Two Clinical Case Reports of Ipsilateral Facial Palsy and Conjugate Gaze Palsy caused by Foville Syndrome & Foville-Millard-Gubler Syndrome (동측 안면마비와 측방주시마비를 일으키는 Foville Syndrome & Foville-Millard-Gubler Syndrome 치험 각 1례)

  • Objective : Foville syndrome & Foville-Millard-Gubler syndrome is a disease characterized by facial palsy, ipsilateral conjugate gaze palsy and ipsilateral internal strabismus. There has never been a clinical report of this disease in oriental medicine. Since gradual improvement has been experienced with oriental medical treatments based on facial palsy and paralytic strabismus in this case, it is worthwhile to introduce this disorder and report the case referring to clinical record. Methods : 75-year-old female patient and 45-year-old male patient suffering right facial palsy, right conjugate gaze palsy and right internal strabismus were treated with acupuncture, herbal medicine, pharmacopuncture and electroacupuncture. Result : Concurrence of right facial palsy, right conjugate gaze palsy and right internal strabismus was diagnosed with Foville syndrome or Foville-Millard-Gubler syndrome which is generally caused by the impairment of PPRF, abducens and facial nerves on the affected side and showed improvement with oriental medical treatments. Conclusion : More accurate diagnosis, more aggressive treatments and further researches are all required on this disease afterwards.

  • Facial palsy is commonly encountered disease in the clinic but bilateral facial palsy is known as rare disease. Type of facial nerve paralysis include unilateral, recurrent ipsilateral, recurrent alternating and bilateral simultaneous palsies. Among the types, the reported incidence of bilateral simultaneous palsy is 0.3~2% of facial paralysis patients. We experienced I case of patient with bilateral simultaneous facial palsy that was concluded as bilateral bell's palsy. Objective : The purpose of this paper is to report the patient with bilateral facial palsy, who improved by oriental medical treatment. Another purpose is to review the current literature and to differential diagnosis of bilateral facial paralysis. Methods and Results : The patient was treated by acupuncture, herb medication and self-massage on facial muscle for 14 weeks. House-brackmann grading score was improved into I/I (Rt/Lt) from IV/IV. Conclusion : Through reviewing some literatures and reports, It is concluded that bilateral facial palsy was related to many other disorders and more ominous than unilateral facial palsy. therefore, its work-up should include a complete neurologic assesment and thorough evaluation. also, we consider that bilateral Bell's palsy can improve by oriental medical treatments.

A Clinical Study on 1 Case of Patient with Bilateral Simultaneous Bell's Palsy Treated by Hominis Placenta Herbal-Acupuncture (자하거(紫河車) 약침치료를 이용한 양측 동시성 Bell's palsy 환자 1례에 대한 증례보고)

  • Objective : This study was carried out to investigate the progress of bilateral simultaneous facial palsy and the effect of Hominis Placenta herbal-acupunture and the other oriental medical therapies. Method : We used two methods to research the progress of disease. 1. Diagnosis - Facial muscle test, Taste test, Hearing test, Photographies, Lab-finding 2. Treatment - Acupuncture, Herbal-acupuncture, Electroacupuncture, Herb-med Edema rate, Pain endurance, WBC, Hemoglobin, Platelet, Total protein, Albumin, Globulin, RA factor, CRP Results : The onset of Rt. facial palsy was earlier than Lt. facial palsy 3days. The reaction on the treatment of Rt. facial palsy was more dull than Lt. facial palsy. In terms of treatment period, Rt. facial palsy was very longer than Lt. facial palsy. Conclusion : According to the above results, we discoveried that Hominis Placenta herbal-acupunture and the other oriental medical therapies had good influence on the bilateral simultaneous facial palsy. In the future, we should endeavor to know influence between Rt. and Lt. face in case of bilateral simultaneous Bell's palsy.

  • 1. Objectives Facial Palsy(Bell's palsy) is a common disease in oriental medicine. In Sasang Constitutional Medicine, Taeumin have a basic condition by Ganyeol as their inherent symptomatic phamacology. This case is a study about Taeumin's facial palsy in Ganyeol conditions. 2. Methods In treatment of facial palsy, especially in acute stage, We prescribe Yeoldahanso-tang(熱多寒少湯) when a patient with facial palsy is diagnosed as Taeumin by their own characters. 3. Conclusions This case-study shows an efficient results by using Yeoldahanso-tang(熱多寒少湯) in treatment of facial palsy compared with other known treatments.

