Exercise With a Novel Digital Device Increased Serum Anti-influenza Antibody Titers After Influenza Vaccination

  • It has been reported that some exercise could enhance the anti-viral antibody titers after vaccination including influenza and coronavirus disease 2019 vaccines. We developed SAT-008, a novel digital device, consists of physical activities and activities related to the autonomic nervous system. We assessed the feasibility of SAT-008 to boost host immunity after an influenza vaccination by a randomized, open-label, and controlled study on adults administered influenza vaccines in the previous year. Among 32 participants, the SAT-008 showed a significant increase in the anti-influenza antibody titers assessed by hemagglutination-inhibition test against antigen subtype B Yamagata lineage after 4 wk of vaccination and subtype B Victoria lineage after 12 wk (p<0.05). There was no difference in the antibody titers against subtype "A." The SAT-008 also showed significant increase in the plasma cytokine levels of IL-10, IL-1β, and IL-6 at weeks 4 and 12 after the vaccination (p<0.05). A new approach using the digital device may boost host immunity against virus via vaccine adjuvant-like effects.

  • We present an algorithm for automatic anatomically adaptive image enhancement of digital chest radiographs. Chest images were exposed using digital radiography system with a 0.143 mm pixel pitch, l4-bit gray levels, and 3121 ${\times}$ 3121 matrix size. A chest radiograph was automatically divided into two classes (lung field and mediastinum) by using a maximum likelihood method. Each pixel in an image was processed using fuzzy domain transformation and enhancement of both the dynamic range and local gray level variations. The lung fields were enhanced appropriately to visualize effectively vascular tissue, the bronchus, and lung tissue, etc as well as pneumothorax and other lung diseases at the same time with the desired mediastinum enhancement. A prototype implementation of the algorithm is undergoing trials in the clinical routine of radiology department of major Korean hospital.

Analysis of the Anatomy of the Venae Comitantes Related to the Digital Artery (손가락동맥과 동반정맥 구조에 대한 해부학적 연구)

  • Purpose: Many descriptions of the digital arterial anatomy including skin territory of the finger have been published. Relatively few studies on venous architecture of the finger have been performed in this area, in part, attributable to the technical difficulties encountered in dissecting small vessels. The purpose of this study is to present the precise microsurgical anatomy of the vein related to the digital artery and venae comitantes of the components. Methods: Arterial and venous anatomy of their relation to the fingers were examined in 38 specimens of two fresh cadavers and 36 clinical cases. All specimens were evaluated grossly, surgical microscopically, or / and light microscopically to observe the three & two-dimensional structure of the artery and joining vein, evidence of the venae comitantes, and venous valve. Results: No longitudinal venae comitantes along the digital artery were found in any specimens. The size of the venae comitantes of each digital artery was much smaller than other vein, but always existed any level of digital artery. One or two venae comitantes in the digital artery ran spiral, oblique, helical, fibrillar, or irregular branched shape. The authors also found the vein of the finger, that had bicuspid valves, but not in venae comitantes. Conclusion: Recently, venous outflow problem rather than arterial circulation is the most common cause tissue failure after microvascular surgery in the hand. Sometimes, if it is not recognized early, there is an increased risk of tissue damage and loss. The authors concluded that this study presents a useful knowledge for the characterization of the venous structure and evidence for venae comitantes like a venule in the digital artery at varying levels of the finger.

Principles for evaluating the clinical implementation of novel digital healthcare devices (첨단 디지털 헬스케어 의료기기를 진료에 도입할 때 평가원칙)

  • With growing interest in novel digital healthcare devices, such as artificial intelligence (AI) software for medical diagnosis and prediction, and their potential impacts on healthcare, discussions have taken place regarding the regulatory approval, coverage, and clinical implementation of these devices. Despite their potential, 'digital exceptionalism' (i.e., skipping the rigorous clinical validation of such digital tools) is creating significant concerns for patients and healthcare stakeholders. This white paper presents the positions of the Korean Society of Radiology, a leader in medical imaging and digital medicine, on the clinical validation, regulatory approval, coverage decisions, and clinical implementation of novel digital healthcare devices, especially AI software for medical diagnosis and prediction, and explains the scientific principles underlying those positions. Mere regulatory approval by the Food and Drug Administration of Korea, the United States, or other countries should be distinguished from coverage decisions and widespread clinical implementation, as regulatory approval only indicates that a digital tool is allowed for use in patients, not that the device is beneficial or recommended for patient care. Coverage or widespread clinical adoption of AI software tools should require a thorough clinical validation of safety, high accuracy proven by robust external validation, documented benefits for patient outcomes, and cost-effectiveness. The Korean Society of Radiology puts patients first when considering novel digital healthcare tools, and as an impartial professional organization that follows scientific principles and evidence, strives to provide correct information to the public, make reasonable policy suggestions, and build collaborative partnerships with industry and government for the good of our patients.

