  • The purpose of this study is to evaluate the usefulness of automatic exposure control (AEC) by analyzing entrance surface dose (ESD) and entropy on using automatic exposure and manual exposure. The experimental method was to measure the dose by placing a semiconductor dosimeter on the Rando Phantom for the Pelvis, Abdomen, Skull, and Chest regions. The DICOM file was simultaneously acquired and then entropy was analyzed by using Matlab. As a result, when using the automatic exposure control, dose of all sites was lower than manual exposure's dose and entropy was high. In addition, paired t-test was performed for each item and p<0.05 was found in each item. In conclusion, the use of automatic exposure control can be a useful method to contribute to the optimization of the exposure dose and the image quality by reducing the amount of unnecessary radiation amount and information loss that can occur in X-ray examination.

  • In this study, we analyzed radiation dose and MTF with setting of Ion chamber and changing kVp so that we are able to suggest acquiring optimized diagnostic images and minimizing patient dose. we assumed right lateral decubitus position among chest decubitus projection and set 7 combination of Ion chamber. By changing kVp(100, 110, 120, 130kVp), we exposed x-ray five times respectively and calculated average value after measuring entrance dose. we input the entrance dose value to PCXMC Monte carlo simulation tool and calculated organ dose and effective dose. Then we did physical image evaluation with MTF for the purpose to compare image quality. As a result, the high kVp, entrance dose is reduced. As change of ion chamber, when selecting second ion chamber, both organ dose and effective dose were the lowest. In contrast, selecting first ion chamber was the highest. MTF is superior to set second Ion chamber and using 120 kVp. Consequently, when taking chest right lateral decubitus using Digital radiography, the optimized combination which have both reducing dose efficiently without declining image quality and aquring good qualified image is set 120 kVp and selecting second Ion chamber.

  • Pulmonary arteriovenous malformations (PAVMs) are direct communications between pulmonary arteries and pulmonary veins, resulting in right-to-left shunts that may cause cyanosis, dyspnea, and digital clubbing. Neurological complications such as intracerebral hemorrhage or brain abscess may result from cerebral thrombosis or emboli. In most cases, they remain unrecognized until the late teenage years. Here, we report a case of a 6-year-old boy who presented with perioral cyanosis, digital clubbing, and dyspnea on exertion. A plain chest X-ray showed a focal nodular opacity in the right lower lobe (RLL), and a diagnosis of a large PAVM in the RLL was confirmed by chest computed tomography. A right lower lobectomy was successfully performed without any complications. Although their incidence in children is low, PAVMs should be suspected as a possible cause of cyanosis and dyspnea of non-cardiac origin, and should be treated promptly to prevent further neurological complications.

  • DR has had an important fact not only in the department of radiology but also in productivity or work efficiency of a whole hospital. The environment of DR has more various parameter than CR, so it is able to supply high quality of medical services. The current environment of radiology department in each hospital has been changed from Film-Screen system to DR through Full-PACS. This hospital which uses Full-PACS became to study the proper condition of CR and DDR and how the image quality of them is expressed among general photographing systems in the DR environment. From this experiment, the image quality of DDR is better than CR under the same exposure condition. And in the DDR system, the score of image which uses AEC is a little higher than the score which doesn't use it. Especially it can be known that the function of AEC of DDR is useful to improve the image quality in the part of skull and chest. (The function of AEC : It is the tool that detects the ionized current of x-ray which goes through objects with using the ion chamber which is in the detector. Also it controls the examination of X-ray when the proper density is reached.) Because the proper degree of density can be represented by this system, the photographing can be taken much easily without consideration of the exposure condition with the thickness of various objects. From the result of this experiment, it can be known that the selection of proper exposure condition plays an important rule to gain good Image Quality. More researches will be necessary about DDR system which has potential ability in the future.