A Study on the Position and the Pattern of the Facial Palsy According to Sex and Age (성별 및 연령에 따른 안면신경마비 출현 양상에 관한 연구)

  • Facial palsy is a general nerve disease which is occurred in a lateral incomplete or complete facial palsy. But it makes man restrict a social activity and so it is very important that we know the prognosis factors of a patient for medical care. We want to investigate the position and the patterns of the facial palsy according to sex and age. In this study, we performed the excitability test of the facial nerve on 103 patients (male 45 and female 58) which diagnosed as the Bell's palsy having an acute facial palsy, and the rate of the facial palsy was 42% (43 case) in right oculi and 58% (60 case) in left oculi. The facial palsy was appeared widely from young to elderly patients and especially, the rate of the facial palsy was high in age groups from 40 to 50.

The Clinical Observation of Facial Palsy Sequela (안면신경마비 후유증에 대한 임상적 고찰)

  • Objectives : In order to obtain the clinical type of facial palsy sequelae and try to make the treatment protocols for each, I observed patients who visited Gunpo-Wonkwang oriental medicine center with Bell’s palsy sequelae that were treated over three months. Methods : I make the value standard of muscle paralysis, contraction, synkinesis and acquired the results as follows. Results and Conclusions : 1. The distribution of age and sex was as follows : females of 41-50 years were the most common demographic, females of 51-60 years and males of 31-40 years were the second, males of 51-60 years were the third, females of 21-30 years and males of over 60 years were the fourth, and males of 41-50 years were the fifth. 2. The distributions of period of disease were as follows : 3-6 months was the most, 12-18 months was the second, 6-12 months and over 24 months was the third, and 18-24 months was the fourth. 3. The sequelae distributions of disease were as follows. In the group of 3-6 months, 12 persons (80%) showed palsy and atrophy, 10 persons (66.6%) showed synkinetics. In the group of over 6 months, all patients showed muscle palsy, muscle atrophy and synkinetics. All groups showed lower sensitivity of muscles, but the group of 18-24 months and the group of over 24 months showed more. Tinnitus was shown by the groups of 12-18 months and 3-6 months. Facial muscle pain was shown by the group of3-6 months only, Crocodile's tear was shown by the groups of 18-24 months and over 24 months. 4. The total palsy rates of sequela patients and palsy rates by muscle for disease period were as follows. The total palsy rate was 27.94%; the palsy rates for the group of 6-12 months and the group of over 24 months was lower than the total palsy rate. The rates of the groups of 3-6, 12-18, 18-24 months were higher than the total palsy rate. The palsy rate of zygomatic minor, levator labii superior muscle was higher than the total palsy rate for all groups. 5. Synkinetics manifestation rates by disease period were as follows. Total synkinetics manifestation rate was 73.81 %; the manifestation rate of the group of 6-12 months was lower than total synkinetics manifestation rate. For the groups of 12-18, 18-24, and over 24 months it was more than the total synkinetics manifestation rate. The group of over 24 months, total synkinetics induced by orbicularis oculi muscle and orbicularis oris muscle. 6. Facial muscle atrophy rates by disease period were as follows. Total atrophy rate was 5.26%; in the groups of 6-12, 18-24, over 24 months, the atrophy rates were higher than the total atrophy rate. The groups of 3-6 and 12-18 months showed lower than the total atrophy rates, while the atrophy of the levator palpebrae superioris muscle and levator palpebrae inferioris muscle was higher than in other groups.

Changes of Infrared Thermographic Findings in Bilateral Bell's Palsy Treated with Stellate Ganglion Block -A case report- (양측성 안면마비에서 적외선 체열 영상 촬영으로 평가한 성상신경절 차단의 치료 효과 -증례 보고-)

  • Facial nerve palsy is not a serious disease, but it can be both upsetting and disabling for patients. More than half of the lesions of facial nerve palsy fall into the category termed Bell's palsy. It is very rare to find a representative case of bilateral Bell's palsy. Here we report the changes of infrared thermographic findings in bilateral Bell's palsy treated with a stellate ganglion block (SGB). A 45-year-old female patient who had a right facial palsy which developed 2 weeks before. Steroid administration and acupuncture was not effective and so she was referred to pain clinic. The right facial palsy was cured after 19th right SGB. Twenty eight days after the onset of the right facial palsy, left facial palsy also developed and cured completely with a left SGB. Serial infrared thermograms were performed. The hypothermias on the affected side improved symmetrically by the end of the treatment.

