Estimation of risks for social exclusion in persons with psychosocial disabilities : a comparison between persons with psychosocial disabilities and those with other types of disabilities (정신장애인의 사회적 배제에 대한 위험 추정 : 타 장애집단과의 비교)

  • The purpose of the study was to conduct an empirical study on the scope and level of social exclusion experienced by persons with psychosocial disabilities. The Wave 6 data of the Panel Survey of Employment for the Disabled were used to investigate the current status of social exclusion in life areas including income, education, work, housing, health, social network, social participation, and discrimination (N=4,161). A series of logistic regressions were executed, with the psychosocial disability(PD) group being the reference; the reverse odds rations of different disability groups were compared against the PD reference group (OR=1). The results showed that compared with all others, the PD group was about 2 to 11 times more likely to have risks in income, work, and housing; and that compared to other groups except for the autism/developmental disability group, they were about 6 to 10 times more likely to have risks in social network, social participation and discrimination. In conclusion, the authors urged the need for legal mandates to have community based mental health services and welfare services for the disabled more available to the PD groups and the necessity to eliminate various discriminatory legislations that are violating human and social rights of the persons with PDs.

Research of Usability Test on Disabled Welfare Vehicle for Guardians and Passengers of Disabled People (장애인 보호자 및 탑승자를 대상으로 한 장애인 복지차량 사용성 평가 연구)

  • This research is to anticipate problems of Ray's disabled experiment welfare vehicles for 17 drivers with disability, improving them. In addition it is to provide the criteria for ergonomic design based on perspective of drivers with disability by investigating 18 items of usability test. The results are as follows. First, satisfaction degree of Ray, disabled experiment welfare vehicle, was shown to be 3.88 which is higher than normal vehicles whose degree is 3.20. This showed that the disabled experiment welfare vehicle is the one with much improvement. Second, so as to develop a welfare vehicle it needs to take into account some factors including rear slope, wheelchair locker, seat belt, safety grip, and high roof. Third, in case of rear slope, high roof and width of manual or automatic wheel chairs should be considered and motor-operated device should also be taken into account for safety lockers, which make a wheelchair firmly fixed. Moreover, motor-operated seat and rear slope can be chosen for either of the disabled and the elderly.

Determinants of Poverty Among the Working Poor with Disabilities (근로장애인 빈곤 결정요인에 관한 연구)

  • Although linkages between poverty and disability are often noted, until recently they have not been systematically examined in Korea. Many people with disabilities tend to become poorer because they lack access to jobs. And income support programs are not sufficient. But more severe is people with disabilities in labor market tend to be poor. The purposes of this study were to explore the characteristics of working poor with disabilities and to identify the major determinants of poverty. For this, '2005 Survey on the Workers With Disabilities' was used. The data was analyzed by descriptive statistics, t-test, logistic regression. The results showed that gender, age, education level, marital status, region where they live, the number of the household member, the severity of disability, the onset of disability are significant predictors in determining their poverty status. Also employment status, occupation type, industry type, numbers of years in current job are major determinants of their poverty status. Finally, based on these results several policy implications were presented.

Computer Adaptive Testing Method for Measuring Disability in Patients With Back Pain

  • Most conventional instruments measuring disability rely on total score by simply adding individual item responses, which is dependent on the items chosen to represent the underlying construct (test-dependent) and a test statistic, such as coefficient alpha for the estimate of reliability, varying from sample to sample (sample-dependent). By contrast, item response theory (IRT) method focuses on the psychometric properties of the test items instead of the instrument as a whole. By estimating probability that a respondent will select a particular rating for an item, item difficulty and person ability (or disability) can be placed on same linear continuum. These estimates are invariant regardless of the item used (test-free measurement) and the ability of sample applied (sample-free measurement). These advantages of IRT allow the creation of invariantly calibrated large item banks that precisely discriminate the disability levels of individuals. Computer adaptive testing (CAT) method often requiring a testing algorithm promise a means for administering items in a way that is both efficient and precise. This method permits selectively administering items that are closely matched to the ability level of individuals (measurement precision) and measuring the ability without the loss of precision provided by the full item bank (measurement efficiency). These measurement properties can reasonably be achieved using IRT and CAT method. This article aims to investigate comprehensive overview of the existing disability instrument for back pain and to inform physical therapists of an alternative innovative way overcoming the shortcomings of conventional disability instruments. An understanding of IRT and CAT method will equip physical therapist with skills in interpreting the measurement properties of disability instruments developed using the methods.

The Effect of Disability Acceptance Mediated with Policy Trust on the Life Satisfaction: Group Comparison of Disability Type (장애수용이 정책신뢰도를 매개로 삶의 만족에 미치는 영향: 장애유형별 집단 비교)

  • The purpose of this study is to analyze the effect of disability acceptance mediated with policy trust on the life satisfaction and group difference of disability type. For this purpose, the raw data(2nd wave, 2st survey) of Panel Survey of Employment for the Disabled(PSED) conducted by the Korea Employment for the Disabled were used. Among the respondents, the data of 2,378 of the external physical disabled and 571 of mental disabled were analyzed. For the analysis, the structural equation model was used to verify the effect of mediation and group difference. The result of the study were as follows. First, disability acceptance of the disabled people effect on the life satisfaction by policy trust, and this was verified with partial mediated model. Second, In both groups, the external physical disabled and metal health disabled, disability acceptance and policy trust were the predictors of life satisfaction. Third, the difference in path coefficients between the two groups of external physical disabled and mental disabled was identified. Based on the results of this study, action plans were discussed to improve the life satisfaction of the disabled.