  • The full-field digital mammography (FFDM), which has been known as a digital breast imaging system, carries out more outstanding performance than the screen-film mammography in overall image quality, skin & nipple, description of pectoral muscle and expression of micro-calcification. Thus, in this thesis, I perform experiments for both the enhancement of image quality and accurate estimation of the result in question, when detecting the very tiny-sized lesions in mammography. The image of digital breast X-rays is the important diagnostic tool for detecting early breast cancer and micro calcification lesion. The experiment of how much compression rate has an effect on the result of diagnosis in the case of microcalcification lesion, with JPEG2000 40:1 compression and over 50% enlargement led to obscure or definitely unacceptable diagnostic results is performed. And in another study of assessment of PSNR degree. I recognized the importance of standardized management system in mammography, where not to mention the accurate reading of the image has the most crucial role in diagnosis

A Bibliometric Analysis of Global Research Trends in Digital Therapeutics (디지털 치료기기의 글로벌 연구 동향에 대한 계량서지학적 분석)

  • To analyse the overall research trends in digital therapeutics, this study conducted a quantitative bibliometric analysis of articles published in the last 10 years from 2014 to 2023. We extracted bibliographic information of studies related to digital therapeutics from the Web of Science (WOS) database and performed publication status, citation analysis and keyword analysis using R (version 4.3.1) and VOSviewer (version 1.6.18) software. A total of 1,114 articles were included in the study, and the annual publication growth rate for digital therapeutics was 66.1%, a very rapid increase. "health" is the most used keyword based on Keyword Plus, and "cognitive-behavioral therapy", "depression", "healthcare", "mental-health", "meta-analysis" and "randomized controlled-trial" are the research keywords that have driven the development and impact of digital therapeutic devices over the long term. A total of five clusters were observed in the co-occurrence network analysis, with new research keywords such as "artificial intelligence", "machine learning" and "regulation" being observed in recent years. In our analysis of research trends in digital therapeutics, keywords related to mental health, such as depression, anxiety, and disorder, were the top keywords by occurrences and total link strength. While many studies have shown the positive effects of digital therapeutics, low engagement and high dropout rates remain a concern, and much research is being done to evaluate and improve them. Future studies should expand the search terms to ensure the representativeness of the results.

A Review of the Domestic Study Trends on Obstetrics & Gynecological Diseases by Using Digital Infrared Thermal Imaging as a Diagnosis Instrument (한방여성의학 영역에서 진단 도구로 적외선 체열 검사(Digital Infrared Thermal Imaging, DITI)를 활용한 국내 연구 동향 분석)

  • Objectives: The purpose of this review is to analyze the domestic study trends of clinical use of Digital Infrared Thermal Imaging (DITI) as a diagnosis instrument on obstetrics and gynecological diseases in Korean literature. Methods: We searched for clinical studies using DITI in the journal of Korean obstetrics & gynecology, National Digital Science Library, Research Information Sharing Service, Korean Medical Database up to May 2020. After searching studies, we selected studies and analyzed according to disease. Results: 18 clinical studies were published in two kinds of academic journals. There were 4 studies about dysmenorrhea and menopausal symptoms, 2 studies about postpartum disease and hypercryalgesia, 1 study about amenorrhea, premenstrual syndrome, leukorrhea, infertility, polycystic ovary syndrome and hot flush. Conception vessel was frequently used for DITI measurement, followed by stomach meridian. The temperatures of abdomen (CV4, CV12), face (HN3), upper and lower limb (PC8, LU4, LR3, ST32) were commonly measured. Conclusions: This study shows that DITI could be one of effective diagnosis instrument for obstetrics and gynecological diseases. More well-designed clinical studies using DITI will be needed.

구내 방사선 촬영에 있어서 Film과 Digital sensor에 따른 피폭선량의 차이에 대한 비교

  • 목적 : 기존 구내 방사선 촬영에서 사용되었던 Film에서의 노출선량과 Digital Sensor를 이용한 구내 디지털 촬영에서의 노출선량을 비교하여 현재 광범위 하게 사용 되어지고 있는 Digital Sensor가 환자의 피폭선량을 감소하는데 기여하는 정도를 알아본다. 대상 및 방법 : 치아 우식증이 없는 5개의 구치부 치아를 선택하여 석고 블럭에 매식한 후 교합면과 인접면에 우식병소들을 형성하였다. 이를 필름(Kodak Insight; IS, Kodak Co, USA)과 Digital Sensor(Kodak RVG 6000; Kodak Co, USA)에 XCP Instrument(Rinn Co, USA)를 사용하고, 전면에는 조직등가물질인 Acrylic Resin Block 20mm를 설치하였다. Acrylic Resin Block의 관구 측면 에서는 조사선량계를 부착하여 단계별로 변화시키는 노출조건에 대한 조사선량을 측정하였다. 그리고 이렇게 얻어진 영상을 3명의 방사선학 전공의와 1명 의 보존과 전공의가 평가를 하였다. 결과 : Film과 Digital Sensor를 가지고 촬영한 영상을 분석한 결과 노출선량에서도 Digital Sensor는 Film 노출선량과 비슷한 결과를 도출해 낼 수 있었다. 그러나 Digital Sensor로 촬영된 영상의 경우에는 Film 경우보다 좀더 효과적으로 조사선량을 판단 할 수 있었으며, 또한 영상 판독시 필름보다 폭 넓게 응용 할 수 있었다. 결론 : 본 연구에서는 선량 변화에만 의존하는 것이 아니라 Digital Sensor로 촬영시 영상 조절을 병행한다면 좀더 정확하고 효과적인 진단 활동에 도움이 될 수 있을 것으로 사료된다.

봉독약침을 이용한 반사성 교감신경 이영양증의 치료 -치료 전후 적외선체열영상 비교-

  • Objectives : The purpose of this case in so report the patient with Reflex sympathetic dystrophy, who is improved by Bee venom. Method : We treated the patient with Bee venom who was suffering from Reflex sympathetic dystrophy, using Digital Infrared Thermographic Imaging and Verbal Numerical Rating Scale(VNRS) to evaluate the therapeutic effects. We compared the temperature of the patient body before and after treatment. Result and Conclusion : We found that Bee venom had excellent outcome to relieve pain, atrophy and ankle joint ROM, and that Bee venom also had clinical effect on hypothermia on the Digital Infrared Thermographic Imaging.
