Updating Korean Disability Weights for Causes of Disease: Adopting an Add-on Study Method

  • Objectives: Disability weights require regular updates, as they are influenced by both diseases and societal perceptions. Consequently, it is necessary to develop an up-to-date list of the causes of diseases and establish a survey panel for estimating disability weights. Accordingly, this study was conducted to calculate, assess, modify, and validate disability weights suitable for Korea, accounting for its cultural and social characteristics. Methods: The 380 causes of disease used in the survey were derived from the 2019 Global Burden of Disease Collaborative Network and from 2019 and 2020 Korean studies on disability weights for causes of disease. Disability weights were reanalyzed by integrating the findings of an earlier survey on disability weights in Korea with those of the additional survey conducted in this study. The responses were transformed into paired comparisons and analyzed using probit regression analysis. Coefficients for the causes of disease were converted into predicted probabilities, and disability weights in 2 models (model 1 and 2) were rescaled using a normal distribution and the natural logarithm, respectively. Results: The mean values for the 380 causes of disease in models 1 and 2 were 0.488 and 0.369, respectively. Both models exhibited the same order of disability weights. The disability weights for the 300 causes of disease present in both the current and 2019 studies demonstrated a Pearson correlation coefficient of 0.994 (p=0.001 for both models). This study presents a detailed add-on approach for calculating disability weights. Conclusions: This method can be employed in other countries to obtain timely disability weight estimations.

  • The purpose of this study would like to analyze statically significant difference for low back-pain frequency of mother after development-disability children. Seven nursery children with disability conducted survey from 122 mothers cared children with disability. Survey data was obtained from April 14. 2006 to May 23. 2006. The results were as follows: According to walking-existence, assistance walking, and disability-degree, low back pain incidence frequency of mothers were statically significant difference, (p<0.05). Low back pain incidence frequency of walker-ability population was 51.4%, but low back pain incidence frequency of walker-disability population was 80.0%. then low back pain incidence frequency of mothers to walking-existence was differed amount. Disabled not statically significant difference to encephalopathy and disability-type1 and disability -type2 (p>0.05). children with disability-degree and assistance walking benchmarked low back pain disability-measure. Low back pain degree not relevancy statically significant. Physical load was statically significant difference between Oswestry's low back pain score and reach effect to child-cared(p<0.05). As development-children with disability of disable degree, Mother appeared to highly low back pain frequency rate and appeared to large reach effect child-cared owing to physical load of low back pain. So hereafter, location and person request to approach with more clinical and objectively. As approach result, it will help to stress solution of children with disability owing to develop to low back pain class and family capable strengthening program and so on.

장애아 보육시설의 물리적 환경에 대한 설문조사연구 (Questionnaire Survey on the Physical Environment of Child Care Centers for Children with Disability)

  • Eater Seal, the child care experts, observes that children with disability significantly benefit from receiving appropriated care along side their peers. Typically developing children serve as role models from which children with disabilities learn age-appropriated communications and social behavior. Therefore the children with disability can actively participate in well-designed child care settings. But unfortunately in Seoul, Korea, only 2% of children with disability are taken care of in child care centers. And even the disabled children at the child care centers are mostly mentally or emotionally disabled because the building, programs and services of the centers are restrictive to the children with severe physical disabilities. This study reviewed Korean domestic physical environment of child care centers for children with disabilities. Questionnaire survey was conducted to 103 centers located in Seoul by mail. Our survey revealed that they need more areas for nursing room and special rooms and the accessibility to buildings has to be improved. Also, there are no elevators in all buildings surveyed. Accessibility to toilet is noted as significant problem, especially wet floors in toilets. In a nursing room, a space for psychological rest and special education is needed for children with mental disability. Technical specifications on how to make buildings and facilities accessible for the children with disability should be developed. The goal of this study is to provide basic information to develop domestic design guidelines to ensure that the child care centers are safe, convenient, and usable for everyone possible.