Study of the Relation of Autonomic Nerve System and Peripheral Facial Palsy by the Heart Rate Variability (HRV(Heart Rate Variability)를 통한 말초성 안면신경마비와 자율신경실조의 상관성 연구)

  • Objectives : This study was designed to evaluate the effect of inbalance of autonomic nerve system on peripheral facial palsy patients in comparison of Heart Rate Variability results of healthy control group with that of peripheral facial palsy group. Methods : The peripheral facial palsy patiens who visited the Dongguk university Bundang oriental medicine hospital from April to June in 2005 were tested Heart Rate Variability items which are Mean Heart Rate(MHRT), Standard Deviation of all the Normal RR intervals (SDNN), Root Mean Square of Successive Differences Between The Normal Heart Beats(RMSSD), Total Power(TP), Low Frequency, High Frequency, ratio between the Low Frequency and High Frequency power(LF/HF ratio). The Heart Rate Variability test results of peripheral facial palsy group were compared with the HRV test results of healthy control group who didn't have nervous disease, musculoskeletal disease, heart problem and pain for the past 6 months. Results : 1. MHRT was not significantly different between peripheral facial palsy group and healthy control group(P=0.18). 2. SDNN was not significantly different between peripheral facial palsy group and healthy control group(P=0.41). 3. RMSSD was not significantly different between peripheral facial palsy group and healthy control group(P=0.93). 4. TP was not significantly different between peripheral facial palsy group and healthy control group(P=094). 5. LF and HF were not significantly different between peripheral facial palsy group and healthy control group((P=0.34, 0.30). 6. LF/HF ratio was significantly different between peripheral facial palsy group and healthy control group(P=0.04). Conclusion : LF/HF ratio is generally used as autonomic nerve system evaluation and there was significantly statistical difference between peripheral facial palsy group and healthy control group.

  • Bell's palsy is an acute facial paralysis caused by peripheral facial nerve dysfunction. The aim of this study is to assess the efficacy of Korean medicine in the treatment of three patients who were hospitalized and diagnosed with Bell's palsy within 2 weeks of coronavirus disease 2019 (COVID-19) infection. The patients were administered with Korean medicine treatments, steroids, and antiviral drugs. Moreover, the Korean medicine treatments include acupuncture, pharmacopuncture, moxibustion, physical therapy, and herbal medicine. Symptom improvement was evaluated daily using the Yanagihara facial nerve grading system, a facial function evaluation tool. Furthermore, it was suggested that the patients affected by Bell's palsy after COVID-19 infection may have a slower improvement in their treatment progress compared with those without COVID-19 infection in the acute stage.

A Clinical Study of Oriental-Western Medicine Treatment on Facial Nerve Paralysis (말초성 안면신경마비의 한양방 협진치료에 관한 임상적 연구)

  • Objectives : Facial palsy is not uncommon disease and most patients with facial palsy are peripheral type, as in Bell' s palsy. In western medicine, oral steroid is the mainstay of the treatment. Recently, oriental-western treatment became one of the alternative modality for the treatment of the facial palsy. However, the treatment result and the degree of patient' s satisfaction were not evaluated. In this study, we tried to characterize the clinical characteristics, short-term recovery rate and degree of patient' s satisfaction after oriental-western medicine treatment on facial palsy of peripheral type. Methods : Between May 2008 and December 2008, we examined 16 patients who presented with facial palsy of peripheral type in Dongguk University Medical Center. Clinical characteristics and recovery rate was analyzed by retrospective chart review. The degree of patient' s satisfaction was measured by 5 point scale. Results : The causes of facial palsy were Bell' s palsy (87.5%) and the Ramsay-Hunt syndrome (12.5%). The highest age groups of facial palsy were 6th and 7th decades. Most frequent accompanying symptom was postauricular pain. After oriental-western medicine treatment, 10 patients (62.5%) showed recovery of facial palsy better than House-Brackmann grade 2. 11 patients (68.8%) were satisfied with the oriental-western medicine treatment. Conclusions : Considering the degree of patient' s satisfaction and treatment result, we believe that oriental-western treatment could be safe and reliable protocol for the treatment of facial nerve palsy of peripheral type.