  • Visual evaluation of chest radiograph images is the most practical and effective method. This study compared the Body Mass Index, waist circumference, and mAs with chest radiographs of 351 women. The Bureau of Radiological Health method was used to evaluate the image quality of chest X-ray images by anatomical and physical methods. The average age of the subjects was $30.17{\pm}4.73$ and the average waist circumference was $66.91{\pm}4.67cm$. The mean Body Mass Index value was $20.21{\pm}2.23$, the mean value of mAs was $3.04{\pm}0.78$, and the mean value of Bureau of Radiological Health was $79.83{\pm}8.45$. When the Body Mass Index value increased, waist circumference and mAs mean value increased. The mean value of Body Mass Index was statistically significant(p<0.05) in Group 4 compared to Groups 1 and 2, with increasing Body Mass Index. Exposure control of the automatic exposure control system is considered to be well performed according to body thickness or Body Mass Index at the time of chest radiography. As the Body Mass Index increases, the thickness of the body increases and the breast thickness of the woman also increases. Therefore, it is considered that the exposure amount is changed by the automatic exposure control device to affect the image quality.

  • The most critical point in the medical use of radiation is to minimize the patient's entrance dose while maintaining the diagnostic function. Low-energy photons (long wave X-ray) among diagnostic X-rays are unnecessary because they are mostly absorbed and contribute the increase of patient's entrance dose. The most effective method to eliminate the low-energy photons is to use the filtering plate. The experiments were performed by observing the image quality. The skin entrance dose was 0.3 mmCu (copper) filter. A total of 80 images were prepared as two sets of 40 cuts. In the first set (of 40 cuts), 20 cuts were prepared for the non-filter set and another 20 cuts for the Cu filter of signal + noise image set. In the second set of 40 cuts, 20 cuts were prepared for the non-filter set and another 20 cuts for the Cu filter of non-signal image (noisy image) with random location of diameter 4 mm and 3 mm thickness of acryl disc for ROC signal at the chest phantom. P(S/s) and P(S/n) were calculated and the ROC curve was described in terms of sensitivity and specificity. Accuracy were evaluated after reading by five radiologists. The number of optically observable lesions was counted through ANSI chest phantom and contrast-detail phantom by recommendation of AAPM when non-filter or Cu filter was used, and the skin entrance dose was also measured for both conditions. As the result of the study, when the Cu filter was applied, favorable outcomes were observed on, the ROC Curve was located on the upper left area, sensitivity, accuracy and the number of CD phantom lesions were reasonable. Furthermore, if skin entrance dose was reduced, the use of additional filtration may be required to be considered in many other cases.

  • This convergence study analyzed the effectiveness of digital radiography system of copper(Cu) filter in the added filtration for the removal of lower energy radiation through dose and image evaluation. We were analyzed from April to June 2015 result of the examination. Cu filter was applied to each non, 0.1, 0.2, 0.3 mm according to change of kV and mAs and doses were evaluated. Image quality was evaluated by PSNR, MAE, MSE, CNR, SNR and qualitative analysis was performed by seven items for resolution and contrast from chest x-ray criteria of national cancer checkup. The absorbed doses with Cu were lowered by 16-88 % than non-filter but the gaps decreased as kV increased. PSNR were over 30 dB and all significant and CNR and SNR were superior with non-filter but in the qualitative analysis, there were different statistical significant according to each item. The score of 0.1 mm filter was high at pulmonary blood vessel observation and in the 0.3 mm Cu, there were no statistical signigicant except high density and full of air portion. Cu filter can improve image quality with lower radiation dose using better radiation quality and correction power at digital radiography system.