국민건강면접조사를 이용한 한국인의 DALE (Disability-Adjusted Life Expectancy)에 관한 연구 (Study of Disability-Adjusted Life Expectancy(DALE) Using National Health Interview Survey in Korea)

  • Objectives : To measure DALE (Disability-Adjusted Life Expectancy) in Korea to find out how long Koreans live in a state of full heath. Methods : DALE was calculated using the life table of 1999 and the disability prevalence from the National Health Interview Survey (NHIS), which was conducted with a sample of 13,523 households in 1998. The disability prevalence was measured using the annual prevalence of the long-term limitation of activities, which were divided into classes 1, 2, 3, 4, 5 and 6 according to the severity of the limitation. The disability weights were measured for each 6 class by conducting a survey of 16 healthcare professionals. The severity-adjusted disability prevalence was calculated by multiplying the disability prevalence of each class by the disability weights respectively. Healthy life years lost due to disability was calculated by multiplying the life expectancy by the severity-adjusted disability prevalence. Finally DALE was measured as the life expectancy minus healthy life years lost due to disability. Results : DALE for 1999, which refers to the expectation of equivalent years of good health, were 72.5, 69.5 and 75.3 years, for total, for males and for females, respectively. The percentages for DALE out of the life expectancy were 95.8, 96.6 and 94.4% for total, for males and for females, respectively. Conclusions : DALE is a newly developed indicator, which could effectively show the healthy life expectancy of populations. A greater notice and use of DALE would be expected as life expectancies increase and the quality of life changes in Korea.

한국인의 활동장애가 없는 건강여명에 관한 연구 (A Study on Korean Disability-Free Life Expectancy)

  • 이 논문은 건강수명의 개념을 도입하여 우리 나라 국민의 건강수준이 어느 정도인지를 규명하고 있다. 평균수명으로는 수명의 양적인 측정은 가능하지만 수명의 질적인 측면의 측정이 배제되기 때문에 일부 선진국가에서는 국민들의 사망과 상병상태를 결합한 활동장애가 없는 건강여명(Disability-Free Life Expectancy)을 산출하여 이용하고 있다. 우리 나라 국민의 활동장애가 없는 건강여명을 산출하기 위하여 사망자료로는 1989년도 간이생명표를 이용하였으며 상병자료로는 대표성을 고려하여 표본의 크기가 가장 큰 1989년 국민건강조사를 이용하였다. 주요 결과를 보면 우리 나라 남자의 경우 0세에서 활동장애가 없는 건강여명은 60.48년이었으며, 여자는 남자보다 3.2년이 더 높은 63.80년으로 나타났다. 연구자에 따라 활동장애에 대한 정의나 측정방법이 다르기 때문에 국제간 정확한 비교는 곤란하지만 우리 나라의 활동장애여명을 외국과 비교해보면 단기이환으로 인한 활동장애여명은 비교적 높지만, 장기이환으로 인한 활동장애여명은 선진국에 비해 낮은 수준이라 할 수 있다. 앞으로 우리 나라에서도 국민의 건강수준을 질적 양적 측면에서 정확히 평가하기 위하여 필요한 기초자료가 주기적으로 생산되어야 할 것이다.

장애 중증도 수준에 따른 장애 유형이 고용에 미치는 영향: 장애인고용패널조사를 중심으로 (Effect of Disability Types by Disability Severity Levels on Employment: Based on the Employment Panel Survey for the Disabled)

  • 목적 : 본 연구의 목적은 장애 중증도 수준뿐만 아니라 장애 유형 또한 함께 고려하여 장애인의 고용현황을 분석함으로써 기존의 장애 중증도 수준에 기반한 의학적 손상을 기준으로 하여 직업적 능력을 고려하는 것에 한계가 있음을 지적하는 데에 있다. 연구방법 : 장애인고용공단(2019)에서 조사한 장애인 고용패널조사 2차 웨이브 4차 조사 자료를 사용하였다. 독립변수는 장애인의 장애 중증도와 장애 유형, 종속변수는 고용여부로 하여 장애 중증도에 따른 장애 유형의 고용가능성 오즈비를 로지스틱 회귀분석으로 산출하였다. 결과 : 장애정도가 심한 군은 고용가능성은 관련 변수들을 보정했을 때, 신체외부장애 유형에 비해 신체내부장애의 고용가능성이 0.413(95% CI: 0.271-0.629)배로 유의하게 낮았고(p<.001), 장애정도가 심하지 않은 군에서는 신체외부장애에 비해 신체내부장애가 0.475(95% CI: 0.327-0.690)배로 고용가능성이 낮았다(p<.001). 결론 : 장애 중증도 수준이 같다 하더라도 장애 유형에 따라 고용가능성이 달라질 수 있음을 확인하였다. 장애 유형과 중증도를 모두 고려하여 고용가능성의 편차를 줄일 수 있는 판정 기준의 마련이 필요하다.

