiratory Syndrome among Nursing Students (간호대학생의 중동호흡기증후군에 대한 지식, 태도 및 예방행위 실천 간의 관계)

  • Purpose: This study aimed to investigate the relationships between the level of knowledge, attitude and compliance of preventive behaviors of the Middle East respiratory syndrome (MERS) among nursing students. Methods: The study sample consisted of 219 nursing students. Through the use of a structured questionnaire, data collection was conducted from June 1st to June 30th 2015. The descriptive statistics, independent t-tests and Pearson's correlation coefficient were employed to analyze the data. Results: The score of the MERS-related knowledge was 9.15 out of 13. The participants had a positive attitude toward the MERS. The level of MERS-related knowledge was high in the case of temporary school closure or break in clinical practice because of the clinical training in hospitals that MERS occurred (t= 2.42, p= .016). The compliance level of the preventive behavior for MERS was high in female students (t= -2.11, p= .036), in the case of temporary school closure or break in clinical practice due to the clinical training in hospitals that MERS occurred (t = 3.29, p= .001), and in students that had MERS-related education for prevention (t = 2.80, p= .006). The MERS-related knowledge was positively correlated with the level of compliance for preventive behavior (r= .18, p= .009). Conclusion: To prevent MERS infection in nursing students, the level of knowledge on MERS should be enhanced so that they can practice preventive behaviors against it. Additionally, the MERS infection control education should include etiology and treatment products based on the MERS response guideline issued by the Centers for Korea Disease Control and Prevention.

Prevalence and Factors related to Irritable Bowel Syndrome in University Students (대학생의 과민성 장 증후군의 유병률과 영향요인)

  • Purpose: The purpose of this study was to identify the prevalence of irritable bowel syndrome and factors related to irritable bowel syndrome (IBS) in university students. Methods: A cross-sectional, descriptive study was conducted using a self-report structured questionnaire with 420 university students. Data collection was done during May and June, 2017. The data were analyzed using descriptive statistics, a t-test, $x^2$ test, and binary logistic regression analysis with IBM SPSS/WIN 22.0 program. Results: The prevalence for IBS was 24% among Korean university students. Family history of IBS (OR=2.98, 95% CI=1.32~6.70), history of stomach and intestinal disease (OR=2.24, 95% CI=1.36~3.77), sleep quality (OR=2.01, 95% CI=1.16~3.48), mild anxiety (OR=2.75, 95% CI=1.19~6.33) and more than moderate anxiety (OR=2.02, 95% CI=1.02~4.00) were found to be factors related to IBS. Conclusion: The prevalence of IBS was high among university students, and sleep quality and anxiety significantly influenced prevalence of IBS in these students. Therefore, interventions to decrease symptom of IBS must focus on management of sleep quality and anxiety.

Combination of Brain Cancer with Hybrid K-NN Algorithm using Statistical of Cerebrospinal Fluid (CSF) Surgery

  • The spinal cord or CSF surgery is a very complex process. It requires continuous pre and post-surgery evaluation to have a better ability to diagnose the disease. To detect automatically the suspected areas of tumors and symptoms of CSF leakage during the development of the tumor inside of the brain. We propose a new method based on using computer software that generates statistical results through data gathered during surgeries and operations. We performed statistical computation and data collection through the Google Source for the UK National Cancer Database. The purpose of this study is to address the above problems related to the accuracy of missing hybrid KNN values and finding the distance of tumor in terms of brain cancer or CSF images. This research aims to create a framework that can classify the damaged area of cancer or tumors using high-dimensional image segmentation and Laplace transformation method. A high-dimensional image segmentation method is implemented by software modelling techniques with measures the width, percentage, and size of cells within the brain, as well as enhance the efficiency of the hybrid KNN algorithm and Laplace transformation make it deal the non-zero values in terms of missing values form with the using of Frobenius Matrix for deal the space into non-zero values. Our proposed algorithm takes the longest values of KNN (K = 1-100), which is successfully demonstrated in a 4-dimensional modulation method that monitors the lighting field that can be used in the field of light emission. Conclusion: This approach dramatically improves the efficiency of hybrid KNN method and the detection of tumor region using 4-D segmentation method. The simulation results verified the performance of the proposed method is improved by 92% sensitivity of 60% specificity and 70.50% accuracy respectively.

Jeju Animal Shelter abandoned animals status and actual condition analysis (제주동물보호소 유기동물 현황 및 실태분석)

  • This study investigated the status and analyzed of stray dogs, stray cats admitted to the Jeju Animal Shelter during the year of 2015 to 2019, and the infection rates of specific diseases for abandoned dogs. In addition, in 2017 to 2019, the collected intake and outcome data were reviewed to analyze shelter capacity in terms of housing capacity (monthly daily average intake, required holding capacity, and adoption-driven capacity), staff capacity (staff hours required for daily care). Out of 24,557 dogs admitted to the shelter, owners of 1,808 dogs (7.4%) visited the shelter and found their lost dogs, while 3,612 dogs (14.7%) were adopted to new families. However, the number of puppies that were euthanized was the highest at 12,266 (49.9%), and 6,876 (28%) died either death from disease or natural causes because they were detained in shelters. The monthly daily average (MDA), which is one of the indicators for efficient population management of Jeju Animal Shelter, was found to be 17.4 for abandoned dogs and 1.7 for abandoned cats. Seasonal variations were observed for MDA, RHC, MDA adoptions, ADC, and RSDC, indicating that various strategies are needed for shelter management. This study was performed to analyze and interpret meaningful statistics for improving the efficiency of animal shelters in Jeju. However, inconsistent collection of animal statistics limited data compilation. Creation of a basic animal statistics matrix with reference to well-designed matrices from recognized professional animal shelters is essential.