  • The purpose of this study was to investigate the features of CsI:Tl and $Gd_2O_2S$ detectors with an indirect conversion method using phantom in the DR (digital radiography) system by obtaining images of thick chest phantom, medium thickness thigh phantom, and thin hand phantom and by analyzing the SNR and CNR. As a result of measuring the SNR and CNR according to the thickness change of the subject, the SNR and CNR were higher in CsI:Tl detector than in $Gd_2O_2S$ detector when the medium thickness thigh phantom and thin hand phantom were scanned. However, when the thick chest phantom was used, for the SNR at 80~125 kVp and the CNR at 80~110 kVp in the $Gd_2O_2S$ detector, the values were higher than those of CsI:Tl detector. The SNR and CNR both increased as the tube voltage increased. The SNR and CNR of CsI:Tl detector in the medium thickness thigh phantom increased at 40~50 kVp and decreased as the tube voltage increased. The SNR and CNR of $Gd_2O_2S$ detector increased at 40~60 kVp and decreased as the tube voltage increased. The SNR and CNR of CsI:Tl detctor in the thin hand phantom decreased at the low tube voltage and increased as the tube voltage increased, but they decreased again at 100~110 kVp, while the SNR and CNR of $Gd_2O_2S$ detector were found to decrease as the tube voltage increased. The MTF of CsI:Tl detector was 6.02~90.90% higher than that of $Gd_2O_2S$ detector at 0.5~3 lp/mm. The DQE of CsI:Tl detector was 66.67~233.33% higher than that of $Gd_2O_2S$ detector. In conclusion, although the values of CsI:Tl detector were higher than those of $Gd_2O_2S$ detector in the comparison of MTF and DQE, the cheaper $Gd_2O_2S$ detector had higher SNR and CNR than the expensive CsI:Tl detector at a certain tube voltage range in the thick check phantom. At chest X-ray, if the $Gd_2O_2S$ detector is used rather than the CsI:Tl detector, chest images with excellent quality can be obtained, which will be useful for examination. Moreover, price/performance should be considered when determining the detector type from the viewpoint of the user.

  • We analyzed image factors to determine the characteristic factors that need for intelligent replenishment system of the auto film processor. We processed the serial 300 sheets of radiographic films of chest phantom without replenishment of developing and fixation replenisher. We took the digital data by using film digitizer which scaned the films and automatically summed up the pixel values of the films. We analyzed characteristic curves, average gradients and relative speeds of individual film using densitometer and step densitometry. We also evaluated the pH of developer, fixer, and washer fluid with digital pH meter. Fixer residual rate and washing effect were measured by densitometer using the reagent methods. There was no significant reduction of the digital density numbers of the serial films without replenishment of developer and fixer. The average gradients were gradually decreased by 0.02 and relative speeds were also gradually decreased by 6.96% relative to initial standard step-densitometric measurement. The pHs of developer and fixer were reflected the inactivation of each fluid. The fixer residual rates and washing effects after processing each 25 sheets of films were in the normal range. We suggest that the digital data are not reliable due to limitation of the hardware and software of the film digitizer. We conclude that average gradient and relative speed which mean the film's contrast and sensitivity respectively are reliable factors for determining the need for the replenishment of the auto film processor. We need more study of simpler equations and programming for more intelligent replenishment system of the auto film processor.

  • Filter absorbs low-energy X-ray to increase the average energy and reduces patient exposure dose. This study investigates if the materials of Mo and W could be used for the digital imaging device CR by conducting image assessment and dose measurement of SNR, FOM and histogram. In addition, measurement of beam quality was conducted depending on the material of the filter, and at the same time, a proper combination of filters was examined depending on the change in tube voltage (kVp). In regard to entrance skin dose, Mo filter showed the dose reduction by 42~56%, compared to Cu filter. Moreover, Mo filter showed higher transmission dose by around 1.5 times than that of Cu filter. In image assessment, it was found that W was unsuitable to be used as a filter, whereas Mo could be used as a filter to reduce dose without decline in image quality at the tube voltage of 80 kVp or higher. As tube voltage increased, 2.0 mm Al+0.1 mm Mo almost had a similar histogram width to that of 2.0 mm Al+0.2 mm Cu. Therefore, Mo filter can be used at relatively high tube voltage of 80 kVp, 100 kVp and 120 kVp. The SNR of 2.0 mm Al+0.1 mm Mo did not show any significant difference from those of 2.0 mm Al+0.2 mm Cu and 2.0 mm Al+0.1 mm Cu. As a result, if Mo filter is used to replace Cu filter in general radiography, where 80 kVp or higher is used for digital radiation image, patient exposure dose can be reduced significantly without decline in image quality, compared to Cu filter. Therefore, it is believed that Mo filter can be applied to chest X-ray and high tube voltage X-ray in actual clinical practice.