  • 본 연구는 장애수용이 정책신뢰도를 매개로 삶의 만족에 미치는 영향을 검증하고, 이 관계가 장애유형에 따라 집단 간 차이가 있는지 살펴보고자 하였다. 이를 위하여 한국장애인고용공단이 2017년 실시한 장애인고용패널조사 2차웨이브 2차년도 원자료를 활용하였다. 응답자 중 신체외부장애인 2,378명과 정신적장애인 571명의 자료를 추출하여 분석하였다. 분석방법은 매개효과 검증 및 집단 간 차이 검증을 위해 구조방정식모형을 활용하였다. 연구결과는 다음과 같다. 첫째, 장애인의 장애수용은 정책신뢰도를 매개로 삶의 만족에 영향을 미치는 것으로 나타났으며, 부분매개효과가 검증되었다. 둘째, 신체외부장애인과 정신적장애인 두 집단 모두 장애수용과 정책신뢰도가 삶의 만족 예측요인이었다. 셋째, 신체외부장애인과 정신적장애인 두 집단 간의 경로계수에 차이가 확인되었다. 본 연구결과를 토대로 장애인의 삶의 만족 향상을 위한 실천방안에 대해 논의하였다.

지역사회 재가 노인의 기능장애 위험요인 (Risk factors for functional disability among community dwelling elderly)

  • Objectives: The purpose of this study was to investigate risk factors for chronic ADL, IADL disability. The study explored clinical and socio-demographic risk factors of functional status decline. Methods: Data from the Survey of Living Condition of Elderly 3-year panel study were analyzed. The study subjects were 5,928 community-dwelling people aged 65 years or older who were no disability in ADL and IADL at baseline. Predisposing factors, pathology, impairment, and functional limitations were regarded as risk factors. Logistic regression analysis was used. Results: During the 3-year study period, 3.9% participants developed chronic ADL disability, 9.4% participants were IADL disabled. After controlling for predisposing factor, the best predictors for ADL disability at 36 months were fall as a pathology factor, cognitive decline, disability judgement, lower limb functional limitation. Comorbidity, fall, cognitive decline, disability judgement, lower limb and upper limb functional limitation were risk factors for IADL disability. Conclusions: Health promotion program focusing elderly is essential to prevent ADL and IADL disability. Mobilizing physical activity should be included in health promotion program for elderly.

장애인관련 정보시스템의 성과평가 및 개선 방안에 관한 연구 (The Study on Performance Evaluation and Improvement of the Information System for the Disabled)

  • Purpose: The study tried to suggest the ways to improve the national disability welfare information projects with evaluating the present condition of the projects. Method: Firstly, the study analyzed the disability information system and disability websites. Secondly, the study found out the problems of the registration of disability and management system through the situation analysis. Finally, the study conducted a survey of the disabled and the information system workers and performance evaluation of the information system. Result: The study offered a proposal of The Disability Unity Management System(DUMS) for the disability registration and management. When the disabled visit the hospital or the public health center, their information is registered automatically in The Disability Unity Management System(DUMS), which helps not to miss out the disability registration. Through this system, they are given the proper service which they need, and the information can be shared with the facilities providing the service for the disabled. Conclusion: The Disability Unity Management System(DUMS) contributes significantly in avoiding the repetition of the support for the disabled and improving the quality of service.

요통기간에 따른 손상, 장애, 심리적 요인들의 상관성 비교 (Comparison of the Relationship Between Impairment, Disability and Psychological Factors According to the Difference of Duration of Low Back Pain)

  • The purpose of this study was to investigate the correlations between pain intensity, physical impairments, disability, and psychological factors according to the difference in duration of low back pain. This study was a cross-sectional survey of 102 participants with low back pain, divided into two groups equal in number: The first group consisted of patients with acute and subacute low back pain, while the second group consisted of patients suffering from chronic low back pain. The results showed that gender, age, pain intensity, physical impairment, disability and Fear-Avoidance Beliefs (FABs) for work activities were not significantly different between two groups. FABs for physical activities of the first group were significantly more prevalent than in the second group. More than moderate correlations were found between pain intensity, physical impairment, and disability in the first group. Less than moderate correlations were found between pain intensity, physical impairment, disability, FABs, and depression in the second group. These findings suggest that we must consider psychological factors in the treatment of patients with chronic low back pain. Regression analyses revealed that pain intensity and FABs for work activities significantly contributed to the prediction of disability in the first group. Also, pain intensity and FABs for physical activities significantly contributed to the prediction of disability in the second group. Pain intensity was most important predictor of disability in two groups.