The Effect of Proprioceptive Neuromuscular Facilitation Respiratory Muscle Strengthening Exercise on Pulmonary Function and Disability Level in Chronic Low Back Pain Patients (고유수용성신경근촉진법 호흡근 강화 운동이 만성 허리통증 환자의 폐 기능과 장애 수준에 미치는 영향)

  • Purpose: This study investigated the effects of proprioceptive neuromuscular facilitation (PNF) respiratory muscle strengthening exercise on the pulmonary function, back pain, and disability level of patients with chronic low back pain (CLBP). Methods: There were 24 CLBP patients randomly divided into the experimental group (n = 12) who took part in PNF respiratory muscle strengthening exercise and the control group (n = 12) who performed a mock treatment. Both groups performed their respective interventions for 15 min five times per week for four weeks. The pulmonary function was measured using a portable spirometer. The back pain of the participants was assessed using a visual analogue scale. The Oswestry disability index was conducted to measure disability level. A paired t-test was performed to compare within-group changes before and after the PNF respiratory muscle strengthening exercise. Differences between the experimental and the control groups were analyzed using an independent t-test. For all tests, the level of statistical significance was α = 0.05. Results: The within-group change in pulmonary function was significantly different in the experimental group (p < 0.05), and the levels of back pain and disability were significantly different between the groups (p < 0.05). There was also a significant between-group difference in pulmonary function, back pain, and disability level after intervention (p < 0.05). Conclusion: These results suggest that PNF respiratory muscle strengthening exercise enhances pulmonary function and reduces levels of back pain and disability in patients with CLBP.

Local Self-organization Ordinance Analysis for the Abolition of Disability Rating System (장애등급제 폐지를 위한 지방자체단체 조례분석 -장애인복지법상 장애등급을 적용하고 있는 조례를 중심으로-)

  • The purpose of this study is to develop a plan to improve the status of local autonomous body ordinances applying disability grades according to welfare law for people with disabilities. The results are that First, 215 of the total municipalities applied the disability grade, and 1,071 cases were applied to the disability welfare law. Among them, 60.04% (643 cases) of the ordinance that can be divided into severe (1-3 grade) and mild (4-6 grade) were found, and 39.96% (428 cases). A new service eligibility standard that reflects factors such as the needs of persons with disabilities, social environment, etc. needs to be developed. Second, since the exemption reduction service accounts for more than 40% of all projects, it is necessary to set the standards considering the other standards within the scope of not decreasing the total service amount when revising the ordinance. Based on these results, We will provide important basic data for establishing new service provision criteria to replace the disability level at the local government level.

  • The number of disabled person is increasing, and also the demand of welfare is increasing. In the developed country, the range of disabled person is broad, because the definition of disability is different among countries. On 10/Apr, 2007, the law to forbid discrimination against disabled person was announced and then it will be operated on 11/Apr, 2008, in Korea. It will need to evaluate conflict between this law and underwriting guideline. There is obvious difference between disabled and non-disabled on risk selection of medical and environmental aspect. More than 50% of disabilities are related to the diseases rather than accident. The role of underwriter is to evaluate the future risk of morbidity (admission and/or operation etc). The insurance company is required to make new product for disabled person and the underwriter has to examine the application of disabled without discrimination, and with respect for the disabled as a human right.

Chromosomal Microarray Testing in 42 Korean Patients with Unexplained Developmental Delay, Intellectual Disability, Autism Spectrum Disorders, and Multiple Congenital Anomalies

  • Chromosomal microarray (CMA) is a high-resolution, high-throughput method of identifying submicroscopic genomic copy number variations (CNVs). CMA has been established as the first-line diagnostic test for individuals with developmental delay (DD), intellectual disability (ID), autism spectrum disorders (ASDs), and multiple congenital anomalies (MCAs). CMA analysis was performed in 42 Korean patients who had been diagnosed with unexplained DD, ID, ASDs, and MCAs. Clinically relevant CNVs were discovered in 28 patients. Variants of unknown significance were detected in 13 patients. The diagnostic yield was high (66.7%). CMA is a superior diagnostic tool compared with conventional karyotyping and fluorescent in situ hybridization.

  • Depression has a direct relationship with lower quality of life for elderly with cerebral vascular accidents(CVA). Daily hassles worsen depression for elderly with CVA disability and thus an effective intervention is necessary. This research examined the direct influence of daily hassles on depression and the mediation effect of family support among 161 elderly with CVA disability living in Seoul using Structural Equation Modeling. The direct effect of daily hassles on depression was statistically significant (p < .001) while family support was shown to be a mediator between daily hassles and depression. The mediation model of family support was examined using $x^2$ difference and Soble test. As a result, family support was proven to be a partial mediator. Implications are discussed.
